Human Paraoxonase 1(PON1)의 유전자 다형성에 따른 중풍환자의 습담 변증과의 상관성 연구

Objective : In the present study, we investigated genetic distribution of eight single nucleotide polymorphisms of PON1 between Dampness and Phlegm and non Dampness and Phlegm pattern identification(PI) among Korean stroke patients. Materials and Methods : One hundred forty stroke subject without Dampness and Phlegm and fifty eight stroke subjects with Dampness and Phlegm were participated in this study. After informed consents, eight single nucleotide polymorphisms(SNPs) in PON1 of each subjects were identified by DNA sequencing and primer extension method and statistical analysis was performed to determine the significant difference between Dampness and Phlegm and non Dampness and Phlegm groups. Results : Among anthropometric characteristics and blood parameters, waist circumference and total cholesterol were significantly higher in Dampness and Phlegm. Among 8 SNPs of PON1, frequency of M allele and subjects with M allele in L55M SNP were significantly higher in Dampness and Phlegm group (p=0.0032 and p=0.0053, respectively) but subjects with T allele in C-2033T SNP were lower in Dampness and Phlegm group(p=0.0302). Effect of L55M and C-2033T on Dampness and Phlegm were 3.07% and 1.75%, respectively. Conclusion : Our results suggest that L55M SNP in exon and C-2033T in promoter region of PON1 maybe affect to Dampness and Phlegm pattern identification. However, further study should be carried out to find out the detailed mechanism how L55M and C-2033T can affect Dampness and Phlegm stroke patients.
중풍 환자의 습담 설진과 고지혈증의 연관성에 관한 연구

Objectives : This study was aimed to clarify the relationship between the tongue diagnosis of dampness-phlegm and hyperlipidemia in acute stroke patients. Methods : We analyzed the data of 1405 patients with acute stroke in 10 oriental medical hospitals from November 2006 to December 2008. We classified patients into two groups, dampness-phlegm and non dampness-phlegm by tongue diagnosis such as the white coating of the tongue, thick coating of the tongue, swollen tongue and teeth printed tongue which is oriental medical diagnosis. And We analyzed their characteristics with type of stroke and lipid blood level. Results : 1. There was no significant difference of stroke type between the dampness-phlegm tongue diagnosis group and the non dampness-phlegm tongue diagnosis group. 2. The ratio of SVO was higher in dampness-phlegm tongue diagnosis group (76.06%) than the non dampness-phlegm tongue diagnosis group (61.26%). 3. According to the blood test, the dampness-phlegm tongue diagnosis group showed higher in total cholesterol, HDL cholesterol, LDL cholesterol and triglyceride than the control group. 4. Total cholesterol and HDL cholesterol were significant higher in dampness-phlegm tongue diagnosis than the control group. Conclusion : According to the analysis, the relationship between the tongue diagnosis of dampness-phlegm and hyperlipidemia in acute stroke patients were more clarified. Based on these results, more prospective studies are to be done with more clinical data.
한국형 중풍 변증 표준시안의 습담 변증 지표에 대한 연구

Objectives : This study was done to investigate clinical frequency and correlation among the indicators of dampness-phlegm pattern identification settled by tentative Korean Standard Differentiation of the Symptoms and Signs for Stroke. Methods : The subjects were 147 hospitalized patients with stroke, and a list of registry was made for each of them. Among the five types of pattern identification, fire-heat, dampness-phlegm, blood-stasis and deficiency of Qi and of Yin, those that have shown a high frequency in dampness-phlegm type were categorized as the dampness-phlegm pattern group. Frequency of dampness-phlegm indicators was compared with those from the non-dampness-phlegm pattern group. Correlations among dampness-phlegm indicators were also studied. Results : 1. Dampness-phlegm pattern group included 26 patients out of 147. 2. Among the indicators of dampness-phlegm pattern. those, in order of highest frequency, were 'tiredness or sluggishness', 'white Coated tongue' and 'sputum'. 3. In comparing dampness-phlegm pattern group with non-dampness-phlegml group, the indicators such as 'lightheadedness', 'nigrescence', 'sputum', 'dermatic dysaesthesia' showed significantly high frequency. 4. Among the indicators, 'sputum' and 'tiredness or sluggishness', 'sputum' and 'yellow coated tongue', and 'white coated tongue' and 'yellow coated tongue' showed significant correlation. 5. In investigation of the correlation of scale in symptoms, various results such as positive correlation and negative correlation were obtained. Conclusion : In this study, more sensitive indicators of dampness-phlegm pattern identification were found. Based on these results, it is suggested that a more practical Korean Standard Differentiation of the Symptoms and Signs of Stroke would be established through continuous clinical studies by giving weight on each specific type of pattern identification.
경동맥 초음파를 이용한 뇌경색 환자의 내경동맥 협착도와 습담변증(濕痰辨證)의 관련성 연구

