        임상한의사 처방의 약물 배합 빈도 분석

        Objectives : Since its enactment in 1987, the 56 standard prescriptions covered by insurance have remained unchanged from its original version. In this study, we tried to discover most frequently used herbal combinations by analyzing prescriptions used in actual clinical settings. Methods : We have built Structured Query Language to analyze herbal combination and progressed this analysis through analyzing the frequencies of medicinal herb combinations in medical prescription slips. Results : We have found out that traditional Korean medical doctors use about 13 herbs in a prescriptions and usually use 253 kinds of herb. And We have found out the most frequently used herbal combination. Conclusions : In this study, We can suggest new method to decide what do we need on insurance prescriptions.

        골관절염 환자의 한약 양약 투여 현황 및 안전성 연구

        Objectives: The combined use of herbal and Western medicine is increasing. However, herbal medicine is highly likely to interact with Western medicine making it important to understand the effects of co-administration. This study investigates the ratio of patients who take Western medicine with herbal medicine, the types of medicines commonly prescribed together, and the results of hospital examinations. Methods: We investigated patients who were hospitalized at Kyung Hee University Korean Medical Hospital for at least one day from January 1, 2010 to December 31, 2017. There were some inclusion criteria. First, we chose patients aged 19 and over. Second, we chose patients who were diagnosed with osteoarthritis (OA) with diagnosis codes M13, M15, M17 according to KCD-7. Third, patients had liver function tests, renal function tests, and general hematology tests performed at least two times during hospitalization. Results: Among a total of 131 OA patients, 32 (24.4%) patients were treated with herbal-Western medicine combination therapy. The most commonly prescribed herbal medicine was Daegalwhal-tang, and the most commonly prescribed Western medicine was celecoxib. In the laboratory findings, all liver function tests, renal function tests, and general hematology tests showed no difference compared to admission day. There were also no differences between herbal medicine single treatment and herbal-Western medicine combination treatment. Conclusions: From these results, we suggest that herbal medicine single treatment and herbal-Western medicine combination treatment for OA patients does not cause adverse effects.

        한방비계내과학 내 중요 본초 및 처방 분석 연구

        Objective : This study aims to study core herbs and formulas in Internal medicine on Spleen system, to enhance efficiency in teaching Internal medicine on Spleen system, Herbalogy, Formula science, and to increase integration of the courses. Methods : Frequency notion, which was generally used in previous studies, was used in this study along with network analysis. Results : Frequently used herbs, herbs with high centrality, frequently combined herbs and core formula were found in this study. The herb with the highest frequency and centrality was ‘Citri Unshius Pericarpium’, and ‘Atractylodis Rhizoma Alba - Citri Unshius Pericarpium’ was the most frequent herb combination. The results of network analysis showed a total of 5 herbal communities of combination. Conclusion : Core herbs were found based on the frequency notion, which is a traditional analysis method. Also, core herbs, herbal combinations, formulas that can that may be overlooked when using frequency notions were found by using network analysis. The results may lead to enhancing efficiency in the education of Internal medicine on Spleen system, Herbalogy, Formula science courses and the integration of courses.

      • Antiplatelet and antithrombotic effect of Phyllostachys pubescens leaves and Mume Fructus combination

        Background: Agents currently used for the treatment and prevention of thrombosis have a number of side effects. We conducted this study to develop antithrombotic agents from herbs that are used in food. Methods: The 80% (v/v) ethanol extracts of Phyllostachys pubescens leaf (PL) and Mume Fructus (MF) and their combinations—2:1 (PM21), 1:1 (PM11), and 1:2 (PM12)—were evaluated on rat platelet aggregation induced by adenosine diphosphate (ADP) in vitro and on arteriovenous shunt thrombosis after 3 days of oral treatment in rats in vivo. Results: At 100 μg/mL, PM21 and PM11 inhibited in vitro ADP-induced aggregation by 44.0 ± 4.3% and 30.0 ± 3.2%, respectively, whereas PL, MF, and PM12 weakly or scarcely inhibited ADP-induced aggregation by 3.9 ± 3.2%, 13.0 ± 2.7%, and 5.2 ± 1.3%, respectively. The IC50 values of PM21 on ADP-, collagen-, and thrombin-induced platelet aggregations were 135.6 ± 7.4 μg/mL, 142.7 ± 5.8 μg/mL, and 186.5 ± 9.7 μg/mL, respectively. In an in vivo rat arteriovenous-shunt thrombosis model, thrombus weight was significantly decreased after the oral administration of 400 mg/kg PL (27.8 ± 3.0%, p < 0.01) or MF (35.2 ± 2.1%, p < 0.01), and with a good accord to the in vitro results, the combination of PL and MF in the ratio of 2:1, PM21 (60.9 ± 1.2%, p < 0.001), showed a superior antithrombotic effect to those of individual extracts. At dosages of 200 mg/kg, 100 mg/kg, and 50 mg/kg, PM21 dose-dependently decreased thrombosis weight (ED50, 314 mg/kg). Conclusion: These results suggest that combination preparations of PL and MF, especially their 2:1 combination, can increase antiplatelet and antithromboticeffects more than PL and MF alone, offering evidence for a potential novel combination antithrombotic therapy.

