        Introduction for the Questionnaires for Restless Legs Syndrome

        Restless legs syndrome refers to a common sensory-motor sleep disorder that affects sleep and quality of life, and it belongs to sleep-related movement disorder. Since the diagnosis of restless legs syndrome depends on the patient's subjective symptoms, it is challenging to differentiate restless legs syndrome from other neurological diseases such as polyneuropathy or radiculopathy. Therefore, systematic and detailed assessment of the clinical presentation of restless legs syndrome plays a pivotal role in diagnosis and follow-up. Here, we introduce the questionnaires for diagnosis and assessment of restless legs syndrome.

        Restless legs syndrome can be diagnosed by clinical criteria of the international restless legs syndrome research group. However, if the diagnosis is unclear, polysomnography could be needed. Restless legs syndrome can be often accompanied with periodic limb movement on polysomnography. On the other hand, for the diagnosis of periodic limb movement disorder, polysomnogeraphy is mandatory. This article summarizes recent reports about polysomnography with restless legs syndrome and periodic limb movement disorder.

        만성 신장병 환자의 하지불안증후군에 대한 작약감초탕 치험 1례

        Restless leg syndrome is a nervous system disorder that causes an overpowering urge to move one’s legs. Symptoms of restless leg syndrome usually worsen when one tries to fall asleep and can prevent sufficient sleep. Restless leg syndrome is common in patients with chronic kidney failure and can be caused or worsened by chronic kidney failure and hemodialysis. Various medications can treat restless leg syndrome, though the long-term use of medications can cause augmentation and adverse effects. In addition, the use of dopamine agonists is limited in patients with chronic kidney failure. This is because the dose of administration should be controlled for patients with chronic kidney failure, and the treatment effect has not been clearly proven. This study reports the case of a 56-year-old male diagnosed with chronic kidney failure complaining of uncomfortable leg sensations. The patient underwent Korean medicine treatment using Jakyakgamcho-tang. The IRLS, NRS, and AIS scores were evaluation tools during treatment. This study suggested significantly improved symptoms through the individual interventions of Jakyakgamcho-tang in a restless leg syndrome patient with chronic kidney failure.

        하지불안증후군을 경험하는 혈액투석 환자를 위한 하지복합운동 프로그램 적용 효과

        Purpose: This experimental study with nonequivalent control group and pre/post-tests aims to investigate the effects of a complex leg exercise program on severity of restless legs syndrome, sleep quality, depression, and physical performance in patients with maintenance hemodialysis. Methods: Patients in the experimental group were provided with the leg exercise program three times a week for eight weeks, a total of 24times, through watching videos during hemodialysis from August 1, 2017 to October 25. The experimental group and control group consisted of 22 people each. Data were analyzed using descriptive statistics, x<sup>2</sup> test, Fisher's exact test, and independent t-test utilizing SPSS/WIN 22.0. Results: After providing with the leg exercise program, these were level of restless legs syndrome and depression were significantly decreased in the experimental group (t=2.79, p=.032; t=0.53, p=.036, respectively). However, sleep quality and physical performance did not have significant difference between the two groups (t=0.02, p=.947; t=1.74, p=.957, respectively). Conclusion: A complex leg exercise program appears to be beneficial in improving restless legs syndrome and depression in maintenance hemodialysis patients with restless legs syndrome.

        Objective: To assess and compare restless legs syndrome (RLS) prevalence according to International RLS Study Group (IRLSSG) essential criteria and Cambridge-Hopkins diagnostic questionnaire (CH-RLSq) in Korean population sample. Method: We selected a stratified random population sample of Koreans over age 19 and evaluated them with Cambridge-Hopkins diagnostic questionnaire to identify restless legs syndrome. We also assessed RLS prevalence according to IRLSSG criteria using questions in Cambridge-Hopkins diagnostic questionnaire. Results: The 1-year prevalences of RLS according to CH-RLSq and IRLSSG essential criteria were 0.5% and 5.5%, respectively. Most individuals (93.0%) with RLS according to IRLSSG essential criteria were excluded in RLS diagnosis according to CH-RLSq by reporting muscle cramp (47.7%) or positional discomfort (82.8%). Conclusion: RLS diagnosis according to IRLSSG essential criteria may include individuals reporting similar symptoms of RLS.

        Is There an Association between Restless Legs Syndrome and Urticaria?

