Hypoallergenic and Physicochemical Properties of the A2 β-Casein Fraction of Goat Milk

Goat milk has a protein composition similar to that of breast milk and contains abundant nutrients, but its use in functional foods is rather limited in comparison to milk from other sources. The aim of this study was to prepare a goat A2 β-casein fraction with improved digestibility and hypoallergenic properties. We investigated the optimal conditions for the separation of A2 β-casein fraction from goat milk by pH adjustment to pH 4.4 and treating the casein suspension with calcium chloride (0.05 M for 1 h at 25°C). Selective reduction of β- lactoglobulin and αs-casein was confirmed using sodium dodecyl sulphate-polyacrylamide gel electrophoresis and reverse-phase high-performance liquid chromatography. The hypoallergenic property of A2 β-casein fraction was examined by measuring the release of histamine and tumor necrosis factor alpha from HMC-1 human mast cells exposed to different proteins, including A2 β-casein fraction. There was no significant difference in levels of both indicators between A2 β-casein treatment and the control (no protein treatment). The A2 β-casein fraction is abundant in essential amino acids, especially, branched-chain amino acids (leucine, valine, and isoleucine). The physicochemical properties of A2 β-casein fraction, including protein solubility and viscosity, are similar to those of bovine whole casein which is widely used as a protein source in various foods. Therefore, the goat A2 β-casein fraction may be useful as a food material with good digestibility and hypoallergenic properties for infants, the elderly, and people with metabolic disorders.

A Gram-negative bacterium was isolated from mushroom media that markedly reduces the level of extracellular toxins (i.e., tolaasins) produced by Pseudomonas tolaasii, the most destructive pathogen of cultivated mushrooms. The HC1 strain was selected as detoxifying tolaasin by bioassay on potato and it was identified Pseudomonas sp. by the cultural, morphological and physiological characteristics, and analysis of the 16S rRNA.. The isolated bacterium is saprophytic but not parasitic nor pathogenic to cultivation mushroom. The isolated bacterium for P. tolaasii cell, was sufficient for detoxification in vitro. Inoculation of the isolated bacterium prevents the development of bacterial disease in Pleurotus ostreatus, Flammunia velutipes and Agaricus bisporus. Control efficacy of brown blotch of strain HC1 treatment was 69, 68 and 55% on Agaricus bisporus, Flammulina velutipes and Pleurotus ostreatus, respectively. The suppressive bacterium may be useful in future for the development of biocontrol system and the construction of genetically modified edible fungi resistant to the disease caused by P. tolaasii.
The Clinical Features of Multidrug-Resistant Bacteria Induced Pyogenic Liver Abscess

Aims: Pyogenic liver abscesses (PLA) remain a potentially significant clinical problem. Although multidrug-resistant organisms (MDROs) are increasingly being observed worldwide, little is known about the incidence and clinical features of MDR bacteria induced PLA. Aim of this study is to provide characteristics of PLA with MDR isolates. Methods: A retrospective study of patients diagnosed with PLA from 2008 to 2017 in Hallym University Medical Center in Korea was performed to characterize PLA. The demographic and clinical features, laboratory and imaging findings, management and clinical outcomes, antimicrobial susceptibility profile of causative bacterial species were studied. Results: Overall, 745 patients were included. 53.1% of PLA aspirate cultures were positive, with Klebsiella pneumonia (71% of positive cultures) being the main organism. MDROs were found in 53 (7.1%) patient, extended-spectrum beta-lactamase (ESBL) producing Gram-negative bacteria was most common (77.3% of MDROs; Klebsiella pneumonia 41.5%, Escherichia coli 58.5%). The group with MDR isolates showed old age (62.3±15.1 vs. 68.9 ±14.3, P=0.002), a lower systolic/ diastolic blood pressure, hypoabluminemia (3.4±0.6 vs. 3.2±0.5, P=0.005), a higher alkaline phosphatase levels (248.1±226.8 vs. 326.9±312.2, P=0.018), a longer hospitalization (23.3±14.4 vs. 29±13.8, P=0.005) and a longer parenteral antibiotic treatment period than the non- MDR group (n=612). Biliary origin of liver abscess, previous history of hepatobiliary procedure and concomitant malignant disease were associated with MDRO induced liver abscess. In multivariate analysis, previous history of hepatobiliary procedure (relative risk, 2.73; 95% CI, 1.124 to 6.645; P=0.027) and concomitant malignancy (relative risk, 3.81; 95% CI, 1.539 to 9.47; P=0.004) were risk factors. Moreover, in-hospital mortality rate was higher in of MDR group (13.2% vs. 3.8%, P=0.007). Conclusions: Our study demonstrates that patients with MDR bacteria induced PLA may have poor prognosis. Thus, microbiological diagnostics is pivotal to tailor individual treatment regimen, especially in the patient with previous history of hepatobiliary procedure and concomitant malignancy.

