Purpose: To determine the efficacy of immediate pars plana vitrectomy as the primary treatment for acute endophthalmitis in patients with a visual acuity (VA) of hand motion (HM) or better. Methods: A total of 149 patients who were referred to a single center for acute endophthalmitis after cataract surgery over the 13-year study period were retrospectively analyzed. Only patients presenting with a VA of at least HM were included. Patients were initially treated with either primary vitrectomy or intravitreal antibiotic injection alone, and their visual outcomes and reintervention rates after initial treatment were compared. Results: There was no significant difference in the proportion of good (final VA ≥20 / 40) and poor (VA ≤ counting finger) visual outcomes between the groups. However, subgroup analysis of patients with a VA of HM (92 eyes) showed that the incidence of reintervention (14 of 72 eyes [19.4%] vs. 9 of 20 eyes [45.0%]) and poor visual outcomes (10 of 72 eyes [13.9%] vs. 8 of 20 eyes [40.0%]) were lower after prompt vitrectomy than after intravitreal antibiotic injection alone (p = 0.019 and p = 0.022, respectively). For those with a VA of at least counting finger, no significant difference was observed between the groups. Conclusions: For patients with endophthalmitis presenting with a VA of HM, performing a prompt vitrectomy reduced the incidence of reintervention and poor visual outcomes than the administration of intravitreal antibiotics alone. Our results suggest that primary vitrectomy for patients with endophthalmitis presenting with a VA of HM could be more beneficial than intravitreal antibiotic injection alone.

(Background and Purpose) Museum is transforming into a complex cultural space that elevates the value of the experience, beyond simply providing information. Accordingly, specialized museums showcase various cultures and must apply effective directing media that allow visitors to enjoy professional culture freely. The Korea National Arboretum Forest Museum provides biological resource services encompassing the history, culture, and environmental ecology of forests. However, aging facilities and a one-dimensional permanent exhibition environment pose challenges in delivering in-depth information and providing the experience of forest culture. This study aims to derive the direction of the Forest Museum’s exhibition media production to convey forest-related knowledge to visitors in an interesting way. (Method) This study investigated and classified the types of exhibition media and media in the spatial direction of major museums by examining prior research and conducting literature reviews, and identified the current status of exhibitions, collections, and exhibition topics in the Forest Museum. Based on this investigation, we selected national and public institutions with similar exhibition themes and identified the characteristics of exhibition directions and media by subject. This study also sets the path for the Forest Museum’s exhibition media based on the analysis. (Results) The results derived from the analysis of target institutions in the fields of history, environmental ecology, and folk culture were as follows: First, the field of history must create a unique interactive video space wherein dynamic images and dioramas are combined with various media technologies to enhance the audience’s intuitive understanding. Second, environmental and ecological fields should introduce augmented reality (AR) technology for audiences to indirectly experience inaccessible areas and gain a deep understanding of the environment and ecology. Third, the folk culture field should enhance the audience’s understanding by using current collections and various video media, and improve the immersive environment through experience with the five senses. (Conclusion) By analyzing nine selected national and public institutions, the direction of the Forest Museum’s exhibition media production to promote the enjoyment of forest culture was derived. This study confirms that exhibitions should be planned in different fields, with the relevant media and design adopted to convey forest culture in depth, wherein multiple factors―such as history, environmental and ecological factors, and folk culture―are complexly intertwined. For the Forest Museum to be transformed into a center for enjoying forest culture in the future, a space composition plan that considers exhibition production media should be implemented when remodeling exhibitions.

Background: Recently, mutation-specific antibodies have been developed for detecting epidermal growth factor receptor (EGFR) mutations by immunohistochemistry (IHC). This study was designed to determine the significance of the type of specimen used for analysis (biopsy vs. resection), and to evaluate any correlation between mutation-specific antibodies and total EGFR protein (tEGFR) expressions. Methods: IHC using mutation-specific antibodies for E746-A750 deletion (DEL) and L858R point mutation (L858R) was performed in biopsies or tissue microarrays (TMA) of resected tumors from a total of 154 patients with lung adenocarcinoma. tEGFR was also investigated by IHC. Results were then compared with DNA sequencing data. Results: Molecular-based assays detected EGFR mutations in 62 (40.3%) patients, including 14 (9.1%) with DEL, and 31 (20.1%) with L858R. IHC with two mutation-specific antibodies demonstrated a homogeneous staining pattern, and showed overall (biopsy/resection) sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of 75.6% (78.3%/72.7%), 94.5 % (90.9%/96.3%), 85% (78.3%/88.9%), and 90.4% (90.9%/89.7%), respectively. tEGFR showed low sensitivity, specificity, PPV, and NPV, with no correlation with either DEL-specific or L858R-specific antibodies. Conclusions: IHC using EGFR mutation-specific antibodies proved to be a reliable screening tool for identifying EGFR mutations in both biopsied and resected specimens with pulmonary adenocarcinoma, especially when the tumor cells in biopsy samples are not sufficient for molecular biology techniques.

Recently, there have been emerging research activities in classifying the Sasang constitution objectively by machine-based methods. The concordance rate of the classification by the pulse wave features was, however, only around 50% up to now. In this paper, we introduce a novel classification algorithm that can promote the accuracy substantially at the expense of the non-classifiable subgroup as a byproduct. For instance, with the pulse wave features alone, we show that female/male subject group in their 20s can be classified into the Sasang constitution group with the concordance rate of 68.4%/65.5% for a subgroup of 57/29(31%/15%) subjects out of 184/195, by leaving the other subjects as the non-classifiable group. Next, we show that the pulse diagnosis has been used only as a supportive tool in determining one's constitution, and consequently the accuracy of the concordance ratio by the pulse wave features alone cannot exceed a finite value, which we estimate to be about 60%.

First Record of the Biting Midges, genus Atrichopogon (Diptera: Ceratopogonidae) from Korea

Recently in the course of examining Korean diptera, we found a species, Atrichopogon femoralis Tokunaga placed in the subfamily Forcipomyinae Meigen. Forcipomyinae is most specious in the family Ceratogonidae, and comprise about 5,566 extant species placed in 103 genera in four subfamilies (Borkent and Wirth, 1997) from almost all the zoogeographic regions. Genus Atrichopogon is second largest genus, with about 520 species (Borkent, 2012). Currently 39 valid species of 6 genera belonging to 3 subfamily are known in Korea. The specimens were collected from intermountain area and reservoir of Chungcheongnam-do, Cheonan-si, and Gyeongsangbuk-do, Gyeongju-si by black light trap (BLT) in 2013. We report an occurrence Atrichopogon femoralis Tokunaga for the first time in Korea, and of which diagnosis, description, distribution and photo images are presented. For further researches, we also provide a key to genera of Korean Ceratopogonidae.
