        Heat stroke is a life-threatening illness characterized by an elevated core body temperature that rises above 40% and central nervous system dysfunctions that results in delirium, convulsions, or coma. Heat stroke is subdivided into two forms, classic and exertional. Since strenuous exercise is one of the major exacerbating and precipitating factors, the incidence of exertional heat stroke is high among young adult, especially military personnel undergoing military training. Exertional heat stroke commonly causes rhabdomyolysis, lactic acidosis, acute renal failure, shock and pulmonary edema. We report a case of heat stroke with rhabdomyolysis, acute renal failure, disseminated intravascular coagulopathy(DIC) after strenuous exercise. A 18 year old man was admitted to the hospital because of loss of consciousness. He was a wrestler and he had tried to lose 6kg in weight by jogging and strenuous exercise on that summer day with hot temperature. The development of this illness was favoured by much clothes inappropriate for that environmental condition with a purpose to enhance sweating, and by restricted intake of water and food. On admission, the blood pressure was 80/50mmHg, and body temperature was 39.4℃ with stuporous mentality. Laboratory finding were as follows : in complete blood cell count, hemoglobin was 16.7g/dL and platelet count was 158,000/ mm³. In blood chemistry, AST 1061U/L, ALT 201U/L, BUN 30mg/dL, Cr 2.2mg/dL, LDH 941IU/L, CK 696IU/L were resulted. In arterial blood gas analysis, pH 7.39, pCO₂ 30mmHg pO₂ 86mmHg, HCO₃ 18.4mEq/L, O₂ saturation 96.7% were resulted. In urinalysis, specific gravity 1.025, protein 3+, erythrocyte 2+ were resulted. In brain imaging study, brain CT finding was normal. After appropriate treatments with rapid cooling, hydration and supportive care, he improved with alert mental status. On 2nd hospital day, in laboratory result, thrombocytopenia and coagulopathy were appeared and renal insufficiency was aggravated. So he was treated with platelet concentrate and fresh frosen plasma for DIC, and enough hydration for appropriate urine volume preservation. After all, On 22nd hospital day, he fully recovered and discharged. Nowadays, with followed for some duration, he is wellbeing with no complications.

        프레탈^(R)정(실로스타졸 50mg)에 대한 실로졸^(R)정의 생물학적 동등성

        Bioequivalence of two cilostazol tablets, the Pletaal^(R)(Korea Otsuka Pharmaceutical Co., Ltd.) and the Cilozol^(R)(Hanmi Pharmaceutical Co., Ltd.), was evaluated according to the guideline of KFDA, Sixteen normal male volunteers(age 20~29 years old) were divided into two groups and a randomized 22 cross-over study was employed. After two tablets containing 50㎎ of cilostazol were orally administered. blood was taken at predetermined time intervals and the concentration of cilostazol in plasma was determined with an HPLC method using UV detector. The pharmacokinetic parameters(C_(max), T_(max) and AUC_(t)) were calculated and ANOVA was utilized for the statistical analysis of parameters. The results showed that the differences in C_(max), T_(max) and AUC_(t) between two tablets were 4.99%, 1.74% and 7.68%, respectively. The powers(1-β) for C_(max), T_(max) and AUC_(t) were83.92%, 80.12% and 85.03%, respectively. Detectable differences(Δ) and confidence intervals were all less than 20%, and confidence interval of all the parameters were also less than 20% at the significance level(α) of 0.05. All of these parameters met the criteria of KFDA for bioequivalence, indicating that Cilozol^(R) tablet is bioequivalent to Pletaal^(R) tablet.

        원자력 사고후 쌀알과 배추내 ^90Sr 및 ^137Cs 농도 예측

        원자력 사고후 시간경과에 따른 작물체내 핵종 농도를 보다 현실적으로 예측할 수 있는 방법을 수립하고 단위 건침적에 대하여 백미와 배추내 ^90Sr 및 ^137Cs의 50 년간 농도를 예측하였다. 비생육기 사고의 경우 농도는 두 핵종 모두 시간경과에 따라 서서히 감소하였고 50년 내내 ^90Sr이 ^137Cs보다 높았다. 생육기 사고의 경우 처음 1년간 농도는 비생육기 사고에 비해 대체로 ^90Sr은 최고 30 배, ^137Cs은 최고 1,000 배 정도까지 높았다. 50 년간의 누적농도는 백미에서는 비 생육기 사고의 경우 ^90Sr이, 생육기 사고의 경우 ^137Cs이 더 높았으나 배추에서는 어느 경우에나 ^90Sr이 더 높았다. 생육기 사고시 50년간의 누적농도에 대한 지배적 경로는 대체로 ^90Sr의 경우 뿌리흡수, ^137Cs의 경우 작물체 직접오염이었다. 재부유의 영향은 무시할 수 있을 정도였다. 예측 결과에 입각하여 사고 조건별로 대책의 방향을 제시하였다. A method of more realistically predicting radionuclide concentrations in crop plants varying with time after a nuclear accident was established to estimate 50 years' concentrations of ^90Sr and ^137Cs in polished rice seeds and Chinese cabbage for unit dry deposition. After non-growing season accidents, concentrations of both nuclides decreased gradually with time and ^90Sr concentrations were higher than those of ^137Cs throughout the whole period. Radionuclide concentrations in the 1st year after growing season accidents were on the whole higher than those after non-growing season accidents by factors of up to 30 for ^90Sr and up to 1,000 for ^137Cs. In polished rice seeds, the 50 years-integrated concentration was higher for ^90Sr than for ^137Cs after non-growing season accidents, whereas the opposite was true after growing season accidents. In Chinese cabbage, however, it was higher for ^90Sr than for ^137Cs after both types of the accident. Generally speaking, the dominant pathway for the integrated concentration after the growing season accident was root uptake for ^90Sr and direct plant contamination for ^137Cs. The effect of resuspension was negligible. Based on the predicted results, the direction of planning countermeasures was suggested for various accident conditions.

