        백혈병환자에서 Liposomal Amphotericin B로 치료된 파종성 Fusarium 감염 1예

        Fusarium spp.는 사람에서 드물게 각막염, 조갑진균증, 상처나 화상부위감염 등을 유발하는 것으로 알려져 있으나 최근에는 면역저하자에서 기회감염의 원인균으로 증가하고 있는 추세이다. 면역저하자에서 파종성 감염이 있는 경우에 예후는 치명적이나, 증례가 많지 않고 진단방법이 제한되어 있어 조기 진단과 신속한 치료가 쉽지 않다. 그러나 특징적으로 피부병변의 동반이 흔하여 면역저하자에서는 피부 병변의 면밀한 관찰과 함께 Fusarium 감염을 조기에 의심하여 피부생검이나 배양을 하는 것이 진단에 도움이 되리라 생각된다. 파종성 Fusarium 감염의 치료로는 감염 부위의 국소적 외과적 절제와 전신적 항진균제의 사용이 필요하고 현재까지는 amphotericin B가 비교적 효율적인 항진균제로 알려져 있으나 면역저하자에서 이에 반응을 하지 않는 경우가 많다. 저자들은 급성골수성백혈병 환자에서 관해유도화학요법 시행 후 호중구감소상태에서 피부병변과 폐침윤이 발견되었고 피부의 중심부 궤양을 동반한 구진성 병변에서 배양검사를 통해 파종성 Fusarium 감염을 진단하였으며 liposomal amphotericin B (AmBisome )를 사용하여 성공적으로 치료한 예를 경험하여 보고하는 바이다. Fusarium spp., basically a superficial pathogen, is a newly emerging fungal pathogen of opportunistic infections in immunocompromised patients. At present, although Fusarium spp. are relatively resistant to amphotericin B, the combination of amphotericin B and surgical debridement appear to be optimal treatment for disseminated infection. Recently we experienced a 32-year-old neutropenic patient after induction chemotherapy for acute myelocytic leukemia presented with skin lesions and infiltrations in both lungs. We diagnosed with disseminated fusariosis by skin culture and successfully treated the patient with liposomal amphotericin B. We emphasize a high index of suspicion for skin lesions especially in immunocopromised patients.

        The diagnostic requirements were suggested and explained regarding the systems of differentiation of symptoms and signs in the third year study of standardization and unification of the terms and conditions used for diagnosis in oriental medicine. The systems were as follows : - analyzing and differentiating of epidemic febrile disease - analyzing and differentiating in accordance with the Sasang constitution medicine based on four-type recognition - differentiation of disease according to pathological changes of Chong and Ren channels - standards for diagnosis of women's disease - standards for diagnosis of children's disease - standards for diagnosis of motor and sensor disturbance (-muscle. born, joint, etc.) - standards for diagnosis of neuropsychiatric disease - standards for diagnosis of five sense organ disease - standards for diagnosis of external disease The indivisual diagnosis pattern was arranged by the diagnostic requirements in the following order : another name, notion of diagnosis pattern, index of differentiation of symptoms and signs, the main point of diagnosis, analysis of diagnosis pattern, discrimination of diagnosis pattern, prognosis, a way of curing a disease, prescription, herbs in common use, disease appearing the diagnosis pattern, documents. The standards for diagnosis of each disease was arranged by the diagnostic requirements in the following order : another name, notion of disease, the main point of diagnosis, analyzing and differentiating of disease, analysis of disease, discrimination of disease, prognosis, a way of curing and prescription of disease, disease in western medicine appearing the disease in oriental medicine, documents.

        The CRM used in evaluation of effectiveness, calibration or analysis equipment when you analyse in material of rice powder or similar matrix. Analytical items were γ-HCH, p,p'-DDE, endosulfan-2, samples of achieved pertreatment analized each component indivisually. Control mode of MS used selected ion monitering(SIM). GC is 6890GC of Agilent Technology company and MS is JMS 700 of Jeol company. MS operating condition was Electron impact ionization with 70eV. 300A, GC operating condition was Column : Rtx-5MS 30m*0.25m ID*0.25um, Oven Temp 90℃(3min)-20℃/min-280℃(2min), Injection port 250℃, splitless, Carrier Gas He with 1mL/min. Water contents was 12.534% and dry mass correction factor 1.143. CRM certified values of the experiment result were p,p'-DDE 0.165±0.011,mg/kg, γ-HCH 0.0129±0.0011mg/kg, endosulfan-2 0.0216±0.0027mg/kg. The numbers following the symbol ± are the numerical values of expanded uncertainties, U=ku_(c) of measurement values preceding the symbol. The level of confidence for the expanded uncertainties are 96% with k=2.

