        침습성 아스페르길수스증 진단을 위항 갈락토만난 항원검사의 유용성 평가

        Background : Invasive aspergillosis (IA) is an important cause of morbidity and mortality among immunocompromised patients. However, 1A is difficult to diagnose, especially in the aforementioned patient group. Recently, galactomannan assay (GMA) using enzyme immunoassay has been introduced in Korea. We evaluated its diagnostic usefulness in the diagnosis of 1A and we analyzed the results according to the underlying diseases. Materials and Methods : All patients who underwent GMA during the period from October 2007 to June 2008 were evaluated retrospectively. According to the criteria of European Organization for Research and Treatment of Cancer/Mycoses Study Group, IA was classified into four clinical categories: proven', probable', possible', and 'non' IA. Patients with 'proven' and 'probable' IA were used as the reference standards for IA. GMA was performed using Platelia Aspergillus EIA (Bio-Rad, Hercules, CA, USA). Optical density index ≥0.5 was considered positive. Results : Of the 144 patients who underwent GMA, two patients were classificed as proven' IA and sixteen patients were probable' 1A. Overall sensitivity, specificity, positive predictive value, and negative predictive value of the assay were 50% (95% confidence interval [C195), 26-74%), 88% (C195, 81-93%), 38% (CI95, 19-59%), and 93% (CI95, 86-97%), respectively. Among the 99 patients with underlying hematologic diseases, GMA showed 67% (C195, 35-90%) sensitivity and 89% (C195, 80-94%) specificity, whereas in 45 patients with underlying diseases other than hematologic diseases, sensitivity and specificity of the assay were 17% (CI95, 0-64%) and 87% (CI95, 73-96%), respectively. Conclusions : GMA showed high specificity, irrespective of the patient population. However, sensitivity of GMA was low and the assay was less sensitive in patients with underlying diseases other than hematologic diseases than in patients with hematologic diseases.

        Morin exerts cytoprotective effects against oxidative stress in C2C12 myoblasts via the upregulation of Nrf2-dependent HO-1 expression and the activation of the ERK pathway

        <P>In the present study, we investigated the cytoprotective efficacy of morin, a natural flavonoid, against oxidative stress and elucidated the underlying mechanisms in C2C12 myoblasts. Our results indicated that morin treatment prior to hydrogen peroxide (H2O2) exposure significantly increased cell viability and prevented the generation of reactive oxygen species. H2O2-induced comet-like DNA formation and gamma H2AX phosphorylation were also markedly suppressed by morin with a parallel inhibition of apoptosis in C2C12 myoblasts, suggesting that morin prevented H2O2-induced cellular DNA damage. Furthermore, morin markedly enhanced the expression of heme oxygenase-1 (HO-1) associated with the induction and phosphorylation of nuclear factor-erythroid 2-related factor 2 (Nrf2) and the inhibition of Kelch-like ECH-associated protein 1 (Keapl) expression. Notably, these events were eliminated by transient transfection with Nrf2-specific small interfering RNA. Additional experiments demonstrated that the activation of the Nrf2/HO-1 pathway by morin was mediated by the extracellular signal-regulated kinase (ERK) signaling cascade. This phenomenon was confirmed with suppressed Nrf2 phosphorylation and consequently diminished HO-1 expression in cells treated with a pharmacological inhibitor of ERK. Collectively, these results demonstrated that morin augments the cellular antioxidant defense capacity through the activation of Nrf2/HO-1 signaling, which involves the activation of the ERK pathway, thereby protecting C2C12 myoblasts from H2O2,-induced oxidative cytotoxicity.</P>

        Elevational Distribution of Breeding Bird Communities in Seoraksan National Park, Korea

        In this study, the elevational distribution of breeding birds in Seoraksan National Park, Korea was investigated. Forty-six species of birds were documented from line transect surveys taken from Seorakdong at 230 m above sea level (a.s.l.) to the Daechungbong summit at 1708 m asl. Birdspecies richness and diversity were highest in Seorakdong and lowest at the Daechungbong summit. As elevation increased, bird species richness and diversity decreased, with a humped-shape trend being observed between 700 m and 1200 m a.s.l. Stepwise analyses revealed that breeding bird species diversity indices were significantly negatively correlated with elevation (r<sup>2</sup> = 0.327, P < 0.001) and positively correlated with vegetational coverage (r<sup>2</sup> = 0.324, P = 0.046). Higher elevations supported fewer birds than low and intermediate elevations. Projections at the local scale, including data on behavior and habitat use by birds, will be necessary for optimal conservation and management of the bird communities in Seoraksan National Park.

