      • Dimesogenic compounds consisting of cholesterol and azobenzene‐based moieties: dependence of liquid crystal properties on spacer length and fluorination of the terminal chain

        <P>The liquid crystalline (LC) properties of two series of non-symmetric dimesogenic compounds consisting of cholesterol and azobenzene-based moieties interconnected by ?-oxyalkanoyl spacers of varying length are compared: one series (AOC-n) has an octyloxy chain attached to the azobenzene mesogen unit while the other (AOCF-n) has a perfluoroheptylmethyloxy chain. In general, compounds bearing the fluorinated alkoxy chain exhibited LC properties over a much broader temperature range than those with the alkoxy chain. In addition, the AOC-n series exhibited the chiral smectic C (SmC*), smectic A (SmA) and cholesteric (N*) phases depending on the length of the central spacer, whereas the AOCF-n series favoured the formation of only the SmA phase with the N* phase completely suppressed. Both series showed an odd–even dependence of the isotropization temperature on spacer length.</P>

        Hydration Effect on the Intrinsic Magnetism of Natural Deoxyribonucleic Acid as Studied by EMR Spectroscopy and SQUID Measurements

        The hydration effect on the intrinsic magnetism of natural salmon double-strand DNA was explored using electron magnetic resonance (EMR) spectroscopy and superconducting quantum interference device (SQUID) magnetic measurements. We learned from this study that the magnetic properties of DNA are roughly classified into two distinct groups depending on their water content: One group is of higher water content in the range of 2.6-24 water molecules per nucleotide (wpn), where all the EMR parameters and SQUID susceptibilities are dominated by spin species experiencing quasi one-dimensional diffusive motion and are independent of the water content. The other group is of lower water content in the range of 1.4-0.5 wpn. In this group, the magnetic properties are most probably dominated by cyclotron motion of spin species along the helical π -way, which is possible when the momentum scattering time (τ k) is long enough not only to satisfy the cyclotron resonance condition (ωcτ k > 1) but also to induce a constructive interference between the neighboring double helices. The same effect is reflected in the S-shaped magnetization-magnetic field strength (M-H) curves superimposed with the linear background obtained by SQUID measurements, which leads to larger susceptibilities at 1000 G when compared with the values at 10,000 G. In particular, we propose that the spin-orbital coupling and Faraday's mutual inductive effect can be utilized to interpret the dimensional crossover of spin motions from quasi 1D in the hydrate state to 3D in the dry state of dsDNA.

