Evidence-based recommendations are included for the use of antiplatelet agents for noncardioembolic stroke. Changes in the guidelines necessitated by new evidence will be continuously reflected in the new guideline.

Physicians’ Attitudes Toward Guidelines for Stroke: A Survey of Korean Neurologists

Background and Purpose: Clinical practice guidelines (CPGs) are regarded as an essential guidance tool for practicing physicians. We surveyed physicians in Korea in order to evaluate their attitudes toward the Korean CPGs for stroke. Methods: We obtained participation agreement for our survey from 27 centers of the 33 most actively contributing to the Korean Stroke Registry. A total of 174 neurologists participated in a questionnaire interview regarding their attitudes toward CPGs for stroke. Results: Of 174 participating neurologists, 65 (37.4%) were stroke neurologists. The average age was 33.6±7.1 and 49 (28.2%) were female. Most of the respondents held positive attitudes and opinions regarding the use of the guidelines, whereas only a small percentage (14.9%) responded negatively. More than 60% of the physicians in the survey reported adherence to the Korean CPGs in dyslipidemia management for the secondary prevention of stroke. Conclusions: The positive attitudes and opinions toward the guidelines imply that physicians’ attitudes should not be regarded as a potential barrier to the implementation of Korean CPGs for stroke practiced by general physicians.

To evaluate the characteristics of permeability in weathered residual soil, flexible wall permeameter tests are performed using undisturbed samples. The Influence of hydraulic gradient and confining pressure on the permeability of weathered residual soil is analyzed. To compare the characteristics of permeability between weathered soil and sand, similar tests are performed using Jumoonjin sand. Also, piping model tests are performed to investigate the piping resistance of weathered residual soil. As a result, weathered residual soil shows very dependable permeability on hydraulic gradient and very large resisting ability against piping compared with sand.

B a c k g r o u n d: Somatosensory evoked potential (SSEP) is valuable for the evaluation of the central pathway. However, peripheral neuropathy sometimes renders the test useless by preventing the conduction from reaching the CNS. We postulated that the peripheral conduction problems could be overcome by proximal stimulation in SSEP and wanted to verify this in the study. Methods: Twenty patients with diabetic sensorimotor polyneuropathy were included. SSEP was elicited by stimulating the median and posterior tibial nerves. We compared the effect of distal and proximal stimulations in each SSEP in the aspect of presence/absence and various latencies of resultant waves. Results: Among the 40 cases, proximal stimulation caused reappearance of subsided waves in 10 cases (25%). In the median nerve SSEP, proximal stimulation made EN1 and CN2 visible which were not evident when distally stimulated. In the posterior tibial nerve SSEP, there was also improvement of forming waves when proximally stimulated. Conclusions: In the diabetic polyneuropathy, proximal stimulation of SSEP is more effective than the conventional distal stimulation in evaluating central pathway.
후천성 탈수초성 다발신경병증에서의 운동유발전위 및 체성감각유발전위 연구

Background and Objectives: The proximal and distal nerve segments are preferentially involved in acquired demyelinating polyneuropathies (ADP). This study was undertaken in order to assess the usefulness of motor evoked potential (MEP) and somatosensory evoked potential (SSEP) in the detection of the proximal nerve lesion in ADP. Methods: MEP, SSEP and conventional NCS were performed in 6 consecutive patients with ADP (3 AIDP, 3 CIDP). MEP was recorded from abductor pollicis brevis and abductor hallucis using magnetic stimulation of the cortex and the cervical/lumbar spinal roots. SSEP were elicited by stimulating the median and posterior tibial nerves. Latency from cortex and cervical/lumbar roots, central motor conduction time (CMCT), EN1-CN2 interpeak latency were measured for comparison. Results: MEP was recorded in 24 limbs (12 upper and 12 lower limbs) and SSEP in 24 limbs (12 median nerve, 12 posterior tibial nerve). F-wave latency was prolonged in 25 motor nerves (25/34, 73.5%). Prolonged CML and PML were found in 41.7% (10/24) and 45.8% (11/24), respectively. Interside difference (ISD) of CMCT was abnormally increased in the upper extremity, 66.7% (4/6 pairs) in case of CML-PML. EN1-CN2 interpeak latency was abnormally prolonged in one median nerve (1/10) and LN1-P1 interpeak latency was normal in all posterior tibial nerves. Conclusions: MEP and SSEP may provide useful information for the proximal nerve and root lesion in ADP. MEP and SSEP is supplemental examination as well as complementary to conventional NCS.

Pressuremeter test estimates deformational properties of soil from the relationship between applied pressure and the displacement of cavity wall, and the results reflect the in-situ stress condition and the structure of soil particles. This study suggests the overall process of test and analysis for the evaluation of nonlinear degradation characteristics of shear moduli, based on the reloading curve of pressuremeter test. The method estimates the maximum shear modulus, taking into account the difference between the stress states around the probe in reloading and that of the in-situ state, and then combines the degradation characteristics of shear moduli taken from reloading curve. This procedure derives the shear moduli in overall strain range. Pressuremeter tests were carried out in various ground conditions using large calibration chamber, together with various reference tests. Shear moduli taken from pressuremeter tests were compared with bender element test and resonant column test results. 프레셔미터 시험은 하중에 따른 변위 곡선에 기초하여 지반의 변형특성을 평가할 수 있는 시험 방법으로 현장의 응력 상태 및 토체의 입자 구조를 그대로 반영하여 지반의 비선형적인 변형특성을 평가할 수 있는 장점을 지니고 있다. 본 연구에서는 교란의 영향을 최소화 할 수 있는 하중 재재하 곡선을 이용하여 전단탄성계수의 감쇠 특성을 평가할 수 있는 시험 및 해석 방법을 제안하고 있다. 하중 제하-재재하 과정에서 발생될 수 있는 원지반과의 응력 차이를 고려하여 최대 전단탄성계수를 산정하고, 재재하 곡선의 감쇠 특성과 결합하여 변형률 크기에 따라 감쇠되는 전단탄성계수의 특성을 평가하고 있다. 이를 위하여, 대형 압력 토조를 이용하여 다양한 지반 조건에서 프레셔미터 시험을 수행하였으며, 벤더 엘리먼트 시험 및 공진주 시험을 통하여 전단탄성계수 감쇠 특성을 평가, 비교하였다.
