        부산지역 일부 중학생의 구강건강지식과 치과공포 척도의 연관성

        The purpose of this study is to examine the reduction of dental fear and to understand the level of dental fear and oral health knowledge and to analyzed the correlation between oral health knowledge and dental fear. This study conducted the survey of 281 middle school students who have a dental visit experience with structured questionnaire from 8 to 9 June, 2012. The data were analyzed by using SPSS 20.0ver and was verified .05 level of significance. As a result, girls than boys feel more dental fear, the better the perceived oral health status of the lower dental fear. In treatment avoidance factor ‘Going to the dental hospital for fear of putting off the date has never been treated’ showed the highest, ‘muscle tension’ was the most commonly experienced during dental physiological response. Stimulating factors induce fear was ‘When you hear the sound of bur running’. The higher oral health knowledge decreased dental fear. Therefore, dental fear of middle school students was associated with oral health knowledge. 본 연구는 부산광역시 소재 중학교 학생 중 치과의료기관의 방문경험이 있는 학생 281명을 대상으로 하였으며, 일부 중학생들의 치과이용에 있어 구강건강지식과 치과공포 수준을 파악하고, 구강건강지식이 치과공포와 연관성이 있는지 분석하여 치과공포의 감소에 도움을 주고자 하였다. 자료수집은 2012년 6월 8일부터 9일까지 구조화된 설문지로 조사하였고, 수집된 자료는 SPSS 20.0 ver을 이용하여 유의수준 .05에서 검증하였다. 그 결과, 남학생보다 여학생이 치과공포를 더 많이 느끼는 것으로 나타났으며, 주관적 구강건강상태가 좋을수록 치과공포는 낮은 것으로 나타났다. 진료회피요인에서는 ‘치과에 가는 것이 두려워 치료날짜를 미룬 적이 있다’가 가장 높게 나타났으며, 치과진료시 가장 많이 경험한 생리적 반응은 ‘근육긴장’이었으며, 공포유발 자극요인으로는 ‘치아삭제용 기구가 돌아가는 소리를 들을 때’였다. 구강건강지식이 높을수록 치과공포는 감소하였으며 그 결과, 치과공포는 중학생들의 구강건강지식과 연관성이 있음을 알 수 있었다. 향후 치과공포를 감소시킬 수 있는 프로그램을 마련하여 중학생들이 치과방문에 소홀히 하지 않도록 하며, 구강건강지식을 향상시킬 수 있도록 치과위생사들의 구강보건교육 프로그램을 잘 활용할 수 있도록 하여야 할 것으로 생각된다.

        체계적 문헌고찰을 통한 국내 치과공포 연구 동향분석 (2007-2017)

        Objectives: The purpose of this study was to investigate the overall research trends and factors influencing dental fears in the last 10 years (2007-2017) and provide recommendations for future research. Methods: The literature review focused on dental fear research in Korea. Inclusion criteria of selected studies were as follows: studies that examined fear in middle school students or older, studies undertaken between 2007-2017, and studies that examined trends and factors relating to dental fears. Results: Findings from this literature review showed that the most commonly used tool for measuring dental fear was the Dental Fear Survey (DFS). A lack of trust towards the dentist increased dental fear among patients. Women managed fear better than men. Past pain experiences increased dental fear. The most frequent intervention for reducing fear was sound (41.7%). Most of the intervention studies demonstrated a reduction of fear, with the exception of interventions using ear plugs. Fear was increased in studies involving ear plugs. Conclusions: Based on the results of the study, specific measures should be taken to alleviate past pain experiences, such as the fear of anesthetic injections and sensations of the dental drill. Continuous research is needed to reduce dental fear.

        Factors Affecting Dental Fear in Korean Adolescents

        Purpose: The purpose of this study was to evaluate the dental fear experienced among Korean adolescents and to identify the relevant factors. Materials and Methods: In order to compare the level of dental fear depending on the subjects’ previous experience, descriptive statistical analysis was performed. Gender- and grade-dependent evaluation was performed according to the presence of their previous dental visit and dental fear. Subjective oral health status was also investigated. In order to determine the factors affecting dental fear, logistic regression analysis was performed. Result: Among the total of 333 subjects who had experienced dental fear, females were found to experience 1.766 times greater dental fear than males (P=0.007). The worse subjective perception of their oral health was associated with increase in the experience of dental fear by 1.245-fold (P=0.047). Conclusion: The dental fear was likely to be formed during the visit to the dentist's office or through previous experience of dental treatment. Therefore in order to reduce the fear associated with dental treatment in adolescents, establishemnt of a proper environment in the dental clinic and a patient management program are necessitated.

