        중환자실 전문간호사의 전문간호행위 분류와 수행분석

        Purpose: The purpose of this study was to reclassify the advanced nursing practices of critical care nurse practitioners(CCNPs) in intensive care unit and measure the time and frequency of CCNP's activities. Method: Practices of ICU nurses are divided into RN's and CCNP's practices by a panel of ICU nursing experts. Each practice of CCNP is defined and CCNP's working time and service frequencies are monitored in general hospitals. Result: Practices of CCNP were classified into 4 domains and 32 practices. Fourteen practices by CCNPs were completed in 10 minutes and the other 12 practices consumed 10-30 minutes. A priority of practice in respiratory therapy was given to artificial airway management, management of tracheostomy patient, lower respiratory care, and the priority of CRRT was management of anticoagulation. Conclusions: Advanced nursing practices of CCNPs were recognized from those of RNs. A further research of CCNPs practices should be extended to other advanced practices and it is required to evaluate economic value of advanced nursing practice in the national health insurance system.

        저체중 또는 과체중 청소년에서 캠프 프로그램을 이용한 영양 교육효과에 관한 연구

        This study was performed to evaluate the effectiveness of a nutrition education program developed for nutritionally imbalanced adolescents. A summer nutrition camp was held for 23 overweight and 16 underweight subjects. Its effectiveness with regard to was evaluated at the end of the camp and 6 months later. Nutrition knowledge, nutritional attitude, food behavior, nutrient intake, exercise habit and ideal body figures desired by the subjects. The results showed significantly higher nutrition knowledge scores at the end of the camp as compared to those obtained prior to the camp program, and these scores were maintained for at least six months. Nutrition attitude scores also improved after the education program, and these improved scores also lasted for 6 months. However, the food behavior scores measured 6 months after the education program were not significantly different from those obtained prior to the camp. Also, the exercise habit, the ideal body figures and the body figures desired by the subjects remained unchanged. When nutrient intakes of subjects were assessed before the program and 6 months later, the mean daily vitamin C intake was significantly increased after the education program. Also, the intake of iron from plant food sources increased in the overweight subjects, while less iron from animal source were consumed by the underweight subjects. Both groups tended to consume more vegetables and fruits 6 months after the education program which may have contribute to the higher vitamin C and plant-based iron intakes. These results indicate that a 4-day nutrition education camp program sustained changes in nutrition knowledge and nutrition attitude for 6 months. The increased intake of vegetables and fruits was also achieved through this education program. However, changes in dietary behavior in adolescents may require repeated education.

        야뇨증 환아들의 심리사회적 특성에 대한 다기관 연구 : 행동 및 정서 문제를 중심으로

        Objectives : The aim of this study was to examine the behavioral and emotional problems associated with nocturnal enuresis in Korean children. Methods : Three hundred eighteen children with nocturnal enuresis, together with their parents, completed the Child Behavior Checklist (CBCL), Disruptive Behavior Disorder Scale according to DSM-IV (DBDS), Children's Depression Inventory (CDI), State-Trait Anxiety Inventory for Children (STAIC), and Piers-Harris Children's Self-Concept Scale (PHCSC). Ninety-three normal students were selected as the control group. Results : Compared to the normal control group, the mean scores with regard to the withdrawn, social problems, attention problems, delinquent behavior, aggressive behavior, externalizing problems and total problems profiles were significantly higher in the nocturnal enuresis group according to the CBCL results. The nocturnal enuresis group also scored significantly higher in the ADHD and ODD profiles of the DBDS. The nocturnal enuresis group was more depressed and anxious than the control group according to the results of the CDI and STAI. The mean score of the PHCSC was significantly lower in the nocturnal enuresis group when compared to the normal control group. Conclusion : The results of this study suggest that children with nocturnal enuresis in Korea have clinically relevant behavioral and emotional problems. The findings support the link between nocturnal enuresis and psychopathology in Korean children.

