        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

      • 만성 화농성 중이염의 세균학적 고찰

        The chronic suppurative otitis media is one of the most common disease in the otologic field. And the works on bacterial distribution of chronic suppurative otitis media were studied by many otologist, but there is some difference. The bacteriologic study was done on 126 cases of chronic suppurative otitis media who visited the Department of Otolaryngology, Chosun University Hospital between January 1989 and December 1989 and the following conclusions were obtained. 1) In sex distribution, the male had higher incidence rate than female (M: 56.3%, F:43.7%). 2) In age distribution, 3rd decade (34.1%), 2nd decade (34.1%), were most prevalent. 3) In duration of the disease, the hightest frequence was below 5 years duration 4) Among 126 cases, 98 cases(77.8%) were unilateral (right ear was 47 cases(37.3%) and left ear was 51 cases(40.5%))and 28 cases(22.2%) were bilateral. 5) The 136 cases(88.3%) showed the positive results under culture, but 18 cases(11.7%) didn't growthed. Among 136 cases 95 cases(61.7%) had single infection and the 41 cases(26.6%) had mixed infection. 6) The most frequently isolated bacteria was staphylococcus(38 strains, 27.9%) and next were proteus(27 strains, 19.9%) and pseudomonas (22 strains, 16.2%). 7) The Staphylococcus was sensitive to cephalothin(94.7%), amikacin(78.9%) and resistant to penicaIin(63.2%). 8) The Proteus was sensitive to amikacin(92.6%), cephalothin(88.9%) and resistant to minocycline(81.5%) and gentamicin(70.4%). 9) The Pseudomonas was sensitive to amikadn(86.4%), gemtamicin(59.1%) and resistant to minocycline(90.9%), ampicillin(86.4%) and cephalothin(86.4%).

      • 경상남도 체육공원의 활성화 방안에 관한 연구 : 창원, 마산, 진해, 김해지역을 중심으로

        본 연구는 최근 생활체육의 장으로 널리 활용되고 있는 도내 체육공원의 현황을 파악하고 시설에 대한 미비점과 문제점, 시설이용에 대한 애로점, 시민들의 요망사항 등을 고려하여 체육공원 이용 활성화 방안을 알아보기 위한 것이다. 이를 위하여 창원·마산·진해·김해 지역의 체육공원 중 다양한 계층이 고르게 사용하고 있는 13곳의 체육공원을 선정하여 직접 현장에 가서 현황을 파악하였으며 또한 일반 시민을 대상으로 설문조사를 실시하였다. 이러한 내용을 조사·분석한 결과 다음과 같은 결론을 얻을 수 있었다. 1. 체육공원에 대한 행정관청 주도의 적극적은 홍보가 필요하다. 2. 체육공원 설치시 그 지역의 이용지를 예상하여 체육공원을 설계하여야 한다. 3. 연령이나 남·녀, 개인 및 집단, 환경 등을 고려하여 위치를 선정하고 다양한 사용자에 대하여 개인적으로 운동욕구나 흥미를 줄 수 있는 운동기구의 설치 및 확충이 요구된다. 4. 운동기구의 정확한 명칭, 운동요령, 운동효과, 체력기준표의 설치가 필요하며 설치 후 세심한 관리가 요구된다. 5. 운동효과를 극대화하기 위한 운동기구의 배치가 요구된다. 6. 화장실, 음수대(세면대), 차양막, 담소장소 등 부대시설의 확충이 요구된다. 7. 학교, 직장체육시설, 아파트 단지 내 소규모 체육공원의 설치와 필요할 경우 사설 시설과 연계할 수 있는 방법을 모색하여야 한다.

      • 실험실적 당뇨성 흰쥐에서 acetaminophen의 대사에 관한 연구

        Acetaminophen is one of the most important drugs used for the treatment of mild to moderate pain when an anti-inflammatory effect is not necessary. Phenacetin, a prodrug that is metabolized to acetaminophen, is more toxic than its active metabolite and has no rational indications. In this study, we were observed activities of free radical generating enzymes, free radical scavenging enzymes and glutathione-related enzymes as well as detoxification mechanism of against acetaminophen-induced hepatotoxicity in STZ-induced hyperglycemic rat. Activities of cytochrome p450, cytochrome b5, aminopyrine demethylase and aniline hydroxylase were changed by the treatment acetaminophen in hyperglycemic rats. It was also observed that activities of conjugating enzymes as sulfotransferase and UDP-glucuronyltransferase was slightly changes against acetaminophen-induced in hyperglycemic rats. Acetaminophen results in the significantly decrease in the level of hepatic glutathione concentration and √-glutamylcysteon, synthetase activities in hyperglycemic rats. And, glutathine S-transferase, superoxide dismutase and glutathione peroxidase activities were significantly increased against acetaminophen-inducedas in hyperglycemic rats. Glutathione reductase and catalase were not affected for experimental state. Furthemore, acetaminophen highered serum alanine, aspartate aminotransferase, sorbitol dehydrogenase, √-glutamylpeptidase, alkaline phosphatase and lactate dehydrogenase activities in hyperglycemic rats.         Fabrication of newspaper-based potentiometric platforms for flexible and disposable ion sensors

