        주관절 신장성 운동에 따른 손목관절 변인이 상완 근 활동성에 미치는 영향

        The study was conducted for the purpose of examining dynamic interaction which expansibility exercise caused by external power influences on muscles by analyzing variables of wrist joint through effects which wrist joint has on change of activity of biceps brachii during 2 segments and compulsive expansion motions through analysis of activation of muscles according to bending and stretching action of wrist joint. When compulsive expansion exercise of elbow joint, the effect of change of elbow joint angle and factor of wrist joint on activity of biceps brachii muscle was(F-5.23, p<.0l) according to bending, standing neutral and stretching of wrist joint showing significant influence, and change of elbow joint angle greatly influenced on activity of biceps brachii muscle (F=13.09, p<.001>, which means significant difference between them. The interaction between factor of wrist joint and change of elbow joint angle showed significant difference, verifying that tere was no effect of factor of wrist joint and change of elbow joint angle on activity of biceps brachii muscle(F=1.34, p>.05). triceps brachii muscle was F=.63, p>.05according to bending, standing neutral and stretching of wrist joint, showing no significant difference, and the effect of change of elbow joint angle on activity triceps brachii muscle was F=18.61, p<.001, showing quite significant difference. This showed that activity of triceps brachii muscle was influnced by change of elbow joint angle. Also the interaction between factor of wrist joint and change of elbow joint angle and activity of triceps brachii muscle was F=3.21, p<.001, showing greatly significant difference. It means that activity of triceps brachii muscle was influenced by factor of wrist joint and change of elbow joint angle.

        팔꿉관절 측방향 일반촬영에서 아래팔뼈 각도 변화에 따른 영상 분석 : 위팔뼈 위관절융기와 팔꿉관절 평가

        Clear overlapping of the bilateral epicondyle and proper separation of the elbow joint are crucial for obtaining accurate lateral general radiographs of the elbow. However, due to the complex anatomical structure of the elbow, achieving optimal positioning is challenging, leading to the need for repeated x-ray examinations. Therefore, the purpose of this study was to investigate the angle of the forearm in patients where accurate lateral images of the elbow joint can't be obtained after vertical incidence using a styrofoam device during elbow joint lateral x-ray imaging. Twenty patients were enrolled in our study following the established protocol. First, a vertical x-ray at an angle of 0° between the forearm and the table was taken (control group). Here, if the lateral image of the elbow joint was deemed inadequate, the forearm angle was adjusted using custom-made styrofoam supports with 5° and 10° inclinations (experimental groups). For the evaluation method, two assessors utilized a 5-point Likert scale to assess the images. The reliability of the assessments was analyzed using Cronbach's alpha coefficient. As a result, patients with inadequate overlap of the bilateral epicondyle and separation of the elbow joint in the initial examination (control group) were able to obtain the best images when setting a 10° angle between the forearm and the table. The subjective evaluation was 1.6 ± 0.8 points at 0°, 2.7 ± 0.8 points at 5°, and 4.4 ± 1.3 points at 10°, respectively. The reliability analysis for the angles of 0°, 5°, and 10° yielded Cronbach's alpha values of 0.867, 0.697, and 0.922, respectively. In conclusion, when it is not possible to obtain accurate images using the conventional position and X-ray beam direction, it is considered that by initially acquiring images with an angle of 10° between the forearm and the table, and gradually decreasing the angle while obtaining images, it would be possible to achieve the optimal image while reducing the number of repeat examinations.

        주관절 등척성 운동에 따른 손목관절 변인이 팔 근육 활성도에 미치는 영향

        The purpose of this study was to examining dynamic interaction which expansibility exercise caused by external power influences on muscles by analyzing variables of wrist joint through effects which wrist joint has on change of activity of arm muscles during 2 segments(Elbow Joint & Wrist Joint) and compulsive expansion motions through analysis of activation of muscles according to bending and stretching action of wrist joint. These signals were compared with %RVC(Reference voluntary contraction) which was normalized by IEMG(Integrated EMG) The results can be summarized as follows : When compulsive isometric exerciseof elbow joint, the effect which change to elbow joint angle in 90, 135, 180 and factor of wrist joint(Flexion, Neutral, Extension) had on activity of biceps brachii, triceps brachii, brachioradialis, flexor carpi radialis was showing no significant difference.

        도서형 전박 피판을 이용한 개방된 주관절의 피복 : A REPORT OF 3 CASES

        The management of soft tissue defect of the elbow is a difficult problem. Open elbow joint can inevitably result in leakage of synovial fluid and subsequent joint infection. Soft tissue defect in this area can cause a severe functional disability due to continuous destruction of supporting structures of the joint, and can cause healing of dislocation and fracture of the joint more difficult. Therefore, an immediate and adequate coverage of the joint and early exercise of the joint is very important for preservation of good function. Recently we have successfully managed three patients of open elbow joint due to trauma and pressure sore with proximally-based radial forearm island flaps, so we have found that this procedure is very useful for the reconstruction of open elbow joint.

