Positional Reproducibility and Effects of a Rectal Balloon in Prostate Cancer Radiotherapy

Despite the increasing use of the rectal balloon in prostate cancer radiotherapy, many issues still remain to be verified objectively including its positional reproducibility and relevance to treatment morbidity. We have developed a custom rectal balloon that has a scale indicating the depth of insertion and dilates symmetrically ensuring positional reproducibility. Fifty patients with prostate cancer treated by definitive 3D-conformal radiotherapy (3D-CRT) or intensity-modulated radiotherapy (IMRT) with rectal balloon were analyzed. Each of first five patients undergone computed tomography (CT) three times with a rectal balloon. The positional reproducibility was tested by Intraclass Correlation Coefficient (ICC) from the CT-to-CT fusion images. Planning variables and clinical acute toxicities were compared between when or not applying balloon. An ICC of greater than 0.9 in all directions revealed an excellent reproducibility of the balloon. Rectal balloon improved considerably the mean dose and V45Gy-V65Gy in plan comparison, and especially in 3D-CRT the rectal volume exposed to more than 60 Gy dropped from 41.3% to 19.5%. Clinically, the balloon lowered acute toxicity, which was lowest when both the balloon and IMRT were applied simultaneously. The rectal balloon carries excellent reproducibility and reduces acute toxicity in 3D-CRT and IMRT for prostate cancer.

전투차량의 음향 스텔스를 위한 배기 소음 저감 장치 개발

최근 진보된 과학 기술로 전투 체계의 양상이 변화하고 있다. 정밀도와 성능이 높은 센서와 무기를 이용하여, 재래식 전투에서 보기 어려웠던 장거리 정밀 탐지 기술이 보편화 되고 있다. 이로 인해 아군의 전투 차량이 적군의 장거리 타격 기술에 노출되는 위험성이 증대되고 있다. 고출력의 엔진이 탑재된 전투 차량의 큰 배기 소음은 적에게 탐지되기 쉬우며 승무원의 생존을 위협하는 요인 중 하나이다. 이런 문제를 해결하기 위해서 최근에 음향 스텔스 기술에 대한 관심이 증가하고 있다. 음향 스텔스 기술은 잠수함 그리고 전투차량과 같은 장치에서 발생하는 음향 신호를 차단하거나 억제하여 적에게 노출되는 상황을 미연에 방지하고, 노출이 되더라도 신호가 왜곡되어 적의 상황 판단을 지연 시키는 기술이다. 전투차량 운용시 병력의 생존성 증대 및 무결한 임무 완수를 목적으로, 본 연구에서는 전투차량의 음향 스텔스를 위한 배기 소음 저감 장치에 대한 설계 방법론을 제시한다. 먼저 전투차량의 배기 소음을 측정하여 주파수별로 저감하여야 하는 소음 저감 레벨을 획득하고, 이를 바탕으로 소음 저감 장치가 달성해야 하는 목표 삽입 손실(Insertion loss)을 결정한다. 음향 메타물질과 생성적 설계 기법을 활용하여 넓은 주파수 범위에 높은 소음저감 대역을 형성하는 배기 소음 저감 장치를 설계한다. 설계한 소음 저감 장치를 제작하여 실험적 방법으로 설계 결과를 검증한다.

The purpose of this paper is to illustrate and explain the business cycle in Korean housing sector, using the Markov switching analysis. To analyze the markets, the Kookmin Bank's housing price index (HPI) and the sub-indices are used. This study found four high volatility regimes for the HPI and six for the Gangnam (southern part of Seoul) apartment price index. Furthermore, it is identified that there is a possibility of the ripple effect which starts from Gangnam area through the comparison of high volatility regimes. In the Busan HPI three regimes are found, reflecting the regional characteristics. Analyzing the sub-indices of the two leading districts in Seoul (Seocho-gu and Nowon-gu), the results are found to be consistent with those of the former indices. In general, however, the downward high volatility regimes of the housing sector caused by US sub-prime mortgage crisis could not be captured through the observations of the indices. This implies a possible limitation of the applicability of the indices. 본 연구의 목적은 마르코프 스위칭 분석을 이용하여 우리나라의 주택시장의 경기변동을 제시하고 설명하고자 하는데 있다. 이를 위해 국민은행의 주택가격지수와 그 하위지수들을 이용하였다. 분석결과 주택가격지수에서는 4개의 고변동성 국면을 관찰할 수 있었으며, 강남지역 아파트지수에서는 6개의 고변동성 국면을 확인할 수 있었다. 또한 강남지역에서 야기된 물결효과의 존재 가능성을 고변동성 국면의 비교를 통하여 확인할 수 있었다. 부산 주택가격지수에서는 지역특성에 따른 3개의 고변동성 국면이 전개되었음을 확인할 수 있었다. 하위지수인 서울의 대표적인 두 지역(서초구와 노원구)의 아파트지수를 대상으로 분석하여도 같은 맥락의 결과를 얻을 수 있었다. 그러나 전반적으로 지표상 미국의 서브프라임 모기지 위기가 발생했을 때에 하락의 고변동성 국면을 포착할 수 없어서 주택가격지수의 유용성에 한계가 있음을 발견하였다.

이공학(理工學) 일반 분야의 데이터베이스는 요 및 해 사이 충실(充實)해졌으나, 화학(化學) 의학(醫學) 약학(藥學) 분야의 데이터베이스에 비하면 그 층(層)은 두껍다고 말할 수 있다. 아직 널리 사용되고 있는 데이터베이스의 종류(種類)도 실제로는 한정(限定)되어 있고, 일부를 제외(除外)하곤 거의가 외국제(外國製)이다. 본고(本稿)에서는 데이터베이스 시스템마다 제공(提供)되고 있는 주요한 데이터베이스의 종류(種類)와 특징(特徵), 이용(利用)의 요점과 활용법을 소개하였다. 현재로는 아직 문헌정보(文獻情報)의 활용이 중심이 되겠지만, 연구(硏究) 개발활동(開發活動)의 효율화를 증진시키기 위해선 팩트 데이터가 불가결(不可缺)하다. 채산적(採算的), 기술적(技術的)으로는 어려운 면도 있겠으나, 이 분야에 있어 상용(商用)의 팩트 데이터베이스를 가능한 한 시급히 정비(整備)할 필요가 있을 것이다. General branches of science and engineering now enjoy being substantially repleted in their databases. However, comparing the databases of such branches with chemistry or medical science and pharmacology, the strata do not seem to be fully equipped yet. As a matter of fact, the variety of such databases in practical application is still somewhat limited. Moreover, these databases are, with a few exceptions, prepared abroad. In this paper varieties and features of databases now available on the market for each database service system, as well as crucial points for practical use are introduced. At present, it is focussed more or less on the application of bibliographical information. Yet, it would be inevitable to equip them with factual data in order to increase the efficiency of research and development activities. Of course, it is still not easy to solve problems for better profitability and higher technical achievement. The author wishes to emphasize the necessity to fully equip the commercial factual database for said branches as quickly as possible.
