        단일 대학병원 폐렴 입원환자의 Levofloxacin 사용 적정성 평가

        Background : Despite recent advances in diagnosis and treatment, pneumonia causes great morbidity and mortality and entails high healthcare costs. Levofloxacin is one of the recommended drugs for the treatment of pneumonia. However, the increase in the use of levofloxacin may lead to an increase in resistance to this medication. The purpose of this study was to evaluate the rate of levofloxacin use to prevent resistance to the drug as per the developed criteria (referenced with Micromedex®, KIMS® and KFDA) and to provide information for implementation of pharmaceutical intervention for the optimal intravenous-to-oral (IV-to-PO) switch. Methods : This study was conducted within pneumonia patients discharged from the pulmonology ward at the Ulsan University Hospital June 1, December 31, 2018. The evaluation criteria were as follow: treatment dose, duration of the treatment, potential drug-drug interactions (pDDIs), evaluation of implementation of essential examinations and adverse drug reaction (ADR) monitoring, and the optimal IV-to-PO switch. The electronic medical record (EMR) database was used to retrospectively collect data. Results : In total, 125 patients were included, of whom 17 were later excluded from the evaluation of the treatment period, and optimal IV-to-PO switch from death during the treatment. Dose adjustment according to renal function was required in 59 (47.2%) patients, four of whom received a suitable dose. The treatment period was appropriate in 66.7% patients. Also, 34.4% patients did not receive concomitant drugs that could interact with levofloxacin, and the implementation rate of essential examination was high except for therapeutic drug monitoring. Additionally, 46.3% of patients were suitable for the optimal IV-to-PO switch. Although no ADRs were reported during ADR monitoring, several symptoms including itching, and elevated LFT were recorded by the EMR. Conclusion : Through evaluating of use of levofloxacin in pneumonia patients, we confirmed that practical mediation by pharmacists and improvement of the system are necessary.

        Zinc-Triggered Induction of Tissue Plasminogen Activator and Plasminogen in Endothelial Cells and Pericytes

        Cerebral amyloid angiopathy (CAA) is common in patients with Alzheimer’s disease (AD) and may contribute to cerebralhemorrhage. We previously demonstrated that tissue plasminogen activator (tPA) and plasminogen (PLG) accumulated at theperiphery of compact amyloid-cored plaques and in the walls of CAA-containing blood vessels in the brains of Tg2576 mice, awidely used AD mouse model. We had also observed that zinc-triggered tPA and PLG induction were observed in mouse corticalcultures. Because zinc also accumulates in amyloid plaques and blood vessel walls in AD brains, we examined whether zinc increasesmRNA and protein levels of tPA and PLG in brain endothelial cells and pericytes. Four hours after the exposure of brain endothelialcells (bEnd.3) to 40 μM zinc, the mRNA and protein expressions of tPA and its substrate PLG were significantly increased. In the caseof brain pericyte cultures, increases in tPA and PLG expression were also detected 2 hr after treatment. However, amyloid-β (Aβ)1-42oligomers did not augment tPA and PLG expression in bEnd.3 cells and pericytes, suggesting that zinc but not Aβ induces tPA andPLG accumulation in CAA found in the AD brain.

