        耳鍼을 活用한 禁煙鍼 施術이 吸煙靑少年들에게 미치는 影響

        Recently WHO death from smoking-related illness would increase more than a thousand in the future. But in spite of various harms, smoking cigarette in developing countries is increasing especially in the adolescents. So in order to evaluate whether auricular acupuncture therapy is effective to quit smoking, clinical examination was carried out. And the results showed that age of starting smoking become lower. The cause of smoking is curiosity, relaxation, envy and comradeship. For this reason they want to stop smoking but feel it is difficult to do. Cigarette smoking didn't show any significant change after auricular acupuncture in both control and therapy. And physical and psychological change did not present any significance. However, auricular acupuncture therapy was appeared to be effective to stop smoking. So with these results, further systemic research to examine the effect of auricular acupuncture is seem to be necessary.

        Cerebral mycotic aneurysms have been noted in 2-10% of cases of bacterial endocarditis and account for 2.5-6.2% of all intracerebral aneurysms. Mycotic aneurysms were reported to occur more frequently in the course of acute endocarditis rather than late in the course of subacute disease. Symptomatic mycotic aneurysms are now uncommon. Early or late hemorrhage correlates better with the presence of pyogenic arteritis and uncontrolled S, aureus infection. Early cerebral hemorrhage is usually associated with S. aureus arteritis, is not amenable to surgery, and carries a high mortality. Control of the infection dramatically decreases the risk of embolism in infective endocarditis patients. Recurrent emboli are not common after adequate antimicrobial treatment of the underlying infection. Recently, we experienced a case of infective endocarditis in which angiographically demonstrated mycotic aneurysm had been resolved with adequate antibiotic therapy.

        만성신부전 환자의 자율신경 기능장애

        Objectives: Impaired autonomic function in patients with chronic renal failure has been well documented in a number of studies to assess the degree of cardiovascular autonomic dysfunction and to assess the relationship with plasma catecholamines. The purpose of the present study was to evaluate the prevalence of autonomic dysfunction and to determine the effect of autonomic dysfunction on the increment of plasma catecholamine, dialysis-induced hypotension and hypotension during chronic dialysis. Methods: We measured the degree of autonomic damage and the concentration of plasma catecholamines in 20 patients on maintenance hemadialysis, 12 predialysis patients with chronic renal failure and 20 normal controls using a standardized battery of five cardiovascular reflex tests, Results: 1) In normal controls, 70% of cases had a normal or early parasympathetic abnormalities however in patients with chronic renal failure, 45.2% of patients had severe abnormalities. The prevalence of autonomic dysfunction was 62.5% and there was significant correlation between sympathetic and parasympathetic score in patients with chronic renal failure. 2) Although overall autonomic function was not different in two chronic renal failure groups, the magnitude of heart rate response to Valsalva maneuver was increased and the magnitude of fall of blood pressure in response to standing-up was reduced in dialyzed patients compared with nondialyzed petients. 3) Patients with autonomic dysfunction was older and had higher postdialysis concentration of plasma norepinephrine than those with normal autonomic function. 4) In dialyzed patients, predialysis concentration of plasma norepinephrine at rest varied widely and was significantly related to the duration of dialysis. Postdialysis concentration of norepinephrine was significantly correlated with the degree of parasympathetic damage. 5) There were no significant differences in autonomic damage or plasma catecholamines whether dialysisinduced hypotension and hypotension in chronic hemodialysis or not. Conclusion: Disturbances of autonomic nerve system are common in chronic renal failure with distinct abnormalities of parasympathetic function and additional sympathetic dysfunction. Elevated plasma norepinephrine seems to be related to the compensatory response of sympathetic nerve system to parasympathetic damage. Impairment of autonomic function does not appear specifically related to dialysis-induced hypotension or hypotension in chronic dialysis.

        Malaria is the world's most important parasitic infection. Although it has been erw5cated from temperate zones including Korea, increasing numbers of travellers visit tropical malarious countries and imported malaria becomes important medical problem in the developed countries. In Korea with increasing travellers to malaria-endemic area, the incidence of imported malaria shows rising tendency same as the developed countries. It becomes important to personal general personal protective measures and chemoprophylaxis to trevellers, when employed in appropriate manner, that can be highly effective in preventing malaria. We recently experienced a case of in Plasmodium vivax malaria with delayed manifestations due to inadequate chemoprophylaxis. A 53- year-old woman with history of trevel to East Africa 4 months ago and chief complaint of fever was diagnosed as tertian malaria. She had irregularly taken prophylatic antimalarial during travel and had not taken it after return but should have taken it at least 4 weeks after return.

