        Early Recognition of Pediatric Strokes in the Emergency Department: Epidemiology, Clinical Presentation, and Factors Impeding Stroke Diagnosis in Children

        Purpose: Strokes are challenging to diagnose in pediatric emergency departments (EDs) as level of suspicion is low and atypical presentations are common. We analyzed clinical features, epidemiology and factors of delayed identification in arterial ischemic strokes (AIS) and hemorrhagic strokes (HS). Methods: Single-center retrospective cohort study of children aged between 29 days and 18 years old diagnosed with stroke between July 2016 to June 2021. Results: Among 36 children, 11 (30.5%) had AIS, 25 (69.4%) had HS. Median age for AIS was 9 years (interquartile range [IQR], 2 to 9) and HS 9 years (IQR, 1 to 11.5) (P=0.715). Focal neurological deficit was seen in 72.7% of AIS and 20% of HS (P=0.006). Only 18.2% of AIS and 52.0% of HS presented within 6 hours of symptoms. Median time from symptom onset to ED presentation was 24 hours (IQR, 12 to 28) for AIS and 7 hours (IQR, 1.8 to 48) for HS (P=0.595). Most (85.6%) arrived by own transport. Median time from presentation to neuroimaging was 7 hours (IQR, 0.9 to 7) for AIS and 4.8 (IQR, 1.3 to 16.8) hours for HS (P=0.376). Eleven patients, 9/25 (36.0%) HS and 2/11 (18.2%) AIS, did not have stroke as differential diagnosis at ED (P=0.714). Common initial diagnoses were viral illness or headaches. On univariate analysis, age <1 (odds ratio [OR], 17.5; 95% confidence interval [CI], 1.2 to 250.4; P=0.035) and absence of focal neurological deficit (OR, 13.091; 95% CI, 1.5 to 117.9; P=0.022) were significant factors for delayed identification. Conclusion: Index of suspicion for pediatric strokes among caregivers and clinicians should be increased. Public awareness campaigns are recommended.

        N6-Methyladenosine modification (m6 A) of circRNA-ZNF638 contributes to the induced activation of SHF stem cells through miR-361-5p/Wnt5a axis in cashmere goats

        Objective: The objective of this study was to investigate the effects of N6-Methyladenosine modification-circRNA-zinc finger protein 638 (m6A-circRNA-ZNF638) on the induced activation of secondary hair follicle (SHF) stem cells with its potential mechanisms in cashmere goats. Methods: The m6A modification of ZNF638 was analyzed using methylation immunoprecipitation with real-time quantitative polymerase chain reaction technique in SHF stem cells. The effects of circRNA-ZNF638 on the induced activation of SHF stem cells in m6A dependence were evaluated through the overexpression of circRNA-ZNF638/its m6Adeficient mutants in circRNA-ZNF638 knockdown SHF stem cells. The competitive binding of miR-361-5p to circRNA-ZNF638/Wnt5a 3′- untranslated region was analyzed through Dual-luciferase reporter assay. Results: The m6A-circRNA-ZNF638 had significantly higher transcription at anagen SHF bulge of cashmere goats compared with that at telogen, as well as it positively regulated the induced activation of SHF-stem cells in cashmere goats. Mechanismly, m6A-circRNA-ZNF638 sponged miR-361-5p to heighten the transcriptional expression of Wnt5a gene in SHFstem cells. We further demonstrated that the internal m6A modification within circRNAZNF638 is required for mediating the miR-361-5p/Wnt5a pathway to regulate the induced activation of SHF stem cells through an introducing of m6A-deficient mutant of circRNAZNF638. Conclusion: The circRNA-ZNF638 contributes the proper induced activation of SHF-stem cells in cashmere goats in m6A-dependent manner through miR-361-5p/Wnt5a axis.

        Numerical Modeling of Creep Degradation of Natural Soft Clays under One-dimensional Condition

        Creep degradation is a common phenomenon of natural soft clays. This paper focuses on developing a one-dimensional constitutive model considering the influence of bond degradation on the creep behavior for natural soft clays. First, conventional oedometer creep tests are studied and a creep based structure indicator ϖ denoting the difference between the creep coefficient of the reconstituted samples and that of intact samples is proposed. Then, the creep coefficient of intact clay is formulated by this indicator and the intrinsic creep coefficient corresponding to reconstituted clay. This formula is then incorporated into a one-dimensional creep model to describe the creep degradation behavior induced by bond degradation. The model parameters can be determined in a straightforward way from oedometer tests which leads to an easy application of the model for practice. Furthermore, coupled with the consolidation theory, the model is used to simulate oedometer tests at constant strain rate as well as long-term creep tests on sensitive Batiscan clay. The destructuration effects on the evolution of creep coefficient are analyzed. The comparisons between experimental and numerical results show that the proposed model can precisely describe the creep degradation behavior induced by destructuration of natural soft clay under one-dimensional loading.

      • A Method of Pseudo 3D Video Reconstruction Based on 2D Video Sequences

        A method of pseudo 3D video reconstruction based on 2D video sequences is proposed, which transforms the conventional 2D video into their stereo version using original image and depth-map image. The red-component map is shifted left to obtain the left-view according to the parallax information. Because the left image has holes, the depth-map preprocessing and novel hole-filling simplified algorithm are used to fill the holes effectively. The experimental results show that the proposed algorithm generates 3D visual view with the same image quality as well as faster rendering speed compared with the reference hole-filling algorithm. The method adapts to the stereo pair synthesis for 3D.