        일부 성인의 치과 공포감 관련 요인

        Objectives: The purpose of the study was to investigate the related factors to dental fear in some adults. Methods: A self-reported questionnaire was filled out by 320 adults in Seoul and Gyeonggi-do from April to June, 2013. Except 14 incomplete answers, data were analyzed by SPSS 18.0 program. The questionnaire consisted of general characteristics of the subjects(7 questions), dental fear related factor(18 questions), distrust for dentist(15 questions), and distrust for dental hygienist(11 questions). The instrument for dental treatment fear was Dental fear Survey(DFS) adapted from Kleinknecht and partly modified by Choi. Out of 20 questions, 18 questions were reconstructed and score by Likert 5 scale. Higher score means higher dental fear. Cronbach alpha was 0.959 in the study. The distrust for dentist and dental hygienist was adapted from Choi and reconstructed and score by Likert 5 scale. Higher score means higher distrust for dentist and dental hygienist. Cronbach alpha was 0.937 in distrust for dentist and 0.874 in distrust for dental hygienist in the study. Results: Those who experienced dental pain tended to have dental fear. The reasons for dental fear were as follows; bad breath(2.96 out of 5 points, hereafter represented as of 2.96/5), dental caries(2.88/5), missing teeth(2.87/5) and tooth pain(2.77/5). The distrust for the dentist was the main dental fear in the adults. Conclusions: The dental fear was closely related to gender, experience of dental pain, oral symptoms and distrust for the dentist. As the psychological pain in the patients was mainly influenced by the trustful atmosphere, it is necessary to make the patient easy before treatment.

        일부 고등학생의 개인의 기질적 특성과 치과 관련 지식에 따른 치과공포감에 관한 연구

        This study is to analyze the personality trait and dental fear of high school students. This study is conducted with a total of 603 first grader at high school in DaeGu city from 1st September to 30th October, 2009. In prevention and dental treatment knowledge category, the average score of female are higher than male. The concern of oral health, the experience of oral health education and the number of toothbrush are significantly different in the knowledge category. In dental fear category, the number of dental pain, the visiting is treatment, the recognize of dental state, the number of toothbrush and the change of dental visiting date are significantly different. In the prevention knowledge according to dental fear state, low fear group 7.03 is higher than high fear group. In the dental treatment knowledge according to dental fear state, high fear group 7.15 is higher than low fear group. In personal temperament, there are not significantly relationship between prevention knowledge, dental treatment knowledge and dental fear. These findings are basis to development of education programs which were included prevention and dental treatment knowledge before visiting dental clinics for reducing dental fear.

