      • Multiple myeloma 환자에서 Bortezomib치료 후 발생한 마비성 장폐색증 1례

        Bortezomib is the first clinically available proteasome inhibitor that is clinically effective in the treatment of multiple myeloma. A proteasome inhibitor acts through multiple mechanisms to arrest tumor growth, tumor spread, and angiogenesis. The main adverse effects of bortezomib are gastrointestinal symptoms, cytopenia, fatigue, and peripheral neuropathy. To date, severe paralytic ileus has not been reported as a toxic effect of bortezomib treatment in multiple myeloma. Bortezomib is a novel agent that has only been used clinically for 30 months, so a need exists to further evaluate its toxicity. We report a case of grade Ⅲ (NCI CTCAE v3.0) or grade Ⅳ(SWOG toxicity criteria) paralytic ileus in a 65-year-old man with relapsed multiple myeloma who underwent one cycle of single-agent bortezomib treatment scheduled in a 21-day cycle (1.3 mg/㎥ as a single i.v. bolus on days 1, 4, 8, and 11).

        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

      • 지붕경사각 변화에 따른 막구조의 풍압 변화 특성

        This paper describes on the results of 1:400 geometrical laboratory modeling of wind-induced point pressure on the membrane roof structure of Stadium with variable roof slopes. The wind flow for flat ground surface was simulated in a boundary wind tunnel at Kumoh National Institute of Technology. The roof slope of membrane structure changed into 5 types. The locations of wind pressures taps are distributed on the same upper and lower part of membrane roof. The resultant wind pressure for each measurement points are analysed on 2 categories. One is acting on upper direction, the other is lower direction. The results showed that in the case of upper direction the changes of wind pressures on the membrane roof are not dominant in regard to changing roof slope, but in lower direction wind pressures are increased at the rear part of roof with changing roof slopes.

      • CATCH22의 분자유전학적 특성에 관한 연구

        To investigate molecular genetic characteristics of CATCH 22 syndrome, we studied the correlation between phenotype and target gene screening and mode of inheritance using FISH (fluroscence in situ hybridization) and STRP(short tandem-repeated polymorphism). Hemizygosity for a region of 22q11.2 was found in 21 CATCH 22 patients. 2 of 19 family (11%) had familial CATCH 22 syndrome and affected 2 parents were mothers. No familial cases were found among CATCH 22 syndrome with absent thymus/ DiGeorge syndrome. The conotruncai cardiac anomalies such as Tetralogy of Fallot and vascular anomalies were common among CATCH 22 syndrome. Both the STRP maker screening(D22S941) and FISH(D22S75) were efficient and useful for the detection of chromosome 22q11 deletion and the evaluation of parent of origin but show some heterogeniety. Markers D22S75 and D22S941 appear to be consistently hemizygous in patients with deletions, so it was very useful in screening the patients with the conotrucal cardiac anomalies.

