Background and purpose: Arterial stiffness is an important, independent determinant of cardiovascular risk. Pulse wave velocity (PWV) has been used as a valuable index of arterial stiffness and as a surrogate marker for atherosclerosis. The Framingham risk score was developed using categorized risk factors to predict the 10 year absolute risk of developing coronary heart disease (CHD). This algorithm is established using recommended guidelines for blood pressure, total cholesterol, and high density lipoprotein cholesterol in addition to age, smoking history and history of diabetes. Tongxinluo(TXL) has been shown to have anti hyperlipidemic activity and anti atherogenic effects. To determine its efficacy and safety, we examined whether TXL improves PWV, ABI, Framingham score, blood pressure, and lipid profile in high risk group of cardiovascular diseases. Subjects and methods: 49 subjects with the high risk of cardiovascular diseases were recruited. Subjects were administered TXL with the dose of 1110mg three times a day for 8 weeks. baPWV, ABI, Framingham risk score, Blood pressure and serum lipid profile were assessed at baseline and after 4 and 8weeks. Results: Total cholesterol, LDL cholesterol, triglyceride, total lipid and phospolipid significantly decreased after 4 weeks of medication. Total cholesterol, total lipid and phospolipid significantly decreased after 8 weeks of medication. There were no significant changes in Framingham risk scores, ABI, PWV and blood pressure. On safety assessment, there were no adverse effects, hepatic or renal toxicity. Conclusion: We suggest that TXL is a safe and useful herbal medicine for hyperlipidemia and as for anti-atherognic effects, further research would be necessary.

Objectives : Metabolic syndrome (MetS) is associated with increasing risk of cardiovascular morbidity and mortality but the risk of stroke associated with the MetS is less well established. The objective of this study was to evaluate the prevalence of MetS in stroke patients. Methods : We analyzed the data of 806 patients with acute ischemic stroke from Hanbang Stroke Registry supported by the Ministry of Science and Technology of Korea. Subjects' enrollment was done in the oriental medical hospitals of 4 universities located in the metropolitan region from April 2007 to August 2008. We analyzed all registered data and evaluated the prevalence of MetS and specific components of MetS. Results : The prevalence of MetS in stroke patients according to American Heart Association/National Heart Lung and Blood Institute (AHA/NHLBI) criteria was 59.6%. The prevalence according to the International Diabetes Federation (IDF) criteria was 41.6%. The prevalence of female patients was higher than that of male patients (71.8% vs 51% by AHA/NHLBI criteria; 61.7% vs 27.7% by IDF criteria). The prevalence of central obesity, low HDL cholesterol and hypertension was higher in female patients. There was no significant relationship between MetS and stroke subtype. Conclusions : This work gives us the basic data on the association between MetS and risk of stroke. There should be the prospective cohort studies to evaluate MetS and the risk of stroke.

Oxidative stress is one of the major reasons for brain aging and neurodegeneration. Ethanol and acetaldehyde increase the levle of oxidative stress in brain tissue resulting in aging and neurodegeneration related alcoholic dementia. Ginkgo biloba extracts are used as therapeutic and preventive agent for dementia. Here, it was investigated whether Ginkgo biloba extract show the effectiveness against ethanol- and acetaldehyde-induced oxidative stress in rat brain. Ethanol and acetaldehyde increased the level of oxidative stress by about 35% to 50% in rat brain tissue. However, Ginkgo biloba extracts reduced the level of ethanol- and acetaldehyde-induced oxidative stress. This result might reveal the link between the effectiveness of Ginkgo biloba extracts on oxidative stress and its effectiveness on alcoholic dementia.
위암 환자에서 Chronotherapy 의 부작용에 대한 고찰

Backgroud/Aims: Gastric cancer is one of the most common malignancy in Korea. In many reports, it has been known that chemotherapy does not improve survival rate of patients with gastric cancer. However, some Japanese reports showed that chemotherapy was effective. Side effects of chemotherapy increase with escalating dosage of chemotherapy drugs. There are some reports that chronotherapy could reduce the side effects and improve the survival rate in patients with gastric or colorectal cancer. We studied the side effects of chronotherapy in patients with gastric cancer. Methods: Fourteen patients with previously untreated gastric cancer received chronomodulated 5-FU ( 900 mg/m2/day, day 1-5) and cisplatin ( 90 mg/m2, day 1). Thirteen patients of control group received 5-FU ( 600 mg/m2/day, day 1-5) and cisplatin ( 60 mg/m2, day 1) during the daytime. The side effects of the two groups were evaluated for nausea and vomiting, diarrhea, peripheral neuropathy, mucositis, leukopenia, anemia, thrombocytopenia, and serum creatinine. Results: Compared to the control group, chronotherapy-treated group showed statistically significant decrease in nausea and vomiting ( p= 0.037), but not in others. Conclusions: Although only the incidence of nausea and vomiting was decreased significantly in the chronotherapy group, chronotherapy could be effective in reducing the side effects of chemotherapy in patients with gastric cancer considering that higher doses of drugs were used in the chronotherapy-treated group. Objectives : This pilot study is designed to reveal the effect of Carthami-semen acupuncture treatment on chronic daily headache. Methods : 1. All the voluntary subjects were diagnosed as CDH. 2. Carthami-semen acupuncture is applied 2 times a week for 4 weeks. 3. The patients were assessed by Headache impairment test score(HIT), Short Form 36 (SF-36) Health Survey, and Headache Free Days(HFD) before and after 4 weeks treatment, and two weeks the treatment finished. Results : The result were as follows ; 1. HIT score decreased from $65.3{\pm}5.5$ to $53.1{\pm}9.4$, and furthermore declined to $49.5{\pm}8.0$ after 2 weeks the treatment finished. 2. HFD increased from 19.1% to 31.5% during the treatment time. Moreover it showed continuous effectiveness after 2 weeks the treatment finished. 3. SF-36 showed significant improvement on all of the QOL items. Conclusions : This pilot study suggests that Carthami-semen acupuncture treatment has an effect on the CDH patients.
공복혈당수치와 운동유발전위의 상관관계에 대한 후향적 분석 : 중추운동신경과 말초운동신경의 비교

Objectives : Peripheral neurodegeneration occurs in diabetes mellitus (DM), both sensory and motor nerve. but we don't know exactly if DM affects central nerve pathway for all studies. Electrophysiologic study is one of the most important diagnostic tools for diabetic neuropathy. Electroneurography and electromyography are usually used. but evoked potentials (EP) is more sensitive to small nerve fiber damages and useful for central nerve evaluation in addition to peripheral nerves. Most diabetic neuropathy studies by EP have been performed with somatosensory evoked potentials (SSEP). In contrast, the objective of this study is to investigate if DM targets central motor neurons by assessing the relation between fasting blood sugar (FBS) and motor evoked potentials (MEP) latency. Methods : We inspected the medical records of 34 patients who had MEP tests during admitting days. The latency from cervical portion to abductor pollicis brevis was used as peripheral motor conduction time (PMCT). and the latency from vertex to cervical portion was used as central motor conduction time (CMCT). Then, they were correlated to FBS using correlation analysis. Results : There was a significant linear relation between FBS and PMCT (Pearson's correlation coefficient r=0.487, p<0.01), but a poor linear relation between FBS and CMCT (Pearson's correlation coefficient r=-0.l97. p>0.05). Conclusions : This study suggests that prolonged latencies of MEP in DM may be due to peripheral neuropathy rather than dysfunction of central motor pathway. therefore the clinical use of MEP to diabetic neuropathy has to be divided segmentally.