      • 단일 기관에서 경험한 소아청소년기 Kikuchi-Fujimoto병의 임상적 고찰

        Objectives: Kikuchi-Fujimoto disease (KFD) is characterized by lymphadenopathy and fever, and is usually self-limited. This study analyzed the clinical characteristics of pediatric patients with KFD. Methods: This retrospective, observational, single-center study was conducted in South Korea from March 2008 to October 2015. KFD was diagnosed based on clinical, radiological or histological findings and excluded when there were any other causes of lymphadenopathy. Medical records were reviewed for clinical and laboratory manifestations. Results: A total of 35 cases were included. The mean patient age was 12.1±2.9 years (range, 5 to 17 years); the male-to-female ratio was 1:0.8. The main clinical manifestations were cervical lymphadenopathy and fever in 34 cases (97%). The mean duration of fever was 12.2±8.3 days (range, 2 to 37 days). We noted enlargement of lymph nodes in the cervical, mesenteric (n=5, 14%), axillary (n=2, 6%), and inguinal (n=1, 3%) regions. Hepatosplenomegaly, loss of appetite, and rash were observed. On laboratory examinations, elevation of ferritin, leukopenia, and positivity for anti-nuclear antibodies were frequently observed. Twelve patients underwent biopsy and 23 cases were diagnosed by radiological findings. The mean duration of hospitalization for all cases was 7.9±2.9 days (range, 3 to 13 days) and steroids were administered in 10 cases. KFD recurrence was observed in 2 cases (5.7%) with the time to relapse of 7 months and 4 years. There were no cases with systemic lupus erythematous or other autoimmune disease. Conclusion: KFD should be considered in pediatric patients with lymphadenopathy and prolonged fever. Patients with KFD should be monitored for recurrence and the development of autoimmune disease.

        지식기반사회의 학교 교육과정 개발을 위한 7차 초등 국어 교과서 분석

        이 연구는 지식기반 사회에서의 학교 교육과정 개발을 위한 선행 연구로서, 국가 수준에서 개발된 7차 초등 국어와 교육과정이 초등 국어 교과서로 '어떻게', '얼마나' 잘 녹아들어 있는가를 분석하는 데 목적이 있다.특히, 학습자 중심 교수-학습의 일환으로, 현 7차 교육과정이 새롭게 시도한 수준별교육과정이 실제 초등 교과서에 어떻게 구현되고 있는가를 살펴보았다.이를 위해 먼저, 국가 고시 교육과정 문서의 교육 목표, 교육 내용, 교육 방법, 교육 평가 부분을 살펴보고, 이에 근거하여 교과서에의 반영 정도를 분석하였다.교과서 분석은 크게 '구성과 제시형식 분석'과 '내용 분석'으로 나뉘고, 내용 분석은 다시 목표, 내용, 방법, 평가로 나뉘어 전개되었다.분석결과, 교육과정 문서상의 목표와 내용은 교과서로 번역되는 과정에서 단편적이고 분절된 활동이나 사례들로 제시되고 있어, 교육과정과 교과서간의 연계성에 더 주의를 기울일 필요가 있음이 드러났다.방법적인 측면에 있어서도 교육과정 문서가 표방한 '자기주도적 학습'에 부합되지 않는 방식으로 교과서가 전개되는 경향이있고, 평가 부분에 대한 반영도 미흡한 것으로 나타났다.또한 7차 교육과정이 의도한 심화ㆍ보충형 수준별 교육과정은 외견상으로 교과서에 반영된 듯 보이나 실질적으로는 개인차를 반영한 것이라고 평가하기 어려운 것으로 나타났다. The purpose of this study was to analyze the Koream Language Arts Textbooks for the 3rd graders developed in accordance with the 7th national curriculum.The analysis was proceeded focused on the following.(1) whether the consistency between the textbooks and the national curriculum exits in terms of goals and objectives, content, learning activities, and evaluation; (2) whether the textbooks are relevantly reflected the idea of the differentiated curriculum newly adopted in the 7th national amended curriculum. The major findings were as follows.(1) The textbooks for speaking/listening, reading, and writing were separately developed.Considering the goals and objectives of the Korean Language Arts curriculum such as creative ability, the relevance of separating the textbooks need to be discussed in the future amendation for the national curriculum.(2) The idea of the differentiated curriculum was reflected on the organization of the textbooks in a superficial way.The idea was reflected in ways of piecing the contents into pieces and have the students repeat the same pieces.