No effective antiviral therapy is proven yet, but clinical use of ribavirin (RBV) has been tried. We investigated the antiviral effect of RBV against SFTSV in vitro. Methods: To test for cytotoxicity of RBV, Vero cells were treated with different concentrations of RBV (3.90 to 500 μg/mL, two-fold dilution) and analyzed by cell viability MTS assay 48h post-infection. To determine antiviral activity of RBV against SFTSV, Vero cells were infected with SFTSV strain Gangwon/Korea/2012 at 100 TCID50 (50% tissue culture infective dose) per well in a 96-well plate, and RBV was added at the concentrations showing no or minimal cytotoxicity. Viral RNAs were extracted from the culture supernatants and quantifi ed using one-step real-time reverse transcription- PCR to amplify the partial large segment of SFTSV. Statistical analysis was done by one-way ANOVA with Tukey`s post hoc test. Results: Cytotoxicity due to RBV was not observed at RBV concentration =31.3 μg/ mL. Viral RNAs at 24h post-RBV treatment were reduced with increasing RBV concentrations (1-32 μg/mL), compared with those of mock-treated cells (P <0.01, Figure). Half maximal inhibitory concentration (IC50) of RBV was 3.69 μg/mL at 24h post-RBV treatment. Conclusions: Our study shows that RBV has antiviral effect against SFTSV in a dose-dependent manner. Further studies are required to evaluate the effi cacy of RBV in SFTS.

        정규 뇌파검사에서 볼수 있는 자연수면

        The purpose of this study were to investigate the patterns of sleep appearances and to find out conditions for easier occurrence of natural sleep during routine EEG examination. The subjects of this study were randomly selected 200 patients who had undergone EEG examinations at Soonchunhyang University Hospital EEG laboratory. The results were as follows ; 1) Half of the cases(50%) showed sleep EEG patterns and 56% of them(56 cases) showed sleep stage Ⅱ patterns. 2) Sleep latency of those who reached only sleep Ⅰ was significantly longer than that of those who reached sleep stage Ⅱ. 3) Sleep was induced within the first 15 minutes of EEG records in 86.4% of all the sleepers. Afterwards, the sleep induction rate was abruptly decreased. 4) Ninety percent of the sleepers showed the EEG patterns of sleep stage Ⅱ within the first 15 minutes of sleep. 5) When the subjects were divided into two groups by the time of EEG recording, i.e, morning and afternoon groups, there was no difference in the rates of sleep induction and appearance of sleep stage Ⅱ. However, sleep latency was significantly shorter in the afternoon group. 6) EEG patterns of sleep stages 3 and stage 4 and REM sleep were not observed in any subjects studied.

        The plenty of availability and high precision of GPS CORS is the reason why it become important more and more in the fields of surveying widely. In this study. I extracted the arbitrary point's coordinate which is using GPS CORS data. now served in RINEX FORMAT via Inter-Net, with observation network of the existing triangulation and GPS CORS data. Then. with this arbitrary point as reference station. RTK GPS was performed. And I will study VRS-GPS concept which reduces the time and cost in the fields of surveying.

      • 등척성 운동 시 요추의 각도에 따른 중앙주파수와 토크의 특성

        Fatigue is the decline in force produced as a result of prolonged muscle activity. Localized muscle fatigue can be identified by a shift toward low in the frequency components of the EMG signal, typically represented by a fall in the median frequency. Previous studies show that a shortened muscle developes a higher fatigue than elongated muscles. The purpose of this study was to investigate the time-related change of median frequency and torque during maximal isometric back extension exercises at different exercise angles (0˚, 12˚, 36˚, 72˚). Twenty healthy subjects (mean age = 24.35 ± 2.70) were evaluated in this study. Median frequency was extracted from the EMG signals by fast Fourier transform (FFT). Initial median frequency and the slope of median frequency change over time were computed from linear regression analysis. Pearson's product moment correlation was used to quantify the relationship between scope of median frequency and torque. The results were as follows: 1) Significant differences in y-intercepts of torque regression equation with respect to exercise angle were shown. However, there were no differences in the slopes of the median frequency and torque, and y intercept of the median frequency among exercise angles. 2) There was no significant correlation between slope of median frequency and torque. 3) But there was moderate correlation between median frequency and torque at each exercise angle. In conclusion, the exercise angle during maximal isometric back extension exercise is not a direct effect on slope of median frequency and torque. But results showed that median frequency and torque shift were highly correlated in all subjects.

