Drug interactions between herbal medicines and pharmaceutical drugs through concurrent use of them may increase or decrease the pharmacological and toxicological effects of either component. Especially, the possible adverse effects that may arise from drug interactions between herbal medicines and pharmaceutical drugs are equally worryig. Plausible cases of herbal medicine-pharmaceutical drug interactions include : bleeding when warfarin is combined with Dansam(Salvia miltiorrhiza), ginkgo(Ginkgo biloba), garlic(Allium sativum), or Danggui(Angelica sinensis); mild serotonin syndrome in patients who mix St John's wort(Hypericum perforatum) with serotonin-reuptake inhibitors; potentiation of oral and topical corticosteroids by liquorice(Glycyrrhiza glabra); decreased blood concentrations of prednisolone when taken with the herbal preparation(Shosiho-Tang); and increased blood concentrations of cyclosporine and calcium antagonists felodipine when taken with the grapefruit juice. The intestinal bacteria, Eubacterium sp. and Bifidobacterium sp., participate in the metabolism of active herbal medicine ingredients, glycyrrhizin, sennoside and baicalin, etc. Since antibiotics and bacterial preparations affect the bacterial population in intestinal bacterial flora, metabolism of the active herbal medicines in the bacterial flora may be altered by their combined administration. Many reports of herbal medicines-pharmaceutical drug interactions are sketchy and lack laboratory analysis of suspect preparations. Health-care practitioner should caution patients against mixing herbal medicines and western drugs

북한에서의 제약시장 생성과 확대 요인

This paper analyzes the factors of the creation and expansion of the pharmaceutical market in North Korea after the economic crisis of the 1990s in terms of ‘supply’. As a result of the analysis, the factors for the creation and expansion of the pharmaceutical market are as follows. First, the economic crisis led to paralysis of the operation of state-run pharmaceuticals, resulting in overheating of drug demand. This creates a pharmaceutical market by attracting state-owned pharmaceutical workers and individual merchants as suppliers. Second, as the demand for medicines increased, the pharmaceutical market attracted individual technicians to drug manufacturers (suppliers).Pharmaceutical manufacturing has led to the voluntary marketization of state-owned companies by deriving new markets centered on the region where the pharmaceutical industry base is established, and promoted market division and drug market development across the country. Third, the drug supply network gradually expands to the overseas market, which was the main reason for the health and medical policy that actively promoted the production of Oriental drugs.Initial capital such as facilities and materials is essential to produce Oriental Pharmaceuticals, which highlighted domestic medicinical herb exports as a source of earnng foreign currency.Various agents linked to China exported North Korean herbs and imported Chinese medicines in large quantities, resulting in the expansion of the pharmaceutical market. Since the inauguration of the Kim Jong-un administration, overseas joint pharmaceutical companies and pharmacies have entered Pyongyang due to market-friendly policies, and private pharmacies belong to the local governments are spreading. These phenomena implie that the fee-based drug supply is progressing in North Korea, and suggests the possibility of cooperation in the pharmaceutical industry where the two Koreas can coexist in the future. If South Korea imports high-efficiency North Korean medicinal herbs at low prices and invests in pharmaceutical equipment and technology in North Korea, the country will respond to the diversification of drug demand by developing domestic market and further entering the international market, and establishing domestic and foreign supply chains to cope with covid- 19, etc., are expected to be effective in inter-Korean cooperation. *Key word : State controlled pharmaceutical market, pills, antibiotics, manufacturing, division of labor, herbal exports, inter-Korean cooperation. [국문요약] 본 논문은 1990년대 경제위기 이후 북한에서의 제약시장 생성과 확대요인을 ‘공급’의 측면에서 분석하였다. 분석 결과 제약시장의 생성 및 확대 요인은 다음과 같다. 첫째, 경제위기는 국영제약 운영의 마비로 이어져 의약품 수요의 과열현상을 초래했다. 이는 국영제약 종사자와 개인 상인들을 공급자로 유인해 제약시장 생성을 불러왔다. 둘째, 의약품 수요의 증대에 따라 제약시장은 개인 기술자들을 의약품 제조자로(공급자)로 유인하였다. 의약품 제조는 제약공업 기반이 구축되어 있는 지역 중심으로 새로운 시장을 파생시키며 국영기업의 자발적 시장화를 불러왔으며 전국적 범위에서 시장 분업과 제약시장 발달을 촉진하였다. 셋째, 의약품 공급 네트워크는 점차 해외시장으로 확대되는데, 이는 고려약 생산을 또 다시 적극 추진한 보건의료정책이 동인으로 작용했다. 고려약을 생산하려면 설비와 자재 등 초기자본이 필수적인데, 이는 약초 수출을 외화벌이 원천으로 부각시켰다. 중국과 연계된 다양한 주체들이 북한산 약초를 수출하는 동시에 중국 의약품을 대량 수입해 제약시장 확대를 초래했다. 김정은정부 출범 이후 시장 우호적 정책으로 인해 해외 합작 제약회사들과 약국들이 평양에 들어서고, 지방정부 명의 개인약국이 확산되는 추세는 사실상 북한에서의 의약품 유료화가 진전되고 있음을 함축하고 있으며 향후 남북이 상생하는 제약산업 협력 가능성을 시사한다. 효능이 뛰어난 북한산 약초를 저가로 수입하고, 제약설비와 기술을 투자하면 북한 내수시장을 성장시키고 나아가 국제시장에 진출함으로써 북한 스스로 의약품수요의 다변화에 대응해 국내외 공급망을 구축해 자립성을 추구하는 북한 당국의 의도에 긍정적 영향을 미칠 것이다. 또한 코로나19 등 남북보건협력에 적극 나서도록 지평을 넓히는 중요한 초석이 될 것으로 본다. *주제어 제약시장, 알약, 항생제, 개인제조, 약초수출, 중국시장, 남북협력.