        일부지역 성인 환자의 주관적 건강인식과 구강건강관리행태가 치과공포감에 미치는 영향

        The purpose of this study was to examine the oral health care and self-rated health status of adults visiting dental clinics in the region of Busan and influential factors for their dental fear from January to May, 2017, in an effort to obtain some information on how to relieve fear. As for dental fear, adults who were in their 30s, who were homemakers and whose monthly income ranged from 4 to 4.99 million won. In regard to fear according to oral health care, dental fear caused by the treatment avoidance factor was stronger among the adults who didn't receive dental checkups and who brushed their teeth in the wrong way. Dental fear that was attributed to the physiological reaction factor and the stimuli-inducing factor was severer among the adults who didn't receive dental checkups and who had no scaling experience. Overall fear was severer among the adults who didn't receive dental checkups and who changed their toothbrushes every four or more months. The factors that affected dental fear were self-rated health status, self-rated oral health status, gender, age, whether to receive dental checkups on a regular basis or not, and oral health education experience. Therefore in order to alleviate dental fear, self-rated health status should be improved, and the kind of system that encourages regular dental checkups and provides oral health education should be prepared. 본 연구는 2017년 1월부터 5월까지 일부지역치과에 내원하는 성인들을 대상으로 구강건강관리행태와 주관적 건강인식수준을 파악하고 치과공포감에 미치는 영향요인을 파악하여 치과공포감을 낮추기 위한 기초자료를 마련하고자 한다. 치과공포에서 회피요인은 여자, 40대, 고졸이하, 주부, 300-399만원, 생리적 반응요인은 여자, 40대, 주부, 200-299점, 자극유발유인 여자, 30대, 주부, 전체공포수준은 30대, 주부, 400-499만원에서 높았다. 구강건강관리행태에 따른 공포감에서 치료회피요인은 구강검진을 하지 않고, 칫솔질방법이 틀린 경우, 생리적 반응요인과 자극반응요인은 검진을 하지 않고, 스케일링 경험이 없는 경우, 전체공포감은 검진을 하지 않고, 칫솔교환시기가 4개월 이상인 경우에서 치과공포감이 높았다. 치과공포에 영향을 미치는 주관적 건강인식수준, 주관적 구강건강인식수준, 성별, 나이, 정기검진유무, 구강보건교육 경험유무가 영향을 미쳤다. 따라서 치과공포감을 줄이기 위해서는 주관적 인식을 개선하고 정기검진과 구강보건교육을 위한 시스템이 마련되어야 할 것이다.

        Prevalence of dental anxiety in 10-14 years old children and its implications

        Background: The aim of this study was to provide insight on dental fear amongst schoolchildren and evaluate the association between caries experience and fear of dental procedures. Methods: A sample size of 250 students (both sexes) of ages 10-14 years were enrolled in the study. Before dental examination, each participant was informed about the study and given the Children's Fear Survey Schedule - Dental Subscale (CFSS-DS) questionnaire. Children who scored greater than 38 were included in the 'with dental fear' group and those who scored less than 38 were assigned to the 'without dental fear' group. All oral check-ups were carried out on the school premises according to WHO criteria. Results: There were 105 children (42%) who experienced dental fear. As CFSS-DS scores increased, scores on the Decayed, Missing and Filled Surfaces Index (DMFS) also increased. Scores were highest on "injections" followed by "dentist drill" and "feeling of choking". Children were significantly less anxious about items of dental treatment if they had experienced that particular form of treatment. Female participants were found to be more dentally anxious than the male participants. Conclusions: The data revealed dental fear in 10-14 years old children and showed that dental fear scores decreased with increase in age and experience.

        Dental Fear Survey 척도를 이용한 치과위생사들의 치과공포감 및 관련요인

        Objectives: The purpose of the study is to investigate the dental fear and related factors of dental hygienists using dental fear survey. Methods: A self-reported questionnaire was completed by 279 dental hygienists from January 15 to March 28, 2015. The study instrument was dental fear survey. Data were analyzed using SPSS 19.0 program by t-test, one way ANOVA and stepwise multiple regression analysis. The questionnaire included general characteristics of the subjects, subjective oral health status, use of dental care services, subjective oral health status, and recent dental treatment. Results: Alcohol consumption was the most influential factor to the dental fear. The subjective status of oral health and direct pain sense also contributed to the dental fear. Conclusions: To reduce the dental fear, it is important to have high confidence toward the dental treatment performance in the dental hygienists.

        The relationship between subjective oral health and dental fear in Korean adolescents

        Background: This study is aimed to evaluate the level of fear and to reduce the overall fear, thereby enabling patients to receive treatment via timely visits. Methods: In a survey conducted by 460 South Korean middle school students, we used 453 data that faithfully responded to the survey. Multiple regression analysis was conducted to investigate the factors influencing subjective oral health and dental fear. The significance level used for statistical significance was ${\alpha}=0.05$. Results: The level of fear was higher for upper grade, female students. The factors affecting dental fear were higher for gingival bleeding and dental pain. Regarding factors for dental fear affecting subjective oral health, lower fear of puncture needle and tooth removal tool resulted in higher subjective oral health. Conclusions: The study found that adolescents had higher fear of dental care when they had gingival bleeding and tooth pain. Gingival bleeding is a symptom of early gingival disease and dental pain is likely due to advanced dental caries. These results suggested that it is necessary to have a program to reduce dental fear and anxiety as well as a program to prevent dental diseases through regular periodic screening and education.