        Clinical and Prognostic Significance of O<sup>6</sup>-Methylguanine-DNA Methyltransferase Promoter Methylation in Patients with Melanoma: A Systematic Meta-Analysis

        Tumor suppressor gene O<sup>6</sup>-methylguanine-DNA methyltransferase (MGMT) promoter methylation has been reported in melanoma. However, the clinical and prognostic significance of MGMT promoter methylation in patients with melanoma remained to be determined. A systematic search was performed to identify eligible papers published. The overall odds ratios (ORs) or hazard ratios and their 95% confidence intervals were calculated. Final 12 eligible publications involving Caucasian population were performed in this study, including 1,071 metastatic melanoma patients, 154 primary melanoma patients, and 211 normal controls. MGMT promoter methylation was significantly higher in primary or metastatic melanoma than in normal controls (p<0.05). No difference of MGMT promoter methylation was found in primary and metastatic melanoma (p=0.432). When metastatic melanoma was compared to normal controls, subgroup analysis showed the correlation between MGMT promoter methylation and different sample materials (tissue: OR=7.01, p<0.001 and blood: OR=12.04, p=0.005). MGMT promoter methylation was not associated with response to drug therapy and the prognosis in overall survival and progression-free survival for multivariate analysis. Our results show that MGMT promoter methylation may be correlated with the increased risk of primary or metastatic melanoma. Based on blood samples, MGMT promoter methylation may become a noninvasive biomarker for the detection of metastatic melanoma. Further additional clinical studies are necessary.         爲 “文學道路” 正名

        "The road of literature" has recently become a pejorative term. Behind such a phenomenon lurks a prevalent assumption: that English education is a matter of ingesting information, of mastering technique, of acquiring facts and know-how, whereas literature is a soft option, an indulgence or a mere trimming to decorate the hard center of the market-oriented syllabus. In order to counter such a trend, we need first of all to address the following theoretical issues: What is the primary task of an English department in China? Is the teaching of English literature relevant to the Chinese market economy today? If yes, in what way is it relevant? How does one persuade a member of modern Chinese society that there is a profit in reading, say, Chaucer and Shakespeare in harkening to any non-Chinese and, "worse", non-modern non-Chinese voices? While re-examining those questions, we need to improve some of our teaching methods that are still prevalent today, in order to make sure that literature is given its due status in English teaching programmes.

        Dynamic File Grouping for Load Balancing in Streaming Media Clustered Server Systems

        A dynamic file grouping strategy is presented to address the load balancing problem in streaming media clustered server systems. This strategy increases the server cluster availability by balancing the workloads among the servers within a cluster. Additionally, it improves the access hit ratio of cached files in delivery servers to alleviate the limitation of I/O bandwidth of storage node. First, the load balancing problem is formulated as a two layers semi-Markov switching state-space control process. This analytic model captures the behaviors of streaming media clustered server systems accurately, and is with constructional flexibility and scalability. Then, a policy iteration based reinforcement learning algorithm is proposed to optimize the file grouping policy online. By utilizing the features of the event-driven policy, the proposed optimization algorithm is adaptive and with less computational cost. Simulation results demonstrate the effectiveness of the proposed approach.

        m6A-mediated lncRNA NEAT1 plays an oncogenic role in non-small cell lung cancer by upregulating the HMGA1 expression through binding miR-361-3p

        Background Lung cancer is the most common primary malignant tumor of the lung, and 85% of lung cancer is non-small cell lung cancer (NSCLC). The N6-methyladenosine (m6A) and long noncoding RNAs (lncRNAs) have been widely reported to participate in the development of non-small cell lung cancer. Objective However, the potential molecular mechanisms of m6A-regulated lncRNAs in NSCLC still need further investigation. Methods The expression levels and the role of lncRNA NEAT1 in NSCLC tissues or cells were measured by RT-qPCR, Western blot, cell counting kit 8 (CCK-8), flow cytometry assay. RNA immunoprecipitation (RIP) was used to measure the levels of m6A modification of NEAT1. Bioinformatics analysis and dual-luciferase reporter gene assay were detected the relationship between miR-361-3p and NEAT1/HMGA1. Mouse xenograft tumor models were established to confirm the effects of lncRNA NEAT1 in vivo. Results In this study, we verified whether m6A-modified lncRNA nuclear enriched abundant transcript 1 (NEAT1) is involved in NSCLC progression via miR-361-3p/HMGA1 axis. Firstly, we found that lncRNA NEAT1 was upregulated in NSCLC, and was associated with a poor survival in NSCLC patients. Methyltransferase like 3 (METTL3)-mediated m6A modification stabilized and upregulated NEAT1 expression. Next, function experiment indicated that depletion of METTL3 and NEAT1 induced cell apoptosis and inhibited cell proliferation, epithelial-mesenchymal transition (EMT). Likewise, in vivo experiments further supported the oncogenic role of NEAT1 in NSCLC. In addition, the molecular mechanism was uncovered in our study, and we found that lncRNA NEAT1 promoted the expression of high-mobility group AT-hook 1 (HMGA1) by sponging miR-361-3p and then promoted tumorigenesis of NSCLC. Conclusion In conclusion, our findings demonstrated that METTL3-mediated m6A modification accelerated NSCLC progression by regulating the NEAT1/miR-361-3p/HMGA1 axis, which provides important targets for the treatment of NSCLC.
