Schwann cells(SCs), an important component of the peripheral nervous system, intract with nerous to mutually support growth and replication for the peripheral nerve regentation. Recently, ading SCs to the lumen of guidance channel is widely tried to improve regeneration or to make regeneration possible over otherwise irreparable gaps. however, it is not easy to isolate and multiplicate SCs as much as enough to help the axonal regeneration. For the allogeneic SCs source for tubular nerve guidance, we developed a little bit improved technique of harvesting and multiplicating SCs. by culturing dispersed dorsal root ganglia in specially designed medium with growth factors and serial processing, we repeatedlly generate relatively homogenous SC cultures. Our technique was compared with other methods of literature using immunostaining methods such as GFAP, S100, BDNF and the total SC count assessment at different time interval after primary culture.

The surgery of oral and maxillofacial area poses the risk of cranial nerve damage such as trigeminal nerve or facial nerve. Inferior alveolar nerve is prone to damage in the third molar extraction, implant installation, orthognathic surgery, open reduction and rigid fixation, and tumor ablation surgery. On the other hands,facial nerve is likely to be damaged or sacrificed with trauma or parotidectomy. In case of inferior alveolar nerve injury, the incidence is reported to be about 1.3%. The nerve function will almost recover in minimal damage, but it won't recover at last in total damage of a part of nerve unit. In latter cases, nerve regeneration in intended by allograft as nerve substitute or various route of merve condit. But the recovery with autograft is believed to be most relialbe mrthod in the rapair of long-span(longer than 15㎜)nerve defect. We have performed autologous sural nerve graft in the repair of nerve defect, which is caused by resection of benign or malignant tumor. Hereby we report the method of nerve harvesting, recovery of defected peripheral nerve and the complications of donor site with the discussion of sural nerve anatomy.

Purpose: The prevalence of macrolide-resistant Mycoplasma pneumoniae (MRMP) has increased worldwide. The aim of this study was to estimate the proportion of MRMP in a tertiary hospital in Korea, and to find potential laboratory markers that could be used to predict the efficacy of macrolides in children with MRMP pneumonia. Methods: A total of 95 patients with M. pneumoniae pneumonia were enrolled in this study. Detection of MRMP was based on the results of specific point mutations in domain V of the 23S rRNA gene. The medical records of these patients were reviewed retrospectively and the clinical course and laboratory data were compared. Results: The proportion of patients with MRMP was 51.6% and all MRMP isolates had the A2063G point mutation. The MRMP group had longer hospital stay and febrile period after initiation of macrolides. The levels of serum C-reactive protein (CRP) and interleukin-18 in nasopharyngeal aspirate were significantly higher in patients who did not respond to macrolide treatment. CRP was the only significant factor in predicting the efficacy of macrolides in patients with MRMP pneumonia. The area under the curve for CRP was 0.69 in receiver operating characteristic curve analysis, indicating reasonable discriminative power, and the optimal cutoff value was 40.7 mg/L. Conclusion: The proportion of patients with MRMP was high, suggesting that the prevalence of MRMP is rising rapidly in Korea. Serum CRP could be a useful marker for predicting the efficacy of macrolides and helping clinicians make better clinical decisions in children with MRMP pneumonia.
Protective Effecf of Brazilin on Cisplatin Nephrotoxicity

Cisplatin is one of the most effective antitumor agents currently available for cancer therapy. However, its clinical use has been limited by its severe side effects, especially nephrotoxicity. Therefore, brazilin, which has a radical scavenging effect, was given intraperitoneally to evaluate the effect on cisplatin nephrotoxicity in rats. Remarkable protective effects against nephrotoxicity of cisplatin were observed when brazilin was administered to rats simultaneously with cisplatin. Hepatotoxicity induced by combination treatment of cisplatin and brazilin was evaluated by measuring serum glutamic pyruvate transaminase and serum glutamic oxalate transaminase. Combination treatment did not affect the levels of sGPT and sGOT, and any combination treatment did not induce metallothionein in kidney. Brazilin which has radical scavenging effect directly reduced nephrotoxicity of cisplatin in vivo. Thus, it seems that nephrotoxicity of cisplatin was caused by free radicals. The present results indicate that brazilin, when it is given with cisplatin, may provide protection against cisplatin nephrotoxicity in rats.