        3관능성 BPA-3MA를 기질로한 광중합형 치면열구전색재의 물성 연구

        The BPA-3MA(bisphenol-A trimethacrylate) in which one of two hydroxy group was substituted by methacrylate was synthesized by reaction of Bis-GMA with methacryloyl chloride. To investigate the possibility to apply BPA-3MA as matrix of composite resin or pit and fissure sealant, BPA-3MA(3MA) or Bis-GMA(GMA) as matrix, TEGDMA as diluent, CQ as photosensitizer, EDMAB as photoinitiator and 5㎛ pyrogenic silica as filler were used for making experimental sealants. In addition, these experimental sealants were compared to a commercial Concise L/C White sealant(CLW, 3M Co, U.S.A.) for flexural strength, diametral tensile strength, amount of abrasion, surface hardness, water sorption and solubility, and degree of conversion. The flexural strength of CLW was the highest value but there was not significant difference with 3MA. Diametral tensile strength of all 3 groups were similar(p>0.05) and the amounts of abrasion of 3MA and GMA were less than that of CLW(p<0.05). The surface hardness of 3MA was higher than that of GMA(p<0.05) but similar to that of CLW. The water sorption of 3MA was less than that of GMA and CLW(p<0.05) and the solubilities of 3MA and CLW were than that of GMA(p<0.05). GMA and CLW were relatively polymerized well as compared to 3MA. From these results, pit and fissure sealant using trifunctional BPA-3MA as matrix had superior physical and mechanical properties to existing Bis-GMA sealant. Furthermore, it will be possible to develop composite resin having superior properties to existing composite resin.

      • 구개부 소타액선에 발생한 호산성 과립세포종

        호산성 과립세포는 타액선을 포함하여 갑상선, 부갑상선 등 주로 선조직에서 발견되는 커다란 과립상의 호산성 상피세포를 말한다. 호산성 과립세포종은 모든 타액선 종양중 약 1.5%의 비율로 발생하는 비교적 드문 양성 종양이며, 주로 노년의 이하선에서 발생하고 악하선과 소타액선에서는 드물게 발생한다. 임상적으로 크기는 약 3-4 ㎝이며 잘 분화된 낭으로 싸여있다. 조직학적 검사에서 광학현미경상 호산성 과립세포로 채워진 세포질이 관찰되며 전자현미경상 과립을 함유한 다량의 비대된 미토콘드리아가 관찰된다. 호산성 과립세포종은 본질적으로는 양성 종양이며 치료를 위하여 완전한 절제가 추천된다. 국소적인 재발은 드물며 다발성인 경우에는 재발하기도 한다. 증례는 38세의 남자 환자로서 구개부의 소타액선에 발생한 약 2.0×2.5 ㎝ 크기의 무통성의 종괴로 절제술을 시행하였으며, 광학현미경과 전자현미경을 이용한 조직병리 검사상 호산성 과립세포종으로 진단되었으며, 수술 후 특별한 합병증 없이 치유되었다. 저자들은 구개부의 소타액선에서는 아주 드물게 발생하는 호산성 과립세포종에 대하여 진단 및 치료를 시행하고 양호한 결과를 얻었기에 문헌 고찰과 함께 보고하는 바이다. Oncocytes are large, granular, eosinophilic epithelial cells mainly found in many glandular tissue including salivary glands, the thyroid, parathyroid, pituitary, adrenal gland, testicle, fallopian tube, liver and stomach etc. Oncocytoma, which originate from oncocytes are uncommon benign neoplasm that present approximately 1.5% of all salivary gland tumors. They typically occur in parotid gland of elderly patients and rarely in submandibular gland or minor salivary gland. On gross examination, they are usually 3 to 4 ㎝ is size and possess a well-defined capsule. Histologically oncocytoma peculiarly consisted of oncocyte which has cytoplasm, filled with eosinophilic granules on light microscopy and a large number of enlarged and morphologically altered mitochondria by electron microscopy. Oncocytomas are essentially benign tumors and complete excision is recommended. Local recurrence is extremely rare, but when it occurs, it may be multifocal. We report a case of oncocytoma raised from minor salivary gland of palate in 38 years old male patient with literature.
