색소 과다 침착만으로 조기 발견한 소아 부신백질이영양증 1례

X 연관 부신백질이영양증(adrenoleukodystrophy, X-ALD)는 성염색체 Xq28에 위치한 ABCD1 유전자의 돌연변이에 의해 발생하며, 매우 긴 사슬 지방산(saturated very long chain fatty acids, VLCFA) 운반의 장애로 뇌와 척수의 백질과 부신 피질을 포함한 모든 조직에 VLCFA가 쌓이게 되어 중추와 말초 신경 조직 내에 탈수초(demyelination)가 진행되고, 부신 피질의 기능 저하가 나타나게 된다. X-ALD의 임상적 증상은 매우 다양하나, 크게 3가지 표현형으로 나누어 대뇌 부신백질형성 장애(cerebral ALD), 부신 척수 신경병증(adrenomyeloneuropathy), 부신 피질 기능 저하 즉 애디슨병(Addison's disease only ALD)으로 나눌 수 있다2). 부신 피질 기능 저하 증세만을 보이는 Addison's disease only ALD의 경우 주로 2세에서 7.5세에 발생하며, 증세는 구토, 위약, 혼수, ACTH 분비 증가에 따른 색소 과다 침착(hyperpigmentation)으로 나타나고 발생 당시에는 신경학적 증세가 동반되지 않는다. 저자들은 구역, 구토, 탈수, 저혈당 및 저혈압 등의 증상 없이 색소 과다 침착이 부신 피질 기능 저하의 유일한 증세였던 환아를 혈장 VLCFA 검사를 통해 X-ALD로 진단하였고, ABCD1 유전자 분석 검사에서 c.1992-2A>G 변이를 확인하였다. 국내에서 색소 과다 침착만을 보이는 환아가 X-ALD로 진단받은 보고는 찾을 수 없었으며, 국내에서 보고되지 않은 돌연변이로 이를 보고하는 바이다. X-ALD 환아의 장기적인 예후 예측과 자세한 상담을 위해서 유전자형과 표현형의 상관관계에 대한 더 많은 연구가 필요하며, 이를 위해 유전자형에 따른 증상과 예후에 대한 자료가 공유되어야 할 것이다. The X-linked adrenoleukodystrophy (X-ALD) is a peroxisomal disease by defects of ABCD1 gene on chromosome Xq28 leading to accumulation of saturated very long chain fatty acids (VLCFA), progressive demyelination and adrenal insufficiency. A 4-year-old boy was visited hospital with the chief compliant of hyperpigmentation beginning at 2-years old. Serum adrenocorticotropic hormone (ACTH) and cortisol concentration were compatible with adrenal insufficiency. The elevated plasmatic concentration of VLCFA and genotype analysis with sequencing of ABCD1 gene established the diagnosis of X-ALD. Brain MRI showed no abnormal high signal intensity on the white matter. Steroid replacement was started with good response. He initiated Lorenzo's oil with restriction of VLCFA by reducing the intake of fatty foods. The author highlight the importance of suspecting of X-ALD in the etiology of primary adrenal insufficiency as the first sign of the disease.

미래의 정보전의 환경 하에서 병사들은 전투 상황 중에 각종 정보를 획득하고 이에 신속히 대응하는 하나의 체계(system)로서 전투 임무를 담당하게 될 것이며, 미래의 병사들은 각종 첨단 장비들을 신체에 부착하고 전투에 임하게 될 것이다. 첨단 디지털 기기들을 추가로 휴대하고도 기동성과 움직임에 저해받지 않고, 오히려 정보력 상승으로 더 나은 전투력을 갖추기 위해 장치 구성품들을 의복에 내장시킨 ``스마트 전투복``을 입을 것으로 생각된다. 본 연구는 정확한 모션 캡쳐 솔루션을 지원하는 VICON사의 Motion Capture System을 이용하여 동작에 따른 체표변화를 측정하고 해석하여 적절한 신체 부위에 기기를 배치함으로써 미래 전장에 적합한 스마트 전투복 디자인을 개발하는 것을 목표로 한다.

니트 인형의 유형 분석과 디자인 개선방안에 관한 연구

Soft and comfortable images from knit structures and materials has been applied for dolls which are connected to expectation of sensitivity restoration from those. Knitted dolls are divided to four types which are hand-needle knitted dolls, crochet dolls, machine knitted dolls and socks dolls. Through researching artists and brands of knitted doll, they can be analyzed to 3 type of expression which are represent of realistic expression, transform or exaggerated expression and expression as a character. In order to development of making and selling knitted dolls, five improvements were suggested. Firstly, knit material improvement is needed. Secondly, the improvement of management aspects are required. Thirdly, there may be mentioned the extended application of knitted item. Fourthly, it offers possibilities of various ways for upcycling. Finally, I propose for the availability of seamless knit using seamless knitting machine. By review about the new beauty of fashion knitted dolls through this study, the possibilities of a new perspective and meaning to the fashion and textile product will be expand.