곰팡이 독소인 ochratoxin A(OTA)는 신장독성, 최기형성, 발암성 및 면역독성을 나타내며, 식품, 곡류 및 정육등에 잔류한다고 알려져 있다. 최근 우리나라의 된장, 간장 등 발효식품에서도 OTA의 전반적인 위해성 평가의 일환으로 OTA의 독성 표적장기인 신장에 초점을 맞추어 신장독성 감소 방법을 제시하고자 한다. 신장독서을 감소시키기위한 대상물질로는 1) 기존에 독성 감소 물질로 알려진 phenylalanine(Phe)2) phenylalanine과 aspartic acid로 구성된 가미료인 아스파탐(Asp),3)녹차의 성분이며 free radical scavenger 및 antioxdant 작용이 있는 polyphenol 70ㄴ(PP),4)최근수명연장 효과가 있고 특히 신장질환에 대한 예방효과가 있다고 알려진 알로에 추출물(AE)을 선택하였다. 신장독성을 유발시키기 위하여 OTA를 2.0mg/kg의 용량으로 2주간 연속 경구추여하였다. Phe (40mg/kg,i.p.)과 Asp(25mg/kg,p.o)은 OTA(2.0mg/kg,p.o)와 병용투여하였으며, PP(200mg/kg,p.o.)는 OTA 투여 2주전부터 AE (501ng/kg, i.v.)은 3일전부터 전처리하여 OTA (2.Omg간9, f.a)와 2주간 병용투여하였다. 신장독성의 확안은.혈청중 BUN, creatinine치 및 뇨중 y-glutamyltranspeptidase와 』ㄴacetyl-,5-D-slucoaarninidase의 팔성을 측정하였고 신장에 대한 조직병리 검사를 실시하였다. 실험결과, OTA를 2주간 2.Omg/kg용량으로 투여한 결과 신장 독성이 유발되었으며,독성 감소물질로 사용한 4개의 화합물 모두 혈액 및 뇨 중 신장독성 지표를 유의성 있게 감소시켰다. 조직병리 검사 결과 OTA에 좌하여 신장의 근위세뇨관에 변성이 유발되었으며, 4개의 화합물 처리군에서는 변성이 관찰되지 않았다. 이러한 결과펄부터 Phe, Asp, PP 및 A료는 모두 OTA에 의한 신장독성을 감소션킬 수 있으며, OTA에 의한 신장독성 유발에는 Phe콘 대한 경쟁작용 및 free radical 생성치 관여되어 있음을 확글할 수 있었다. In this study we investigated the protective effects of aspartame (Asp), phenylalanine(Phe), polrphenoB 705(PP) and aloe estract (AE ) on the nephrotoxicity induced by subacute exrolurewere admiai'ftered to Spragu,e-Dawley rats simultaneously with 07,c, (2.Omg/kg, f.a) for 2 weeks. PP (200mg/kg, f,o.) and AE (SOmg/Etg, i. u.) were pretreated before adminfstration of OTA, for 2 weeks and3 days, respectivelf. Using e]Ixymuria, BUN level, creatfneinia and histophathologic examination as indices of renal damage, we observed that all of four compvnds preveBted the nephrotoxic effects inducedby OTA. It seems that stnlct,ural analogues of OTA such as Asp and Phe have better protective effecton the nephrotoxicity of OTf. than aBtioxidants. These results indicate that 1) formation of free radicaland lipid peroxidation are like?y to be involved in the nephrotoxicity of OTA in vivo,2) Asp, PP and AEare might be used for prevent,ion of renal lesions in cases of ochratoxicosis.

목적: 소아과 전문의들의 소아 천식 이해 정도와 진단 및 치료 경향에 대해 알아보고 소아 천식에 대한 의사 재교육에 필요한 기본적인 자료를 얻고자 이 연구를 시행하였다. 방법:소아과 학회 회원 주소록에서 무작위로 선정한 개원의 및 봉직의, 수련병원 전문의 1,200명에게 천식의 진단, 치료와 예후에 관련된 문제로 구성된 설문지를 발송하여 회수하고 그 결과를 분석하였다. 결과:응답율은 19.8%였으며, 응답자는 남 140명, 여 98명으로 총 238명이었 Purpose: Korean Academy of Pediatric Allergy and Respiratory Disease suggested Korea Guideline for Diagnosis and Management of Childhood Asthma(KGDMCA) in 1994 and updated it in 1999. 시스플라틴에 의한 LLC-PK1의 알파-메틸글루코스 흡수 감소 기전

We have previously shown that determination of glucose uptake using alpha-methylglucose(alpha-MG) is very sensitive and rapid parameter for the assessment of loss of cellular function in renal cell line(LLC-PK1). The present study was designed to elucidate the mechanism of inhibition of alpha-MG uptake and the intracellular site of toxic action of cisplatin(CIS). LLC-PK1 cells were exposed to various concentrations(5mcM-lOOmcM) of CIS for 5 hrs or 24 hrs and alpha-MG uptake was determined. Mitochondrial function was evaluated by measuring intracellular ATP content and MTT reduction. The activities of marker enzymes for the basolateral membrane(Na+-K+ ATPase) and brush border membrane (alkaline phosphatase: ALP) were also measured. CIS treatment significantly inhibited the alpha-MG uptake in a time- and dose-dependent manner above 25mcM for 5 hrs. Intracellular ATP content and MTT reduction were affected by 24 hr-treatment of 50mcM CIS. The activities of Na+-K+ ATPase and ALP were significantly decreased at 10mcM and 5mcM of CIS for 24 hrs, respectively. The incubation with CIS for 5 hrs had no effects on the intracellular ATP content, MTT reduction and the activities of marker enzymes up to 100mcM. These results partly indicate that inhibition of alpha-MG uptake by CIS may not be attributed to the disturbance of mitochondrial function or inhibition of the activity of Na+-K+ ATPase and can be resulted from direct effect of CIS on the Na+/glucose cotransporter in brush border membrane. This study shows that additional mechanistic information, indicating the intracellular site of nephrotoxic action, can be gained by coupling the alpha-MG uptake and ATP content or the activity of Na+-K+ ATPase.