        대한방사선종양학회지 게재 논문의 통계적 오류 현황

        목 적: 본 연구는 대한방사선종양학회지 게재 논문의 통계 오류 현황을 파악하고 이에 근거한 문제 제기를 통해 학회지의 학술적 발전에 기여하고자 하였다. 대상 및 방법: 2006∼2007년 사이에 대한방사선종양학회지에 게재된 총 77편의 논문을 연구 대상으로 하였다. 각각의 논문에 적용된 통계 방법론의 적정성 평가는 통계점검표를 활용하였다. 통계점검표에는 연구의 종류, 통계기법의 사용 범위, 각 논문에 적용된 통계 기법의 종류, 통계적 기법 적용의 타당성 항목이 포함되었다. 통계 오류는 ‘생략의 잘못’과 ‘시행의 잘못’ 항목으로 나누었다. 한 논문에서 서로 다른 항목이 여러 가지 관찰된 경우 각각 횟수로 측정하였다. 같은 항목이 2회 이상 측정된 경우 1회로 횟수를 측정하였다. 통계 전문가가 개별 논문을 대상으로 통계점검표를 작성하였다. 일차 평가자가 방사선종양학 전문가가 아닌 것에서 올 수 있는 평가 오류가 있을 수 있으므로 개별 통계점검표는 간행위원회에서 한 차례 더 세부 점검을 하였다. 작성된 통계점검표의 통계 분석은 SAS (version 9.0, SAS Institute, NC, USA) 소프트웨어를 이용하였고 빈도분석을 시행하여 각 항목의 빈도와 백분율을 산출하였다. 결 과: 총 77편 중 원문을 다운로드 할 수 없었던 4편을 제외한 73편의 게재 논문을 대상으로 평가하였다. 증례보고는 5편, 원저 논문은 68편이었다. 대상 논문 중 46편의 논문에서 통계적 추론을 사용하였고, 16편의 논문에서 단순한 기술통계를 사용했으며, 11편의 논문에서는 통계적 기법을 사용하지 않았다. 추론통계를 사용한 46편의논문에서 사용된 추론통계의 횟수는 각각 분할표분석 17회(37.0%), 비교통계분석 23회(50.0%), 회귀분석 7회(15.2%), 상관분석 5회(10.9%), 생존분석이 27회(58.7%)였다. 통계 기법을 활용하여 연구 결과를 분석할 때 통계적용의 오류가 없는 논문은 19%였다. ‘생략의 잘못’은 34편(50.0%)의 논문에서 총 50회 관찰되었다. ‘시행의 잘못’은 35편(51.5%)의 논문에서 총 47회 관찰되었다. ‘생략의 잘못’과 ‘시행의 잘못’이 모두 발견된 논문은 21편 (30.9%)이었다.결 론: 대한방사선종양학회지에 게재된 논문에서 통계 분석 과정의 다양한 영역에 걸쳐 크고 작은 통계적 오류가 있음을 확인하였다. 향후 대한방사선종양학회지 투고 논문의 심사 과정에서 통계 오류에 관한 적절한 심사를 추가하는 것이 필요하다고 판단된다. Purpose: To improve the quality of the statistical analysis of papers published in the Journal of the Korean Society for Therapeutic Radiology and Oncology (JKOSTRO) by evaluating commonly encountered errors. Materials and Methods: Papers published in the JKOSTRO from January 2006 to December 2007 were reviewed for methodological and statistical validity using a modified version of Ahn’s checklist. A statistician reviewed individual papers and evaluated the list items in the checklist for each paper. To avoid the potential assessment error by the statistician who lacks expertise in the field of radiation oncology; the editorial board of the JKOSTRO reviewed each checklist for individual articles. A frequency analysis of the list items was performed using SAS (version 9.0, SAS Institute, NC, USA) software. Results: A total of 73 papers including 5 case reports and 68 original articles were reviewed. Inferential statistics was used in 46 papers. The most commonly adopted statistical methodology was a survival analysis (58.7%). Only 19% of papers were free of statistical errors. Errors of omission were encountered in 34 (50.0%) papers. Errors of commission were encountered in 35 (51.5%) papers. Twenty-one papers (30.9%) had both errors of omission and commission. Conclusion: A variety of statistical errors were encountered in papers published in the JKOSTRO. The current study suggests that a more thorough review of the statistical analysis is needed for manuscripts submitted in the JKOSTRO.

        자가면역성 다선증후군 1 예

        The polyglandular autoimmune syndrome designates the dysfunction of endocrine and nonendocrine system involving two or more organs on the basis of an autoimmune mechanism. The autoimmune nature of these diseases has been based on the presence of lymphocytic infiltration in the affected gland, organ specific autoantibodies in the serum, cellular immune defects and an association with the HLA DR/DQ genes or immune response genes. This syndrome is usually classified into three classes and their etiology or pathogenesis is still not completely understood. A 28-year-old woman developed vitiligo and insulin dependent diabetes mellitus during the treatment of Graves' disease with antithyroid drug. She had a tendency of spontaneous ketonemia and serum c-peptide levels were low(0.21, 0.16ng/mL: fasting and glucagon stimulated). Thyrotrophin binding inhibitor immunoglobulin and pancreas iIslet cell cytoplasmic antibody were positive. We report here a case of polyglandular autoimmune syndrome, type III manifesting Graves' disease, vitiligo, and insulin dependent diabetes mellitus(J Kor Soc Endocrinol 10: 418-423, 1995).

