      • 생활체육활동이 장애인의 태도와 만족도에 미치는 영향

        This research analysed the vital statistical feature, the daily life physical activities attitude, the life satisfaction , the types of daily life physical activities compared with the disabled' attitude end satisfaction using a stratified cluster sampling method on 214 the orthopedically handicapped persons, the mentally retarded, the visually impaired, the deals, the cerebral lesion retard who are people over sixteen years old, live in Busan, Gyeongsangnamdo, Ulsan, Daegu, Gyeongbuk, and are participating in or interested in the daily life physical activities. The conclusions are as follows: First, there are significant differences among the attitudes and satisfactions of daily life physical activities according to vital statistical feature. Second, there are significant differences among the attitudes on whether the disabled are participated in the daily life physical activities or not. Third, there are significant differences among the life satisfaction and the attitude related physical activities according to the frequency of daily life physical activities participation. Forth, there are significant differences among the concerned requirements according to type of retardation.

        소아의 기계적 환기요법

        Mechanical ventilation in children has some differences compared to in neonates or in adults. The indication of mechanical ventilation can be classified into two groups, hypercapnic respiratory failure and hypoxemic respiratory failure. The strategies of mechanical ventilation should be different in these two groups. In hypercapnic respiratory failure, volume target ventilation with constant flow is favorable and pressure target ventilation with constant pressure is preferred in hypoxemic respiratory failure. For oxygenation, fraction of inspired oxygen(FiO2) and mean airway pressure(MAP) can be adjusted. MAP is more important than FiO2. Positive end expiratory pressure(PEEP) is the most potent determinant of MAP. The optimal relationship of FiO2 and PEEP is PEEP≒FiO2×20. For ventilation, minute volume of ventilation(MV) product of tidal volume(TV) and ventilation frequency is the most important factor. TV has an maximum value up to 15 mL/kg to avoid the volutrauma, so ventilation frequency is more important. The time constant(TC) in children is usually 0.15-0.2. Adequate inspiratory time is 3TC, and expiratory time should be more than 5TC. In some severe respiratory failure, to get 8TC for one cycle is impossible because of higher frequency. In such case, permissive hypercapnia can be considered. The strategy of mechanical ventilation should be adjusted gradually even in the same patient according to the status of the patient. Mechanical ventilators and ventilation modes are progressing with advances in engineering. But the most important thing in mechanical ventilation is profound understanding about the basic pulmonary mechanics and classic ventilation modes.

        The related topics with sports psychology were selected from the reports on the Korean Journal of Physical Education, Physical Fitness of Korean athletes, the reports presented at the Japanese Society of Physical Education Congress, Research quarterly, and quest in the United States. Then the topics were consideringly analysed, being classified into the subjects of Development, Learning, Adjustment, Instruction, Physiological Psychology and Expressive Dance. Following results were examined as the tendency and its transition of the research in sports psychology. 1. By 1976, the research of Sports Psychology in the United States tended to take much attention to the field of Adjustment. However, since 1977, the research in the field of Learning increased. This tendency is due to the introduction of ANOVA applied by statistics to sports. 2. Japanese research also took much attention to the subjects of Adjustment and Learning. However, since the latter half of the 1970's, the research advanced with the subject of Learning, which is coincided to the tendency in the United States. 3. The research of sports psychology in Korea tends to take too much attention to the field of adjustment. Such a phenomenon is caused by two facts that the research area includes the most broad field of adjustment of all the other subjects, and that the data collection is subceptible by questionaire. 4. Recently, the research of sports psychology is focused on the field of learning.

        A review on particle dynamics simulation techniques for colloidal dispersions: Methods and applications

        Colloidal dispersions have attracted much attention both from academia and industry due to industrial significance and complex dynamic properties. Accordingly, a variety of attempts have been made to understand the complicated physics of colloidal dispersions. Particle dynamics simulation has been playing an important role in exploring colloidal systems as a strong complement to experimental approaches from which it is hard to get exact microscopic information. Our aim is to provide a well-organized and up-to-date guide to particle dynamics simulation of colloidal dispersions. Among diverse particle dynamics simulation techniques, we focus on Brownian dynamics, Stokesian dynamics, multi-particle collision dynamics, and self-consistent particle simulation techniques. First, the concept of the simulation techniques will be described. Then, for each simulation technique, pros and cons are discussed with a broad range of applications, including concentrated hard sphere suspensions and biological systems. It is expected that this article helps to identify and motivate research challenges.