        This paper examines antenna diversity combining system using the Maximal Ratio Combining(MRC) as a combining method and Channel State Information(CSI) Viterbi decoding strategy combined with the antenna diversity technology to overcome frequency selective fading caused by multipath channels. Also we investigated estimated CSI by using 2 consecutive long codes in the PLCP preamble for Viterbi decoding algorithm. The proposed new decoding strategy outperforms over a single antenna system and gets additional gains over a non-CSI antenna diversity combining receiver.

        The purpose of this study is to present quantitatively the influence of variables that had the largest effect on the changes in suspended solids(SS), which would cause turbid water phenomenon, among water quality factors of the non-point pollution source, and then to develop a multiple regression equation of SS and predict the water quality of ungaged watersheds so as to provide basic data to establish efficient management plans for SS which flow in rivers and lakes. To identify the correlation of SS with the amount of rainfall and the state of land use, a simple correlation analysis and a simple regression analysis were conducted respectively. Finally, a multiple regression analysis was conducted to provide that SS were set as dependent variables while the amount of rainfall, paddy fields and dry fields were set as independent variables. As a result, the amount of rainfall had the most significant influence on changes in SS, followed by dry fields and paddy fields. In addition, the multiple regression equation was developed to predict SS in unmeasurable watersheds.

        정신지체 학생의 구강실태 및 우식활성도에 관한 연구

        본 연구의 목적은 정신지체 학생의 구강건강 실태 및 치아우식 활성도를 평가하여, 학생들의 구강건강 증진을 위해 필요한 교육 방안을 모색하고, 실제적인 구강건강 관리 향상을 위한 기초 자료를 마련하고자 하는 것이었다. 광주광역시 소재 S 정신지체특수학교 유아부부터 고등부까지의 학생을 대상으로 조사를 실시하였다. 구강건강실태를 평가하기 위한 구강검사는 213명에 대해 시행하였으며, 치아우식활성도 검사는 197명에 대해 시행하였다. 치아우식활성도 평가를 시행한 197명의 정신지체 학생 중 우식활성이 고도인 학생은 33명(16.77%)으로 나타났으며, 13세 이하가 그 이상의 연령에 비해 치아우식활성도가 높았다. 치아우식활성도의 경중도와 치은염 유무와는 관련성이 없는 것으로 나타났다. 정신지체 장애학생의 치아우식활성도 경중과 치아우식경험지수와의 상관성을 평가한 결과, 치아우식활성도가 증가할수록 우식경험치율과 우식경험치지수가 높은 양상을 보였으며, 특히 유치의 경우 우식경험유치율 및 우식경험유치지수는 치아우식활성도와 높은 상관성을 보였다. The purpose of this study is to evaluate oral health status, caries-inducing bacterial activity of the mental retardation students, and developing motivation method for improving oral hygiene management by clarifying the relationship between caries- inducing bacterial activity and oral health status of mental retardation students. Caries experience indices, caries susceptibility test, gingival health evaluation, and oral hygiene management and oral health survey were performed. Results were as follows: 1. The DMFT index of disabled students(12 years) was 2.07. 2. The gingival inflammation was occurred more frequently in older ages. 3. There also was a high positive correlation between caries incidences and the results of caries-inducing bacterial activity test especially in deciduous teeth. This result suggested that the Dentocult SM mutans test as a caries activity test is a reliable method for measuring the status of dental caries in mental retardation students. Because it would motivate the mental retardation students to care more actively for their oral hygiene if they learn how much dental caries-inducing bacteria are living in their mouth by observing the bacterial colony on the cultured test strip, it can become a possibly efficient educational tool for the mental retardation students.

        Fuzzy controller does not derive special quality spinning expression for system, and uses rules by value expressed by language. It is used extensively to non-linear, plant which mathematical modelling is difficult etc... Fuzzy control algorithm of AMR that is used by this research applies obstacle position, distance of obstacle, Progress direction of robot, speed of robot, Perception area of sensor, etc.. by fuzzy control and decide steering angle of robot. An Autonomous Mobile Robot(AMR) performs duty by sensing a recognized situation and controlling suitably. The existing algorithm has some advantages that it is possible to express the obstacle exactly and the robot is sensitive to the change of environment. However, this algorithm, needs to control repeatedly according to the modelling and working environment that requires a great quantity of calculations. In this paper, We supplement shortcoming and designed direction algorithm of AMR using fuzzy controller. The existing algorithm has some advantages that it is possible to express the obstacle exactly and the robot is sensitive to the change of environment However, this algorithm needs to control repeatedly according to the modelling and working environment that requires a great quantity of calculations.
