      • 고빌리루빈혈증을 동반한 자가면역성 간염 1례

        자가면역성 간염은 대개 만성 경과를 가지며, 혈중 자가면역항체와 혈청 글로불린치의 상승, 그리고 조직학적으로 괴사 염증성 변화를 특징으로 하는 질환으로 아직 정확한 병인이 밝혀져 있지 않은 상태이다. 발병연령은 대개 젊은 영자(15-25세)에서 호발한다. 이 질환은 급성 간염의 임상경과를 보일 수 있으나, 심한 급성 간염이나 전격성 간염으로도 나타날 수 있는데, 이 경우 아주 나쁜 예후를 보인다고 한다. 치료는 자가면역성 간염 임상 경과의 다양성이나 병인, 병리기전의 불확실성에도 불구하고 대개 steroid 치료에 반응하는 것으로 알려져 있으며 대개 80%의 관해율을 나타내며 궁극적으로 간경변으로의 진행을 막을 수 있는 것으로 보인다. 저자들은 58세 남자에서 발생한 급성의 경과를 가지고 심한 황달을 동반한 자가면역성 간염을 경험하였기에 문헌고찰과 함께 보고하는 바이다. Autoimmune hepatitis (AIH) is a chronic necroinflammatory liver disorder of unknown cause associated with circulating autoantibodies and a high serum globulin level. The age of onset of AIH show a peak between the age of 15 and 25 years. AIH can develop and be manifested as acute hepatitis, but severe form of acute hepatitis or fulminant hepatic failure has a poor prognosis. Although AIH is likely to progress from chronic active hepatitis to cirrhosis, steroid therapy can control the disease activity, prolong survival, improve the quality of life , and defer liver transplantation. In the present report we describe a 58-year-old man who admitted because of progressive jaundice and fatigue. He was diagnosed with AIH from laboratory test result showing positivity for antinuclear antibodies, anti-smooth muscle antibodies, and negativity for hepatitis viral markers and from liver biopsy. Steroid therapy, oral administration of prednisolone, was effective in improving the liver function test. Following liver biopsy 6 months after onset shows markedly improved necroinflammatory activity.

      • 2차원 및 3차원 초음파를 이용한 태아표면의 해부학적 선별 검사에 관한 연구

        Objective : Our purpose of this study was to determine the clarity and consuming time of fetal surface screening in thirty-eight fetuses from 24 to 32 weeks of gestation by use of two and three-dimensional ultrasonography. Methods : From May 1998 to July 1998, we performed the fetal surface screening of the fetal face, extremities, and spine. Thirty-eight fetuses of 24-32 weeks of gestation (thirty-seven normal fetuses and one abnormal fetus) were studied by use two-dimensional (AI 5200S, 3.5-5.0MHz; Acoustic Imaging Technologic Cooperation, U.S.A.) and three-dimensional ultrasonography (Volusion 530D, 3.5-5.0 MHz ; Kretz-Medicine, Korea). On the fetal face, we evaluated both orbits, nose, lips and brow. Both hands and feet, five digits and toes should be seen in this study. We evaluated cervical spine and lumbosacral junction of the spine. Results : The maternal average age of 38 patients was 29.9 years and the average gestational age was 29.7 weeks. The results of fetal surface screening were as follows : face (both orbits 89.5%, lips 92.1%, nose 92.1%, and brow 89.5%), extremities (both hands 68.4% and feet 73.7%), and spine (cervical spine 94.7% and lumbosacral junction 97.4%) by use of two-dimensional ultrasonography. By use of three-dimensional ultrasonography, results were as follows : face (both orbits 76.3%, lips 81.6%, nose 81.6%, and brow 78.9%), extremities (both hands 76.3% and feet 78.9%), and spine (cervical spine 71.1% and lumbosacral junction 73.7%). Consuming time for two-dimensional ultrasonography were 4.7 minutes for face, 11.6 minutes for extremities, and 5.7 minutes for spine. Using three-dimensional ultrasonography results were 12.8 minutes for face, 17.9 minutes for extremities, and 11.7 minutes for spine. Average consuming time of all fetal anatomic screening was 22.0 and 42.4 minutes by use of two and three dimensional ultrasonography. In one abnormal fetus had cleft lip and polydactyly of right foot which were detected all by three-dimensional ultrasonography, but we couldn't detect the polydactyly by two-dimensional ultrasonography. Conclusion : Three-dimensional ultrasonography was more time-consuming method, but we suggested that three-dimensional ultrasonography was superior than two-dimensional ultrasonography in fetal surface screening.

        In recent years, gonorrhea has been pandemic and remains one of the most common STDs in the world, especially in developing countries. For the detection of a more effective therapeutic regimen and assessing the prevalence of Penicillinase Producing Neisseria gonorrhoeae(PPNG), we have been trying to study the patients who have visited the Venereal Disease Clinic of Choong-Ku Public Health Center in Seoul since 1980 by menas of the chromogenic cephalosporin method. In 1998, 93 strians of N. genorrhoeae were isolated, among which 60(64.5%) were PPNG. The prevalence of PPNG in Seoul, which had been decreased to 39% in 1996 after a peak of 74.3% in 1993, is increased to 64.5% in 1998.