        <P><B>Abstract</B></P> <P>Paper-based materials have attracted a great deal of attention in sensor applications because they are readily available, biodegradable, inexpensive, and mechanically flexible. Although paper-based sensors have been developed, but important obstacles remian, which include the retention of chemical and mechanical stabilities when paper is wetted. Herein, we develop a simple and scalable process for fabrication of newspaper-based platforms by coating of parylene C and patterning of metal layers. As-prepared parylene C-coated newspaper (PC-paper) provides low-cost, disposable, and mechanically and chemically stable electrochemical platforms for the development of potentiometric ion sensors for the detection of electrolyte cations, such as, H<SUP>+</SUP> and K<SUP>+</SUP>. The pH and K<SUP>+</SUP> sensors produced show near ideal Nernstian sensitivity, good repeatability, good ion selectivity, and low potential drift. These disposable, flexible ion sensors based on PC-paper platforms could provide new opportunities for the development of point-of-care testing sensors, for diagnostics, healthcare, and environment testing.</P> <P><B>Graphical abstract</B></P> <P>Flexible and disposable USB-type ion sensors were fabricated using newspaper-based platforms by coating of parylene C and patterning of metal layers, showing a near-Nernstian reponse, a wide detection range, a rapid response time, repeatability, selectivity, and stability.</P> <P>[DISPLAY OMISSION]</P>

        Clinical Analysis and Red Flag Signs in Pediatric Headache According to Age

        Purpose: The clinical characteristics of headaches vary by age among pediatric patients. Red flag signs are key factors in differentiating secondary headaches and should be considered in the context of the patient’s age. Methods: This study involved a retrospective chart review of pediatric patients presenting with headaches. Patients were categorized by age into three groups: pre-school age (under 6 years), school-age (6 to 12 years), and adolescence (over 12 years). Demographic data, headache characteristics, laboratory findings, and neuroimaging results were evaluated. Overall, 17 potential red flags were assessed. Results: A total of 687 patients were included, of whom 102 were of pre-school age, 314 were school-aged, and 271 were adolescents. The frequency of overweight/obesity was found to increase with age. The pre-school age group experienced a shorter period from symptom onset to presentation and a briefer duration of pain. In contrast, adolescents displayed a longer period from symptom onset, a greater frequency of headaches occurring at least three times per week, and a higher rate of headache episodes lasting over 3 days. Children under 6 years old were more commonly diagnosed with secondary headaches than older children. Across age groups, secondary headaches were suspected when systemic symptoms such as fever were present, when the headache had a sudden onset, when the patient responded poorly to medication, or when abnormal neurological signs and symptoms were observed. Conclusion: The clinical features of pediatric patients vary by age group. Clinicians should consider red flag signs in the context of patient age and individual characteristics.

      • Extremely Fast Self-Healable Bio-Based Supramolecular Polymer for Wearable Real-Time Sweat-Monitoring Sensor

        <P>Sensors with autonomous self-healing properties offer enhanced durability, reliability, and stability. Although numerous self-healing polymers have been attempted, achieving sensors with fast and reversible recovery under ambient conditions with high mechanical toughness remains challenging. Here, a highly sensitive wearable sensor made of a robust bio-based supramolecular polymer that is capable of self-healing via hydrogen bonding is presented. The integration of carbon fiber thread into a self-healing polymer matrix provides a new toolset that can easily be knitted into textile items to fabricate wearable sensors that show impressive self-healing efficiency (>97.0%) after 30 s at room temperature for K<SUP>+</SUP>/Na<SUP>+</SUP> sensing. The wearable sweat-sensor system-coupled with a wireless electronic circuit board capable of transferring data to a smart phone-successfully monitors electrolyte ions in human perspiration noninvasively in real time, even in the healed state during indoor exercise. Our smart sensors represent an important advance toward futuristic personalized healthcare applications.</P> [FIG OMISSION]</BR>

      • High performance flexible pH sensor based on polyaniline nanopillar array electrode

        <P><B>Abstract</B></P> <P>Flexible pH sensor technologies have attracted a great deal of attention in many applications, such as, wearable health care devices and monitors for chemical and biological processes. Here, we fabricated flexible and thin pH sensors using a two electrode configuration comprised of a polyaniline nanopillar (PAN) array working electrode and an Ag/AgCl reference electrode. In order to provide nanostructure, soft lithography using a polymeric blend was employed to create a flexible nanopillar backbone film. Polyaniline-sensing materials were deposited on a patterned-nanopillar array by electrochemical deposition. The pH sensors produced exhibited a near-Nernstian response (∼60.3mV/pH), which was maintained in a bent state.
