        근막 결손의 복원과 자궁천골인대 고정을 통한 전자궁적출술 후 질탈출의 성공적 교정

        목적 : 본 연구는 전자궁적출술 후 질탈출을 부위-특이적인 결함의 개념에 입각하여 치골경부 근막과 직장질 격막의 연결을 통한 골반내 근막의 복원과 자궁천골인대 고정술을 이용한 질탈출의 성공적인 수술적 교정의 방법과 결과를 보고하고자 한다. 연구 방법 : 1999년부터 2001년 8월까지 자궁적출술 후 질궁탈출 증상으로 문화병원 산부인과를 내원, 탈출된 질의 근막을 재건시킨 다음, 이를 자궁천골인대에 고정시킴으로써 질의 부위-특이적인 교정술을 받은 3명의 환자를 대상으로 하였다. 3명중 1명은 개복술을 하였고 2명은 내시경 수술을 하였다. 결과 : 환자의 연령 분포는 49-67세였으며, 수술시간은 평균 113분 (105-125)이었다. 세 환자 모두 수술 후 질의 상태가 해부학 및 기능적으로 정상이었으며, 9일째 특이사항 없이 퇴원하였다. 수술 후 HRT를 시행한 1명을 포함한 3명 모두 각각 24개월, 12개월, 7개월간의 추적 검사에서도 특이한 합병증이나 재발 없이 좋은 경과를 보였다. 결론 : 골반 근막 결손의 복원과 자궁천골인대 고정술은 전자궁적출술 후 질탈출의 교정에 매우 안전하고 효과적이며, 기능적인 방법으로 사료되며, 자궁천골인대를 질탈출 교정에 이용한 연구는 본 연구가 국내에서 처음이다. Objective : To report the method and efficacy of the site specific-defects repair in the endopelvic fascia and uterosacral ligament suspension for the correction of posthysterectomy vaginal vault prolapse. Methods : This study was performed in 3 patients with posthyterectomy vaginal vault prolapse who underwent the surgery for the reconstruction of endopelvic fascia and the correction of vault prolapsed vagina by using uterosacral ligament suspension at the department of Obstetrics and Gynecology in Moon-Hwa Hospital. Among them, 1 patient was treated by laparotomy, and 2 patients were treated by laparoscope. Results : The age of patients ranged from 49 to 67 years. The mean operation time was 111 minutes (105-120 minutes). The status of postoperative vagina was normal in all 3 patients, who were discharged 9 days after operation without symptoms. No complication or recurrence were found in them despite long-term follow-up for 24 months, 12 months, 7 months, respectively after operation. One patient has been treated with HRT therapy. Conclusion : The reconstruction of the defect of endopelvic fascia and uterosacral ligament fixation may be very safe, effective, and functional in the correction of vaginal vault prolapse. This study reports the treatment of vaginal vault prolapse using uterosacral ligament within Korea for the first time.

        In recent years, gonorrhea has been pandemic and remains one of the most common STDs in the world, especially in developing countries. For the detection of a more effective therapeutic regimen and assessing the prevalence of Penicillinase Producing Neisseria gonorrhoeae(PPNG), we have been trying to study the patients who have visited the Venereal Disease Clinic of Choong-Ku Public Health Center in Seoul since 1980 by menas of the chromogenic cephalosporin method. In 1998, 93 strians of N. genorrhoeae were isolated, among which 60(64.5%) were PPNG. The prevalence of PPNG in Seoul, which had been decreased to 39% in 1996 after a peak of 74.3% in 1993, is increased to 64.5% in 1998.

      • 낙동강 중류부에서의 최적 수질관리 기법의 개발

        Optimum water quality management in the midstream of the Nakdong river was studied. Deterministic water quality model RQ1 and stochastic water quality model RQ2 have been developed. RQ1 contains two codes, RQ1-1 for hydraulic analysis and RQ1-2 for water quality analysis. RQ2 consists of Monte-Carlo simulation for the DO analysis in river. A water quality simulation using RQ1 and RQ2 in the midstream of the Nakdong river has been made. The computed concentration of DO and BOD at several points have good agreements with the observed data. BOD-DO profiles in this basin and the probability distribution for DO concentration at some points have been predicted in 1996 and 2001. This study provides the optimum water quality management planning in the future using the deterministic and stochastic water quality model.

      • Lagrangian Transport Model에 의한 擴散方程式의 數値解析

        The mathematical modeling of the dispersion behaviour in the river was studied. The solution approach to the convective-diffusion equation uses a Lagrangian reference frame which greatly simplifies the mathematics of solution procedure and dramatically reduces errors caused by numerical dispersion. The dispersion analysis of the conservative material was made and the accurrcy of the model was also discussed. The model should be calibrated by field data, and expected to be developed for the simulation of the various pollutants such as BOD, DO, nitrogen, phosphorous and of the unsteady flow in the river.

        Many causes of rhabdomyolysis have been reported in the literature. Technetium-99m methylene dipho-sphonate (Tc-99m MDP) bone scintigraphy is considered to be the most sensitive and effective modality in defining the extents of disease involvement and follow-up of patients with rhabdomyolysis. In this paper, we reviewed two cases of generalized rhabdomyolysis related to seizure activities and two cases of localized rhabdomyolysis related to an excessive exercise. In addition other causes of rhabdomyolysis and possible mechanisrns of Tc-99m MDP uptake are discussed briefly.