Background: Chronic atopic dermatitis (AD) shows many histopathologic features of psoriasis, such as epidermal hyperplasia, prominent T lymphocyte and dendritic cell infiltration. Recent findings suggest that Asian AD patients might have a blended phenotype of AD and psoriasis. However, little is known about this issue. Objective: We investigated clinical and immunohistopathological characteristics of Korean adult patients with AD showing psoriasiform features histopathologically. Methods: A total of 59 Korean male patients (19.9±3.2 years) with chronic AD enrolled. Clinical, laboratory and histopathological features were compared between AD patients with psoriasiform features and those with typical histologic pattern of AD. In addition, immunohistopathological characteristics were analyzed using TH17 axis-associated cytokines. Results: Fifteen patients (25.4%) showed a more psoriasiform histologic appearance, with regular elongation of rete ridges and parakeratosis. Although all patients were characterized by erythematous excoriated patches, patients with psoriasiform features often showed clearer demarcation of lesions, with more prominent scaling. Despite the presence of increased IgE levels in both groups, differences and similarities of immunohistopathological characteristics were found in the AD lesional skin. Conclusion: Our findings will enable us in-depth understanding of phenotypic characteristics of Korean AD.

In this study, the length and area of t he artifact were quantitatively evaluated according to the direction of the magnetic susceptibility artifact. Using a self-made magnetic susceptibility artifact phantom the frequencyencoding direction in the 3.0T MRI system was changed in axial, coronal, and sagittal planes using T2 fast spin echo (FSE). The results showed a difference in length when the frequency encoding was changed from anteroposterior (AP) to right-to-left (RL) in the axial, RL to superior-inferior (SI) in coronal and AP to SI in sagittal planes (p < 0.05). In addition, the difference occurred when the magnetic susceptibility artifact area was measured according to the frequency-encoding direction conversion (p < 0.05). However, additional studies of magnetic susceptibility artifacts using various types of screws and plates are needed. This study provides basic data to prevent magnetic susceptibility artifact due to screw fixation.

Aims: The Baveno VI consensus recommended that screening endoscopy can be safely avoided in patients with a liver stiffness (LS) < 20 kPa and platelet count >150x10<sup>9</sup>/L. Also, the Expanded Baveno VI criteria was proposed to spare more endoscopies. We aimed to validate and revise these criteria with other non-invasive markers in ruling out high-risk varices. Methods: Patients with compensated advanced chronic liver disease (cACLD), which is defined as LS ≥10 kPa, and who had endoscopy within 6 months of the transient elastography in 8 centers were included. Patients with hepatocellular carcinoma and those with Child-Pugh score >6 was excluded. Varices needing treatment (VNT) were defined as ≥F2 varices or varices with red color sign. Results: A total of 1218 patients with cACLD were included. Age was 56.0±11.5 years and 778 patients (63.9%) were men. Chronic hepatitis B was the most frequent underlying liver disease (479 patients, 39.3%), followed by alcoholic liver disease (359 patients, 29.5%). VNT were noted in 249 patients (20.4%). With Baveno VI criteria, VNT loss rate was only 1.9%, while sparing endoscopy rate was only 25.7%. With the Expanded Baveno VI criteria, 43.5% of endoscopies were avoided, but VNT loss rate was 5.8%. We explored various other parameters and cutoff values in ruling out VNT. Platelet >110x109/L and LS <20 kPa spared 39.1% of endoscopy with 4.4% of low VNT loss rate. When the LS-spleen diameter to platelet ratio score (LSPS) was applied, LSPS < 14.5 could spare more endoscopies (44.2%) with low VNT loss rate (4.5%). Conclusions: Combination of platelet >110x10<sup>9</sup>/L and LS <20 kPa has VNT loss rate less than 5%, and high endoscopic sparing rate in patients with cACLD. This study also showed LSPS <14.5 as a better strategy with increased number of spared endoscopies while maintaining excellent VNT loss rate.

As one of the major types of lung cancer, non-small cell lung cancer (NSCLC) accounts for the majority of cancer-related deaths worldwide. Treatments for NSCLC includes surgery, chemotherapy, and targeted therapy. Among the targeted therapies, resistance to inhibitors of the epidermal growth factor receptor (EGFR) is common and remains a problem to be solved. MET (hepatocyte growth factor receptor) amplification is one of the major causes of EGFR-tyrosine kinase inhibitor (TKI) resistance. Therefore, there exists a need to find new and more efficacious therapies. Deoxypodophyllotoxin (DPT) extracted from Anthriscus sylvestris roots exhibits various pharmacological activities including anti-inflammation and anti-cancer effects. In this study we sought to determine the anti-cancer effects of DPT on HCC827GR cells, which are resistant to gefitinib (EGFR-TKI) due to regulation of EGFR and MET and their related signaling pathways. To identify the direct binding of DPT to EGFR and MET, we performed pull-down, ATP-binding, and kinase assays. DPT exhibited competitive binding with ATP against the network kinases EGFR and MET and reduced their activities. Also, DPT suppressed the expression of p-EGFR and p-MET as well as their downstreat proteins p-ErbB3, p-AKT, and p-ERK. The treatment of HCC827GR cells with DPT induced high ROS generation that led to endoplasmic-reticulum stress. Accordingly, loss of mitochondrial membrane potential and apoptosis by multi-caspase activation were observed. In conclusion, these results demonstrate the apoptotic effects of DPT on HCC827GR cells and signify the potential of DPT to serve as an adjuvant anti-cancer drug by simultaneously inhibiting EGFR and MET.
