The factor that improve liver stiffness in patients with chronic hepatitis B

Background/Aims: Liver stiffness (LS) as assessed by transient elastography (TE) can change longitudinally in patients with chronic hepatitis B (CHB). The aim of this study was to identify the factors that improve LS. Methods: Between April 2007 and December 2012, 151 patients with CHB who underwent two TE procedures with an interval of about 2 years were enrolled. Ninety-six of the 151 patients were treated with nucleos(t)ide analogues [the antiviral therapy (+) group], while the remaining 55 patients were not [the antiviral therapy (–) group]. The two groups of patients were stratified according to whether they exhibited an improvement or a deterioration in LS during the study period (defined as an LS change of ≤0 or >0 kPa, respectively, over a 1-year period), and their data were compared. Results: No differences were observed between the antiviral therapy (+) and (–) groups with respect to either their clinical characteristics or their initial LS. The observed LS improvement was significantly greater in the antiviral therapy (+) group than in the antiviral therapy (–) group (–3.0 vs. 0.98 kPa, P=0.011). In the antiviral therapy (+) group, the initial LS was higher in the LS improvement group (n=63) than in the LS deterioration group (n=33; 7.9 vs. 4.8 kPa, P<0.001). However, there were no differences in any other clinical characteristic. In the antiviral therapy (–) group, the initial LS was also higher in the LS improvement group (n=29) than in the LS deterioration group (n=26; 8.3 vs. 6.5 kPa, P=0.021), with no differences in any other clinical characteristic. Conclusions: A higher initial LS was the only factor associated with LS improvement in patients with CHB in this study. (Clin Mol Hepatol 2015;21:32-40)

Carbon fiber reinforced plastic (CFRP) has many desirable qualities, including being lightweight and very strong. These characteristics have led to its use in applications ranging from small consumer products to vehicles. Circular and square CFRP members were fabricated using 8ply unidirectional prepreg sheets stacked at different angles ([+15°/-15°]4, [+45°/-45°]4 and [90]8, where 0° coincides with the axis of the member). Based on the collapse characteristics of a CFRP circular member, the collapse characteristics and energy absorption capability were analyzed. Impact collapse tests were carried out for each section member. In this study, the impact energies at crossheads speeds of 5.52 m/s, 5.14 m/s and 4.57 m/s were 611.52 J, 529.2 J and 419.44 J (circular member) 2.16 m/s, 1.85 m/s and 1.67 m/s are 372.4 J, 274.4 J and 223.44 J (square member). The purpose is to experimentally examine the absorption behavior and evaluation the strength in relation to changes in the stacking configuration when the CFRP circular members with different stacking configurations were exposed to various impact velocities. In addition, the dynamic characteristics were considered.

CFRP is an anisotropic material which is the most widely adapted lightweight structural member. Formation of specimen is prepared using autoclave and crushing test is utilized pneumatic vertical shock test. The CFRP Circular members were made of 8ply unidirectional prepreg sheets stacked at different angles ([902/02]s, [02.902]2, [90/0]s2, [90/0]2s, [0/90]4, where 0° direction coincides with the axis of the member) and interface numbers (2, 3, 4, 6 and 7). Based on the collapse characteristics of CFRP circular member, the collapse characteristics and energy absorption capability were analyzed. The impact collapse tests were carried out for each section member. In this study, the impact energy at the crosshead speed of 5.52m/s, 4.14m/s and 4.57m/s are 611.52J, 529.2J and 419.44J. The incidence of adrenal incidentalomas has increased because imaging studies are now being more frequently performed, including abdominal sonography, CT and MRI. Although there is only a consensus on the treatment of adrenal incidentalomas from the National Institute of Health (NIH) conference 2003, it is generally accepted that surgical resection is required if there’s any possibility of malignancy or functionality of the adrenal tumor. Abdominopelvic actinomycosis is a rare chronic progressive suppurative disease that is caused by gram-positive bacteria of the genus actinomyces, which is part of the normal flora of the oral cavity and gastrointestinal tract, with low virulence. Herein, we report on a case of adrenal actinomycosis that imitated a huge adrenal tumor in a 39-year-old women, and the adrenal actinomycosis was confirmed histologically only after adrenalectomy. To the best of our knowledge,this is the first Korean case report on actinomycosis that occurred in the adrenal gland

저자들은 축성 척추관절염 환자에서 돌발성 감각신경난청이 저음역에서 편측성으로 발병한 예를 경험하였다. 본 증례의 난청은 증상 발생 24시간 이내 전신적 및 국소적 스테로이드 치료로 회복되었다. 축성 척추관절염환자에서 돌발성 감각신경난청이 동반될 수 있으며 이 경우 조기 스테로이드 치료를 고려해야 한다. Sudden sensorineural hearing loss (SNHL) is rarely presented in patients with several immune-mediated inflammatory diseases. We report a case of sudden SNHL in a patient with axial spondyloarthritis (aSpA). A 29-year-old male with aSpA was admitted for sudden unilateral hearing loss. His aSpA symptom was stable; however, pure tone audiometry revealed that he had SNHL in the left ear at low frequency. His hearing was fully recovered with systemic and local steroid therapy. This case suggests that sudden SNHL may be an extra-articular manifestation of aSpA and thus need prompt steroid therapy in order to restore hearing.

A 36-year-old woman had erythematous confluent maculopatches on her face, trunk, and palm. she was diagnosed as erythema multiforme. After she was treated with dapsone, she presented with jaundice, fever and abrupt aggravation of the skin lesion. She had elevated hepatic enzymes, bilateral cervical lymphadenopathy, and splenomegaly. The possibility of angioimmunoblastic T-cell lymphoma was raised as an exicisional biopsy of her cervical node showed atypical lymphoid hyperplasia with immunohistochemical results compatible with angioimmunoblastic T-cell lymphoma. As her clinical course was highly suggestive of dapsone-induced hypersensitivity syndrome, we assumed that the cervical lymphadenopathy might be a pseudolymphoma after systemic hypersensitivity reaction. In a transjugular liver biopsy no evidence of lymphomatous involvement was found, and she was recovered without chemotherapy but low dose prednisolone for dapsone induced hypersensitivity. Gradually the skin lesion was improved and the cervical lymph node enlargement regressed. Her hepatic enzymes were also normalized in 3 weeks of prednisolone and the corticosteroids were then tapered for a month. In follow-up CT scans after 3 months and 6 months of the disease manifestation, the cervical lymph nodes enlargement was not shown any longer, spleen size was normalized, and no other findings suggestive of lymphoma progression was observed. Here we present a case of lymphoadenopathy mimicking angioimmunoblastic T-cell lymphoma as an adverse reaction of dapsone.
