        급성 미로염으로 진단받은 환자의 한방 치험 1례

        Objectives : The purpose of this study is to report the efficacy of using oriental medical treatment with acute labyrinthitis diagnosed patient.Methods : We experienced one case of acute labyrinthitis diagnosed patient treated with oriental medical treatment. To evaluate the results of this treatment, we measured the hearing recovery, decrease of tinnitus and vertigo. Decrease of tinnitus and vertigo was assessed by THI and Visual Analogue Scale(VAS).Results : VAS of tinnitus decreased from 1.5 to 0.2, and vertigo decreased from 2 to 0. THI decreased from 41 to 25.Conclusions : This study suggests that using oriental medical treatment is effective on Acute labyrinthitis.

        Wernicke Disease is well known of nutriotional disorders Wernicke Disease is charaterized by ataxia, confusion and oculomotor disturbance. We diagnosis one patient, who has ataxia, confusion and oculomotor disturbance as Jusang(酒傷), and gave saenggangunbi-tang, bojungchiseub-tang and samchulgunbi-tang. We observed that clinical symptom and Liver function were improved by these herbal medicine Therefore We consider Wernicke Disease as Jusang(酒傷) and further reports with many case, however, will be needed.

        국내 아메리칸 핏불테리어에서 발생한 열사병 증례 보고

        Heat stroke in a dog is a life-threatening syndrome characterized by a high body temperature over $41^{\circ}C$, by the central nervous system dysfunction, and by multiple organ dysfunction. A 11-month-old male American Pit Bull Terrier was presented to clarify the cause of death to diagnostic laboratory. This dog showed clinical signs such as high body temperature ($42^{\circ}C$), severe tachypnea, hematochezia, epistaxis and hemoptysis after transportation at hot summer time. At necropsy, there were hemorrhages in skin, serosa of stomach and small intestine, and also dark red fluid in lumen of intestine. Histopathologic examination revealed extensive hemorrhages in stomach, muscle, skin, and tongue. In microbiology, pathologic bacteria such as Pasteurella, Boedetella, Salmonella, Clostridium, and circulating virus in Korea such as CDV, CIV, CAV, CHV, CCV, CPIV were negative except CPV. There were many gross and microscopic lesions suggesting hest stroke while pathologic agents and related lesions were not detected. Therefore we diagnosed this case as heatstroke.

        Teaching and learning were carried out long before the word "education" was coined. As teaching and learning became more universal, the word "education" was construed as a social promise, and there was a general consensus as to what it denoted. Many university professors will most likely have great confidence and expertise with respect to "teaching" in their area of specialization, and they believe that they are fulfilling a social promise. However, how much expertise do they have in actually making students "learn"? How concerned are professors about enabling students to utilize their potential and talents to cultivate their learning abilities and to adjust to the different demands of various fields? The same issue arises in medical education. To what extent can professors' teaching heighten students' sense of purpose and motivation to learn? With regard to increasing learners' initiatives, the learning model of constructivism presupposes that learners are active and creative, have their own personalities, and possess unlimited learning potential. The PBL being carried out in medical schools today is a form of study that can take advantage of these aspects of learners. They can maximally widen the range of students' development through many intellectual activities and solve difficult problems by either sharing or critiquing the thoughts and ideas of others. The acts of teaching and learning that have been carried out for thousands of years remain difficult to this day and must be ceaselessly deliberated and researched by experts in the field of education. Just as good teachers are required to produce good learners, we must give ourselves room to rethink the basis of education in order to maximize effective and efficient learning.

        양안에 발생한 급성 황반 신경망막병증 1예

        Purpose: To report a case of bilateral acute macular neuroretinopathy (AMN) following flu-like illness. Case summary: A 42-year-old woman complained of paracentral scotoma in both eyes 2 weeks after an episode of flu. Fundus examination showed several dark, brown-reddish, wedge- shaped lesions in both maculae, and red-free fundus photography confirmed the lesions. Fluorescein and indocyanine green angiography was normal. Amsler grid test demonstrated paracentral scotoma. The multifocal electroretinogram (ERG) response was subnormal, and ocular coherence tomography (OCT) confirmed an impaired photoreceptor layer in both eyes. Under the diagnosis of AMN, the patient was followed- up without treatment. No subjective changes were observed during follow-up, although the damage to the photoreceptor was partially recovered based on spectral-domain OCT (SD-OCT). Conclusions: Multifocal ERG and SD-OCT may be helpful to diagnose AMN. Despite partial recovery of clinical signs on SD-OCT, paracentral scotoma could be sustained for a long period of time.