        신성요붕증 가계에서 바소프레신 V2 수용체(AVPR2) 유전자 분석 : AVPR2 유전자 R202C 돌연변이의 발견

        목적 : 신성 요붕증(Nephrogenic diabetes insipidus, NDI)은 바소프레신(arginine vasoporessin, AVP)에 대한 신세뇨관의 저항성으로 인하여 요농축의 장애를 특징으로 하는 드문 유전성 질환이다. 반성유전형 신성 요붕증은 바소프레신 V2수용체(AVPR2)의 장애에 기인하며, NDI 환자에서 지금까지 다양한 AVPR2의 돌연변이가 보고되었다. 저자들은 임상적으로 반성 유전형 신성 요붕증으로 진단된 가계에서 AVPR2 유전자의 돌연변이를 발견하기 위하여 분자유전학적 검사를 실시하였다. 방법 : 대상환자의 백혈구에서 추출한 DNA로 AVPR2유전자를 polymerase chain reaction-single strand conformational polymorphism(PCR-SSCP)분석하여 이상이 발견된 부분은 클론닝하여 염기서열을 분석하였다. 같은 PCR 산물을 Hae III로 처리하여 PCR-RFLP(restriction fragement length polymorphism) 분석을 하였다. 결과 : AVPR2 유전자를 PCR-SSCP 분석하였을 때 PCR 산물의 정상인과 이동거리의 차이가 발견되어 환아에서 돌연변이가 있고 환아의 어머니는 보인자임을 예측하였고, 염기서열을 분석하여 675번째 염기 A가 G로 치환됨으로 tryptophan이 cysteine으로 바뀌는 R202C 점돌연변이를 발견하였다. 같은 PCR 산물을 PCR-RFLP 분석을 하였을 때 돌연변이로 인한 Hae III의 인지부위의 상실을 확인하였고 환아의 어머니가 이종접합보유자 (heterozygote)임을 확인하였다. 결론 : 저자들은 임상적으로 신성 요붕증으로 확인된 환아와 어머니의 V2 수용체 유전자를 분석하여 R202C 돌연변이를 확인하였다. 신성 요붕증은 진단이 지연되면 성장장애, 정신박약과 사망을 초래할 수 있는 심각한 질환이나, 태생기 또는 신생아기에 진단하면 후유증을 예방할 수 있으므로 조기진단 및 보인자 발견에 분자유전학적 진단 방법을 적극 활용하여야 하겠다. Purpose : Nephrogenic diabetes insipidus (NDI) is a rare X-linked disorder associated with renal tubule resistance to arginine vasopressin (AVP). The hypothesis that the defect underlying NDI might be a dysfunctional renal AVPR2 has recently been proven by the identification of mutations in the AVPR2 gene in NDT patients. To investigate the association of mutations in th AVPR2 gene with NDI, we analyzed the AVPR2 gene located on the X chromosome. Methods : We have analyzed the AVPR2 gene in a kindred with X-linked NDI. The proband and proband's mother were analyzed by polymerase chain reaction-single strand conformational polymorphism(PCR-SSCP) and DNA sequencing of the AVPR2 gene. We also have used restriction enzyme analysis of genomic PCR product to evaluate the AVPR2 gene. Results : C to T transition at codon 202, predictive of an exchange of tryptophan 202 by cysteine(R202C) in the third extracellular domain was identified. This mutation causes a loss of Hae III site within the gene. Conclusion : We found a R202C missense mutation in the AVPR2 gene causing X-linked NDI, and now direct mutational analysis is available for carrier screening and early diagnosis.

      • 한국장애인체육정책의 행정조직체계 개선에 관한 모델 연구

        Recently the Korean government has formally discussed what authorities are more effective in dealing with affairs in relation to sports for the disabled. Thus the purpose of this study is to propose models of the better management of such supports. For the purpose, this researcher reviewed current discussions between Ministries of Health & Welfare and Culture & Tourism about transferring the control of sports for the disabled and investigated foreign nations' administrative organizations and policies of such sports, which can be effectively useful to establishing such organizations and policies of this nation. As a result, the following models were proposed. 1. Proposal 1 : A model of development in the existing system under Ministry of Health & Welfare 2. Proposal 2 : A model of systematic development following the establishment of Korea Association of Sports for the Disabled(tentative) under Ministry of Health & Welfare 3. Proposal 3 : A model of step-by-step development through the transfer of the control of sports for the disabled from Ministry of Health & Welfare to Ministry of Culture & Tourism

        Schwannoma of the Aryepiglottic Folds: A Case Report

        A schwannoma of the aryepiglottic fold is an uncommon benign tumor. We report a rare case of a laryngeal schwannoma. The clinical, computed tomography (CT) and magnetic resonance imaging findings are presented. The tumor was located at the both aryepiglottic folds of the larynx. The CT scan revealed a well defined, delayed, heterogenously, target-like enhancing mass extending from the right aryepiglottic fold to the left aryepiglottic fold. The tumor was removed completely by laryngomicroscopic surgery (LMS). The diagnosis and treatment of this tumor are also discussed.

        Fluoroscopy-Guided Treatment of a Bronchopleural Fistula with a Platinum Vascular Occlusion Coil and N-butly-2-cyanoacrylate (NBCA) : A Case Report

        A bronchopleural fistula (BPF) following a pulmonary resection is relatively rare; however, it has high morbidity and mortality rates, despite advancements in treatment. Treatment options for a BPF include surgical procedures, conservative therapy, medical therapy, and in particular, the use of a bronchoscopy and different glues, coils, and sealants. This paper discusses a case of the treatment of a postoperative BPF, which is not identified on the bronchoscope, using a platinum vascular occlusion coil (microcoil) and N-butly-2-cyanoacrylate (NBCA) under fluoroscopic guidance. We hereby assert that such an approach as a combination might be an improvement to the minimally-invasive treatment methods currently in use.