      • 발달장애학생을 위한 웹기반 의사소통능력신장 프로그램의 개발과 적용 사례

        발달장애학생의 의사소통능력은 단순한 언어문제 이상의 의미를 지닌다. 의사소통능력은 모든 학습의 기초가 되고 결국 인간의 삶의 질을 좌우하게 하는 중요한 요소가 되고 있기 때문에 의사소통능력에 문제가 많은 발달장애학생에게 있어서 의사소통능력 신장은 매우 중요하다. 발달장애학생의 언어 및 의사소통능력의 결함은 사회적 결함과 인지적 결함에 상호 관련성이 있어 다른 사람으로부터의 의사소통을 이해하는 능력과 자신의 생각을 표현하는 능력이 제한되어 다른 사람과의 상호관계를 맺는데 어려움이 많았다. 따라서 발달장애학생의 언어지도는 생활 장면에서 교수하는 것이 가장 바람직하다고 하겠다. 그러나 지금까지 주로 활용된 전통적인 언어지도 방법은 어휘를 증가시키고, 발화의 길이를 널리는 데는 효과가 있었지만, 학습된 내용을 생활 장면에서의 일반화에는 문제점이 있는 것으로 지적되고 있다. 발달장애학생의 언어지도 방법은 일상생활 속에서 학습 내용을 선정하고 이를 실생활 장면 혹은 이와 유사한 환경에서 효과적으로 지도할 수 있는 적절한 교수 방법이 필요하다. 그래서 발달장애학생의 의사소통능력 신장을 위한 환경중심 언어지도 방법을 활용한 웹(Web)기반 언어지도 프로그램을 개발하였다. 웹(Web)기반 언어지도 프로그램은 제7차 특수학교 기본교육과정과 국립특수교육원에서 발간한 ‘발달장애학생의 언어교육 프로그램(1998)’ 분석을 통하여 개인 · 가정생활, 학교 · 사회생활, 경제 · 여가생활 등의 6개 영역으로 구분하여 각 영역별 학습주제를 선정하였으며, 학습주제에 대한 학습의 진행 형태는 학습목표, 낱말학습, 상황학습, 평가로 진행할 수 있도록 하였으나, 학습자의 현재 수행 능력을 고려하여 능력에 따른 진행의 절차를 학습자가 선택할 수 있게 하였다. 본 프로그램을 적용해본 결과 다음과 같은 교육적인 효과를 얻을 수 있었다. 첫째, 웹을 활용한 가상체험을 통하여 적극적이고 흥미 있는 참여를 유도하며, 학습자 중심의 주도적인 학습과 안내자, 촉진자로서의 교사의 역할을 통해 발달장애학생의 의사소통 능력을 신장시킬 수 있었다. 둘째, 교실에서 이루어지는 지금까지의 수업방식을 탈피하여 학습자가 언제, 어디서나, 누구든지 학습할 수 있는 열린 학습체제의 교육환경을 조성할 수 있었다. 셋째, 문자정보, 그래픽, 애니메이션, 사진, 음성, 비디오 등 다양한 형태의 정보가 상호대화 형식으로 활용되며, 수준별 반복학습을 실시함으로써 교수-학습 방법의 개선뿐만 아니라 양질의 수업을 진행하는데 도움을 줄 수 있었다. 본 프로그램을 활용한 언어지도가 발달장애학생의 의사소통능력 신장을 위한 최선의 방법이라고 말할 수는 없으며, 언어지도의 한 가지 방법이라고 말할 수 있다. 따라서 본 프로그램의 활용한 언어지도는 학교에서 교사가 학생의 능력과 흥미를 고려하여 지도한다면 발달장애 학생의 의사소통능력 신장에 많은 도움이 될 것이다. The developmental disability of the metal handicapped children means more than simple linguistic problem. The developmental disability is the base of all the learning and important factor which decides the happiness of the human being finally, so the extension of mutual understanding ability is so important, in the case of the metal handicapped children who have a problem of the mutual understanding ability. In the case of the metal handicapped children, the defects of the mutual understanding and linguistic ability are related with the social defect and the defect of recognition, and the ability of understanding about mutual relations with other people and self-expression ability are limited. So they have many problems in mutual relations with other people. Therefore, the linguistic instruction of the metal handicapped children is recommended to be done in the living. But, the traditional linguistic instruction which's used mainly as yet, is effective in increase of a vocabulary and the length of a locutionary act, but it has a problem in the popularization of the learning in the real life. The method of the linguistic instruction for the metal handicapped children, should be selected as contents in the real life, and the right learning method which can be done under the real life or the similar environment with it, is needed. Because of the above realities, the linguistic teaching web-based program is developed, using the linguistic instruction which is focused on the environment, for the extension of the mutual understanding ability of the metal handicapped children. In the linguistic teaching web-based program, it is divided with 6 field (personal · home life, school · social life, economical · leisure life) and the educational subject classified by each field is selected, by the analysis of 'The linguistic educational program for the children who have developmental disability' published by the 7th basic education course of special school and the national special education organization. Besides, the progress form of learning about the educational subject is directed by educational goal, word learning, situation learning and valuation, but a learner can choose a procedure followed by ability, regarding to his accomplishment ability. As the result of the application of this program, the educational effects that we could get are the following : 1. We could lead a positive and interesting participation by virtual experience through WEB, and the communication ability of the metal handicapped children was extended by a role of a teacher, as a guide and promoter and a leading learning focused on a learner. 2. The teaching method which is done in school now, is dismissed and we could form the educational environment of the learning system that a learner could learn, without distinction of time, a place and a person. 3. The various form of information, like text, graphic, animation, photo, sound, video and so on, is applied as a form of communication. Therefore, the program was useful in not only the improvement of the teaching-learning method, but also the proceeding of the effective lesson. The linguistic teaching which use this program, can not be the best method of the extension of the mutual understanding ability and it's just one method of linguistic teaching. if the linguistic teaching which use this program is conducted, regarding to a ability and a interest of a student, it will be so useful in the extension of the mutual understanding ability, for the children who have developmental disability.

