      • 고려인삼의 주아 발달 경로에 대하여

        본 시험은 인삼의 채종종자의 조직을 관찰하여 잠아의 실체를 조사하였고 묘삼의 잠아특성을 산지별로 조사하여 다경이 되는 잠아의 발달경로를 알아보았다. 그리고 묘삼을 본포에 이식하여 3년생을 채굴하여 4년생이 될 주아의 생성경로를 조사하였다. 1. 묘삼의 주아는 발아종자의 경근천이부에서 피층조직이 융기하여 돔(dome)형태의 원잠아 또는 측잠아가 형성된 후 이들 조직이 분화하여 발달하였다. 2. 묘삼의 단뇌는 90%가 원잠아에서 유래하였고 쌍뇌는 원잠아와 2%정도의 측잠아1개가 발달되어 형성되었으며 3뇌는 원잠아와 측잠아 2개가 발달하여 형성되었다. 3. 본포에서 2년생과 3년생은 주아가 단뇌로 형성되어 있어 2경개체가 발생하지 않으나 4년생은 3년생에서 발달한 쌍뇌 이상의 주아가 있어 2경개체이상이 관찰되는 최초의 연생이었다. 4. 4년생의 주아는 종자에서 형성된 원잠아, 측잠아와 2-3년생에서 뇌두부위에서 형성된 피층잠아에서 유래하는 것으로 고년생으로 갈수록 쌍뇌 또는 3뇌는 원잠아에 비해 피층잠아 유래의 주아가 많았다. We studied the substance of latent bud by surveying the embryo of harvested seed and developmental pathway of latent bud by analysing the characteristics of latent bud of local ginseng lines. We transplanted one year old ginseng seedlings in the field and harvested it out two years later. And then We investigated developmental pathway of the main bud which would be the shoot of ginseng in the fourth year. Main bud of seedling was formed from the region between root and shoot of germinating seed. Primary and axillary latent buds protruded in dome shape from cortex tissue and differentiated into main bud. Ninety percent of single main bud was derived from primary latent bud, twin main buds were derived from primary latent bud and one axillary bud, and triple main buds were derived from primary latent bud and two axillary latent buds. In the field, We could not find 2-stem plant from 2-3 years old plant, however, We found 2-stem plant from 4-year-o1d plant because twin main buds developed from 3-year-o1d plant. Therefore 2-stem plant was observed obviously from the plant at least 4 years old. Main buds of 4-year-old plant were formed from primary and axillary latent bud of seed and cortex latent bud of rhizome from 2-3 years old plant. In older plants, twin and triple main buds were derived more from cortex latent bud than primary latent bud.

