Original Article : High Recurrence Rate of Idiopathic Peptic Ulcers in Long-Term Follow-up

Background/Aims: Our aim was to compare the long-term clinical outcomes of idiopathic peptic ulcer disease (IPUD) with those of Helicobacter pylori-positive and nonsteroidal anti-inflammatory drug (NSAID)-induced peptic ulcer disease (PUD). Methods: Patients with endoscopically diagnosed PUD were retrospectively reviewed. According to their H. pylori-infection status and history of NSAIDs use, patients were categorized into three groups: H. pylori-positive PUD, NSAID-induced PUD, and IPUD. Clinical outcomes were analyzed, and the recurrence rate of PUD was compared among the three groups. Results: A total of 238 patients were enrolled. Those with IPUD, NSAID-induced PUD, and H. pylori-positive PUD comprised of 56, 60, and 122 patients, respectively. The 5-year cumulative incidences of recurrent ulcers were 24.3% (95% confidence interval [CI], 11.6% to 37.0%) in IPUD, 10.9% (95% CI, 2.6% to 19.2%) in NSAID-induced PUD, and 3.8% (95% CI, 0.1% to 7.5%) in H. pylori-positive PUD (IPUD vs NSAID-induced PUD/H. pylori-positive PUD, p=0.43/p<0.001 by log-rank test). In the Cox-proportional hazards model, only IPUD remained as an independent risk factor associated with recurrent ulcers (hazard ratio, 5.97; 95% CI, 1.94 to 18.34; p=0.002). Conclusions: IPUD exhibited a higher recurrence rate than H. pylori-positive and NSAID-induced PUD in long-term follow-up and was an independent risk factor for ulcer recurrence. STUDY DESIGN:: An experimental design was conducted to investigate kinematic changes in 3-dimensional trunk motions between subjects with and without chronic low back pain (LBP) while demonstrating axial rotation. OBJECTIVES:: The purpose of this study was to compare 3-dimensional kinematic data for the upper and lower thorax and the lumbar spine from the axis of the core spine during axial rotation activities in the standing position while considering anthropometric factors in subjects with and without LBP. SUMMARY OF BACKGROUND DATA:: Rotation of the trunk is associated with a large number of LBP cases and surrounding spinal tissue injuries. METHODS:: Fifteen subjects with chronic LBP (5 men, 10 women) and 15 subjects without LBP (9 men, 6 women) participated in this study. The outcome measures included kinematic data of actual trunk rotation angles for the upper and lower thorax and the lumbar regions relative to the core spine (spinal root) in sagittal, coronal, and transverse axes. RESULTS:: The spinal range of motion was significantly different for each spinal region (F=240.25, P=0.001) and axis (F=213.91, P=0.001). There was a significant interaction between the spinal region and the group (F=4.34, P=0.04). There was also a 3-way interaction with the spinal region, the axis, and the group (F=11.04, P=0.001). These results indicated that spinal region and axes are important to consider because the upper thorax demonstrated a significantly greater rotational displacement in subjects with chronic LBP. Among the anthropometric factors, age (F=6.24, P=0.02) interacted with the spinal region and the axis. CONCLUSIONS:: Decreased spinal range of motion in older subjects might result in a stiffened spine in addition to possible poor proprioception from back injuries to passive structures in subjects with chronic LBP.

We previously reported that MSTO-211H cells have a higher capacity to regulate Nrf2 activation in response to changes in the cellular redox environment. To further characterize its biological significance, the response of Nrf2, a transcription factor that regulates ARE-containing genes, on the synergistic cytotoxic effect of clofarabine and resveratrol was investigated in mesothelioma cells. The combination treatment showed a marked growth-inhibitory effect, which was accompanied by suppression of Nrf2 activation and decreased expression of heme oxygenase-1 (HO-1). While transient overexpression of Nrf2 conferred protection against the cytotoxicity caused by their combination, knockdown of Nrf2 expression using siRNA enhanced their cytotoxic effect. Pretreatment with Ly294002, a PI3K inhibitor, augmented the decrease in HO-1 level by their combination, whereas no obvious changes were observed in Nrf2 levels. Altogether, these results suggest that the synergistic cytotoxic effect of clofarabine and resveratrol was mediated, at least in part, through suppression of Nrf2 signaling.

The aim of this study was to determine suitable oxygen transmission rate (OTR) films for modified atmosphere (MA) storage of asparagus spears (Asparagus officinalis L.), and to examine the related changes in asparagus spear quality. Asparagus spears were packed with 10,000, 20,000, 40,000, 70,000, and $100,000cc/m^2{\cdot}day{\cdot}atm$ OTR film treatments for MA storage, and perforated film (conventional storage) was used as the control. The fresh weight loss rate was less than 1% in all OTR film treatments but not in the control. In all OTR film treatments, ethylene content rapidly increased on the first day of storage, and gradually decreased thereafter to $2.0{\mu}L{\cdot}L^{-1}$ by the final storage day. In the 10,000 cc and 20,000 cc OTR film treatments, carbon dioxide content was maintained within the permissible range for asparagus spears under recommended controlled atmosphere (CA) and MA conditions (5-12%). The oxygen content was maintained between 12% and 20% in all OTR film treatments. Hue angles and total chlorophyll content were highest in the 10,000 cc OTR film treatment in both tips and stems. Visual quality was maintained at a saleable level only in the 10,000 cc OTR film treatment until the final storage day (25 days). Based on sensory evaluation, the least off-odor was detected in the 10,000 cc OTR film treatment, and the most in the control treatment. While the soluble solids content decreased with all film treatments, it was maintained at the highest level in the 10,000 cc OTR film treatment. Vitamin C content decreased with all film treatments during storage, but was maintained at higher levels in the 10,000, 20,000, and 40,000 cc OTR film treatments. The firmness of tips and stems increased with storage duration in all film treatments, stems were firmer under the perforated film, 10,000, and 20,000 cc OTR film treatments. Given these results, the 10,000 cc OTR film treatment is considered the most appropriate for MA asparagus spear storage since visual quality and soluble solids content were maintained, and fresh weight loss, vitamin C content, off-odor, and yellowing were suppressed during storage.

In this paper we present numerical performance of the previously researched subspace projection based interference cancellation algorithm. The numerical analysis is based on real GPS data including carrier and doppler effects. The position errors are represented for both narrow band and wide band jamming signals.
Fuzzy Bin-Based Classification for Detecting Children’s Presence with 3D Depth Cameras

<P>With the advancement of technology in various domains, many efforts have been made to design advanced classification engines that aid the protection of civilians and their properties in different settings. In this work, we focus on a set of the population which is probably the most vulnerable: children. Specifically, we present ChildSafe, a classification system that exploits ratios of skeletal features extracted from children and adults using a 3D depth camera to classify visual characteristics between the two age groups. Specifically, we combine the ratio information into one bag-of-words feature for each sample, where each word is a histogram of the ratios. ChildSafe analyzes the words that are normalized within and between the two age groups and implements a fuzzy bin-based classification method that represents bin-boundaries using fuzzy sets. We train and evaluate ChildSafe using a large dataset of visual samples collected from 150 elementary school children and 150 adults, ranging in age from 7 to 50. Our results suggest that ChildSafe successfully detects children with a proper classification rate of up to 94%, a false-negative rate as low as 1.82%, and a low false-positive rate of 5.14%. We envision this work as a first step, an effective subsystem for designing child safety applications.</P>