      • 정전 열 접합을 이용한 Multi-Substrate Bonding

        We performed silicon-to-glass bonding using silicon direct bonding followed by anodic bonding(SDAB). Initial bonding between glass and silicon was caused by the hydrophilic surfaces of silicon and glass ensemble using silicon direct bonding(SDB) method. We found that the bonded specimen using SDAB process had higher strength than one using anodic bonding process only. We performed multiple layer bonding by SDAB, which is glass- silicon -glass as sandwich structure. In the silicon wafer, the (1mm x 2mm x 500μm)-sized cavity was formed by the anisotropic etching of the silicon substrate in EPW(Ethylendiamin-Pyrocatechol-Water). And, the cavity was sealed with glass wafers by SDAB method.

      • 미세공정상에서 전가산기의 해석 및 비교

        In this paper the main topologies of one-bit full adders, including the most interesting of those recently proposed, are analyzed and compared for speed, power consumption, and power-delay product. The comparison has been performed on circuits, optimized transistor dimension to minimize power-delay product. The investigation has been carried out with properly defined simulation runs on a Cadence environment using a 0.25-μw process, also including the parasitics derived from layout. Performance has been also compared for different supply voltage values. Thus design guidelines have been derived to select the most suitable topology for the design features required. This paper also proposes a novel figure of merit to realistically compare n-bit adders implemented as a chain of one-bit full adders. The results differ from those previously published both for the more realistic simulations carried out and the more appropriate figure of merit used. They show that, except for short chains of blocks or for cases where minimum power consumption is desired, topologies with only pass transistors or transmission gates are not attractive. In contrast. the most interesting implementations in terms of trade off between power and delay are the traditional CMOS and Mirror topologies. Moreover, the CPL allow the best speed performance.

        A In Korea when change order was occurred, contract amount may be adjusted according to Government Contract Act. If the requirement of privy is fully accepted, conflict can't arise. Because of a misunderstanding and misappropriation of contract, a difference between the contract condition and job site, an errors & omissions of the design document, the contract disputes has been increased since the Seoul 2nd Metro Project claims alleged by 6 Contractors at Feb. 1998. To mitigate and to resolve effectively construction dispute, training of construction participants, introduction of coercive arbitration clause in public construction, adaption of the lowest bidding system, participation of construction engineer in arbitration process were suggested in this paper.

      • 전류 모드 CMOS 다치 논리 회로를 이용한 8X8 병렬 승산기

        This paper presents new current-mode CMOS multiple-valued integrated circuits for high-density arithmetic systems. The validity and effectiveness of the proposed circuits are verified through the HSPICE simulation. A 8×8-bit parallel multiplier using the proposed circuits with 2V supply voltage is designed with a 0.25㎛ standard CMOS technology. The performance of the proposed multiplier is evaluated to be about 74% smaller than that of a corresponding binary implementation.

      • 사람의 축색 반사성 및 발한의 연구

        발한 기능에 대해 연구하기 위하여 아세티콜린 (acetylcholine, Ach) 이온 투과법에 의해 축색 반사성을 자극하는 정량적 축삭 반사성 발한 검사법 (quantitative sudomotor axon reflex test, QSART)을 발한 반응을 검토하였다. 용적 습도계 QSART를 이용해 Ach 10% 이온 투과법 (2 mA ×5 min)을 하는 동안 직접적으로 활성화된 (directly activated, DiR)땀과 축색 반사성에 (axon reflex, AXR) 의해 땀의 양을 측정했다. 발한 시작점(onset-time)시간, 무스카린 수용체(muscarine receptor)의 직접적인 영향에 의한 땀의 양 그리고 축색 반사성에 의해 매개되는 니코틴 수용체 (nicotine receptor)에 의해 간접적으로 발생되는 (AXR(1) and AXR(2)) 발한 양에 대해 연구하였다. 발한 시작점에 있어서 왼쪽과 오른쪽에서의 상관관계가 높은 것으로 나타났다. (p<0.01). 직접적인 니코틴 수용체의 활동의 자극으로 유도된 발한의 양에 있어서 왼쪽과 오른쪽에서의 상관관계가 높은 것으로 나타났다. (P<0.01). 간접적인 니코틴 수용체의 활동의 자극으로 유도된 발한의 양에 있어서 왼쪽과 오른쪽에서의 상관관계가 높은 것으로 나타났다. (P<0.01). 이 같은 실험결과로 볼 때 말초적 발한 검사에 간단한 방법으로 사료되며 임상의 신경분야등에 유용하게 사용될 것으로 판단된다.

      • 열 관용과 아세티콜린에 대해 발한 반응의 관계

        열대 지방에 살고 있는 사람들은 열대 기후에 영구적으로 거주함으로써 열-관용 (heat -tolerance)의 능력을 가지고 있다. 열대 지방의 원주민들에게서 억제된 열적 발한 의 말초 기작을 알아 보기위해서, 발한 신경지배의 주 전달체인 아세티콜린 (acetylcholine, Ach)에 대한 발한 반응을 이온투과법9quantitative sudomotor axon reflex test, QSART, 말초 발한 반응)또는 뜨거운 물에 다리를 담그는 것 (HW test, 43C, 중앙 발한반응)을 통해 건강한 일본인 (15명0과 아프리카인 (10명) 간에 비교하였다. QSART를 실행되는 동안 직접적으로 활성화된 발한 반응과 축색 반사성 매개에 의한 발한 반응이 일어나게 되고 HW test에서의 발한 반응은 용량습도계에 의해 측정된다. 이러한 실험의 결과들은 적어도 아프리카인의 억제된 열적 발한이 땀샘과 각 땀샘당 땀의 배출량의 증가를 통해 ACh에 대한 땀샘의 민감성이 억제되는 부분에 기여하는 것으로 타나났다.
