        패밀리 레스토랑 가격공정성이 지각가치와재의용 의도에 미치는 영향

        This essay researches the influence of the price fairness are to investigate the relationship between price fairness, various factors and to examine the pricing of family restaurant or the factors to control to family restaurant industry when there are negative perceived value and degree of reuse, so that it establishes the marketing strategy for energizing the family restaurant industry. The implications of the research result are as follows. Firstly, even though the significant relations between all factors of price fairness and perceived value was not investigated in the relationship between price fairness and perceived value, price fairness factors had effect on quality and value perception directly and indirectly and consisting concepts influenced with correlations. Secondly, the relations between price fairness and degree of reuse was carried by perceived value in the relationship between price fairness and degree of reuse by perceived value. 본 연구는 패밀리 레스토랑 가격공정성이 다양한 요인들 간의 영향 관계를 분석하여 패밀리 레스토랑의 가격책정이나, 부정적인 지각가치와 재이용 의도 시 패밀리 레스토랑산업에 적용하여 통제할 수 있는 요인을 탐색하여 패밀리 레스토랑 산업의 시장 활성화를 위한 마케팅전략을 수립하는 데 본 연구의 목적이 있다. 연구의 결과를 바탕으로 확인된 시사점은 다음과 같다. 첫째, 가격공정성과 지각가치와의 관계에서는 가격공정성 모든 요인들의 지각가치 요인들과 유의한 관계가 확인되지 않았으나, 가격공정성 요인들이 품질지각과 가치지각에 직접적으로 영향을 미치고 있으며, 구성 개념 간에는 상호관련성을가지고 영향력을 미치는 것으로 나타났다. 둘째, 지각가치와 재 이용의도 관계에서는 지각가치에 고객이 만족하면 할수록구매의사는 분명한 것으로 밝혀졌다. 지각되어지는 상품이나, 가치, 시설지각에대한 인식의 정도가 높게 나타나면 재이용 의도가 강하게 나타날 수 있음을 확인할 수 있었다. 셋째, 지각가치에 따른 가격공정성과 재이용 의도의 관계에서는 지각가치가 가격공정성과 재이용 의도 간 관계를 매개하는 것으로 나타났다. 실제로 패밀리레스토랑을 이용한 후에는 지불한 비용에 상응하여 제공받은 상품과 서비스의 긍정적인 가치를 통해 재이용 의도에 긍정적인 요소로 작용하고 있으며, 이는 서비스산업의 특성상 제시된 가격이 불공정하다고 지각하여도 재이용 의도에 변함이 없는것으로 판단된다.

        Comparison of Genotypes and Enterotoxin Genes Between Staphylococcus aureus Isolates from Blood and Nasal Colonizers in a Korean Hospital

        In this study, we investigated the genetic background of 70 Staphylococcus aureus isolates (36 methicillin-resistant S. aureus [MRSA] and 34 methicillin-susceptible S. aureus [MSSA]) obtained from blood at a Korean tertiary-care hospital, using spa typing, multilocus sequence typing, and SCCmec typing. In addition, the prevalence of enterotoxin (sea, seb, sec, sed, see, seg, seh, sei, and sek), tst, and pvl genes among the samples was assessed via polymerase chain reaction, and the results were compared with those of 95 isolates of S. aureus obtained from nasal swabs. All MRSA isolates from blood, except one, belonged to three major clones: sequence type (ST)5-MRSA-II, ST72-MRSA-II (or IVA), and ST239-MRSA-III, among which ST5-MRSA-II was the predominant clone. The prevalence of enterotoxin genes in the S. aureus isolates obtained from blood differed significantly from those from the nasal swabs for the sea, seb, sec, and seh gene. In particular, the seb and sec genes were detected exclusively in the MRSA isolates of ST5 or spa-CC002, thereby suggesting the co-adaptation of virulence genes with the genetic background and their contribution to biological fitness.

        광범위 약제내성 Streptococcus pneumoniae에 의한 급성 폐렴구균 파종성 감염 1례: 뇌수막염과 화농성 관절염 동반

        The emergence and spread of antimicrobial resistance threatens the successful treatment of invasive pneumococcal disease (IPD). We report a case of disseminated pneumococcal infection with meningitis, spondylitis, and septic arthritis caused by an extremely drug-resistant strain of Streptococcus pneumoniae, which was resistant to at least one agent in all classes but was sensitive to vancomycin and linezolid. The patient was treated successfully using intravenous vancomycin (1 g every 12 h) and shoulder surgery. The serotype of this isolate was 15A, a nonvaccine type, and multi-locus sequence typing (MLST) revealed ST8279. MLST analysis and antimicrobial susceptibility test revealed that the strain, SMC1710-32, belonged to ST8279, with the same molecular characteristics with drug susceptibility as extensively drug-resistant (XDR) clone 11A-ST8279 in the previous studies. These XDR pneumococcal strains, serotype 11A and 15A possessed identical molecular characteristics including multiple mutated genes involved in very-high-level resistance to various drugs. The difference in serotype was due to large scale recombination for serotype switching. Further surveillance and investigation of the serotype distribution and genotypes of XDR strains are essential to prevent their spread.