        가금류 생균제 개발을 위한 Lactobacillus fermentum YL-3의 분리 및 생리 특성

        국내 환경적 요인을 감안하여 국내의 닭에서 젖산균을 분리하고 각 균주들의 내산성, 내담즙성, 내열성, 병원성균 성장 억제 능력등을 조사하여 가장 성상이 우수한 균주를 선발함으로써 생균제용 균주의 선발과 산업적으로 이용 가능성을 조사하였다. pH 3.5에서 1차적으로 내산성이 있는 139균주를 분리하였으며, 0.3% 담즙에 대한 내성이 있는 111균주를 분리하였다. 산 생성 및 기타 특성을 조사한 후 최종적으로 34 균주를 선발한 후 그 중 24시간에 가장 빠른 성장률을 보이고 pH 2.0에서 3시간동안 정치했을 때 가장 많은 생존율을 나타낸 Lactobacillus fermentum YL-3을 생균제 가능 균주로 선발하였다. YL-3의 내산성은 pH3.0에서 3시간 동안 60%의 생존률을 나타내고, pH 2.5는 50%, pH2.0에서는 40%의 생존율을 나타내었다. 그러나 pH 2.0에서 12시간 동안에도 사멸하지 않고 10^7 CFU/ml의 생균수를 유지하였다. YL-3의 내담즙성은 0.3% 담즙에 대하여 사멸되거나 억제되지 않고 12시간 동안에 성장을 보였다. 또한 0.5, 1.0, 2.0%가 첨가된 액체배지에서 24시간의 배양 후에도 같은 경향을 나타내었다. YL-3의 내열성은 45℃에서는 생균수의 큰 감소 폭을 나타내지 않았지만 그 이상의 온도에서는 급격히 생균수가 감소하는 것을 볼 수 있었다. Sal. enteritidis, E. coli와의 혼합 배양에서 Sal. enteritidis는 혼합 배양한지 15시간만에, E. coli는 16시간만에 억제효과를 나타내었다. This study was performed to screen lactic acid bacteria in domestic poultry for the probiotic use. Among the previously obtained acid tolerant, 139 strains, 11 strains were selected with MRS medium containing 0.3% oxgall. 34 strains of 111 was reselected by Gram-staining and acid producing ability. These strains was identified by MIDI Sherlock Microbial Identification System. Among the identified 34 strains Lactobacillus fermenum YL-3 was selected for the final probiotic use because of the good growth and high survival rate at pH 2.0. 60%, 50% and 40% cells of Lactobacillus fermentum YL-3 survived at pH 3.0, 2.5 and 2.0, respectively. More than 10^7 CFU/ml survived when exposed with the number of 10^8 CFU/ml at pH 2.0 after 12 hr. L. fermenum YL-3 maintained growth in MRS broth containing 0.3, 0.5, 1.0 and 2.0% oxgall for 24 hr. L. fermenum YL-3 showed an inhibitory effect against pathogenic strains of Sal. enteritidis and E. coli O157:H7. In mixed culture with L. fermenum YL-3 Sal. enteritidis lost ability completely in 15 hrs and E. coli O157:H7 in 16hrs.

        한국어 교육에서 ‘-겠’과 ‘-(으)ㄹ 것이다’의 교육 방안 연구

        】이 글은[의지], [추측]의 ‘ -겠-’과‘-(으)ㄹ 것이다’에 대한 기존의 선행 연구와‘ -겠-’과‘-(으)ㄹ 것이다’가 가지는 각각의 의미를 살펴본 것이며 또한 ‘-겠-’이 가지는 통사적 제약도 살펴보았다. ‘-겠-’과‘-(으)ㄹ 것이다’ 는 상태 동사와 결합하느냐 동작 동사와 결합하느냐에 따라 의미가 달라 진다. 이런 상황으로 보건대 이 두 양태 표현의 의미 변별을 학습자에게 교수하는데 있어 교수·학습자 모두 고충을 겪을 것으로 보인다. 따라서 ‘- 겠-’과‘-(으)ㄹ 것이다’ 의 문장의 의미 파악을 좀 더 쉽게 할 수 있는 방법을 모색하였다. 이 연구로 한국어 학습자들이 ‘-겠-’과‘-(으)ㄹ 것이다’를 배우는 데 부담을 덜고 두려움을 해소하여 의사소통 기능 향상에 도움이 되길 바란다. ‘-겠-’, ‘-(으)ㄹ 것이다’ connotes conjecture and intention. These two expressions are of high frequency but studies on differentiating the meaning of two have not reached a conclusion due to the ambiguity of the two expressions. It is not difficult to assume that beginning·intermediate level students have difficulty in understanding emotional expressions in conversation. It is certain to be confronted with a problem when expressing their own manner or view, for the learners. This paper studied preceding research on ‘-겠-’, ‘-(으)ㄹ것이다’, which conveys meaning of intention and conjecture, and their respective features. The syntactic restriction of ‘-겠-’ is studied as well. When ‘-겠-’ is used to express intention, the subject should be the first person. Teaching·learning the differentiation in the meaning of these two expressions in the second·third person sentence, is a dilemma. Thus in this paper methods to make learners better comprehend ‘-겠-’, ‘-(으)ㄹ 것이다’ are studied.

        Use of Combined Oral Contraceptives in Perimenopausal Women

        While perimenopausal women have low fecundity, they are still capable of becoming pregnant and the majority of pregnancies occurring during perimenopause are unintended pregnancies. Therefore, even during perimenopause, contraception must be used if unintended pregnancies are to be avoided. However, many perimenopausal women and healthcare providers believe that older people should not take combined oral contraceptives (COC) because doing so may be dangerous. However, to date, there is no evidence that taking COC presents an increased risk of cardiovascular events or breast cancer for middle-aged women as compared to other age groups, and in their recommendations, the Centers for Disease Control and Prevention (CDC) also do not list age itself as a contraindication for COC. Perimenopausal women often experience menstrual irregularity, heavy menstrual bleeding, and vasomotor symptoms. Taking COCs can help control these symptoms and significantly reduce the risk of ovarian cancer, endometrial cancer, and colorectal cancer. The objective of the present review is to examine the usage methods of COC among perimenopausal women and the health issues that may arise from taking COC in perimenopausal women.