        인문사회의학의 의사국가시험 도입에 대한 인식도 조사

        Purpose: This study aimed at investigating the current situation of Medical Humanities and Sociology-related-curriculum in Korean medical schools, and suggesting the courses of study for the contents and methods of the Medical Humanities and Sociology examination, which can be included in the National Examination for Medical Practitioners. Methods: We analyzed Medical Humanities and Sociology-related courses which are offered in Korean medical schools, and a survey was conducted by medical school professors and students and medical journalists. In the survey, the Medical Humanities and Sociology-related courses were divided into 8 parts, and the participants were asked to evaluate the importance of duty, necessity of education, necessity of evaluation and the evaluation method of each part using a seven-point scale. Results: A total of 207 medical school professors and students and 9 medical journalists participated in the survey. The results were similar for the importance of duty and necessity of education of each part, but those for the necessity of evaluation were different. - As a result, there seems to be a gap between the importance of duty and the importance of education of each course. Medical journalists and students group answered differently on the necessity of evaluation of each course was also reserved. Conclusion: It is necessarily recommended to include Medical Humanities and Sociology-related courses such as medical ethics, self-improvement and doctors' social responsibilities in the National Examination for Medical Practitioners.

        보폐정천탕(補肺定喘湯)을 이용한 아토피 피부염 치료(治療)에 관한 임상연구

        The purpose of this study was on effect of Bopejungcheuntang(BPJC) for atopic dermatitis. During 5 months between January and May 2000, BPJC water extract administered to patient suffering from atopic dermatitis, in Dep. of Pediatrics, Dongguk university Pundang Oriental Hospital and this study was conducted for 27 patient (male 17, female 10) The result were as follows 1. The distribution of remedial values were improvement (51.9%), unresponsiveness(37.0%), depravation(11.1%). 2. After dispensing BPJC, ordinal sites of atopic dermatitis were inner space of elbow and knee joint, face and neck, back and abdomen, wrist and ankle. Befor and after dispensing BPJC this ordinal sites were similar but after dispensing BPJC, the other sites except head is improved. Order of symptomatic improvement is itching, licheniasis, erythema, exudate. Both itching and exudate is significantly improved. 3. According to way of water extract, symptomatic improvement is most excellent with general water extract & distiled water, the second is general water extract, the third is distiled water. 4. BPJC effect on atopic dermatitis is not significantly affected by sex, day of falling ill, medical treatment, resident environment, feeding, character and duration of medical treatment.

        Inhibition of mitoNEET induces Pink1-Parkin-mediated mitophagy

        MitoNEET, a mitochondrial outer membrane protein containingthe Asn-Glu-Glu-Thr (NEET) sequence, controls the formation ofintermitochondrial junctions and confers autophagy resistance. Moreover, mitoNEET as a mitochondrial substrate undergoes ubiquitinationby activated Parkin during the initiation of mitophagy. Therefore, mitoNEET is linked to the regulation of autophagy andmitophagy. Mitophagy is the selective removal of the damagedor unnecessary mitochondria, which is crucial to sustaining mitochondrialquality control. In numerous human diseases, the accumulationof damaged mitochondria by impaired mitophagyhas been observed. However, the therapeutic strategy targetingof mitoNEET as a mitophagy-enhancing mediator requires furtherresearch. Herein, we confirmed that mitophagy is indeed activatedby mitoNEET inhibition. CCCP (carbonyl cyanide m-chlorophenylhydrazone), which leads to mitochondrial depolarization, inducesmitochondrial dysfunction and superoxide production. This, inturn, contributes to the induction of mitophagy; mitoNEET proteinlevels were initially increased before an increase in LC3-Ⅱ proteinfollowing CCCP treatment. Pharmacological inhibition ofmitoNEET using mitoNEET Ligand-1 (NL-1) promoted accumulationof Pink1 and Parkin, which are mitophagy-associated proteins,and activation of mitochondria–lysosome crosstalk, in comparisonto CCCP alone. Inhibition of mitoNEET using NL-1, or mitoNEETshRNA transfected into RAW264.7 cells, abrogated CCCP-inducedROS and mitochondrial cell death; additionally, it activated theexpression of PGC-1α and SOD2, regulators of oxidative metabolism. In particular, the increase in PGC-1α, which is a majorregulator of mitochondrial biogenesis, promotes mitochondrialquality control. These results indicated that mitoNEET is a potentialtherapeutic target in numerous human diseases to enhancemitophagy and protect cells by maintaining a network of healthymitochondria.

        환자안전 교육과정 개발 사례 연구

        Patient safety is achieved through systematic improvement based on the knowledge and willingness of medical professionals. A systematic longitudinal curriculum for patient safety is essential to prepare medical students and professionals. The purpose of this article is to introduce our experience with a 'workshop for developing a patient safety curriculum' and to compare the results with previous studies. The workshop comprising 15 medical professors and patient safety experts met for 2 days. The Consensus Workshop method was applied, collecting opinions from all of the members and reaching consensus through the following stages: context, brainstorm, cluster, name, and resolve. The patient safety curriculum was developed by this method, covering patient safety topics and issues, and teaching and assessment methods. A total of 7 topics were extracted, 'activities for patient safety, concepts of patient safety, leadership and teamwork, error disclosure, self-management, patient education, policies.' Issues, teaching methods, and assessment methods were developed for each topic. The patient safety curriculum developed from the workshop was similar to previous curricula developed by other institutions and medical schools. The Consensus Workshop method proved to be an effective approach to developing a patient safety curriculum.
