Eosinophilia accompanied by eosinophilic invasion and organ dysfunction may develope idiopathic hypereosinophilic syndrome. Any organ can be involved including bone marrow, lung, skin, heart, gastrointestinal tract and nervous system. Cough, dyspnea, pleural effusion or chest pain are common pulmonary manifestation, and they may be attributed to parenchymal infiltration, pulmonary embolism or heart failure. We report a 43-year-old woman with idiopathic hypereosinophilic syndrome involving bone marrow, skin, and lung. The patient developed acute dyspnea and chest pain. High resolution CT demonstrated multiple wedge-shaped segmental involvement with pleural effusion thought to be a pulmonary infarction or heart failure. Echocardiography could not find any abnormality. Lung biopsy showed interstitial eosinophilic infiltration with increased eosinophils in BAL fluid. She was treated with high dose corticosteroid and hydroxyurea. Within few days, most of her symptoms disappeared and chest radiography nearly cleared up.
장무지굴건 건초염에 대한 건내시경적 치료의 임상적 결과 및 합병증

Purpose: To report the clinical outcomes and complications of flexor hallucis longus (FHL) tendoscopy using 3 portals. Materials and Methods: Between January 2012 and April 2013, 10 patients (10 ankles) received tendoscopic surgery for the treatment of FHL tenosynovitis. Patients complaining of pain and tenderness along the course of FHL despite over 6 months of conservative treatments were indicated for surgery. The mean age was 41.7 years (range: 18-57) and the follow up period was 12.7 months (range: 6-20). Tendoscopy was performed using posteromedial, posterolateral, and plantar portals. Clinical evaluations included preoperative and postoperative visual analogue scale (VAS), American orthopaedic foot and ankle society (AOFAS) score, and patients' satisfaction. Results: Tendoscopic findings included tenosynovitis in 10 cases, degenerated vinculae in 6 cases, stenosis of the tendon at its entrance into the fibro-osseous tunnel in 5 cases, and degenerative partial tendon tear in 3 cases. Two cases had associated symptomatic os trigonum and 3 cases had posterior ankle impingement syndrome. Preoperative pain decreased from median VAS 6 (range: 4-10) to 2.1 (range: 1-5) at the last follow up and AOFAS score improved from 50.1 (range: 36-63) to 82.1 (range: 61-89) (p<0.05). Nine patients were satisfied or very satisfied with the outcome. Injury of the lateral plantar nerve occurred in one case. Conclusion: FHL tendoscopy using 3 portals is a feasible and useful minimal invasive surgical technique for the management of FHL tenosynovitis.
북경대학 소장 한대의간(漢代醫簡)과 노관산 의간(老官山醫簡)의 비교 연구

Objectives : Overlapping contents between two recently discovered Han period bamboo slats, the so-called "Beidahanjian" and the "Liushibingfang" have been identified. This study aims to present new knowledge that could be inferred from the concordance of these two texts. Methods : The most recent original texts of the medical part of the Beidahanjian and medical texts excavated from the Laoguanshan in addition to the Liushibingfang were compared with each other to determine identical parts. The meaning of these concordances was explored. Results : Identical sentences in two verses in the Beidahanjian and the Laoguanshan were identified. Conclusions : The Beidahanjian is a credible Western Han period text, of which the medical bamboo slats are likely to comprise an independent text that is a combination of ancient folk prescriptions and those of doctors. 목적: 근래에 발견된 두 종의 한대 문헌인, 소위 “북대한간”과 “육십병방” 사이에 중복된 내용이 확인되었다. 본 논고에서는 이러한 두 문헌의 일치를 통해 유추 가능한 신지견을 제시하고자 한다. 방법: 최근까지 발표된 북대한간의 의학 영역 원문과 육십병방을 비롯한 노관산 출토 의학 문헌 원문을 상호 대조하여 일치하는 부분을 확정하고 이러한 일치가 갖는 의의를 합리적으로 탐색해 보았다. 결과: 북대한간과 육십병방의 2개 조문에서 문장의 일치가 확인되었다. 결론: 북대한간은 날조의 혐의가 없는 신뢰할 만한 전한시대 문헌이며 북대한간의 의학 분야 죽간은 고대 민간 처방과 의가(醫家) 처방이 혼재된 형태의 단일 문헌일 가능성이 크다.
사출성형 CAE와 머신러닝을 이용한 스파이럴 성형품의 중량 예측

In this paper, we intend to predict the mass of the spiral using CAE and machine learning. First, We generated 125 data for the experiment through a complete factor design of 3 factors and 5 levels. Next, the data were derived by performing a molding analysis through CAE, and the machine learning process was performed using a machine learning tool. To select the optimal model among the models learned using the learning data, accuracy was evaluated using RMSE. The evaluation results confirmed that the Support Vector Machine had a good predictive performance. To evaluate the predictive performance of the predictive model, We randomly generated 10 non-overlapping data within the existing injection molding condition level. We compared the CAE and support vector machine results by applying random data. As a result, good performance was confirmed with a MAPE value of 0.48%.
