Evaluation of BacT/Alert FAN Plus Bottles for the Culture of Peritoneal Dialysate

Background: A major complication of peritoneal dialysis (PD) is peritonitis, and bacterial culture of PD effluent in a blood culture bottle is the preferred technique for diagnosis of peritonitis. In this study, we compared dialysate inoculation and culture using the BacT/AlerTⓇ Fastidious Antimicrobial Neutralization Plus blood culture bottles (FAN Plus; bioMérieux, France) to the conventional centrifugation culture method. Methods: A total of 170 PD effluents were simultaneously processed by the conventional centrifugation culture method and by culture using FAN Plus media with two different inoculation procedures: inoculation after centrifugation and direct bedside inoculation. Results: Of the 52 cultures that were positive on at least one of the culture methods, 27 samples were positive on conventional centrifugation. However, 46 samples showed growth following inoculation into the FAN Plus media after centrifugation, and 47 samples were positive on the direct FAN Plus inoculation method. Using the case definition for PD peritonitis to classify samples, sensitivity of the conventional method was 50.0% (95% CI, 33.7-66.3%), whereas the sensitivity of the FAN Plus media was 78.9% (95% CI, 62.2-89.9%) by inoculation after centrifugation and 86.8% (95% CI, 71.1-95.1%) by direct inoculation. Use of both inoculation methods with FAN Plus media resulted in 92.1% sensitivity (95% CI, 89.2-99.9%). Conclusion: Culture using FAN Plus media demonstrated a superior bacterial recovery rate to the conventional centrifugation culture method. A combination of the two inoculation methods with FAN Plus media is recommended for the best diagnostic yield, while direct inoculation alone can be useful due to its simplicity and cost-effectiveness. Bacillus circulans에 의해 생물전환된 갈락토올리고당의 장내 환경 개선 효과

본 연구에서는 B. circulans를 활용한 생물전환을 통해 생성한 GOS(NeoGOS-P70TM)가 장벽을 구성하는 세포에 미치는 영향을 확인하고자 하였다. 실험 결과, GOS는 장관을 구성하는 세포들의 TJ 형성에 긍정적인 효능을 갖는 것으로 확인되었다. GOS는 체내에서 분비되는 효소에 의해 분해되지 않고 대장에 존재하는 장내 미생물에게 영양원으로 이용될 수 있으며, 장내 병원성 미생물의 군집화를 방해하는 효과를 나타내는 것으로 알려져 있다. 또한 장내 미생물의 대사에 의해 생산되는 단쇄지방산과 같은 물질들은 장관을 구성하는 세포들에 직접적인 에너지원으로 이용될 수 있는 것으로도 밝혀져 있기 때문에 산업적 이용에 대한 수요가 증가하고 있다. 따라서 본 연구 결과는 GOS를 활용한 새로운 개념의 장 건강 기능성식품 개발에 대한 기초자료로 사용될 수 있을 것으로 판단되었다. In a previous study, we reported that galactooligosaccharides (GOS, NeoGOS-P70TM) bioconverted by Bacillus circulans potently improved the clinical symptoms of ulcerative colitis in a mouse model. Here, to elucidate the therapeutic mechanism, we investigated the effect of GOS on the gut environment using human goblet (LS174T) and epithelial cells (Caco-2). Treatment of LS174T with GOS induced potent proliferation and the mRNA expressions of MUC2 and TFF-3, which are related to the production of mucin and tight junction (TJ) proteins. Also, a CLSM-based immunocytochemical study showed that GOS increased mucin production in a dose-dependent manner. To investigate the reinforcing effect of GOS and GOS-supplemented LS174T culture supernatants (containing TFF-3) on gut barrier integrity, we evaluated the mRNA expressions of TJ components, trans-epithelial electric resistance, and FITC-dextran monolayer permeability. The mRNA expression levels of TJ-related genes, such as ZO-1, Occludin, and Claudin-1, were significantly increased by GOS treatment. On the other hand, treatment remarkably enhanced the TEER value of Caco-2 cells and dose-dependently reduced dextran flux. We concluded that GOS stimulates the expressions of MUC2 and TFF-3 in goblet cells and reinforces gut TJ barrier integrity.
Accurate and Rapid Measurement of Glycated Hemoglobin Using HLC-723 G11 Variant Mode

Background: Type II diabetes mellitus causes many complications, and its prevalence continues to increase in Korea. Accurate measurement of glycated Hb (HbA1c) is important because of its usefulness in diagnosis, follow-up, and prediction of prognosis. We tested the analytical performance of the HLC-723 G11 Variant Mode (G11vr; Tosoh Bioscience, Inc., Tokyo, Japan), recently introduced to Korea, in detecting HbA1c. Methods: We evaluated precision, linearity, carry-over, and turnaround time. Using 208 samples, including 108 flagged samples, we compared HbA1c concentrations from four analyzers through correlation analysis: G11vr, HLC-723 G8 Variant Mode (G8vr, Tosoh Bioscience), HLC-723 G11 Standard Mode (G11st, Tosoh Bioscience), and HLC-723 G8 Standard Mode (G8st, Tosoh Bioscience). We used HPLC mass spectrometry (MS) and capillary electrophoresis (CE) to confirm the HbA1c concentrations of 15 additional known Hb variant samples. Results: Repeatability (% CV) in measuring low- and high-concentration controls was 0.57% and 0.35%, respectively; within-laboratory precision was 0.86% and 0.69%, respectively. In a linearity test, the coefficient of determination was 0.9999 (measurement range: 3.64% to 18.59%) for HbA1c. The correlations between G11vr and other analyzers were weaker for flagged samples than for non-flagged samples. The carry-over effect was less than 0.4%. Turnaround time for a single sample was lower in G11vr (one minute) than in G8vr (1.6 minutes). For 15 samples with Hb variants, G11vr HbA1c results were more similar than those of other analyzers to HPLC-MS and CE results. Conclusions: G11vr showed adequate performance and rapid turnaround time in measuring HbA1c.

Flow Cytometric Analysis of T Cells in Hemophagocytic Lymphohistiocytosis

Background: T cell immunophenotypes in patients with hemophagocytic lymphohistiocytosis (HLH) have been described. Downregulation of CD5 or CD7 on T cells has been reported in patients with Epstein-Barr virus (EBV)-positive HLH. As the utility of T cell immunophenotypes as an adjunctive diagnostic or a prognostic marker for HLH has not been evaluated, we analyzed T cell immunophenotypes in HLH patients for this purpose. Methods: We classified 45 HLH patients into three subgroups: EBV-positive HLH (N=27), EBV-negative secondary HLH (N=15), and familial HLH (N=3). We retrospectively characterized downregulation patterns of CD5 or CD7 on activated T cells, using flow cytometry. Overall survival was estimated using Kaplan-Meier curves and compared using the log-rank test. Results: An aberrant immunophenotype, including CD5 and/or CD7 downregulation on T cells, was observed in 55.6% (15/27) of the EBV-positive HLH patients and 100% of the familial HLH (3/3). Only one (1/15, 6.7%) patient with EBV-negative secondary HLH showed an aberrant loss of CD7 antigen on CD8+ T cells. The presence of an aberrant T cell immunophenotype was not related to overall survival in EBV-positive HLH and EBV-negative secondary HLH patients. Conclusions: An aberrant T cell immunophenotype may assist in discriminating EBV-negative secondary HLH and EBV-positive HLH. However, it may not be useful as a prognostic marker.