      • 암환자의 건강문제 분류를 위한 횡단적 조사연구 : 서울시내 일개 종합병원 중심으로

        The impact of cancer on human life has been recognized and the demand for care for the patient with cancer is ever increasing. Yet very little study around these concerns has been reported. The subjects of this study were 388 patients with cancer who were receiving treatment .in the outpatient department of Yonsei University Cancer Center. The study dealt with the health problems experienced while receiving this treatment. The research method used in this study was the structured interview technique using a questionnaire. Data collection was done between Dec. 1. l986 and June 30. 1987. The questionnaire used in this study was a revision of three questionnaires, one developed by D.W. McCaffrey(coping with cancer), one developed by C.A. Morris(self concept as altered by the diagnosis) and one developed by M.J. Yasko(management of symptoms associated with chemotherapy). Content validity was treated with oncologists and oneology nurses & nursing faculty members. The results and recommendation of the study areas follows: 1. Results. 1) Characteristics of the Sample The diagnoses of the subjects were as follow: The most frequent diagnosis was cancer of the stomach(26.5%), followed by breast cancer, lymphoma, rectal / colon cancer and lung cancer. The average age was 48.6 years. There were an equal number of men and women. There were 116(30.4%) subjects who received chemotherapy and surgery for the treatment of their disease of the total number of subjects. 264(69.5%) were treated for less than 1 year, and 116(30.5%) were treated for more than 1 year. The average Length of treatment was 12.7 months. 2) In Summary Health problems were categorized under four areas: physical. emotional, social and economical. The patients with lymphoma and stomach cancer experienced more health problems than any others in this study. According to the age of the patients, those in their 20's complained of physical and emotional problems most frequently and of those in their 30's social-economical concerns were most evident. In female gender. physical and emotional problems were identified more frequently than male patients. And yet male patients indicated greater concern with economic problems than female patients. Regardless of the treatment method or length of treatment health problems in different areas were similar and statistically not significant. However, economic concern was statistically significant(F = 2.52. p< 0.5) in those with receiving a combination of chemotherapy and radiation Marital status and health problems were also statistically significant. That is patients who were divorced, separated or widowed had more physical, psychological and economical problems than those who were married. 2. Recommendations. 1) Since the physical symptoms of nausea / muscle weakness and malnutrition were major health problems, the patient's diet and activity need to be carefully detailed and included in the care plan and further research is needed in order to implement physical, psychological. economical & spiritual interventions. 2) A follow up study should be done using a larger number of subjects and indifferent settings to further validate the health problem checklist for cancer patients.