        향토수목원 및 관광지 조경을 위한 자생식물의 개발과 이용

        要約우리는 우리 지방에 자생하면서 기후풍토에 적응된 아름답고 진귀한 많은 화목류와 교목류 등을 부존하고 있으면서도 이들을 연구 개발하지 않고 주로 도입종과 재배종에만 의 존하고 있는 것이 오늘의 현실이다. 그러므로, 국내외의 광광객이 dyurn하는 즉, 강원도는 물론 우리나라 고유의 정취와 이미지를 줄 수 있는 많은 야생종들을 연구, 개발하여 조경식 물화시켜 대량으로 번식시키는 동시에 일반 재배화함으로써 향토색 짙은 관광한국을 만들어 관광사업의 근본목표인 경재적 효용을 다하기 위해 매우 개발가치가 있는 교복류 37종과 관 목류 73종의 특성, 재배법, 용도 등을 조사하였다.ABSTRACTThis study examined ecophysiological characteristics, cultivation methods and planting uses of tree and shrub species native to Kangwon province and valuable as landscape plants.The results were as followos :1. 73 species selected as shrubs were; Rhododendron brachycarpum, Rhododendron micranthum, Abelia mosanensis, Abeliophyllum distichum, Acanthopanax sessiliflorus, acanthopanax senticosus, Alangium platanifolium var. macrophyllum etc. 2. 38 species selected as trees were ; Cornus controversa, Sorbiis commixta. Mains baccata, Prunus padns, Acer triflorum, Acer tegmentosum, Styrax japonica, Styrax obassia, Sorbiis ainifolia etc. 3. Shade-tolerant species were ; Abies koreana. Rhododendron brach\carpum. Rhododendron micranthum, Primus padus, Sorbus commixta, Acer triflorum, Rhus japonica, Cornus kousa, Kalopanax pictus, Chionanthus retusa, Sorbus ainifolia, Magnolia sieboldii, Sorbaria sorbifolia var. stellipila, Berberis koreana, Lindera obtiisiloba, Rhododendron schlippenbachii and Euonymus alatiis. 4. Species growing well even in less fertile soils were ; Finns thunbergii, Quercus dentata, Koelreuteriapaniculata, Rhus japonica, Quercus acutissima, Albiz^ia julibrissin, Sorbus ainifolia, Staphylea buinalda, Philadelphus schrenckii, Clerodendron Iriclwtomum, Deut^ia coreana, Weigela florida, Zanthoxyiuin schinifolium and Lindera obtusiloba. 5. Salt-tolerant species were ; Finns thunbergii, Quercus aliena, Primus padus, Styrax japonica, Quercus dentata, Kalopanax pictus, Chionanthus retusa, Albizua julibrissin, Tilia amurensis, Forsvthia koreana, Lonicera inaackii, Samplocos chinensis for. pilosa, Clerodendron trichotomum, Sambiicus williamsii var. coreana, Abelia mosanensis, Deutua coreana. Viburnum cariesii, Rhus japonica, Rhododendron yedoense var. poukhanense, Zanthoxylum schinifolium, Callicarpa dichotoma, Rhododendron schlippenbachii and Euonymusalatus. 6. Pollutant-tolerant species were ; Styrax japonica, Prunus padus, Quercus dentata, Kalopanax pictus, Chionanthus retusa, Gleditsia japonica var. koraiensis, Styrax obassia, Forsythia koreana, Spiraea salicifolia, Clerodendron trichotomum. Viburnum sargentii, Viburnum cariesii, Weigela florida and Rhododendron vedoense var. poukhanense.

      • 균열도가 R·C 교량의 수명에 미치는 영향

        There are several causes. the percussion from the exterior, the decline of capacity by circumstances, the crack occured in the interior, the carbonation and soon, that can shorten the span of life of concreted construction. However, the crack itself, as the primary factor of the life-curtailment of concreted construction, is principal and should bring about the erosion of the reinforcing rod and the curtailment of sticking strength between concrete and reinforcing rod and the span of life. The span of life of construction influenced by crack will be treated in this report. Finally, I think that the Crack-Ratio found out by the outside investigation with timeworn R·C bridge should be utilized for deciding bridge's life and danger with statistics·analysis of used-life/carbonation-life, used-life/ fatigue-life.

        There exists a major impediment to learning electricity which may illuminate why direct presentation of scientists' ideas is too aggressive a strategy. This impediment lies in thinking that electricity is used up and in envisaging the light bulb as a consumer. This view is based on a powerful idea which summarises much of our experience; it declares that 'things get used up', 'you cannot get something for nothing', and 'intuitive thinking'. To replace this view with scientific view, we made the analogy model. It is suggested that the analogy model is more effective in teaching electricity circuits than conventional teaching method.

      • 겨우살이 첨가 김치의 발효양상과 항돌연변이 및 암세포성장저해효과

        Fermentation characteristics, and antimutagenic and anticancer effects of mistletoe added kimchi were studied. The changes in pH, acidity and the value of prevalence test of the mistletoe added kimchi during the fermentation were determined. Ames antimutagenicity test against aflatoxin B_(1) (AFB_(1)) and N-methyl-N'nitro-N-nitrosoguanidine (MNNG) in Salmonella typhimurium TA100, and MTT assay using HT-29 human colon carcinoma cells and AGS human gastric adenocarcinoma cells were employed to test the chemopreventive effect of the kimchis. Mistletoe added kimchis were fermented at 15℃ for 1 days and then ripened at 5℃ to reach optimum pH of 4.3. Mistletoe added kimchis fermented slowly compared to the control kimchi when the changes of pH and acidity of the kimchis were compared. Sour flavor of mistletoe in the kimchis was decreased during the fermentation. Mistletoe added kimchis exhibited higher inhibitory effect against the mutagenicities induced by AFB_(1) and MNNG in the Ames test than the control kimchi. Especially, 2% mistletoe added kimchi showed the highest inhibitory effect (75%, 68%) in the Ames test (p<0.05), Two % mistletoe added kimchi showed the highest anticancer effect against HT-29 human colon cells (p<0.05). Mistletoe added kimchis exhibited significantly higher anticancer effect than the control kimchi in AGS cells, too, however, there was no significant different effect among the different levels of mistletoe (0.5∼2.0%) addition (p<0.05). It can be concluded that addition of mistletoe in kimchi increased preservation period of the kimchi and enhanced antimutagenic and anticancer effects of the kimchi.