        급성 외상성 주관절염에 대한 관절강내 봉약침 치험 2례

        Objectives : We got a good effect in two patients not by subcutaneous but by intra-articular bee venom injection treatments at acute traumatic arthritis of elbow joint, so report it. Methods : We used only intra-articular bee venom injection treatments at acute traumatic arthritis of elbow joint. After measured the VAS(visual analogue scale) and ROM(range of motion) of elbow joint, we analyzed bee venom injection effect. Results and Conclusions : The symptoms(swelling, pain) and the functions of elbow joint were quickly improved through intra-articular bee venom injection treatments. One case was improved that Rt. elbow joint flexion was increased from 60° to 150°, extension was increased from 20° to 0°. The other case was improved that Lt. elbow joint flexion was increased from 90° to 150°, extension was increased from 30° to 0°, and both were decreased in VAS. The results suggest that bee venom intra-articular injection has acute pain reduction, anti-inflammation effect.

        Unstable Simple Elbow Dislocation Treated with the Repair of Lateral Collateral Ligament Complex

        Unstable simple elbow dislocation (USED) repair is challenged by the maintenance of joint reduction; hence, primary repair or reconstruction of disrupted ligaments is required to maintain the congruency and allow early motion of the elbow. We evaluated the effectiveness and the outcome of lateral collateral ligament (LCL) complex repair with additional medial collateral ligament (MCL) repair in cases of USED. Methods: We retrospectively reviewed 21 cases of diagnosed USED without fractures around the elbow that were treated with primary ligament repair. In all cases, anatomical repair of LCL complex with or without common extensor origin was performed using suture anchor and the bone tunnel method. Next, the instability and congruency of elbow for a full range of motion were evaluated under the image intensifier. MCL was repaired only if unstable or incongruent elbow was observed. Clinical outcomes were evaluated using the Mayo elbow performance score (MEPS) and radiographic outcomes on last follow-up images. Results: All cases achieved a stable elbow on radiographic and clinical results. LCL complex repair alone was sufficient to obtain the stable elbow in 17 of 21 cases. Four cases required additional MCL repair after restoration of the LCL complex. The overall mean MEPS was 91 (range, 70 to 100): excellent in 12 cases, good in 7 cases, and fair in 2 cases. All 17 cases with LCL complex repair only and 2 of 4 cases with additional MCL repair had excellent or good results by MEPS. Conclusions: USED requires surgical treatment to achieve a congruent and stable joint. If the repair of lateral stabilizer such as LCL complex acquires enough joint stability to maintain a full range of motion, it may not be necessary to repair the medial stabilizer in all cases of USED.

        Clinical Article : 고교 역도선수에서 주관절의 외반 불안정성; 초음파의 유용성

        We evaluated abnormalities in medial portion of elbow in high-school weightlifter compared with the non weightlifter using a stress radiography and ultrsonography. The experimental group(G1) was 26 high school weightlifters with an average age of 17 years old (range, 16.18 years). The control group (G2) were comprised of 25 age matched general students. Both groups received physical examination, simple and valgus stress radiography and ultrasonography on both side of elbow. Physical examination showed 26.9% (14/52 elbows) tenderness and 19.2% (10/52 elbows) valgus laxity in G1, no tenderness and laxity in G2. There were no differences in medial joint gaps on simple radiography (G1, 3.3 mm, G2, 2.7 mm; p>0.05), but the valgus stress view showed 5.6±0.8 mm medial joint gap in G1 and 3.8±0.8 mm in G2 (p<0.001). Ultrasonography in G1, angular deformity was found in 67.3% (36/52) and G2 all in normal (p<0.01). The horizontal distance was an average 4.9±1.23 mm for the G1 and 3.1±0.78 mm for the G2 (p<0.001). Vertical distance of the proximal portion of the ulna was average 0.58±0.94 mm for the G1 and 1.59±0.49 mm for the G2 (p<0.001). In G1, angular deformity of male was 50% (15/30 elbows) and female was 95% (21/22 elbows) (p<0.001). Change of horizontal and vertical distance were larger in female (p<0.05). In conclusion, there were increased incidence of medial elbow joint laxity in high school weightlifter, especially in female, regardless of career. Sustained valgus laxity could be prone to ulnar collateral ligament injury and should be evaluated with ultrasonography-assisted dynamic study.