      • Methicillin-내성 황색포도상구균의 각종 항균제에 대한 내성양상과 Plasmid DNA의 특성

        광주지역의 A병원과 B병원에 입원한 환자의 각종 임상가검물로 부터 분리동정한 황색포도상구균 251주를 19종의 항균제에 대한 감수성 검사를 실시하고 내성인자를 조사하여 다음과 같은 결과를 얻었다. Oxacillin에 대한 내성빈도는 A병원에서 74.2%로 B병원의 46.1% 보다 약 30% 정도가 높았다. 또한 cephalosporine계 항균제와 aminoglycoside계 약제에 대한 내성빈도는 A 병원 분리균주에서 높은 경향을 보였으며, ampicillin(Amc), tetracycline(Tc), chloram-phenicol(Chp), ciprofloxacin(Cpfx)등의 항균제에 대한 내성빈도는 A병원와 B병원에서 분리된 균주간에 유사하였다. 그러나 fosfomycin(Fom)에 대한 내성빈도는 오히려 B병원 분리균주에서 높았다. 10가지 이상의 약제에 내성을 갖는 다제내성균의 출현율은 B병원에서 보다 A병원에서 높았다. A병원와 B병원에서 분리된 MRSA 대부분이 cefamandole(Cmdl)을 제외한 cephalosporin계 약제와 aminoglycoside제제에 대해 내성을 나타내었으며, netilmicin(Net) 과 rifampin(Rfp)에 대한 내성빈도는 A병원에서 분리된 균주에서 높았으며 Fom, Tc, Chp에 대한 내성빈도는 오히려 B병원에서 분리된 MRSA 에서 높았다. 다제내성균의 출현율은 두 병원간에 큰차이가 없었다. MRSA 의 다제내성양상은 Fom, Net, Chp 및 Rfp에 대한 내성에 따라 A병원와 B병원에 서 분리된 균주간의 차이가 관찰되었는데, A병원에서 가장 많이 나타난 내성유형은 AmcCtxCzolLmoxGmTobLcmAmkNetTcEmRfp 및 AmcCtxCzolCmdlLmoxFomGmTobLcmAmkNetTcEmRfp에 중복내성을 갖는 균주가 각각 12균주로 가장 많았으며 B병원에서 분리된 균주에서 많이보이는 내성유형은 AmcCtxCzolLmoxFomGmTobLcmAmkChpTcEm에 중복내성은 갖는 균주가 11주로 가장 많았다. 두 병원에서 분리된 균주 모두 β-lactam계 약제와 aminoglycoside계 약제간의 교차내성을 가지고 있었다. MRSA균주에서 분리한 plasmid DNA profile은 A병원와 B병원에서 분리된 균주에서 약제에 대한 다제내성양상에 따라 차이가 있었으며, A병원 분리균주에서는 108kb, B병원 분리균주에서는 2.8kb의 plasmid를 보유하는 차이가 있었다. Background: Increased isolation of methicillin-resistant Straphylococcus aureus has become one of the important problem in treating staphylococcal infections. The aim of the study was to investigate the epidemiological characteristics of antimicrobial resistance of MRSA's isolated in the Kwangju area. Methods: 251 strains of S. aureus were isolated from various specimens from A-hospital and B-hospital and there resistance patterns against 19 antimicrobial agents and plasmid profiles studied. Results: Methicillin resistance was detected in 74.2% and 46.1% of A-hospital and B-hospital isolates, respectively. Resistance to cephalosporins and aminoglycosides was more frequent in the A-hospital isolates than in the B-hospital isolates, whereas fosfomycin resistance was more frequent in the B-hospital isolates than in the A-hospital isolates. Frequency of resistance to ampicillin, tetracycline, chloramphenicol, ciprofloxacin in A-hospital isolates was similar to B-hospital isolates. Strains with multiple resistance to more than 10 drugs were more frequent in the A-hospital isolates than that in the B-hospital isolates. Among MRSA strains, resistance to netilmicin and rifampin was more frequently encountered in A-hospital isolates, while resistance to fosfomycin, tetracyclin, and chloramphenicol was more frequent in B-hospital isolates. No differences in resistance patterns against other cephalosporins and aminoglycosides were observed between the two hospitals. Fosfomycin, netilmicin, chloramphenicol and rifampin resistances were different between the hospitals in strains with multiple resistance. The most frequently observed multiple resistance patterns observed among A-hospital isolates were AmcCtxCzoILmoxGmTobLcmAmkNetTcEmRfp and AmcCtxCzolCmdlLmoxFomGmTobLcmAmkNetTcEmRfp. Among B-hospital isolates, AmcCtxCzolLmoxGmTobLcmAmkChpTcEm pattern was the most frequent. Conclusion: MRSA strains from both hospitals showed cross-resistance to β-lactams and aminoglycosides. Plasmid profiles from MRSA strains were different, according to the multiple resistance patterns. Characteristically, 108 kb plasmid was detected only in A-hospital-MRSA strains, while 2.8 kb plasmid was detected only in B-hospital-MRSA strains.

        한국인 모발과 내부장기조직간 Cd, Hg, Pb 및 Zn함량의 상호관련성

        In order to study the relationships between trace element concentrations of hair and internal organs, autopsy samples of hair, liver, kidney cortex, kidney medulla, heart, lung, spleen, cerebrum, testis and bone from 162 Korean subjects were analyzed for Cd, Hg, Pb and Zn by inductively coupled plasma atomic emission spectrometry. High significant positive correlations were found between the Cd concentration in hair and bone and Pb in hair and lung. Hair Hg levels were highly significantly correlated with organ Hg levels in the liver, kidney cortex, kidney medulla, heart, lung, spleen, cerebrum, testis and bone. And hair Zn levels were highly significantly correlated with organ Zn levels in the kidney cortex, kidney medulla, spleen, testis and bone.