한방의료기관 조제ㆍ포제 관련 법규에 대한 고찰

Objectives : The purpose of this study is to analyze and identify the problems of current laws and regulations regarding preparation and processing of herbal medicines at hospitals of Korean Medicine. Possible solutions are proposed in the end Methods : Based on the status of hospitals of Korean Medicine and characteristics of Korean Medicine and Korean Medicine industry, I analyze the laws and regulations in regards to preparation and processing of herbal medicines and propose possible solutions. Results : Regulations for the agents, places, and cautions in respect of preparation and processing of herbal medicines are inadequate. Meanwhile, the definition of drug preparation in the Pharmaceutical Affairs Act does not apply to processing and preparation of herbal Medicines at hospitals of Korean Medicine, since herbal medicines accompanies chemical and physical change. Discussions & Conclusions : New regulations for on-site preparation at hospitals of Korean Medicine are necessary. Also, the definition for herbal medicines preparation, which states possible chemical and physical changes of herbal medicines, should be specified in Pharmaceutical Affairs Act.

약사법상 한약제제의 정의 중 한방원리의 의미에 대한 고찰

Objectives : In Pharmaceutical Affairs Act, herbal medicinal preparations are defined as medicines made by Korean medicine principles. But in Act, Korean medicine principles are vague. Thus, there is a request to explain what the Korean medicine principles are. The aim of this study is to suggest implications of Korean medicine principles in definition of herbal medicinal preparations. Methods : With regard for definition of Korean medicine principles, we referred to the domestic and foreign regulations and literature about the history of herbal medicinal preparations. As a result, the meaning of Korean medicine principles was historically examined from various angles. Results : Through this study, we studied the Korean medicine principles from the past medical history. Due to the broad definition of Korean medicinal principles, we tried to extract general ideas of medicine principles first. We also found that we had scientific formulations based on korean medicine principles which could be used in modern society. In the end, we found that numerous medicine principles which include ‘Four qi and five flavors theory’, ‘Processing of medicinals’, ‘Yin and yang theory’, ‘Five phases theory’, ‘Meridian entry of viscera and bowels theory’, ‘Herb Couplet Interaction theory’, ‘Sovereign, minister, assistant and courier theory’, etc. Conclusions : Innumerable principles used from the past existed. These principles were broad and could be used as modern scientific principles. Based on these facts, we illustrated details of Korean medicine principles, hope this principles be widely shared and Korean herbal medicinal preparations be further developed.

한국의 한약제제 생산현황과 발전방안

Recently. the herbal industry has expanded into the herbal dietary supplements industry, the herbal cosmetics industry, etc. But still the main form of herbal drug usage is water extraction- packaging after herb preparation in private clinics. There was no report or data of investigations about industrial herbal preparations. To search for a better plan of herbal drug improvement and popularization. we surveyed the present state of Korean herbal preparation production. We made an industrial herbal preparation list, investigated the total amount of production, the chief items of manufacture. names of prescriptions each item is based on. scales of each production company, etc., and report on them on this article.

However, contrary to the purpose of the system of separation of prescribing and dispensing about the oriental herbal medicine, the Wonoi-Tangjeonsil regulations were introduced under Medical law enforcement rules in 2008. Actually the Wonoi-Tangjeonsil is as well as contrary to the intent of separation of prescribing and dispensing about the oriental herbal medicine, addition to the legal terminology confusion between the preparation and manufacture, discrimination of another doctor and acupuncturist, the violation of Pharmacists and doctors on the Scope of Work, violation of the principle of legal reserves, Violation of several provisions of the Pharmaceutical Affairs Law, etc, there are Various problems about Wonoi-Tangjeonsil. Therefore revision work will be needed for modification and complementary to a portion of which may be in violation of the Pharmaceutical Affairs Law. All of these issues, to trying to enforce separation of prescribing and dispensing about the oriental herbal medicine will be the fundamental solution.

Jageum-Jung, the herbal pharmaceuticals, inhibits the hepatic fibrogenesis as mediated with TGF-β1/smad signaling

Background Herbal prescriptions have various effects and their efficacy is potentiated by the use of combinations of medicinal herbs. Objective Jageum-Jung (JGJ) is a traditional oriental medical prescription composed of five herbs. It has been used for detoxifi cation, and as an anti-inflammatory and antitumor agent. However, the effect of JGJ on hepatic fibrogenesis is unclear. Results We investigated the role of JGJ in TGF-β1/smad signaling, which is implicated in fibrogenesis, and its hepatoprotective effect in CCl 4 -treated mice with liver fi brosis. Treatment of LX-2 cells with TGF-β induced expression of mediators (α-SMA, PAI-1, and MMP-2) of fibrogenesis and activation of proinflammatory cytokines (TNF-α, IL-1β, and IL-6). However, these were downregulated by pretreatment with JGJ. In mice, oral administration of JGJ prevented liver injury induced by CCl 4 , as indicated by decreases in the ALT and AST levels. Conclusions JGJ inhibits hepatic fibrogenesis and TGF-β1/Smad signaling.
