        실내사무환경의 환경성담배연기(ETS)의 지표물질에 관한 연구

        The purpose of this research is to evaluate indoor office environment using the concentrations of nicotine, 3-enthenyl-pyridine(3-EP), and respirable suspended particulate(RSP), which are indicators for environmental tobacco smoke(ETS) and the correlations between indicators and environmental conditions(smoking density, smoking index). The mean air charge per hour (ACH) in smoking rooms was 10.4 and most of the smoking rooms showed non-compliance withe ASHRAE standard value except only one smoking rooms. The concentrations of RSP, 3-EP, nicotine showed log=normal distributions, and became different statistically depending on smoking condition(p〈0.01). The geometric mean concentration of RSP in smoking room was 441.7 ug/㎡ that is far exceeded environmental standard(150 ug/㎡). This implies that fine particulate in smoking room should be carefully controlled considering smoking density and ventilation fate. The mean concentrations of nicotine and 3-EP were 93.4 ug/㎥, respectively. The correlation coefficients between RSP and SI, 3-EP and SI, and Nicotine and SI were 0.67, 0.84 and 0.74, respectively. The correlation coefficient between nicotine and 3-EP, nicotine and RSP, and RSP and 3-EP were 0.76,0.78 and 0.57 respectively.

      • 자연환경방사선에 의한 피폭선량(유효선량당량)의 종합적 해석연구

        자연환경방사선의 세 가지 주요 요소인 지각방사선, 우주선 전리성분 및 라돈과 그 자핵종이 주는 선량을 측정, 해석하였는 바 18개월간에 걸쳐 대전, 수원, 경산 지역에 대하여 수행하고 이를 종합하여 유효선량당량으로 평가하였다. 지각방사선과 우주선 전리성분 측정을 위하여 3"∮×3" Nal(Tl) 섬광검출기와 휴대형 다중파고분석기를 이용한 감마선 분광분석 및 ^7LiF PTEE disk TLD에 의한 적산선량측정을 병행하였고, 공기중 라돈농도의 측정은 CR-39 고체비적검출기를 내장한 라돈컵을 설계제작하여 사용하였으며 비적수 라돈농도 교정인자는 스웨덴 국립방사선 방호연구소의 표준 라돈 챔버내에서 조사하여 결정하였다. 세 지역의 가연방사선에 의한 유효선량당량에는 약간의 수치적 차이는 있었으나 본질적 차이는 찾아볼 수 없었으며 지역 측정치의 분산가중 평균치는 지각방사선 (463±3)uSvㆍy^-1[(52.9±0.3)nSvㆍh^-1], 우주선 전리성분 (287±15)uSvㆍy^-1[(32.8±1.7)nSvㆍy^-1]로 외부 피폭이 (775±9)uSvㆍy^-1[(88.5±1.0)nSvㆍy^-1]였으며 라돈과 그 자핵종에 의한 내부피폭은 (1973±298)uSvㆍy^-1[(225.2±34.0)nSvㆍy^-1]였다. 따라서 자연황경방사선에 의한 여간 총 선량당량은 (2762±296)uSvㆍy^-1[(315.3±34.0)nSvㆍh^-1]로 평가되었는데 이 값들은 UN보고서^(11)에 나온 “정상적 자연방사선 지역”의 값과 비교할 만한 타당한 값으로 나타났다. Assessment of effective dose equivalent from natural environmental radiation that consists of terrestrial gamma­ray, ionizing component of cosmic­ray and alpha­rays of atmospheric radon and its decay products has been carried out for 18 months in Daejeon, Suwon and Kyungsan areas. A 3"∮×3"cylindrical Nal(Ti) scintillation detector in association with portable multichannel analyser and ^7LiF PTFE disk TLDs were used for the measurement of terrestrial gamma­ray and cosmic­ray ionizing component, while self­designed radon­cups with CR-39 SSNTD attached to the inside bottom were used for the integral measurement of at­mospheric radon concentration. Determination of calibration factor for converting track density of CR-39 detector to radon concentration was made by irradiation of radon­cups in the standard radon chamber of Swedish National Institute of Radiation Protection. Although resultant effective dose equivalents of three different measurment points show minor numerical differences, no essential discripancy between the places was found. The variance weighted means of the annual effective dose equivalents of each component of radiation from the tree places came out to be: (463±3)uSvㆍy^-1[(52.9±0.3)nSvㆍy^-1] for terrestrial gamma­ray, (287±15)uSvㆍy^-1[(32.8±1.7)nSvㆍy^-1] for ionizing component of cosmic­ray and (1973±298)uSvㆍy^-1[(225.2±34.0)nSvㆍy^-1] for internal exposure of radon and its decay products. Consequent total effective dose equivalent turned out to be (2762±298)uSvㆍy^-1[(315.3±34.0)nSvㆍy^-1], which if reasonably comparable with that appeared in UNSCEAR Report^[11]as per caput annual effective dose equivalent in “areas of normal back ground radiation.”