        등속성 장비를 이용하여 한국고교야구선수 주관절 근력 평가기준치 설정

        이 연구의 목적은 등속성 장비를 이용하여 고등학교 야구선수들을 대상으로 주관절 근육의 평가 기준치를 설정하는 데 있다. 상지에 의학적으로 문제가 없는 고등학교 야구선수 201명을 대상으로 하였다. 주관절 검사는 HUMAC NORM (CSMI, USA) system을 이용하여 굴곡, 신전운동을 부하속도 60°/sec에서 4회를 실시하였으며, 절대근력(peak torque)와 상대근력(peak torque % body weight)을 측정하였다. Cajori의 5단계(6.06%, 24.17%, 38.30%, 24.17%, and 6.06%)로 구분한 구간추정(interval estimation)방식에 의해 평가 기준치를 나타내었다. 이 연구의 결과에서 각속도 60°/sec에서 주관절의 절대근력(peak torque) 굴근의 평균은 37.88±8.14Nm 신근은 44.59±11.79Nm으로 나타났으며, 상대근력(peak torque % body weight) 굴근의 평균은 50.06±8.66Nm 신근은 58.28±12.84Nm로 나타났다. 각속도 60°/sec에서 주관절 굴근(flexer)과 신근(extensor)의 절대근력(peak torque)과 상대근력(peak torque % body weight)의 평가기준치는 설정하였다. 이 연구에서 분석 된 결과를 토대로 다음과 같이 결론을 이끌어 냈다. 주관절은 야구선수들에게 가장 많이 발생되는 손상임에도 불구하고 국내외 주관절 근력에 관한 연구가 미비한 실정이다. 주관절 손상을 예방하고 경기력 향상을 위해 상대적 근력 수준과 훈련효과를 보다 객관적으로 평가할 수 있는 근력 척도가 필요하다. 주관절의 절대근력과 상대근력의 평가기준치를 제시함으로서 고교선수, 야구지도자, 선수트레이너, 재활전문가 등에게 손상회복 및 재활 복귀 시 유용하게 자료가 될 것으로 생각되며, 객관적인 임상평가 데이터로 활용하는 데 매우 유용한 정보라고 사료된다. The purpose of this study was to establish norm-referenced criteria for the isokinetic strength of the elbow joint inKorean high school baseball players. Two hundred and one high school baseball players participated in this study,none of whom had any medical problem withtheir upper limbs. The elbow flexion/extension test was conducted four times at a speed of 60°/sec. The HUMAC NORM (CSMI, USA) system was used to obtain the values of the peak torque and peak torque per body weight. The results were presented as norm-referenced criterion valuesusing the5-point scale of Cajori which consists of five stages (6.06%, 24.17%, 38.30%, 24.17%, and 6.06%). In the results of this study, the peak torques of the elbow (flexor and extensor?) at an angular velocity of 60° / sec were37.88 ± 8.14Nm and 44.59 ± 11.79Nm, and the peak torque per body weight of the elbow (flexor and extensor?) were50.06 ± 8.66 Nm and 58.28 ± 12.84 Nm, respectively. The reference values of the peak torque and peak torque per body weight of the elbow flexor and extensor were setat an angular velocity of 60° / sec. On the basis of the results analyzed in this study, the following conclusions were drawn. There is a lack of proper studies on the elbow joint strength, even though the most common injury in baseball players occurs in the elbow joint. Therefore, we need to establish a standard muscle strength in order to prevent elbow joint injuries and improve their performance. The criteriaforthe peak torque and peak torque per body weight establishedherein will provideuseful information for high school baseball players, baseball coaches, athletic trainers and sports injury rehabilitation specialists in injury recovery and return to rehabilitation, which can beutilized as objective clinical assessment data.

        상지 굴곡근피로가 주관절의 위치 감각에 미치는 영향

        Background and Purpose : This study was to analysis effects of muscle fatigue on error of elbow joint sense. Methods : A total 19 healthy student(men = 10, women = 9) who did't have any problem of musculoskeletal system in upper extremities participated this study. we divided two groups into young group(n = 10, $19.67{\pm}.5$) and old group(n = 9, $28.56{\pm}1.5$). In order to evoke muscle fatigue of elbow flexor, we used Biodex, and participations performed concentric contraction of elbow flexor 150 numbers as well as we measured error of joint sense using by Biodex. We collected data just after, 30min, 2hour, 24hour after evoked muscle fatigue, and we finanlly acquired average value of three times measured joint sense of elbow joint. And we calculated value of percentage of error of joint sense. We analyzed collected data by repeated ANOVA, ANOVA using by SPSS ver.12.0 program. Result : This study showed that there was no significantly effects between groups and within groups, we could see that there was significantly difference among duration by each group of age, and sex(p<.05). Conclusion : The error of joint position sense presented highest value just period after evoked muscle fatigue compared after 30 min, 2 hours, 24 hours, and we can't find out interaction between duration and age and sex.