      • K형 강관접합부의 편심접합에 관한 실험적 연구

        A finite element analysis and experimental study were performed to investigate the behavior and strength of a connection of tubular steel member considering slenderness ratio and eccentricity. Primary interest here are ultimate strength of connections having eccentricity. In comparison with the current design standard according to which the gravity axes of the main and branch pipes coincide with each other, the inner eccentric joint where the gravity axes of the branch pipes are shifted to the gusset plate side can reduce the gusset plate size. However, in the eccentric joint where the gravity axes do not coincide, the bending moment that acts on the main pipe due to eccentricity causes a problem. As a result, in cases where the slenderness ratio is 40 or less, the buckling strength of the inner eccentric joint was similar to that of a joint without eccentricity. Therefore the possibility of use of inner eccentricity connection to reduce the gusset plate size has been verified.

        톨루엔 노출 근로자에서 요중마뇨산 배설농도로 평가한 호흡보호구 착용효과에 관한 연구

        목적 : 보호구 착용 효과에 대한 실제적, 구체적자료를 제시함으로서 예방목적의 보호구 사용에 대한 근로자들의 인식을 제고시키기 위함이다. 방법 : 서울시내에 소재한 일부 그라비아 인쇄소에서 톨루엔이 함유된 잉크 및 희석제를 취급하는근로자 26명을 대상으로 본 연구를 수행하였다. 유기가스용 방독마스크 미착용 상태와 유기가스용 방독마스크 착용후 작업환경 중 톨루엔 개인노출농도와 요중 마뇨산 시간별 농도를 측정, 비교하였다. 결과 : 그라비아 인쇄소의 공기중 톨루엔 개인노출농도는 유기가스용 방독마스크 미착용일과 착용일에서 각각 147.52+57.34 ppm 및 134.55+52.44ppm으로 통계학적으로 유의한 차이는 없었다. 방독마스크 미착용일파 착몽일의 작업종료 후 요중 마뇨산 농도의 기하평균값은 각각 1.5) 10.33:1 g/L 및0.49(0.14) 하였으며 두 일간에 유의한 차이를 보였단. 방독마스크 미착용일에서 요중 마뇨산 농도는 근무시작 4시간 후(오후 작업시작전, 13:00)부터 유의하게 증가하기 시작하였으며, 착용일에서는 근무종료시까지 유의한 증가를 보이지 않았다. 유기가스용 방독마스크 착용 효과를 톨루엔 대사산물인 요중마뇨산 농도로 평가하면 착용시 농도가 미착용시에비하여 89.3% 감소하였다. 보호구 미착용일에 있어서 공기중 톨루엔 개인노출농도와 요중 마뇨산 농도는 유의한 상관성을 나타났으며 회귀방정식은 Y(요중 마뇨산, g/L) =0. OO7x (공기중 톨루엔, ppm)+0.665로 계산되었다. 결론 : Fit Test등 올바른 보호구 착용 교육을 통한 전 작업시간에 걸친 유기가스용 방독마스크 보호구 착용은 직업적인 톨루엔 노출의 호흡기 흡수를거의 대부분 차단하였다. Objectives : This study was carried out in order to raise workers' recognition of the effects of respirator use by providing concrete and practical data. Methods : Twenty-six workers who dealt with toluene based ink and diluents at a gravure printing office in Seoul were investigated. The toluene exposure level and urinary hippuric acid level were monitored under respirator non-wearing (RNW) and under respiratory wearing (RW) conditions. Results : The mean concentrations of toluene exposed to each worker through the air of gravure printing office were 147.52±57.34 and 134.55±52.44 ppm on respirator non-wearing day and respirator wearing day, respectively. There was no significant difference in toluene exposure concentration for the two days. The mean concentration of urinary hippuric acid in RNW and RW groups were 1.51 (0.53) g/L and 0.49 (0.14) g/L, respectively, as measured at the end of the workday. There was a significant difference seen in urinary hippuric acid concentration between the two days. The urinary hippuric acid concentration began to increase at 4 hours after the start of the workday (13:00) in RNW. However, there was no significant increase until the end of the workday in the case of RW. The urinary hippuric acid concentration in RW reduced 89.3% as compared with the concentration seen in RNW. There was a significant correlation between the toluene concentration and the urinary hippuric acid concentration in RNW. The regression equation was Y (urinary hippuric acid concentration, g/L) = 0.007 ×χ(toluene concentration in the air, ppm) + 0.665. Conclusions : Our results indicate that the respirator intercepted most toluene that may have been absorbed Into the respiratory organs, and suggested that properly wearing a respiratory such as wearing the respirator during the entire workday and performing a proper fit test played an important role in the protection from toluene exposure