        Restless legs syndrome (RLS) is a disease characterized by the urge to move the legs and sleep disturbances. Similarly, chronic spontaneous urticaria (CSU) is a dermatological disease characterized by pruritus and sleep disorders. In this study, we aimed to determine the prevalence and severity of RLS in patients with chronic spontaneous urticaria (CSU) and to compare the quality of sleep of patients with and without RLS in the CSU group using the Pittsburgh Sleep Quality Index. A total of 130 patients with CSU and 100 healthy controls were included in this study. The frequency of RLS, frequency of sleep disturbances, and average score of RLS in patients with CSU were statistically significantly higher than control groups (respectively P = 0.008, P = 0.009, P = 0.004). Subjective sleep quality, sleep latency and habitual sleep efficiency scores in patients with RLS were statistically significantly higher than patients without RLS (respectively P = 0.016, P = 0.007, P = 0.035). We claimed that pruritus of urticaria may decrease the quality of sleep in patients with RLS and it may trigger and worsen the restless legs syndrome. Furthermore, RLS and CSU may share a common etiology.

        Restless legs syndrome (RLS) is a very frequent neurological disorder characterised by sensory and motor symptoms. The diagnosis of RLS basically relies on the presence of 4 clinical criteria defined by the International RLS Study Group. So RLS is can be composed of various pathogenetic mechanisms. This review focuses on the recent developments in pathophysiology of RLS, with particular emphasis on the background of recent developments in genetics, neuroimaging, and animal studies. Neuroanatomical, neurophysiological, neuropharmacological, and neurogenetic findings have to be merged to reveal the secrets of restless legs syndrome.

        Nocturnal Restless Arms and Face Improved by Dopamine Agonist: Is It a Variant of Restless Legs Syndrome?

        The symptoms of restless legs syndrome (RLS) usually involve legs and can extend to other body parts. However, isolated body parts other than leg have rarely been involved. A 32-year-old woman had abnormal sensations of both forearms and face during night, which caused a difficulty falling in sleep. The symptoms were relieved by shaking and rubbing. She experienced a clear benefit from pramipexole, but a recurrence of restlessness was followed in both arms subsequent to pramipexole withdrawal. Finally, she had been prescribed a dose of 0.375 mg, which was well tolerated for 6 years. We report a case of nocturnal restless arms and face without leg involvement, which was like RLS in terms of clinical criteria except lesion site.

        하지불안증후군의 진단과 치료

        Restless legs syndrome (RLS) is a neurological disorder characterized by an irresistible urge to move one's body to stop uncomfortable or odd sensations. It most commonly affects the legs. Moving the affected body part, such as walking or stretching provide relief the urge to move the legs and any accompanying unpleasant sensation partially or totally. RLS is relatively common, affecting 5 to 15 % of the general population, with prevalence rates increasing alongside age. Restless legs syndrome can lead to sleep-onset or sleep-maintenance insomnia, and occasionally excessive daytime sleepiness, all leading to significant morbidity. Dopaminergic systems are known to be strongly related with RLS that are closely linked to CNS iron homeostasis. Besides defective dopaminergic system that is closely related with iron metabolism, genetic factors play a role in early-onset individual with a positive family history. The diagnosis can be made based on the symptom characteristics, differential diagnosis is important because many conditions could mimic RLS symptoms. Dopamine agonists (DAs) have been considered the first-line therapy, but with the growing appreciation of problems associated with long-term treatment, particularly augmentation and impulse control disorder, alpha-2-delta drugs, such as gabapentin, are now considered the first line of treatment in patients with troublesome RLS. In more severe cases, a combination therapy may be required.

        ■ Objectives The purpose of this case study is to report the effectiveness of Gami Ssanghwa-tang on the restless legs syndrome patient. ■ Methods The patient was treated with Korean medicine, including herbal medication Gami Ssanghwa-tang for 26 days and acupuncture for once a week as an out patient. The evaluation was performed using the Korean versions of the International Restless Legs Scale(K-IRLS). ■ Results After 34 days of treatment, the K-IRLS total score improved from 24 to 8. And at the follow up session(7 days after end of treatment), the K-IRLS total score improved to 0. ■ Conclusion This study suggested that herbal medicine, Gami Ssanghwa-tang might be effective in Restless legs syndrome.