        Aspergillus awamori와 Zymomonas mobilis로 구성된 혼합고정화 배양계의 최적 조건

        Gel bead의 효율적인 이용을 목적으로 산소요구성이 전혀 다른 두 균주로 구성된 혼합고정화 배양계의 개발을 시도하였다. 호기성 균으로서 A. awamori, 혐기성균으로서 Z. mobilis를 사용하여 생전분으로 부터 에탄올 생산을 행한 결과는 다음과 같다. 두 균주의 최적 혼합비율은 A. awamori 1.25×10^9 spore/L-gel, Z. mobilis 0.5g cell/L-gel이었다. 배양이 완료된 후의 gel bead 내의 균주 분포는 각각 혐기부와 호기부, 즉 gel bead의 표면부와 중심부로 생육장소가 구분되어 있었다. A-Z계의 배양에서는 에탄올의 수율이 낮았고, 균사가 gel bead로 부터 노출되었다. 배양 개시 후 36시간째에 배양기의 면전을 특수 제조한 check valve가 부착된 실리콘 plug로 교환하여 혐기적 배양을 행한 A-Z 36계에서는 gel bead로 부터 균사의 누출이 상당히 억제되었고, pH가 4.3을 유지하면서, 에탄올의 수율이 대조구 보다 약 2배 높게 나타났다. Co-immobilized mixed culture system(A-Z system) composed of two different oxygen-demanding strains, aerobic(Aspergillus awamori) and anaerobic(Zymomonas mobilis) strains, in a Ca-alginate gel beads was developed to increase ethanol production from raw starch as a carbon source. Optimal mixture ratio of A. awamori and Z. mobilis was 1.25×10^9 pores/L-gel and 0.5g cells/L-gel, respectively. After 120 hours of cultivation, gel beads distinguished oxygen-rich surface for A. awamorifrom oxygen-deficient central part for Z. mobilis. At A-Z culture system, yield of ethanol on glucose, Y_(p/s)=0.18, was very low and there was high leakage of cells from surface of gel beads. At A-Z 36 culture system with changing silicon check valve for cotton plug at 36 hours in A-Z culture system, there was no cell leakage from gel beads, pH was maintained at around 4.3 during cultivation, and yield of ethanol on glucose, Y_(p/s)=0.36, showed 2 times higher than that of control culture system (cotton plug culture).

      • 고온·알칼리성 Bacillus sp.의 Catabolite Repression 저항성 변이주로부터 Cellulase 및 Xylanase의 생산

        고온·알칼리성 cellulase 및 hemicellulase 생성 미생물을 분리할 목적으로 pH 10.3의 PY-CMC배지에서 55℃로 3회 연속 직접배양하여 효소 생성능이 우수한 20여균주를 분리하였고, 그 중에서 2개균주를 최종 선정하여 균학적 성상을 조사한 결과 고온·알칼리성 Bacillus속과 거의 일치하였다. 선정, 동정된 고온·알칼리성 Bacillus sp. F204와 F1020은 α-1, 4-glucose 등에서는 효소 생합성이 왕성하였으나, glucose와 같은 단당류에서는 효소가 전혀 유도되지 않았는데, 이 두 균주는 carbon catabolote repression을 받고 있었다. 2-deoxy glucose는 야생주에서는 강력한 repressor로 작용하였고, 야생주의 catabolite repression을 해제 시키는데 antimetabolite로 작용한다는 것이 밝혀졌다. NTG(500㎍/㎖, 50℃, 20∼60분)와 UV(2분) 처리후 2-DG(20mM)에 직접배양하여 강력한 catabolite repression resistant strain인 M-211과 M-367을 분리하였다. M-211과 M-367은 2-DG를 energy원으로 이용하였으며, glucose에 대하여 CMCase는 1.5%, avicelase 및 xylanase는 약 1.0%까지 저항성을 보였다. 또한 두개의 변이주의 효소 활성은 CMCase 2배, avicelase 1.5배 및 xylanase3배의 증가를 보였으며, 각 효소생합성을 위한 gene expression은 cAMP-independent transcription을 하는 것으로 나타났다. Cellulase and xylanase synthesis of thermophilic alkalophilic Bacillus sp. F204 and F1020 isolated in our lab, were repressed by glucose and induced by compounds structurally related to α-1,4 linkage (avicel cellulose and xylan et. al.). Catabolite repression-resistant mutants were obtained through treatment of NTG(500㎍./㎖), UV(2 min) and enrichment culture in 2-deoxy glucose (20 mM) containing PY-glucose on the wild type strain. T재 catabolite repression resistant mutants were not repressed by glucose and produced cellulase complex and xylansase when in grown in the presence of 1.0% glucose and 30mM 2-DC, which were repressors in wild type. Enzyme production was greater with the mutants (CMCase of 2 times, avicelase of 1.5 times, xylanase of 3 times) than with the wild type strains under various test conditions. T재 mutants M-211 and M-367 were potential catabolite repression resistant mutants and their gene expression was cAMP independent transcription.

