종양세포 검출을 위한 MAGE A1-6 역전사 이중 중합효소연쇄반응법의 민감도 및 특이도 평가

MAGE A1-6 아형을 동시에 검출할 수 있는 공통 시발체를이용한 역전사 이중 중합효소연쇄반응법을 사용하여, 종양검출법으로서의 민감도와 특이도를 평가하고자 하였다. 또한 본 방법의민감도를 기존의 telomerase 측정법 및 세포진검사법과 비교하였다. 2001년 2월부터 2002년 12월까지 암으로 확진된 355명의 환자를 대상으로 각종 암조직과 객담, 유도객담, 기관지 세척액, 흉곽액, 혈액, 소변 및 자궁경부 도말 검체 등을 채취하여 민감도를 평가하였고, 정상조직, 혈액, 소변, 객담 등 총 316예의 검체를 대상으로 특이도를 평가하여 다음과 같은 성적을 나타내었다. 1. 각종 암조직에서 MAGE A1-6 역전사 중합효소연쇄반응법의 평균 양성률은 79%였다. 편평상피암에서는 85% 이상의 양성률을 나타내었고, 선암에서는 약 70%의 양성률을 나타내었다2. 암환자들의 각종 체액에서 MAGE A1-6의 평균 양성률은57%였다. 검체에 따라 혈액 9-27%, 객담 59%, 소변 68%, 유도객담 및 기관지세척액 70% 이상, 흉곽액 및 자궁경부 도말 87%의 양 성 률 을 각 각 나 타 내 었 다 .3. 폐암, 위암 및 두경부암의 정상조직에서 MAGE A1-6 검사법은 0-8.9%의 양성률을 나타내었으며, 정상인의 혈액 및 양성폐질환자의 객담에서 2%, 정상인의 소변에서는 0%의 양성률을나타내었다.4. 26예의 객담을 대상으로 MAGE 역전사 중합효소 연쇄반응법과 telomerase 활성도 검출법 및 세포진 검사법을비교하여 평균 61.5%, 26.9% 및 15.4%의 양성률을 각각 나타내었다.이와 같이 MAGE A1-6 역전사 중합효소연쇄반응법은 지금까지 알려진 검사법 중 암세포 검출에 가장 높은 민감도와 특이도를나타내었다. 임상검체에서 RNA 보존이 잘 이루어진다면, MAGEA1-6 역전사 중합효소연쇄반응법은 각종 암세포 검출에 매우 유용하게 사용될 수 있을 것이다.
Clostridium Perfringens 에 의한 위막성 대장염 1예

A 62-year-old female was adrnitted to the Catholic University Hospital of Taegu-Hyosung with an intracerebral hemorrhage. She was operated on successfully, but developed bacterial pneumonia. She was then treated with sulperazone, tobramycin, and metronidazole for 1 month. After the antibiotic treatment, she suffered from a fever and bloody, mucoid diarrhea for 3 days, and was examined with a sigmoidoscope. The sigmoidoscopic examination revealed yellow patches of ulcerations and swelling covered with thick sero- sanguinous exudate in the distal transverse colon and sigmoid colon. A latex agglutination test for C. was performed on her stool, whereby difficile cytotoxin was negative, however, metronidazole resistant C. perfringens was isolated from anaerobic culture of the biopsied colon tissue. She recovered with 15 days using oral vancomycin treatment. The possibility of C. perfringens as a causative organism of pseuclomernbraneous colitis was discussed.
백서에서 기계적 담도폐쇄에 의한 HDL 대사 변화에 대한 연구

Extrahepatic biliary obstruction in humans and in rats leads to change of the total HDL and its subfractions. To study this mechanism author have used Sprague Dawley rat with transsected common bile duct to measure total HDL and HDL₃ by dual precipitation method. we had observed those rats for 20 days. Other parameters for liver dysfunction also were studied. The results were as follows: 1) HDL₃ is higher concentration than HDL₂ inhuman plaema about over half amount of total HDL but our study in the control rats, total HDL consist of HDL, 89.7±4.5% and HDL₃ 10.1±1.7%. 2) There were significant (statistical) differences between high total HDL levels of postoperative 5th days group (O.G1) or high those of postoperative 20th days group (O.G5) and those of control group (p$lt;0.01, p$lt;0.05) but a linear increase of total HDL levels of operation groups were not observed. 3) The differences of HDL₃ %(HDL₃/T-HDL) between operation groups and control group or either sham operation groups were statistically significant (p$lt;0.01, p0.01) and significant increase of HDL₃ % of operation groups was observed (p$lt;0.05). 4) The change of HDL₃ % of operation groups were observed same pattern in the mechanical biliary tree obstructed rats such as the change of total bilirubin, spleen weight and AST/ALT ratio. These results suggest that high HDL₃ levels can use as an indicator or a diagnostic marker of obstructive jaundice. 5) In studies of bile juice of operation groups, the HDL₃ % of bile juice (84.04%) were higher than those of serum (51.76%) and it menas that the form of nascent HDL from liver is almost HDL₃. Above results concluded that there were significant increase in the level of HDL₃ in mechanical biliary tract obstruction of rats compare to HDL₃ of normal rat and that is probably due to change of apolipoproteins and decrease activity of lecithine cholesterol acyl transferase.
