        Laparoscopic Ventral Hernia Repair

        Laparoscopic ventral hernia repair is performed less frequently than open repair because some ventral hernias are unsuitable for laparoscopic repair and the complications are more severe than those of open repair. However, currently, the incidence of laparoscopic hernia surgery has been gradually increasing. The technique for laparoscopic ventral hernia repair depends on the shape, size, location, number, recurrence, and symptoms of the hernia. Computed tomography (CT) is the most accurate method for identifying these factors. Ventral hernia repair begins with an approach to the peritoneal space. Having adequate space to place the mesh is the most important step in surgery. Cosmetic and medical results of primary closure of the hernia margin are superior to those of the bridging technique in laparoscopic ventral hernia repair. However, if primary closure is not possible, the component separation technique can be used to narrow the defect for primary repair of a ventral hernia. Making the abdominal skin flap during the conventional component separation technique can injure the perforator vessels in the abdominal wall, and an injured perforator shuts down the blood supply to the subcutaneous tissue of the abdomen, which then becomes necrotic. To prevent such complications, a perforator-preserving technique can be performed, such as the laparoscopic and posterior component separation techniques. Complications of laparoscopic ventral hernia repair include seroma, hemorrhage, intestinal injury, mesh infection, and recurrence. Mesh infection is one of the most severe complications that sometimes requires reoperation. To prevent infection, it is necessary to minimize contact between the mesh and skin during the surgical procedure.

        시신경척수염범주질환에서 재발뇌병변에 의해 발생한 기면병과 난치구토

        We report a 26 year-old female who initially presented with hypersomnia and visual disturbance with preceding upper respiratory infection. She was diagnosed as neuromyelitis optica spectrum disorder (NMOSD) with the presence of anti-AQP4 antibody. Eight months later, she experienced nausea and vomiting refractory to conventional therapies, which was proved correlated with a lesion of area postrema on brain magnetic resonance imaging. These might be significant clinical manifestations in NMOSD and may widen the clinical spectrum of the disease.

        Clinical and Pathological Features of Korean Patients with DNM2-Related Centronuclear Myopathy

        Background and Purpose Centronuclear myopathy (CNM) is characterized by the presenceof central nuclei within a large number of muscle fibers. Mutations of the dynamin 2 gene (DNM2)are common causes of autosomal dominant or sporadic CNM. The aim of this study was to characterize the clinical and pathological features of CNM relative to the presence of DNM2 mutations. Methods Six patients with clinical and pathological features of CNM were recruited. Detailedclinical and pathological findings were analyzed according to the presence of DNM2 mutations. Results We detected DNM2 mutations in four of the six sporadic CNM patients, and identifiedthe following distinct clinical and pathological features in those patients with DNM2 mutations:preferential involvement of the distal lower limbs, typical nuclear centralization, and radially distributed sarcoplasmic strands in muscle pathology. In contrast, those without DNM2 mutationsexhibited rather diffuse muscular involvement, and nuclear internalization and myofibrillar disorganization were more pronounced features of their muscle pathology. Conclusions These findings suggest the presence of specific features in Korean CNM patients. A detailed clinical and pathological examination of CNM patients would be helpful for molecular genetic analyses of this condition.

        이형접합 COL6A1 돌연변이에 의한 Bethlem 근육병가족

        Collagen-VI-related myopathies are caused by mutations in the COL6A1, COL6A2, and COL6A3 and are known to have a wide phenotypic spectrum, including Bethlem myopathy, Ullrich congenital muscular dystrophy, intermediate phenotype, and limb-girdle muscular dystrophy. These patients present with joint hyperextensibility and/or contractures as well as skin changes and muscle weakness, and so clinicians need to notice those extramuscular symptoms in order to achieve a correct diagnosis. We describe the clinical, pathological, and radiological features in a family with Bethlem myopathy caused by a COL6A1 mutation.

        Surgeon Custom-Made Iliac Branch Device to Salvage Hypogastric Artery during Endovascular Aneurysm Repair

        Endovascular salvage of the hypogastric artery using iliac branch device (IBD) duringendovascular aortic aneurysm repair (EVAR), offers less invasive alternative solution tosurgery to prevent pelvic ischemia. We have performed the first Korean surgeon custommadeIBD for this purpose to overcome the limitation of unavailability of the devices inKorea. Four patients with abdominal aortic aneurysm with bilateral common iliac arteryaneurysm (CIAA) were treated using custom-made IBDs from October 2013 to December2013. IBD was created in back table before EVAR operation using TFLE Zenith iliac limbstent graft (Cook Inc.). Three V12 (Atrium, Inc.) one Viabahn (Gore, Inc.) were used forbridging between IBD and target hypogastric artery. With this modification of IBDprocedure, exteriorize the guide wire without snare device is possible which offers anotherbenefit in terms of reducing medical costs comparing to commercial IBD. All operationswere successful without any device related complications or postoperative endoleaks. During the mean follow up of 3 months, all IBD were patent without clinicalcomplications. Surgeon custom made IBD is feasible and useful to preserve pelvic perfusionespecially in the situation of limited commercial IBD availability in many countries. Longtermfollow-up is needed to evaluate stent graft patency and IBD-related complications.