        바이오 필터에 의한 벤젠과 에틸렌 처리실험에 대한 고찰

        A biofilter study was conducted by changing inlet concentration and residence time for the removal of gaseous benzene and ethylene. In addition, carbon dioxide producted from the biofilters was investigated. Over 96% of benzene was removed at the residence times of 2 and 4.3 min, and inlet benzene concentrations of 220~200ppm. The ethylene biofilter was capable of achieving ethylene removal efficiency as much as 100% at a residence time of 14 min, and inlet concentrations of 99~290 ppm. At a steady state, the carbon dioxide of 409~611 ppm was produced with an ethylene inlet concentration of 290 ppm. Most of benzene and ethylene were degraded at lower part of the biofilters where more microbial activity occurred.

      • K형 강관접합부의 보강효과에 관한 실험적 연구

        In current design, the strength formula for a Gusset-Tube connections stiffened with rib-plate is proposed only in the limited case because of its complexity of the behavior. So in this study, experimental study and finite element analysis were performed to investigate the behavior and strength of a Gusset-Tube connections stiffened with rib-plate. Because of limitations in loading capacity, the specimens were fabricated with 1/3 scale of full size. Tensile coupon tests and stub column tests were also carried out. Of primary interest here are the stiffened effect of Gusset-Tube connections considering lateral force ratio, gusset plate length, eccentricity, stiffened length. Consequently, we intend to obtain accumulated database to propose practical strength equation and suggest rational reinforcing method of Gusset-tube connection.

        Determination of biokinetic constant for volatile organic compounds(VOCs) is important because the biokinetic constants as biodegradation parameters play an significant role in mathematical design and modeling of bioremediation of VOCs. The objective of this study was to compare biokinetic of VOCs under aerobic and unaerobic conditions. After comparison of normalized k for VOCs on the basis of trichloroethylene(TCE) with methanotrophs, dichloromethane(DCM) and chloroform(CF) were very well degraded hydrocarbons whereas cis 1,2-DCE and 1,1,1-TCA were poorly degraded hydrocarbons under the aerobic conditions.

      • 전자선 증착장치 및 ITO 박막의 제작과 그 특성에 관한 연구

        태양 전지의 제작시 투명 전극막으로 쓰이는 ITO 박막을 제조하기 위하여 cathode 접지 방식의 전자총을 가진 정전 편향 집속형의 전자선 증착 장치를 제작하였다. 가속전압이 0∼15kV, 필라멘트 전류가 0∼28A, 두 전극판 사이의 간격이 11mm, anode와 deflector 사이의 간격이 30mm인 최적 실험조건하에서 생성된 전자선 spot의 크기는 길이가 20mm 폭이 1mm였으며, 전자선의 출력은 약 400W였다. 이 전자선 증착 장치를 이용하여 1600℃까지의 증발 온도를 가진 도체와 절연체 등을 쉽게 증착시킬 수 있었으며, 만드어진 ITO 박막은 약 5000Å의 두께에서 10^-1∼10^-2Ω·㎝의 비저항과 95%의 광투과율을 갖는다. An electron beam evaporator with a cathode grounded type electron gun was designed and constructed in order to deposit ITO thin films for photovoltaic applications. The optimum condition was observed with electode spacing of 11mm, 30mm between anode and deflector, a cathode inner diameter of 3mm and an anode inner diameter of 5mm. As the accelerating voltage varies from 0 to 15kV and ghe filament current from 0 to 28 A, the minimum electron beam spot has a 20mm length, and 1mm width and the output power ranges from 0 to 400W under the above optimum conditions. We can deposit materials whose evaporation temperature is over 1600℃ using this evaporator. And as a result ITO thin film showed a low resistivity of 10^-1∼10^-2Ω·㎝ and the transmittance of 95%.

      • 강판-콘크리트 구조 벽체-지지대 접합부의 거동특성에 관한 연구

        An experimental and analytical study on the behavior of SC(steel plate - concrete) structure's wall-support joint for nuclear power plants was performed. Nine full scale specimens were tested with horizontal monotonic load, all acting in the same plane causing uni-axial moment on the SC structure's wall-support beam joint. The major focus relates to the identification of the behavior and capacity of the SC composite joint system. The effects of variations of the location of support, the thickness of steel-plate and the depth of support were studied. The horizontal force-displacement relationship, steel-plate strains, plate deformation, support strains were observed and analyzed in order to evaluate the behavior of the system.