Objectives : This study was aimed to clarify the relationship between the dampness-phlegm diagnosis and internal carotid artery stenosis by measuring carotid artery sonography in cerebral infarction patients. Methods : One hundred eighty subjects were recruited from the patients admitted to the Department of Internal Medicine at Kyunghee university oriental medical center from September 2008 to July 2010. We assessed one hundred eighty patients' carotid artery sonography data and diagnosed dampness-phlegm by oriental medical diagnosis. then, analyzed their characteristics, risk factor, lifestyle, metabolic syndrome, body mass index, Waist/Hip ratio(W/H ratio) and dampness-phlegm diagnosis. Results : On the demographic variables of the patients, age, smoking, W/H ratio and dampness-phlegm group were significantly higher in severe internal carotid artery(ICA) stenosis group than in the control group. According to the significant difference in dampness-phlegm group, we analyzed dampness-phlegm related index for pattern identifications by ICA stenosis. As a result, sputum, bowel sound, chest discomfort, slippery pulse were significantly higher in the severe ICA stenosis group than in the control group. In multivariate analysis, dampness-phlegm group showed close relationship with severe ICA stenosis group. Conclusion : According to the analysis, significance between dampness-phlegm diagnosed patients group and severe ICA stenosis were clarified. These results can be utilized in the future as a basis material.

Objectives: Epicardial fat is true visceral fat that is known to be associated with metabolic syndrome, high abdominal fat, insulin resistance, coronary artery diseases, low coronary flow reserve and subclinical atherosclerosis. Dampness-Phlegm pattern is one of the pattern diagnosis of traditional Korean medicine. Previous studies showed that Dampness-Phlegm pattern is associated with hypertension, dyslipidemia, metabolic syndrome. This study is intended to find association between Dampness-Phlegm pattern and epicardial fat thickness. Methods: This study was a community-based single center trial. Ischemic stroke patients within 30 days after their ictus were enrolled. Epicardial fat thickness was measured using transthoracic echocardiography. Other measured and obtained variables are medical history, weight, height, body mass index, fasting blood glucose, cholesterol, triglycerol, high density lipoprotein, lipid and low density lipoprotein. Results: Three hundred sixty six were enlisted, and one hundred forty were diagnosed with the Dampness-Phlegm pattern. Dampness-Phlegm pattern group had significantly thicker epicardial fat. Binary logistic regression also showed statistically significant result. Conclusions: This study showed close association between epicardial fat and Dampness-Phlegm pattern. This result suggests a clue to standardization of pattern identification.

Objectives : This study aimed to clarify the relationship between the Oriental medical diagnosis and arteriosclerosis by measuring carotid-ankle vascular index(CAVI) in acute cerebral infarction patients. Method : One hundred thirty-one subjects were recruited from the patients admitted to the Internal Medical Department at Kyunghee University Oriental Medical Center from April 2007 to August 2008. We sorted cerebral infarction patients and assessed one hundred fourteen patients' CAVI data. We diagnosed dampness-phlegm by Oriental medical diagnosis and evaluated stroke type by single or multiple infarctions. then, we analyzed their characteristics with type of stroke, risk factor, lifestyle, metabolic syndrome and dampness-phlegm diagnosis. Result : 1. On the demographic variables of the patients, age, hypertension, hyperlipidemia, multiple infarction group and metabolic syndrome and dampness-phlegm group were significantly higher in the high CAVI score group than in the control. 2. According to the significant difference in the dampness-phlegm group, we analyzed dampness-phlegm related index for pattern identifications by CAVI score. As a result, dark circles, insomnia, headache, white coating tongue. slippery pulse, and rough pulse were significantly higher in the high CAVI score group then in the control. 3. In multivariate analysis, age, hypertension, multiple infarction and dampness-phlegm groups showed a close relationship with the high CAVI score group. Conclusions : According to the analysis, significance between dampness-phlegm diagnosed patients group and high CAVI score were clarified. Moreover, multiple location infarctions also have a relationship with high CAVI score in cerebral infarction patients. These results can be utilized in the future as a basis material.

Object : The aim of this study was to assess the relationship between left ventricular hypertrophy and Dampness-Phlegm diagnosis in cerebral infarction patients. Methods : Among 227 of the total recruited patients, 59 patients were diagnosed as left ventricular hypertrophy. We assessed their general characteristics, risk factors, lab findings and Korean medical diagnosis. We compared the assessed variables between left ventricular hypertrophy group and non left ventricular group. We analyzed the relationship between left ventricular hypertrophy and risk factors. And we also analyzed the relationship between left ventricular hypertrophy and dampness-phlegm diagnosis. Results : 1. The rate of left ventricular hypertrophy in female patients was larger than the rate of male patients. 2. There were more patients finally diagnosed hypertension in left ventricular hypertrophy group. 3. According to the analysis about the rate of Dampness-phlegm related Index for Pattern Identification by left ventricular hypertrophy, Sallow complexion and obesity were significantly higher in the left ventricular hypertrophy than in the non left ventricular hypertrophy group. 4. In multivariate analysis, Dampness-phlegm group showed close relationship with left ventricular hypertrophy. Conclusions : According to the analysis, significance between dampness-phlegm diagnosis diagnosed group and left ventricular hypertrophy were clarified. These results can be utilized in the future as a basic material to be used for diagnosis and management of dampness-phlegm diagnosis on cardiovascular diseases.
