연구목적 : 현재 야뇨증에서 흔히 보이는 문제행동이 야뇨증의 원인인지, 아니면 공존하는 동반된 문제인지에 대한 의견이 분분하다. 저자들은 이존 연구의 대상이 대부분 임상환자였다는 점을 감안하여 비임상군을 연구대상으로 하여 야뇨증 아동에서 흔히 보이는 문제행동이 무엇인지를 알아보고 이를 통하여 야뇨증과 문제행동간의 관련성을 밝히고자 본 연구를 시행하였다. 방법 : 충청북도 영동군 소재의 2개 초등학교 학생 1,421명중 야뇨증이 있는 72명을 환자군으로 하였고, 연령, 학년, 성별을 대비시킨 72명의 정상아동을 대조군으로 하여 수정된 아동행동조사표를 시행하였다. 결과 : 야뇨증 아동에서 불안-공포 척도, 우울 척도, 주의력결핍-과잉활동 척도, 비행 척도의 점수가 의미있게 높게 나왔다. 주의력결핍-과잉활동 척도의 점수는 성별과 학년에 관계없이 항상 의미있게 높게 나왔으면 남자와 고학년에서는 비행 척도가 의미있었고, 여자와 저학년에서는 불안-공포 척도가 의미 있었다. 결론 : 야뇨증 아동에서 다양한 문제행동이 존재한다는 것을 알았고 야뇨증 환자의 치료시 이런 문제행동들에 좀더 주의를 기울이고, 더 많은 관심을 가져야 한다는 것을 아게 되었다. This study was designed to evaluate the emotional and behavioral problems of nocturnal enuresis children(n=72) in general population. And this was compared with normal control children(n=72). The behavioral problems were assessed using a modified child behavior checklist(CBCL) parent form. The results indicated that enuretic children were rated significantly more anxious, fearful, depressed, inattentive, hyperactive, delinquent on the modified CBCL rating when compared to the age-, grade- and sex-matched normal control children. Finally, it was an impressive evidence that there exist many emotional and behavioral problems in children with nocturnal enuresis. KEY WORDS : Enuresis · Behavior · CBCL.
S-458 A Case of spontaneous full recovery from Central Pontine Myelinolysis

Central pontine myelinolysis (CPM) is a neurological disease characterized by destruction of myelin sheaths of brain cells in the pons, usually caused by rapid correction of chronic hyponatremia. Although previous reports on CPM described very poor outcome, recent some reported that relowering of serum sodium within 24hr after symptom onset showed a favor recovery from CPM. However, it is unusual spontaneous full recovery from CPM without relowing of Na. We report a case of 59-year-old woman diagnosed with CPM, and recovered spontaneously. A 59-year-old woman presented to our hospital with scalp laceration. On admission she was alcohol ordor state, but she was alert. She complained nausea and vomiting which had evolved 2 days. Her serum biochemistry showed sodium of 111.1 mEq/L. She was treated with 3% saline. And six hours later, when serum sodium was 119.9 mEq/L, 3% saline was stopped and we started 0.9% saline. At that night, she showed severe emotional lability and disorientation. She didn’t sleep all night. On 2nd admission day, serum sodium was 130.4 mEq/L. We consult to psychiatry about anxiety. She was diagnosed with delirium tremens and started on chlordiazepoxide. On 3rd admission day, serum sodium reached 137.8 mEq/L. On 5th admission day, patient became drowsy. Neurological examination revealed only general weakness. Focal neurologic deficits were not observed. So we suspected drug side effect due to chlordiazepoxide. We decided to reduce chlordiazepoxide. On 9th admission day, her drowsy mental state wasn’t improved. We stopped chlordiazepoxide. Brain MRI was performed, and MRI showed high signal intensity in the central pons and bilateral basal ganglia and thalamus on T2-weighted image and diffusion weighted imaging. She was diagnosed with CPM and EPM. We continued supportive care since then, but she couldn’t speak and couldn’t move her extremities herself for a long time. While continuing supportive care, her drowsy mental state gradually improved. On 22th admission day, she became alert and could handshake. On 26th admission day, she was discharged to convalescent hospital. Three weeks after discharge, when she came to out-patient clinic, she showed full recovered neurologic problem.

The purpose of this study was to elucidate whether the molecular defect of acid-base transporters in renal tubules is related to the functional defect of urinary acidification in distal renal tubular acidosis(RTA). We performed NH₄Cl, furosemide, or bicarbonate loading test to evaluate renal acidification function, and immunohistochemistry using antibodies to H^+ -ATPase, Cl^-/HCO₃^- exchanger(band-3 protein), and Na^+/K^+ -ATPase in kidney tissue in 6 patients with RTA and renal cell carcinoma patients as normal controls. Kidney tissue was obtained either by percutaneous needle biopsy(RTA) or nephrectomy(NC). The results were as follows; 1) In all six RTA patients, proton secretory defect of distal acidification was shown by a failure to lower the urine pH after NHC1 loading or furosemide test or abnormally low urine-blood pCO₂ difference during bicarbonate loading. In two patients with RTA, proximal acidification defect was combined, which was demonstrated by increased fractional excretion of bicarbonate. 2) In mal control, intense H^+ -ATPase and band-3 protein staining was observed in collecting ducts. 3) In distal RTA patients, H6+ -ATPase and band-3 protein staining was not demonstrable or markedly decreased in the intercalated cells of distal nephron. 4) In two patients who had both proximal and distal RTA, H^+ -ATPase staining was markedly decreased in the brush border of proximal tubules as well as the distal nephron. In conclusion, the defect of acid-base transporters in renal tubule was related with the functional defect of urinary acidification in distal RTA.
Analysis of porcine macrophage immune response to antigenic molecules and short chain fatty acids

Macrophages play an important role in both the innate and adaptive immune responses. These include phagocytosis, killing of microorganisms, antigen presentation, and induction of immune cytokines and antimicrobial genes. Macrophage activity is reported to be controlled by diverse exogenous antigenic or endogenous metabolic molecules, and the underlying mechanisms are well documented in human and mouse macrophage cells. Bacterial lipopolysaccharide (LPS) is known to be one of the most potent stimuli activating macrophages through the toll like receptor 4 (TLR4) signaling pathway. There are other antigenic molecules, such as muramyl dipeptide (MDP) and outer membrane protein A (OmpA), that are also known to activate immune cells. On the other hand, short chain fatty acids (SCFAs) such as acetate and butyrate are produced by gut microbiota and control host energy metabolism and signal transduction through GPR receptors. However, there are few studies demonstrating the effects of these molecules in macrophages from domestic animals, including domestic pigs. In this study, we attempted to characterize gene expression regulation in porcine macrophages (PoM2, Pig Monocytes clone 2) following treatment with LPS, MDP, OmpA, and two short chain fatty acids using porcine genome microarray and RT-PCR techniques. A number of novel porcine genes, including anti-microbial peptides and others, appeared to be regulated at the transcriptional level. Our study reports novel biomarkers such as SLC37A2, TMEN184C, and LEAP2 that are involved in the porcine immune response to bacterial antigen LPS and two short chain fatty acids.
