        당뇨병환자에서 뇨증 Fibronectin농도의 증가에 관한 연구

        Plasma fibronectin is an α₂-glyoprotein, which is produced by vascular endothelial cells. Raised level of plasma fibronectin has been observed in diabetic patients particularly in the presence of microvascular complications. However, no available data exist about urine level of this glycoprotein in diabetic patients. We measured urine fibronectin level by ELISA methods in 54 diabetic patients who have microvascular comlications or not. The following results were obtained. 1) Urine fibronectin level(㎍/g creatinine) in diabetic group(1740.0±678.0) is increased compaired with those in normal control group(471.0±59.0). but this results were not significantly different among two groups(P>0.05). 2) Urine fibronectin level(㎍/g creatinine) was significantly increased in patients with nephropathy (6188.0±3144.0) compared with those in normal control group(471.0±59.0) and patients without nephropathy(645.0±251.0)(P<0.001). 3) There were a significant correlation between BUN, creatinine, creatinine clearance, 24hr urine total protein and urine fibronectin level in diabetic patients. Our data suggest that urine fibronectin excretion level might be used as a sensitive guide for diabectic nephropathy.

      • 키토산에 대한 Itaconic acid의 graft 공중합과 그 특성

        Chitosan was prepared from chitin which was abstracted from wasted crab shell. Then itaconic acid was graft-copolymerized onto chitosan using ceric ammonium nitrate(CAN) as a reaction initiator. To investigate the optimal grafting conditions, the influences of several factors on the grafting were studied, i.e., the concentrations of CAN and itaconic acid, the reaction temperature and time. And to find out its flocculation ability, the flocculation test was carried out with a metal plating factory waste water. The state of the graft-copolymer was identified through IR spectra checking. The optimal grafting conditions and flocculation results were shown to be: concentration of ceric ammonium nitrate is ??, reaction temperature is 40℃ and reaction time is 4hrs with 0.25M of the monomer(itaconic acid). Through flocculation tests using chitosan, grafted chitosan(CsIa) and cation, CODcr, metal ions removal rates were measured. The order of superiority is CsIa>Cs>Cation.

      • 장벽경색이 선행한 Henoch-Schonlein 자반증 1례

        Henoch-Schonlein 자반증은 피부, 위장관, 관절, 신장 등을 주로 침범하며 소혈관염을 기본 병리 소견으로하는 전신성 혈관 장애 질환이다. 위장관계 증상으로는 복통, 오심 및 구토, 잠혈, 토혈, 흑색변 등이 있을 수 있고, 이러한 증상은 합병증 없이 회복되는 것이 보통이나 드물게는 장중첩증, 장벽경색 및 괴사, 장천공 등의 심한 합병증을 동반할 수도 있다. 이러한 위장관계 증상들은 특징적인 피부 소견에 선행하여 나타날 수도 있으며, 이때는 급성복증에 대한 감별진단이 중요하다. 저자들은 장벽경색이 선행하여 감별진단에 어려움을 겪고 시험적개복술을 시행한 이후에 특징적인 피부 자반이 발현되어 Henoch-Schonlein 자반증으로 진단할 수 있었던 증례를 경험하였기에 이를 보고하고자 한다. 본 증례는 입원 1일전부터 시작된 복통을 주소로 입원한 5세 남아가 배꼽 주위에 심한 경련성 복통을 지속적으로 호소하였고 이학적 소견상 배꼽주위의 압통, 장음의 감소, 흑색변 등이 있었으며 소장조영술상 공장 부위에 미만성의 불규칙한 점막비후 소견을 보여 제 8병일에 시험적개복술을 시행한 결과 근위 공장에 국한된 장괴사의 소견이 발견되어 절제 및 문합술을 실시 하였다. 병리학적 검사에서는 장벽경색의 소견을 보였다. 수술후에 복통은 완화되었으나 경미하게 지속되었고, 제 13병일경에 급성 부고환염이 병발되었다가 회복되었고 제 15병일경에는 하지에서부터 발현된 자반이 3일간 지속되었다. 결구 Henoch-Schonlein 자반증과 이의 합병증으로 발생한 근위 공장의 장벽경색으로 진단하였다. Colicky abdominal pain as symptom of Henoch-Schonlein purpura occurs in up to 70% of patients, and results from submucosal edema and hemorrhage. Abdominal pain and other GI symptoms such as nausea, vomiting, melena and hematemesis resolve mostly without any complications. But rarely life-threatening GI complications such as intussusception, intestinal mural infarction and necrosis, and massive hemorrhage may develope. In 14% of patients, abdominal pain may precede other symptoms, and creats diagnostic difficulties. We reports a case of the Henoch-Schonlein purpura presented with colicky abdominal pain which resulted from intestinal mural infarction and preceded characteristic purpuric skin lesion by several weeks. This 5 year old male patient complained of colicky abdominal pain, nausea and melena. Exploratory laparatomy was done and revealed intestinal mural infarction in proximal jejunum. One week later operation, purpuric skin lesion develiped in both leg and feet, and then, Henoch-Schonlein purpura was diagnosed.