      • Cs-137 γ-선 조사장치의 방사선장 해석연구

        In reference to the ISO-4037 and ANSI N13.11, appropriateness of the shielding and collimating system, and suitability of the irradiation and its environmental conditions of a 20 Ci Cs-137 gamma-ray irradiator installed in Korea Institute of Nuclear Safety(KINS) were investigated. Upon the installation of the irradiator, condition of collimator alignment and virtual beam cross section were examined by use of a portable He-Ne laser. In order to evaluate the fitness of the secondary(scattered) to primary ratio of the collimated gamma-ray beam from the irradiator in light of ISO and ANSI requirement, the measurements of the secondary to primary ratio were performed by use of a cavity type ion chamber which had been calibrated in a calibration laboratory that maintains reliable traceability. For the theoretical assessment of the gamma-ray field the collimated beam were divided into fifteen sectors and for each sector the secondary to primary ratio and the gamma-ray spectrum was simulated by means of Monte Carlo method. The outcomes were compared with the experimental results, and cause of scattering and the existence of alleged modifying factors in the irradiator were examined.

        주의력결핍/과잉행동장애와 양극성장애 공존 환아군과 주의력결핍/과잉행동장애 환아군의 비교연구

        Objective : Attention deficit hyperactivity disorder (ADHD) is frequently comorbid with a variety of psychiatric disorders. Among them, bipolar disorder (BPD) has especially attracted growing interest. This is partly due to the fact that early onset BPD has been frequently misdiagnosed as ADHD because of symptomatic overlap. The aim of the present study is to find the differences in demographic data and clinical features of ADHD+BPD and ADHD in children and adolescents. Method : Children and adolescents patients participating in the present study have been enrolled at the child psychiatric clinic since 2004. 14 patients meeting the DSM-IV chteria for ADHD+BPD and 19 patients meeting the criteria for ADHD and 15 healthy comparison subjects were recruited. All groups were evaluated through the Schedule for Affective disorders and Schizophrenia for School-Age Children-Present and Life Time version-Korean version (K-SADS-PL-K). Demographic data and clinical charactehstics of the subjects were also collected. Parents were asked to complete the Child Behavior Check List(CBCL) and the Toddler Temperamental Scale (TTS) clinical ratings were obtained using the Young Mania Rating Scale(YMRS), the Children's Depression Iventory (CDI) and the Dupaul ADHD scale. Clinical variables between ADHD+BPD and ADHD were analyzed using the Mann-Whitney U test. YMRS, CDI, Dupaul ADHD scale (inattention and hyperactivity), CBCL, and TTS among the three groups were analyzed using the Kruskal-Wallis test with post-hoc Mann-Whitney U test. Results : 1) ADHD+BPD group had an earlier onset age of ADHD than ADHD group 2) ADHD+BPD group had more co-morbid psychiathc disorders than ADHD group. 3) Compared to ADHD group, ADHD+BPD had more psychiathc family history, especially mood disorders. 4) ADHD+BPD group had prodromal symptoms such as irritability, anger dyscontrol and academic dysfunction, compared to ADHD group who rarely showed prodromal symptoms. 5) ADHD+BPD group had higher ADHD scores than the ADHD group in the Dupaul ADHD inattentive scale. 6) In global functions of CBCL, ADHD+BPD group showed more impaired functions at home and school than ADHD group. 7) In Attention diagnostic system (ADS), ADHD+BPD group had more omission and commission errors than ADHD group, especially in the visual system. 8) In the subtests of the IQ profile, ADHD+BPD group had lower arithmetic and block design scores than ADHD group. Conclusion : Clinicians have to rule out underlying bipolarity in children and adolescents with ADHD who show earlier age of onset and severe clinical features. Bipolarity should be explored intensively in ADHD children and adolescents who have early onset of symptoms and severe functional impairment.

        골 - 슬개건 - 골을 이용한 전방십자인대 재건술시 발생한 수술중 수기오류 및 수술후 합병증

        The most commonly used graft source for anterior cruciate ligament reconstruction is the autogenous bone- patellar tendon-bone graft unit. Despite a good success record, intraoperative technical errors and postoperative complications have been known. Author analyzed intraoperative technical errors and postoperative complications, in 44 patients who were treated at the department of orthopaedic surgery, chosun university hospital from Jan. 1994 to Jun. 1996. The most common intraoperative technical errors was screw-graft divergency in 5 cases. Other intraoperative technical errors were too anteriorly location of femoral tunnel in 1 case, too anteriorly location of tibial & femoral tunnel both in 1 case, graft tunnel mismatching in 1 case, graft pullout in 1 case and partial destruction of posterior cortex of femoral tunnel in 1 case, fracture of the bone plug in 1 case. The most common postoperative complication was patellar tendinitis in 20 cases(45.5%). Other postoperative complications were anterior knee pain in 14 case. (31.8%), patellar crepitation in 12 cases(27.3%), quadriceps atrophy in 9 cases(20.5%), arthrofibrosis in 3 cases and graft failure in 2 cases. Author concluded the causes of intraoperative complications were technical errors, and anteriorly location of graft tunnel is most influenced factor to final fuctional results. To eliminate the postoperative complications, intraoperative technical errors should be avoided and also accelerated rehabilitation should be done.