        L-형 장골이식을 이용한 구순열 비변형의 수정

        Patients with a cleft lip nasal deformities hope for a pleasing nasal profile, a well-defined angular nasal tip, and symmetry of the nose. The main objective of the correction of cleft lip nasal deformity is to achieve nasal symmetry and an improved nasolabial relationship. The present study was carried out to assess the effect and prognosis of correction of cleft lip nasal deformity with L-shaped iliac bone graft. We performed correction surgery of the severe cleft lip nasal deformity with the autogenous L-shaped iliac bone graft in five patients and described the technique of correction. The columella portion of the L-shaped graft provided stabilization, eliminating the see-saw effect of the bridge on the bony fulcrum of the bony bridge, The disadvantage of this technique was stiffening of the nasal tip, which included the distal end of the grafted bone. However, the nasal tip gradually becomes softer, probably due to resorption of the bony edge several months after the operation. We obtained stable and symmetric reconstruction of the supporting elements of the nose. These results suggest that L-shaped iliac bone graft is stable and esthetic reconstruction method for correction of the severe cleft lip nasal deformity

      • 백혈병 마우스 모델의 동종골수이식에서 활성화된 자연살해세포들의 보충이 이식편대백혈병효과와 이식편대숙주반응에 미치는 영향

        배경: 백혈병에서 동종골수이식 (allogeneic bone marrow transplantation)의 성공적 치료 효과를 얻기 위해서는 이식편대숙주반응 (graft-versus-host disease, GVHD) 발생의 극복과 재발의 방지가 중요한 과제이다. 골수를 역류원심성 세포분리 (counterflow centrifugal elutriation, CCE) 방법으로 분리하여 얻은 rotor off (R/O) 세포분획은 T 세포의 수는 적지만 조혈모세포들을 다량 포함하고 있어 동종골수이식에서 주조직적합복합체 (major histocompatibility complex, MHC) 차이를 극복할 수 있고, 이식편의 생착 성공과 GVHD 발생 예방에 효과적이다. 그러나 골수로부터 T 세포를 제거하면 백혈병세포를 공격하는 이식편대백혈병 (graft-versus-leukemia, GVL) 효과가 감소되기 때문에 백혈병 재발의 빈도가 높다. 자연살해세포 (natural killer cell, NK cell)의 보충 첨가는 동종골수이식 후 GVHD 발생을 줄이면서 충분한 GVL 효과를 얻을 수가 있다. 따라서 저자는 분리 후 IL-2로 활성화시킨 NK 세포들을 골수 R/O 세포분획과 함께 백혈병 마우스 모델에 동종이식함으로써 GVHD와 GVL에 미치는 효과를 관찰하였다. 방법: Balb/c (H-2^(d)) 마우스에서 유래된 A20 (murine B-lymphoma/leukemia cell line, H-2^(d)) 백혈병 세포를 이식 2 일 전에 Balb/c 마우스에 주입하고, 치사량의 전신 방사선을 조사한 직후에 Balb/c 또는 C57BL/6 (H-2^(b)) 마우스의 골수 R/O 세포분획을 꼬리정맥을 통하여 주입하였다. 이들은 모두 이식 후 6-8 주 이내에 사망하였다. 동종이식의 대조군 (n=9)에는 1 × 10^(7)의 R/O 세포분획만을 주입하였고, 실험군 (n=9)에는 C57BL/6 마우스의 비장세포들로부터 단클론항체들을 이용한 negative selection방법으로 분리한 후 IL-2로 활성화된 5 × 10^(5)의 NK 세포분획을 1 × 10^(7)의 R/O 세포분획과 함께 주입하였다. GVL 효과의 판정은 이식 후 14 일과 28 일 째 되는 날 마우스에서 골수, 비장, 간 등을 얻어 백혈병 세포들의 침윤을 조직학적으로 관찰하였으며, GVHD의 정도는 육안적 관찰법으로 평가하였다. 결과: R/O 세포분획만을 이식한 대조군의 골수, 비장, 그리고 간 조직에서는 A20 백혈병 세포의 침윤이 각각 89% (8/9), 78% (7/9)와 22% (2/9)에서 관찰되었고, R/O 세포분획과 NK 세포분획을 함께 이식한 실험군에서는 비장과 간을 제외한 골수에서만 89% (8/9)에서 A20 백혈병 세포의 침윤이 관찰되어 두 군 사이에 장기별 분포의 차이를 볼 수 있었다 (P= 0.0001). 한편 GVHD는 두 군 모두에서 경하게 나타나서 유의한 차이는 없었다. 또한 생착 부전으로 사망한 마우스는 없었다. 결론: CCE를 이용하여 T 세포를 제거한 동종골수이식에서 NK 세포의 보충은 GVHD의 악화는 일으키지 않으면서, 백혈병의 진행을 억제하는 GVL 효과를 얻을 수 있었다. Background: Allogeneic bone marrow transplantation (BMT) with T cell-depleted marrow accompanies engraftment failure and relapse of leukemia by a loss of the graft-versus-leukemia (GVL) effect frequently, while it can prevent GVHD. Supplement of NK cells could prevent GVHD and enhance GVL effect in several murine allogeneis BMT models Roter off (R/O) cell fraction obtained by counterflow centriation elutriatio (CCO) contains small number of T cells and many hematopoietic stem cells. The aim of this study was to determine the effect of R/O cell fraction supplemented with IL-2 activated NK cells on GVL and GVHD within the leukemic mouse BMT model. Methods: Inoculation of A20 (H-2d, murine B-lymphoma/leukemia, Balb/c origin) cells into Balb/c mice via the tail vein 2 days prior to lethal total body irradiation (TBI) and infusion of the Balb/c BM or C57BL/6 (H-2b) R/O fraction were performed. It resulted in 100% mortality within 6 to 8 weeks. The irradiated mice in the control group were injected with 1 × 107 R/O cell fraction alone (n=9) and in the experimental group mice were injected with 1 × 107 R/O cell fraction plus 5 × 105 negatively selected IL-2 activated NK cell fractions of the spleens via the tail vein (n=9). On day 14 and 28 after BMT, the bone marrows, spleens, and livers of mice were harvested for histopathologic analysis of the infiltrations of leukemic cells. We then evaluated the GVHD within the mice. Results: A histopathologic study of the recipients receiving R/O fraction alone showed infiltration of leukemic cells, 89% (8/9) in bone marrows, 78% (7/8) in spleens, and 22% (2/9) in livers. The experimental group of mice showed only the infiltration of leukemic cells 89% (8/9) in bone marrows, not in spleens and livers. There were the organ differences of the leukemic cells infiltrations between the two groups (P=0.0001). There were no obvious differences in the GVHD scores between these two groups, and severe GVHD was not observed. There was no engraftment failure among groups. Conclusion: Thus, our findings suggest that R/O cell fraction obtained by CCE and supplemented with NK cells can promote GVL effect without mediating clinically overt GVHD in allogeneic BMT of mouse leukemia.