        편평세포폐암에서 p53 돌연변이의 발현과 TNM 병기와의 상관관계

        Primary lung cancer is now recognized as a major cause of cancer death in Korea as well as the whole world. Non-small-cell lung cancers(NSCLC) comprise about 75% of lung cancer and squamous cell carcinoma is the most common type in Korea. Mutations of p53 gene are common in variety of human cancers, including lung cancer. The p53 gene appears to inhibit the proliferation of cells from the G1 to the S phase of cell cycle and is able to suppress the transformations of cells by other oncogenes, to inhibit the growth of malignant cells in vitro and suppress the tumorigenic phenotype of transformed cells. Alteration or inactivation of p53 by mutation, or by its interactions with oncogene products of DNA tumor viruses, can lead to cancer. These mutations seems to be the most common genetic changes in human cancers. p53 gene mutation is known to be a poor prognostic marker in breast cancer and has significant association with lymph node involvement. But in human lung cancer the association of p53 mutations and TNM stages are controversial. Immunohistochemical staining can detect only mutant p53 protein bacause of markedly prolonged half life of mutant p53 protein. We performed the immunohistoschemical staining on 48 surgically resected and 10 bronchoscopically biopsized specimens of primary squamous cell carcinoma of the lung with monoclonal antibody(DO 7) and analyzed the relationship between the expression of p53 protein and clinical TNM stages. p53 protein was detected in 60.3% of total 58 cases. p53 protein was positive in 52.9% of stage I(17cases), 83.3% of stage Ⅱ(6 cases), 72.2% of ⅢA(18 cases), 42.9% of ⅢB(7 cases) and 50% of stage Ⅳ(10 cases) which revealed no significant correlation between p53 protein detection and stages. There was no significant correlation between p53 protien detection and No and N₁-₃as well Mo and M₁. In conclusion, we couldn't find any significant correlation between p53 protein detection and TNM stages which suggests p53 mutation is frequent in squamous cell carcinoma of the lung but play different role in progression of clinical stages.

      • 키틴계 담체를 이용한 효율적인 효소고정화법 개발

        본 연구에서는 키토산을 담체로하고 5가지 다른 방법에 의해 단백질 가수분해 효소인 trypsin을 선정하여 효율적인 효소고정화법을 개발하고자하였다. 비교한 효소고정화 방법으로, 먼저 가교제의 변화로서 glutaraldehyde와 hexamethylenediisocyanate를 비교였고, 다음으로 프로세스의 변화로서 ⅰ)chitosan bead에 효소를 흡착한 후 glutaraldehyde로 가교하는 방법, ⅱ)chitosan bead에 효소를 흡착한 후 hexamethylenediisocyanate로 가교하는 방법, ⅲ)chitosan과 효소를 혼합하여 bead화하고 glutaraldehyde로 가교하는 방법, ⅳ)chitosan bead를 glutaraldehyde로 1차 가교 하고 효소를 흡착한 후 다시 glutaraldehyde로 2차 가교하는 방법, ⅴ)chitosan bead에 효소 흡착 후 glutaraldehyde로 가교 처리하여 NaOH로 처리하는 방법등으로 비교하여 ⅲ>ⅴ>ⅳ>ⅰ>ⅱ 의 순으로 아미노산 생성효율을 확인하므로써 보다 효율 높은 효소고정화법을 모색하였다. 가교제의 종류에서는 glutaraldehyde가 더 유리하였고, 고정화 가교방법에서는 chitosan과 효소를 미리 혼합하여 bead화한 다음 가교한 것이 기타 다른 방법들에 비해서 고정화 효율이 가장 우수함을 확인하였다. More effective methods for immobilization of enzymes(trypsin) were examined with chitosan beads, through comparison of several different immobilization methods. The methods applied in this experiment are: Firstly with different crosslinking agents : glutaraldehyde and hexamethylenediisocyanate. Secondly with different processes: ⅰ) Adsorption of enzyme on the beads, then crosslinking with glutaraldehyde. ⅱ) Adsorption of enzyme on the beads, then crosslinking with hexamethylenediisocyanate. ⅲ) Mixing chitosan with enzyme, then beads formation followed by crosslinking with glutaraldehyde ⅳ) Treatment with glutaraldehyde, then enzyme adsorption on the beads followed by crosslinking with glutaraldehyde. ⅴ) Adsorption of enzyme on the beads and then crosslinking with glutaraldehyde followed by treatment with 0.01N-NaOH aq. solution The order of superiority is ⅲ>ⅴ>ⅳ>ⅱ>ⅰ described above. The efficiencies of hydrolysis of milk protein by the immobilized enzymes were estimated by spectrophotometry of amino acids produced through hydrolysis.