        유색감자 추출물의 항돌연변이 및 항암 활성

        본 연구는 괴경 내부에 적색 및 보라색의 안토시아닌 색소를 함유한 유색감자의 추출물을 대상으로 S. typhimurium TA98과 TA100 균주의 돌연변이 유발여부를 확인하였고, 직접돌연변이원인 4-nitroquinoline-1-oxide(4-NQO)와 간접 돌연변이원인 bozo(a)pyrene(BaP)에 의해 유발될 수 있는 돌연변이에 대한 항돌연변이 활성과 6종의 인간 암세포주(전립선암세포주: LNCaP, 결장암 세포주: HCT-15와 SW-620, 위암 세포주: ACHN, 폐암 세포주: A549, 백혈병 세포주: MOLT-4F)를 대상으로 SRB 방법을 이용하여 항암활성을 비교하였다. 그 결과 유색감자 중 괴경 내부에 보라색의 안토시아닌을 다량 함유한 자영 품종이 다른 품종에 비해 높은 수준의 항돌연변이활성 및 항암활성을 나타내었으며, 특히 자영 품종의 추출물은 다른 세포주 보다 전립선암 세포주에 대한 항암활성이 특이적으로 우수한 양상을 확인하였다. 자영 품종의 추출물은 5ug/mL 이상의 농도에서 암세포의 증식을 억제할 뿐 아니라, 전립선암 세포주 LNCaP와 PC-3에 대해 세포사멸을 유발하는 결과를 Cell Death Detection ELISA와 TUNEL Assay로 확인하였고, 세포사멸과 연관된 유전자의 발현분석을 western blot으로 확인하였다. 이상의 결과에서 괴경 내부에 안토시아닌을 함유한 유색감자는 괴경 내부의 색상이 백색인 일반감자에 비해 강한 항돌연변이활성 및 항암활성을 나타내므로 유색감자는 기능성이 증대된 식용감자로서의 이용가치가 충분하며, 유색감자를 이용한 기능성식품 소재개발 및 의료산업의 신규소재화가 가능하리라 판단된다. Colored potatoes are an excellent source of dietary polyphenols including anthocyanins. Generally, anthocyanins from fruits and vegetables exhibit anti-carcinogenesis and anti-cancer properties in vitro test. This experiment was conducted to know the effects of colored potato extracts contained anthocyanins on antimutagenic activity and anticancer activity to six human cancer cell lines containing LNCaP (androgen-dependent) prostate cancer cells. Extracts of three colored potatoes ('Hongyoung', 'Jayoung' and 'Jasim') and the white potato ('Superior') cultivars were used in this study. The extracts of three colored potatoes inhibited the mutagenicities induced by direct mutagen such as 4-nitro-quinoline-1-oxide (4-NQO) and another indirect mutagens of bezo(a)pyrene (BaP). Also, the extracts of 'Hoyoung' and 'Jayoung' showed higher antimutagenic activity than 'Jasim' and 'Superior' against to direct or indirect mutagen on both strains of TA98 and TA100. The activity of growth-inhibitory of extract of four potato cultivars were screened by SRB (sulphorhodamine B) method on diverse human cancer cells representing different types of cancers. Among the extract of four potato cultivars, the extract of 'Jasim' showed moderate inhibition on proliferation of LNCaP, ACHN and MOLT-4F cells and did not inhibit the proliferation of other cancer cells. On the other hand, extract of 'Superior' did not inhibit the proliferation of any tested cancer cell lines. However, the extracts of 'Hongyoung and Jayoung' inhibited the proliferation of cancer cells with GI50 values ranging from 2.5 to 30~;~mug/mL . On the basis of the GI50 values, it is clear that LNCaP cells were more sensitive to extracts of colored potato cultivars than other cancer cells. The extract of 'Jayoung' at 30~;~mug/mL were more active and inhibited cell proliferation, and induced apoptosis in LNCaP cells. This result revealed that the extracts of colored potatoes are expected to be good candidate for development into source of antimutagenic and anticancer agent.

        Effect of Abrupt Onsets Attentional Capture by the Color Word in the Stroop Task

        Although the Stroop effect has been considered to be strong evidence of automatic recognition of word, some studies showed that the Stroop effect is modulated by visual attention. In this study, we investigated the role of visual attention in the involuntary word processing of the Stroop task. Three experiments examined whether the magnitude of the Stroop effect is modulated by the color-word’s ability to capture attention in a bottom-up fashion, namely, via an abrupt onset. The Stroop effect was found to be smaller when the color-word distractor appeared as a part of a pre-existing placeholder than when it appeared abruptly in a blank location around the color-patch target (Experiment 1). Furthermore, dilution of the Stroop effect in the presence of an additional, neutral word depended on which of the two words was presented more abruptly (Experiments 2 & 3), suggesting attentional competition between them. These results imply that the color word in the Stroop task is recognized when it captures attention.
