        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

Second, From 1947 to 1957 Korean marathon had succeeded in commercial marathon race. Third, In 1990's the succession of Barcelona(1992) and Atlanta(1996) Olympics and succession of Asian Games(1990,1994,1998). 2.The succession of Russian marathon is classified by two stages. First, from 1946 to 1962 and 1978, the succession of European Championship. Second, the succession of Moscow Olympics.

      • Co(II) 이온과 N₂O₃계 거대고리 리간드와의 착물 형성에 관한 연구

        The stability constants of the complexes of Co(II) ion with N₂O₃-donor macrocyclic ligands such as 1, 15-diaza-3, 4:12, 13-dibenzo-5, 8, 11-trioxacycloheptadecane(NenOdienH₄):1, 15-diaza-3, 4 : 12, 13-dibenzo-5, 8, 11-trioxacyclooctadecane(NtnOdienH₄) have been determined by uv-vis spectrophotometric method in DMSO solution. In order to obtain the enthalpy and entropy changes of complexation reaction of Co(II) ion with these ligands, the stability constants have been measured at 20-60℃. From the results, the followings have been obtained, 1) In DMSO solution, Co(II) ion forms a 1:1 complex with these ligands. 2) The order of stability constants is NenOdienH₄< NtnOdienH₄. 3) The complexation reaction are exothermic and show the positive entropy changes.

        열간단조 후 공냉에 의해 제조된 내마모강의 마모특성

        Wear of blades, buckets, plates and chisel occurred in operation of construction machinery, agricultural equipments, earthmoving equipments and offshore structures cause earlier failure of the components, resulting in the increased energy consumption, cost of maintenance and replacement. To solve the problems, advanced materials with higher resistance to abrasive wear have been attempted to make through alloy design and hot deformation followed by air cooling. 0.25C-1.4Mn-1.0Cr-0.7Ni-0.2Mo-0.08V-0.02Cu steels produced by hot forging were cooled in air to form bainite, showing higher resistance to wear than any other steels with different chemical composition in the sliding wear tests under the applied load of 74N. Higher strain hardening during sliding wear of 0.25C-1.4Mn-1.0Cr-0.7Ni-0.2Mo-0.08V-0.02Cu steels to cause microploughing wear mode seems to be one of the main reasons for showing higher resistance to wear than any other steels in this study.

        귀리 수용성 추출물의 성분과 점도특성

        귀리 단백 다당체의 산업적 활용 가능성을 검토하고자 추출온도(40~60℃), 에탄올 농도(0~20%0 및 pH(5~9) 조건을 조합하여 중심합성계획법에 따라 얻은 귀리 추출물의 이화학적 특성을 분석하였다. 귀리 추출물의 수율은 6.4~17.9% 범위로 추출온도는 증가할수록 에탄올 농도는 감소할수록 증가하였고 중성일 때 보다 알칼리성이나 산성일 때 수율은 증가하는 경향을 보였다. β-glucan 및 단백질 함량은 각각 21.8~38.3% 및 23.8~29.0% 범위였고 추출온도는 낮을수록 에탄올농도는 높을수록 증가하는 경향을 보였으나 pH에 의한 영향은 없는 것으로 나타났다. 귀리 추출물 수용액은 저온 및 고농도일수록 점도가 증가하였으나 일반적으로 낮은 범위였으며, 전단속도에 따라 감소하는 의소성을 나타내었다. 본 연구결과 저농도 에탄올을 용매로 사용하여 귀리 추출물중의 단백 다당체 함량비를 조절할 수 있었고, 그 수용액 또한 식품용으로 적합한 유체 특성을 갖는 것으로 나타나 산업적인 활용가치가 높을 것으로 기대된다. To examine the utilization of the soluble proteoglucan in oats as a nutraceutical ingredient, soluble components were extracted from Oat Bran Concentrate (OBC) by varying the experiment factors such as temperature (40∼60℃), ethyl alcohol concentration (0∼20%), and pH (5∼9), and then analzed gor their compositions and viscosity. The total yield of iat solube extracts ranged between 6.4∼17.9 and the contents of β-glucan, protein, lipid, and ash in oat extracts were 21.4∼38.3%, 23.2∼29.0%, 8.3∼11.8%, and 5.9∼7.9%, respectively. The exreaction yield increased with the extraction temperature, but decreased with the alchohol concentration and pH. The β-glucan and protein contents increased with increasing temperature and decreasing ethaol concentration. However, the pH level did not influenced those contents. The viscosity of 2% oat extract solution ranged between 8.6∼83.1 cp at different share rates, showing the pseudo-plastic flow properties. Results suggested that extraction condition could be controlled the purity of β-glucan and protein and applied commercially to the large-scale process.