        뉴로메드정(옥시라세탐 800㎎)에 대한 뉴라세탐정의 생물학적동등성

        The purpose of the present study was designed to evaluate the bioequivalence of two oxiracetam tablets, Neuromed tablet (Korea Drug Co., reference drug) and Neuracetam tablet (Sam Jin Pharmaceutical Co., test drug), according to the guidelines of Korea Food and Drug Administration (KFDA). Release of oxiracetam from the tablet in vitro was tested using KP Ⅷ Apparatus Ⅱ method with various dissolution media (pH 1.2, 4.0, 6.8 buffer solution and water). Twenty-four healthy volunteers, 23.7 ± 2.4 year in age and 68.9 ± 6.2 ㎏ in body weight, were divided into two groups and a randomized 2×2 cross-over study was performed. After oral administration of a tablet containing 800 ㎎ of oxiracetam, blood samples were taken at predetermined time intervals and concentrations of oxiracetam in plasma were determined using HPLC-MS-MS. The dissolution profiles of two formulations were very similar at all dissolution media. In addition, pharmacokinetic parameters such as AUCt, C_(max) and T_(max) were calculated and ANOVA test was utilized for the statistical analysis of the parameters using logarithmically transformed AUC, and C_(max), untransformed T_(max). The results showed that the differences between two formulations based on the reference drug were 0.42%, 0.45% and -12.58% for AUCt, C_(max) and T_(max), respectively. There were no sequence effects between two formulations in these parameters. The 90% confidence intervals for the log transformed data were within the acceptance range of log 0.8 to log 1.25 (e.g., log0.94 ~ log1.06 and log 0.90 - log 1.07 for AUCt and C_(max), respectively), indicating that Neuracetam tablet is bioequivalent to Neuromed tablet. The major pharmacokinetic parameters, AUCt, and C_(max), met the criteria set by KFDA for bioequivalence indicating that Neuracetam tablet is bioequivalent to Neuromed tablet.

        According as the current needs of low temperature in semiconductor manufacturing process, PECVD using low temperature and high deposition rate is becoming more of a concern. However, PECVD equipment has the defect possibility on the interface between substrates and gate oxide layer, because ions or electrons with much high energy clash the interface. In this study, we embarked on the experiment with Statistical Design of Experiment and then also analyze which parameters influence on the characteristic of silicon dioxide filems. We finally made a constructive proposal for process optimization conditions.

      • 질병의 활동도 지표로써 머리카락 중 미량의 구리와 아연의 분석

        The copper and zinc in scalp hair are determined by the ICP-AES and the relations between the trace elements concentrations with disease activity for the rheumatoid arthritis are investigated. The concentrations of the metals are varied depending upon the distance from the skin. The concentrations at the distal are higher than that of the proximal. Zinc concentration of the rheumatoid arthritis patients is higher than that of the osteroarthritis ones, and the confidence level that the group are different lies 95-99%. However, copper concentration for rheumatoid arthritis patients is lower and the two groups are different with a 50-80% confidence level.