예비교사 전문성 개발을 위한 교원양성기관 교육과정 및 교수-학습의 문제점과 개선 방향

Purpose: This study aimed to identify the problems of curriculum and teaching-learning in teacher education institutions and to find directions for quality improvement for pre-service teachers' professional development. Design/methodology/data/approach: This study conducted FGI with a total of 18 students, professors, and teachers. The collected qualitative interview data were analyzed using constant comparative analysis method. Findings/Results: The curriculum and teaching-learning management of teacher education institutions were disparate from the field of elementary, middle, and high schools, which led to the lack of field experience of pre-service teachers. Value: By comprehensively acquiring diverse opinions and insightful understanding from stakeholders in the field of education, this study will contribute to providing field-based implementation strategies and micro-ideas for innovative changes in the teacher education system.

Primary Adrenal Insufficiency in a Newborn With Adrenal Hypoplasia Congenita Caused by a Mutation of the DAX1 Gene

Adrenal hypoplasia congenita (AHC) is a rare inherited disorder of the adrenal gland caused by deletion or mutation of the dosage-sensitive sex-reversal AHC critical region on the X chromosome, gene 1 (DAX1) gene. The DAX1 gene is expressed in the adrenal cortex, the pituitary gland, the hypothalamus, the testis, and the ovary. Most affected infants present with failure to thrive, salt wasting, and hypoglycemic seizure in early life. Immediate mineralocorticoid and glucocorticoid replacement is essen- tial. Most boys with AHC present with hypogonadotropic hypogonadism, resulting in failure to enter puberty and the need for testosterone treatment. However, a recent study revealed that the onset of puberty in boys with AHC can be variable, ranging from arrested or absent to precocious. We describe a case involving a newborn who presented with primary adrenal insufficiency due to a mutation of the DAX1 gene and was finally diagnosed with AHC.

이 논문은 중등교원 전문성 개발을 위한 교원자격기준의 체계화를 달성하기 위한 과제를 제시하는 데 목적이 있다. 교원양성교육에 대한 국가수준의 질 관리체제와 교원자격기준의 체계적 개발은 직전교육, 수습연수교육, 현직교육, 지도성훈련이라는 교원경력 발달단계의 연계성을 통합함으로써 이루어질 수 있으며, 이를 통하여 중등교원의 지속적인 전문성 개발이 유도될 수 있음을 제시하였다. 더불어서 40개 사범대학의 교직과정 편성현황을 분석하여 운영적 특징과 문제점을 제시하여 향후 교직과정 운영의 질 관리를 위한 기초자료를 제공하고자 하였다.

교육행정의 자연론적 정합론에 대한 분석적 고찰

Since Thomas Greenfield made his celebrated presentation in 1975, there have been a number of theory debates concerned with finding a better way of doing "good science" in educational administration. Scholars in the field do now realize that there are important philosophical questions, especially epistemological issues, underlying the theory debates of educational administration. In recent times, the work of several Australian scholars (specifically Colin Evers and Gabriele Lakomski) termed as "naturalistic coherentism", has been the center of a good deal of attention by researchers in the field of education. They have raised many crucial questions concerning, for example, the nature of educational research, the epistemological justification for "good theory" and more appropriate notions of science for trustworthy educational research. Despite the importance of the theoretical contribution to knowledge in the field of education, made by "naturalistic coherentism", the philosophical background on which naturalistic coherentists have relied has not been explored in great detail, mainly because of its theoretical sophistication. It was shown in this paper that the philosophical pedigree of "naturalistic coherentism" is extremely complex. It consists of various elements that cannot be summarized in a single "ism". It incorporates post-positivism from the philosophy of science. In epistemological terms, it is best labelled holistic coherentism. And, from the field of philosophy of mind, it draws on naturalism, scientific realism and eliminative materialism. In order to identity the philosophical views that have shaped "naturalistic coherntism" I traced it back to ideas developed by American naturalists, such as Quine and the Churchlands.

변혁적 지도성에 대한 비판적 고찰: 분산적 지도성을 중심으로

본 논문은 지도성 연구를 위한 이론적 모델로서 최근 새롭게 부각되고 있는 분산적 지도성의 주요 내용을 고찰하고, 이에 근거하여 지도성 연구의 새로운 방향성을 탐색해 보고자 하였다. 이를 위해서 변혁적 지도성 이론의 주요 내용을 비판적으로 검토하면서 인식론적 한계점을 고찰하였다.<br/> <br/> This article discusses the main ideas of distributed leadership, recently emerged in the field of educational administration, as a new conceptual framework of leadership studies. In order to understand the whole feature of leadership, the distributed perspective argues, all the situational factors by which leadership happens need to be taken into account, since the practice of leadership is 'stretched over' school leaders and the material and symbolic artifacts in the environment.<br/> In the article, the traditional conceptions of leadership studies such as transformational leadership are criticized, since it is based on a narrow view of science termed logical positivism emphasizing ideas of hypothetico deductive approaches and the centralized mindset. It is pointed out that empirical evidence of transformational leadership is shown to be remarkably thin and to be derived from an inadequate conception of leader-follower relations.<br/> In terms of capturing the everyday routine ways of organizational functioning by emphasizing aspects of social and spatial distribution of leadership, it is suggested, proposals for distributed leadership are a promising development that substantially broadens the field of leadership studies.<br/> <br/>