        <P>We report here a new protease activatable strategy based on a polymer nanoparticle platform. This nanosensor delivers chemically labeled matrix metalloproteinase (MMP)-activatable fluorogenic peptides to the specific MMPs of interest in vivo. Intravenous administration of the nanosensor in an MMP-positive SCC-7 xenograft tumor and a colon cancer mouse model verified the enzyme specificity of the nanosensor in vivo.         Nano-sized metabolic precursors for heterogeneous tumor-targeting strategy using bioorthogonal click chemistry <i>in vivo</i>

        <P><B>Abstract</B></P> <P>Herein, we developed nano-sized metabolic precursors (Nano-MPs) for new tumor-targeting strategy to overcome the intrinsic limitations of biological ligands such as the limited number of biological receptors and the heterogeneity in tumor tissues. We conjugated the azide group-containing metabolic precursors, triacetylated <I>N</I>-azidoacetyl-<SMALL> <I>D</I> </SMALL>-mannosamine to generation 4 poly(amidoamine) dendrimer backbone. The nano-sized dendrimer of Nano-MPs could generate azide groups on the surface of tumor cells homogeneously regardless of cell types via metabolic glycoengineering. Importantly, these exogenously generated ‘artificial chemical receptors’ containing azide groups could be used for bioorthogonal click chemistry, regardless of phenotypes of different tumor cells. Furthermore, in tumor-bearing mice models, Nano-MPs could be mainly localized at the target tumor tissues by the enhanced permeation and retention (EPR) effect, and they successfully generated azide groups on tumor cells <I>in vivo</I> after an intravenous injection. Finally, we showed that these azide groups on tumor tissues could be used as ‘artificial chemical receptors’ that were conjugated to bioorthogonal chemical group-containing liposomes via <I>in vivo</I> click chemistry in heterogeneous tumor-bearing mice. Therefore, overall results demonstrated that our nano-sized metabolic precursors could be extensively applied to new alternative tumor-targeting technique for molecular imaging and drug delivery system, regardless of the phenotype of heterogeneous tumor cells.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

        Facile Method To Radiolabel Glycol Chitosan Nanoparticles with <sup>64</sup>Cu via Copper-Free Click Chemistry for MicroPET Imaging

        <P>An efficient and straightforward method for radiolabeling nanoparticles is urgently needed to understand the <I>in vivo</I> biodistribution of nanoparticles. Herein, we investigated a facile and highly efficient strategy to prepare radiolabeled glycol chitosan nanoparticles with <SUP>64</SUP>Cu via a strain-promoted azide–alkyne cycloaddition strategy, which is often referred to as click chemistry. First, the azide (N<SUB>3</SUB>) group, which allows for the preparation of radiolabeled nanoparticles by copper-free click chemistry, was incorporated to glycol chitosan nanoparticles (CNPs). Second, the strained cyclooctyne derivative, dibenzyl cyclooctyne (DBCO) conjugated with a 1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid (DOTA) chelator, was synthesized for preparing the preradiolabeled alkyne complex with <SUP>64</SUP>Cu radionuclide. Following incubation with the <SUP>64</SUP>Cu-radiolabeled DBCO complex (DBCO-PEG<SUB>4</SUB>-Lys-DOTA-<SUP>64</SUP>Cu with high specific activity, 18.5 GBq/μmol), the azide-functionalized CNPs were radiolabeled successfully with <SUP>64</SUP>Cu, with a high radiolabeling efficiency and a high radiolabeling yield (>98%). Importantly, the radiolabeling of CNPs by copper-free click chemistry was accomplished within 30 min, with great efficiency in aqueous conditions. In addition, we found that the <SUP>64</SUP>Cu-radiolabeled CNPs (<SUP>64</SUP>Cu-CNPs) did not show any significant effect on the physicochemical properties, such as size, zeta potential, or spherical morphology. After <SUP>64</SUP>Cu-CNPs were intravenously administered to tumor-bearing mice, the real-time, <I>in vivo</I> biodistribution and tumor-targeting ability of <SUP>64</SUP>Cu-CNPs were quantitatively evaluated by microPET images of tumor-bearing mice.