        약년자 폐암의 임상적 관찰

        N/A Carcinoma of the lung is generally considered as a disease of middle age and elderly. But lung cancer is now so prevalent that patients below the age of 40 years must be increaseing. We reviewed the records of 483 lung cancer patients who had been diagnosed in the department of internal medicine of Seoul National University Hospital from March 1984 to February 1986, and the clinical features of the young patients under 40 years were compared with those of the control group patients over 40 years. The results are summarized as follows; 1) Seven percent of 34 cases of 483 patients were belong to the young age group patients (less than 40 years). The youngest patient was 22 years old. 2) Male to female ratios of young group and the control group were 3.25:1, 5.9:1 respectively (p>0.05). 3) The mean duration of symptoms from the onset to the definite diagnosis was 2.7 months in young group and 2.3 months in the control group (p>0.3). 4) The most common initial symptoms in the young group were dyspnea (26.5%), hemoptysis (20.5%), cough or sputum (20.5/), chest pain (8.8%) which were also the most common initial symptoms in the control group. (p>0.25) The incidence of superior vena cava syndrome was significantly higher in the young age group (23.5%) than in the control group (7.1%) (p<0.05). 5) In the young group, the proportion of adenocarcinoma (35%) was significantly higher than that of the control group (18%) (p<0.05). 6) The proportion of smoker was significantly lower in the young age group (13 cases, 38.2%) than in the control group (77.7%). The histological type of 13 smokers of the young group was squamous cell carcinoma in 12 cases and adenocarcinoma in 1 case. 7) All non-small cell lung cancer patients in young group were belong to stage III. 8) Distant metastasis rate of the young group (74%) was significantly higher than that of the control group (54.8%) (p<0.05).