      • 자율신경계의 활성도 측정을 위한 Data Acquisition System의 개발 및 임상응용

        Background: Power spectrum analysis method is a powerful noninvasive tool for quantifying autonomic nervous system activity. In this paper, we developed a data acquistion system for estimating the activity of the autonomic nervous system by the analysis of heart rate and respiratory rate variability using power spectrum analysis. Materials and methods: For the detection of QRS peak and measurement of respiratory rate from patient's ECG, we used low-pass filter and impedance method respectively. This system adopt an isolated power for patient's safety. In this system, two output signals can be obtained: R-R interval(heart rate) and respiration rate time series. Experimental ranges are 30∼240 BPM for ECG and 15∼80 BPM for respiration. Regults: The system can acquire two signals accurately both in the experimental test using simulator and in real clinical setting. Conclusion: The system developed in this paper is efficient for the acquisition of heart rate and respiration signals. This system will play a role in research area for improving our understanding of the pathophysiologic involvement of the autonomic nervous system in various disease states.

      • 상완골 근위부 골절 치료에서 잠김 압박 금속판의 유용성

        Purpose: To prove the efficacy of locking compression plate (LCP) in proximal humerus fracture. Materials and Methods: Forty four out of 48 patients who had proximal humerus fracture operation with LCP from November, 2004 to June, 2008 and available for follow up for more than 6 months were evaluated. Eighteen cases were male and 26 cases were female with mean age of 56 years (range 29-85 years). Cause of injury was traffic accident in 17 cases, slip down in 20 cases and fall down in 7 cases. Mean interval from injury to operation was 6.8 days (range 2-30 days) and mean operation time was 80. 5 minutes(range 40-130 minutes). Open reduction and internal fixation with LCP using deltopectoral approach was used in all cases. Postoperative clinical evaluation was done using Constant-Murley assessment and bone union and neck-shaft angle was checked using radiographic exam. Results: Bone union was achieved in 41 out of 44 cases at mean interval of 11.2 weeks(range 6-16 weeks). Mean neck-shaft angle was 129.7 degree (range 101-146) with more than 20 degree of angular deformation in 2 cases. For the complication, 2 cases of temporary axillary nerve injury, 1 case of nonunion, 2 case of delayed unions with angulation, 2 case of ankylosis was observed. Conclusion: LCP can prevent reduction loss and minimize soft tissue injury which result in firm fixation and early immobilization. It seem to be very efficient in treating proximal humerus fracture in elderly.

      • 대장암종에서 암관련 유전자들의 Promoter Methylation과 암종 발생 및 진행과의 관게에 대한 연구

        The colorectal corcinoma is one of the most common malignant tumor in Korea and its carcinogenic mechanism has not been elucidated exactly yet. So far, the most common carcinogenic mechanism has been known to be multiple genetic changes. But, the CpG island hypermethylation has been revealed to be one of the most important carcinogenic mechanism second to the genetic changes. The epigenetic changes characteristically imprint and transfer to the next generation without changes of nucleotide sequences. When the CpG island of promoter or 5'- exon 1 of a specific gene is methylated by DNA methyltrasferases (DNMTs), the gene expression is inhibited. Carcinogenic mechansisms by promoter methylation has been well known, but the progression of the cancer has not been elucidated yet. The molecular researches on the carcinogenic mechanisms largely depended on the tumor tissue containing various components of cell, due to difficulties in harvesting pure homogeneous tumor cells. Promoter methylatonof the cancer related genes as APc, hMLH2, p16, DAP-K and MGMT in 50 cases of sporadic colorectal carcinoma was studied. The pure normal colorectal epithelial cells, and cancer cells from superficial and deep invasive portion were harvested respectively by laser captured microdissection(LCM), and DNA was extracted from them. The promoter methylation was studied by methylation specific poymerase reaction (MSP) after bisulfite modification of the DNA and the expression of the genes by immunohistochemistry. The results suggests that promoter methylation of the cancer related genes are closely related with the colorectal carcinogenesis and it occurrs from early phase of carcinogenesis, progressing constantly to the late stage. And the promoter methylation of normal epithelial cells increased by age. The p15, known to be the genes related to hematologic malignancy, was closely related to the colorectal carcinoma as well.

      • 과불소우레탄 공중합체의 합성과 표면특성

        Perflurorourethane copolymers were stnthesized by free-radical copolymerization using a fluorine-containing monomer and urethane monomers as starting materials. The fluorine-containing monomer used was perfluoroalkyl alcohol(FOH), and the urethane monomers used were tolylene 2,4-diisocyanate(TDI), hexamethylene diisocyanate(HDI), and 4,4-methylene bis(phenyl isocyanate)(MDI). The surface properties and surface modification ability of the perfluorourethane copolymers prepared were investigated. The surface free energies of polyethylene(PE) films modified by the perfluorourethane copolymers and their surface modifkcation properties were also investigated.