      • 限界狀態 設計法에 의한 鐵筋콘트리트 二方向 슬라브의 最的設計

        This study attempts to optimize reinforced concrete two way slabs with each spanning condition, based on the concept of ultimate limit state and of serviceability limit state. The variable adoped in this mathematical model consist of design variables including sectional sizes and reinforcement areas per unit width of middle strips and column slrips of slab. The cost function is taken as the objective function in the formulation optimal problem. A number of constraint equations involving flexural stress, reinforcement ratio, deflection and side limit is derived in accordance with BSI cp110 requirements on the basis of limit state design theory. Both objective function and constraints equation derived into terms of design variables and analysis variables generally become high degree nonlinear programming problems. Optimal algorithm that can optimize a structure synthetically earring on both analysis and design of a slab with each spanning condition. The developed algorithm is directly applied to two adjacent edges discontinuous slab, one long edge discontinuous slab, one short edge discontinuous slab and interior panel slab in order to assure validity of the algorithm and the possibility to optimize reinforced two way slabs. From this research it is found that the althorithm developed in this study is applicable to optimization of reinforced two way slabs and it converges to a optimal solution with 2 to 3 iterations regardless of initial variables. The results shows that an economical design can be possible when campared with conventional designs.

        This study was performed to investigate: The ecological difference of two ecotypes "Seushan (northern) and "Cheju(southern)" The effect of temperature (low. high. alternative) and long day treatments on the growth and bulb formation in two ecotypes Translocation of growth regulators and contents of total sugars in garlic plants Identification of isozyme existing in garlic plants by the electrophoretic method. The results are summarized as follows: 1. The experiment of temperature and long day treatments. Sprouting and early growth of the southern type were accelerated by low iemperature treatment but those of long day treatment were not affected. In plant height and leaf sheath length. the southern type was increased more than the northern type until March 20 and the plants that were treated with low temperature were increased considerably as compared with the control There was no difference in the total leaf numbers between two ecotypes but low temperature treatment romoted those as compared to the control. 2. Ecological experiment of two regions The plant height of the southern type was accelerated until March 25 i n Cheju and March 10 in Suweon. but the plant height of the northern type was promoted after April 20. The growth of leaf length and leaf sheath lentth showed no dlfference durting winter. but after March 10 the southern type was increased in Suweon and Cheju. there was no difference in clove number and bulb weight within the regions and seeding time. but those of the southern type were increased as compared with the northern type 3 Analysis of sugar Low temperature treatment tended to increase concetrations of sugar as compard with control and the difference between cultivats was clear Friictosan was increased as the plants grew. especially at the later period of the whole growth with ever): treatment 4 Bioassay at growth regulators Af Rf 0.5-0.6 and Rf 0.4-0.5 Glbgberellin-like substances were detected from the leaf and leaf sheath of northern type and inhibitors were detected at Rf 0.7-0.9 While 5. The change of Isozyme patterns It was noted that the isozyme patterns were similar between two ecotypes in peroxidase. There was no difference at the bands which means acid phosphatase in two types. There were three or four bands which showed Malate dehydrogenase. but also showed the similar tendency according to the cultivars and every parts.

        ‘병동 중심의 처치수가 산정지침 개발’ 등을 통한 간호업무 표준화 및 그 효과

        문제: 병동 처치수가 산정 부정확 및 누락으로 인한 부적절한 재고관리로 진료차질, 불필요한 업무발생 및 수익이 감소한다. 목적: 병동 처치수가 산정 정확화를 위한 업무 표준화 도구 및 시스템을 개발하여 적정 재고관리를 통한 수익증대 및 직무만족도를 향상시킨다. 의료기관: 서울시 종로구에 소재한 대학병원 질 향상 활동: 병동중심의 처치 산정지침 개발 및 수가물품의 적정재고 관리방안을 모색하였다. 개선효과: 병동중심의 처치수가 산정 지침서를 제작하여 업무표준화를 기하였음. 응급청구 품목 및 수량이 ’07년 대비 71%감소하고 타 병동 차용품목이 활동 전에 비해 61% 감소, 수량은 77% 감소함. 응급청구 총소요시간이 활동 전에 비해 ’07년 대비 77% 감소하고 타 병동 차용 총 소요시간이 61% 감소함. 전년 동기간 대비 수익이 4% 증가하였으며 또한 54병동과 보험 심사팀에서 직원들의 직무만족도가 향상되었다.

        韓國産 植物凝集素의 淋巴球凝集에 關한 硏究

        It has been accumulated a number of research works on phytagglutinin (PHA) reported by many investigators. Recently, the utilization of PHA in the medical field is increasing, especially as a lectin for the blood grouping, as a mitogen for artificial culture of the various cells and as a determinant of the receptor specificity of various tumor cells. The purpose of the present paper is to describe PHA can applicated as a leucoagglutinin and as a reagent for lymphocyte cytotoxicity test. 2 grams of ground seed of Korean Celosia cristata L., Vicia amurensis OETTINGEN, Glycine Max. a METT. and ground stems of Fritillaria ussuriensis Max. was mixed well with 100㎖ of pH 7.0 phosphate buffer saline (PBS) respectively, and the extract was treated for 30 minutes in water bath adjusted 56。C, then centrifuged, the supernatant was filtered with millipore filter paper (HA type, size 0.45μm). With the plant extracts, agglutination of human lymphocyte, chicken thymus and bursa cell, and agglutination inhibition with various sugars, and lymphocyte cytotoxicity test were carried out and the following results were observed. All of human lymphocytes, chicken thymus and bursa cells were agglutinated by extracts of Celosia cristata and Vicia amurensis, however, only chicken thymus cells among them were agglutinated by extract of Glycine Max. In the cytotoxicity test of 214 of human lymphocytes with Fritillaria ussuriensis extract, 67% was positive, which showed the extract acts as a anti-HLA lectin, and the frequency suggested that a new specificity of lymphocyte was demonstrated