        Graves' disease occurs in about 3 to 6% of patients with myasthenia gravis, and approximately l% of patients with Graves' disease develop myasthenia gravis, These associations are of interest in view of the frequent association of thymic enlargement with Graves' disease. Further, antibodies against specific receptors, i.e., the TSH receptor and the acetylcholine receptor, are involved in the pathogenesis of the two diseases. We experienced a 20-year-old man who presented with Graves' disease(primary hyperthyroidism with diffuse goiter and antithyroid autoantibodies) and isolated ocular myasthenia gravis. Chest CT revealed homogenously enhanced enlarged thymus. Following restoration of euthyroidism with propylthiouracil, ocular myasthenic symptom persisted. We performed near total thyroidectomy and thymectomy. Histologic examination showed thyroid and thymic hyperplasia. The patient is followed up with anticholinesterase medication after operation. The effects of both thyrotoxicosis and its alleviation on the course of myasthenia gravis is variable, but in the majority of instances, myasthenia is accentuated during the thyrotoxic state and improves when a eumetabolic state is restored, Recently, we experienced a case of concomitant Graves disease and myasthenia gravis, so we reported the case with literature review.

        A Web-based 'Patterns of Care Study' System f or Clinical Radiation Oncology in Korea

        목 적: PCS 연구의 목적은 임상 방사선치료 QA의 표준 연구수단을 구축하고,방사선치료과 정의 기본요소를 결정하고, 협동 임상시험의 기반을 제공하고,외국과의 비교 연구를 위한 기본자료의 구축에 있다.우리나라 임상 방사선종양학의 PCS 연구기법으로서 구축한 웹 기반의 PCS시스템의 개발 및 특성을 보고하고자 한다. 대상 및 방법 : 본 시스템은 메인서버와 외부에 구축한 2중 백업서버로 구성되어 있다. 리눅스 운영 체제 및 MySQL 데이터베이스를 이용하여 프로그램을 완성하였다. PCS 대상이 되는 종양으로서의 개발 우선 순위는 1989년부터 1999년 사이에서 우리나라 전국의 방사선종양학과에서 치료빈도가 많은 순서를 근거로 결정하였다. 결 과: 2003년 초반에 웹 기반의 임상 PCS시스템이 구축되었다. 웹 주소는 www.pcs.re.kr이며, 우 선 유방암,직장암,식 도암, 후두암, 폐암 및 전이성 뇌종양을 대상으로 하였다. 완성된 개발 항목의 수는 1,000항목을 상회하였다.구축된 시스템은 사용자 편의성 충족, 기입 내용 중복 확인, 자료 기밀성, 자료 보존 및 사본 작성,이중 백업,통계 분석 지원 등 을 구현하였다. 문자화된 자료 외에 영상자료도 입력되도록 시스템을 구축하였다.아울러,임상시험승인 관련양식 지원, 무작위 자료 추출,방사선종양학과의 구조적 요소 파악 기능도 갖추었다. 결 론: 본 시스템은 PCS분야에서는 최초로 구축되고 작동되는 웹 기반 시스템이다.구축한 시스템을 통해서 단시간 내에 표본 자료의 수집이 가능하여 시간 및 비용을 절약할 수 있게 되었으나 구축자료의 신뢰도 확인을 별도의 검증과정은 필수적이다. 본 연구팀이 개발한 본 웹기반시스템은 향후 PCS 연구 또는 자료수집이 필요한 유사연구상황에 응용될 수 있는 표준적인 기법으로서의 활용되기를 희망한다. Purpose We report upon a web-based system for Patterns of Care Study (PCS) devised for Korean radiation oncology. This PCS was designed to establish standard tools for clinical quality assurance, to determine basic parameters for radiation oncology processes, to offer a solid system for cooperative clinical studies and a useful standard database for comparisons with other national databases. Materials and Methods: The system consisted of a main server with two back-ups in other locations. The program uses a Linux operating system and a MySQL database. Cancers with high frequencies in radiotherapy departments in Korea from 1998 to 1999 were chosen to have a developmental priority. Results: The web-based clinical PCS system for radiotherapy in www.pcs.re.kr was developed in early 2003 for cancers of the breast, rectum, esophagus, larynx and lung, and for brain metastasis. The total number of PCS study items exceeded one thousand. Our PCS system features user-friendliness, double entry checking, data security, encryption, hard disc mirroring, double back-up, and statistical analysis. Alphanumeric data can be input as well as image data. In addition, programs were constructed for IRB submission, random sampling of data, and departmental structure. Conclusion: For the first time in the field of PCS, we have developed a web-based system and associated working programs. With this system, we can gather sampled at a in a short period and thus save, cost, effort and time. Data audits should be performed to validate input data. We propose that this system should be considered as a standard method for PCS or similar types of data collection systems.