        자발성 점막하 식도 박리 1예

        A 55-year-old male was admitted to our hospital complaining of chest discomfort, dysphagia and severe pain on swallowing. Esophageal manometry identified the lower esophageal sphincter as having no response to swallow. Barium swallow showed typical esophagograms of double barrelled esophagus or mucosal stripe. An endoscopic examination revealed an appearance as if an esophago-tracheal fistula had been made at the level of the upper thoracic esophagus. Also, two slitlike mucosal tears were seen on the lower esophagus. Subsequent fasting and total parenteral nutrition for several weeks failed to bring about any improvement in his symptoms. So, as a treatment, primary closure of the upper opening of the false channel was performed under general anesthesia. Soon after the surgical procedure, the patients symptoms disappeared except for mild dysphagia. He was discharged after oral intake had been judiciously commenced with fluids and soft diets. During his follow-up in the out-patient department, he was examined and found to have no specific symptoms including fever or dysphagia. Moreover, there was great improvement from the massive dissection of the esophagus on esophagogram.

        Pseudomonas sp. S4-14가 생산하는 Alkaline Lipase의 정제 및 효소학적 성질

        각종 균원시료로부터 LAS에 대해 내성이 있는 alkaline lipase를 생산하는 균주를 분리하여 동정한 결과 Pseudomonas sp. S4-14라 명명하였다. Pseudomonas sp. S4-14를 배양하여 ammonium sulfate 침전, DEAE-cellulose, DEAE-sepharose 및 gel filtratoin의 과정을 거쳐 정제하였으며 그 결과 비활성도는 992.15 U/㎎ protein, 수율은 16.1%이었다. 정제효소는 SDS-PAGE 상에서 단일 band로 확인되었으며 분자량은 약 65,000 dalton이었다. 정제된 alkaline lipase의 최적 pH는 10.5였으며, 최적온도는 45℃ 였다. pH 안정성 범위는 8.0∼12.0이었고 온도안정성은 45℃에서 1시간 동안 안정하였으며 50℃에서는 80% 활성을 유지하였다. alkaline lipase의 금속이온에 대한 영향을 검토한 결과 Pb^(2+), Zn^(2+), Fe^(3+) 에 의해 활성이 크게 저해되었고 Ca^(2+)에 의해 활성이 약간 증가되었다. 계면활성제(LAS, AOS, AE-7)와 표백제(perborate)에 대한 영향을 조사한 결과에서는 50 ㎎/ℓ 정도에서 약 50∼60% 정도가 실활되었다. The strain S4-14 which produced alkaline lipase and had resistance against linear alkylbenzene sulfonate was isolated from soil or water samples. The isolated strain S4-14 was identified a species belong to Pseudomonas. Alkalin lipase secreted by Pseudomonas sp. S4-14 was purified by ammonium sufate precipitation procedure followed by DEAE-Cellulose, DEAE-Sepharose and gel filtration chromatohraphies with 995.15 U/㎎ protein and 16.1% yield. The molecular weight of the enzyme was estimated to be 65,000 dalton by SDS-PAGE. The optimum pH and temperature of the purified enzyme was 10.5 and 45℃, respectively. The enzyme was stable at 45℃ for 1 hr and in a pH range from 8.0 to 12.0 for 24 hr at 4℃. The activity of lipase was enhanced by Ca^(2+) while inhibited strongly by Pb^(2+), Zn^(2+) or Fe^(3+). The activity of lipase was inactivated about 50∼60% in the presence of 50 ㎎/ℓ linear alkylbenzene sulfonate, α-olefin sulfonate, alcohol ethoxylate or perborate.

        We represent the way about storage and retrival of software components to reusable after making a module. Spacially, in this paper, we study the way about the retrival using the restricted natual langusge to reduce difficulty of usage of users. The result of this, restriction of grammer for interface between computer and man using natural language is vary larg, but problem of translation speed was not occur and individual differentia about retrival operation between users was absorbed.