        Comparison of virulence between matt and mucoid colonies of Klebsiella pneumoniae coproducing NDM-1 and OXA-232 isolated from a single patient

        Nine Klebsiella pneumoniae isolates coproducing NDM-1 and OXA-232 carbapenemases were successively isolated from a single patient. Although they were isolated simultaneously and were isogenic, they presented different colony phenotypes (matt and mucoid). All nine isolates were resistant to most antibiotics except colistin and fosfomycin. In addition, matt-type isolates were resistant to tigecycline. No differences were detected in the cps cluster sequences, except for the insertion of IS5 in the wzb gene of two matt-type isolates. In vitro virulence assays based on production of capsular polysaccharide, biofilm formation, and resistance to human serum indicated that the mucoid-type isolates were significantly more virulent than the matt-type. In addition, mucoid-type isolates showed higher survival rates than the matt-type ones in infection experiments in the fruit fly, suggesting a higher virulence of K. pneumoniae isolates with a mucoid phenotype. To our knowledge, this is the first report of K. pneumoniae colonies with different phenotypes being isolated from the same sample. In addition, we show that virulence varies with colony phenotype. Dissemination of K. pneumoniae isolates expressing both antibiotic resistance and high virulence would constitute a great threat.

        Post-influenza Pneumonia Caused by the USA300 Community- Associated Methicillin-Resistant Staphylococcus aureus in Korea

        Panton-Valentine leukocidin (PVL)-positive USA300 clone has been the most successful community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) clone spreading in North America. In contrast, PVL-negative ST72-CA-MRSA has been predominant in Korea, and there has been no report of infections by the USA300 strain except only one case report of perianal infection. Here, we describe the first case of pneumonia caused by the USA300 strain following pandemic influenza A (H1N1) in Korea. A 50-year-old man was admitted with fever and cough and chest radiograph showed pneumonic consolidation at the right lower lung zone. He received a ventilator support because of respiratory failure. PCR for pandemic influenza A (H1N1) in nasopharyngeal swab was positive, and culture of sputum and endotracheal aspirate grew MRSA. Typing of the isolate revealed that it was PVL-positive, ST 8-MRSA-SCCmec type IV. The analysis of the PFGE patterns showed that this isolate was the same pulsotype as the USA300 strain.

        First Imported Case of Skin Infection Caused by PVL-positive ST30 Community-Associated Methicillin-Resistant Staphylococcus aureus Clone in a Returning Korean Traveler from the Philippines

        Although pandemic community-associated (CA-) methicillin-resistant Staphylococcus aureus (MRSA) ST30 clone has successfully spread into many Asian countries, there has been no case in Korea. We report the first imported case of infection caused by this clone in a Korean traveler returning from the Philippines. A previously healthy 30-yr-old Korean woman developed a buttock carbuncle while traveling in the Philippines. After coming back to Korea, oral cephalosporin was given by a primary physician without any improvement. Abscess was drained and MRSA strain isolated from her carbuncle was molecularly characterized and it was confirmed as ST30-MRSA-IV. She was successfully treated with vancomycin and surgery. Frequent international travel and migration have increased the risk of international spread of CA-MRSA clones. The efforts to understand the changing epidemiology of CA-MRSA should be continued, and we should raise suspicion of CA-MRSA infection in travelers with skin infections returning from CA-MRSAendemic countries.

        Severity-Adjusted Dexamethasone Dosing and Tocilizumab Combination for Severe COVID-19

        Purpose: Real-world experience with tocilizumab in combination with dexamethasone in patients with severe coronavirus disease(COVID-19) needs to be investigated. Materials and Methods: A retrospective cohort study was conducted to evaluate the effect of severity-adjusted dosing of dexamethasonein combination with tocilizumab for severe COVID-19 from August 2020 to August 2021. The primary endpoint was30-day clinical recovery, which was defined as no oxygen requirement or referral after recovery. Results: A total of 66 patients were evaluated, including 33 patients in the dexamethasone (Dexa) group and 33 patients in thedexamethasone plus tocilizumab (DexaToci) group. The DexaToci group showed a statistically significant benefit in 30-day clinicalrecovery, compared to the Dexa group (p=0.024). In multivariable analyses, peak FiO2 within 3 days and tocilizumab combinationwere consistently significant for 30-day recovery (all p<0.05). The DexaToci group showed a significantly steeper decreasein FiO2 (-4.2±2.6) than the Dexa group (−2.7±2.6; p=0.021) by hospital day 15. The duration of oxygen requirement was significantlyshorter in the DexaToci group than the Dexa group (median, 10.0 days vs. 17.0 days; p=0.006). Infectious complications andcellular and humoral immune responses against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in the convalescencestage were not different between the two groups. Conclusion: A combination of severity-adjusted dexamethasone and tocilizumab for the treatment of severe COVID-19 improvedclinical recovery without increasing infectious complications or hindering the immune response against SARS-CoV-2.