        불면증에서 산조인의 유무에 따른 약물배오의 경향성 고찰

        Objectives : Zizyphi spinosi Semen is widely used for treating insomnia in korea and other oriental country. This study is analyzing combination tendency of Zizyphi spinosi Semen in herbal formula for treating insomnia. Methods : Herbal formulas for treating insomia were searched in "The Prescription Dictionary of Traditional Chinese Medicine", and checked in each literary source. Then, the herbal formula was divided in two groups : FCZ(The formula group containing Zizyphi spinosi Semen) and FWZ(The formula group without Zizyphi spinosi Semen). FCZ and FWZ were compared by following criteria; i) Prevalence of herbal formula by period ii) Prevalence of medicinal material's effects iii) Prevalence of medicinal material iv) Prevalence of the nature of midicinals v) Prevalence of flavor of medicinals vi) Prevalence of meridian entry. Results : 116 Herbal formula met our criteria, and herbal formulas were divided in two groups : FCZ(n=59), FWZ(n=57). The result of prevalence in FCZ is following; Herbal formula prevalence increased as time passed to the Qing(淸) dynasty. Tonifying and replenishing medicinal(補益藥), tranquillizing medicinal (安神藥), and inducing diuresis to drain dampness medicinal(利水?濕藥) are the three most prevalent types of materia medica. In specifically, Panax ginseng radix(人蔘), Liriopis Tuber(麥門冬), Angelicae Gigantis Radix(當歸), Poria Cocos(茯神), Glycyrrhizae Radix(甘草), Poria cocos(茯神), Polygalae Radix(遠志), Thuja Orientalis L.(柏子仁), Paeonia Japonica(白芍藥), and HgS(?砂) are combined frequently. The most prevalent nature of medicinals were 'warm microwarm(溫 微溫)' and flavor of medicinals was normal(平). Meridian entrys that frequently combined were heart(心), spleen(脾), and liver(肝). Conclusion : We found combination tendency of Zizyphi Semen in herbal formula for treating insomnia.

        항암제와 한약재의 병용투여 시 암세포 증식억제 효과에 대한 국내 실험연구 문헌고찰

        Objective : In this study, we searched the experimental research about combined treatment of anticancer drug and herbal medicine for killing or inhibiting proliferation of Cancer cells searched in OASIS and KISS. This study aimed to analyze the experimental research paper about anticancer drug combined treatment with herbal medicine. Methods : We collected the research paper including killing or inhibiting proliferation of Cancer cells in OASIS and KISS using keyword anticancer drug with herbal medicine, tumor suppressor with herbal medicine, inhibition of Cancer with herbal medicine and combined treatment with herbal medicine. Assorting by cancer cells, we analyzed experimental results cancer cell viability, anticancer drug dosage, tumor weight and survival rate. Also, we checked the effects of herbal medicine on cancer and additive effect reducing side effect of anticancer drug. Results : Total 45 studies were selected. 38 studies reported combined treatment of anticancer drug and herbal medicine was more effective than only anticancer drug. The death of cancer cells was synergistically induced by the cotreatment of anticancer drug and herb extracts. The studies suggest that the cotreatment of anticancer drug and herb extracts could reduce side effect of anticancer drug. In addition, some studies reported cotreatment mechanism like apoptotic death signal processes. In combined treatment of anticancer drug and herb extracts, The expression of Fas/Fas L, Bax, Bcl2, Caspase-3 etc.. was markedly increased in cancer cells. Conclusions : Our results suggest that anticancer drug combined treatment with herbal medicine could be efficient for killing or inhibiting proliferation of cancer cells. However, this paper had some limitation as follows: First, collected studies have been published only for korean journal. Second, results of research and effects of combined treatment are not collected objectively. To solve these problems, more objective and balanced studies should be performed.

        아토피 피부염에서 한약 외용제와 양약 병용 치료의 효과: 체계적 문헌고찰 및 메타분석

        Objectives: This study was conducted to evaluate the effect of combination treatment of herbal medicine for external use and western medicine for atopic dermatitis (AD). Methods: We searched randomized controlled trials (RCTs) which assess the effect of combination treatment of herbal and western medicine for AD through 8 electronic databases from the start to December 2022. The data synthesis was conducted by using Review Manager (RevMan, ver.5.4.1) and Cochrane risk-of-bias tool was used to evaluate the risk of bias. Results: 13 RCTs were included. The combination treatment group had significantly higher total efficacy rate(p<0.0001) and lower SCORAD score (p<0.00001) than the western medicine treatment group. The adverse event rate was also significantly lower in the combination treatment than the western medicine treatment group (p<0.0001). But there was no significant difference in recurrence rate (p=0.09). Conclusion: This study demonstrates that the combination treatment of herbal and western medicine could be safe and effective for AD. However, due to limits of included studies such as high heterogeneity between the literature and unclear risk of bias, further studies are warranted.

        Effects of the Combination Herbal Extract on Working Memory and White Matter Integrity in Healthy Individuals with Subjective Memory Complaints : A Randomized, Double-Blind, Placebo-Controlled Clinical Trial

        Objectives The combination extract of four kinds of herbs, Gastrodia elata, Liriope platyphylla, Dimocarpus longan, and Salvia miltiorrhiza, has shown to have memory improving effects in mice. The aim of this study was to investigate the efficacy and safety of the herbal mixture for improving working memory as well as microstructural changes in white matter integrity in individuals with subjective memory complaints. Methods Seventy-five individuals with subjective memory complaints were assigned to receive either placebo (n = 15) or herbal mixture (low-dose group, n = 30 and high-dose group, n = 30) supplementation in an 8-week, randomized, double-blind, placebo-controlled clinical trial. Changes in working memory performance and fractional anisotropy (FA) values reflecting white matter integrity from baseline to 8-week endpoint were assessed. Results The herbal mixture group showed an increase in working memory performance compared to the placebo group (p for interaction = 0.001). In addition, the herbal mixture group showed an increase in FA values in the temporo-parietal regions (corrected p < 0.05), which are crucially involved in working memory function and are among the most affected regions in patients with cognitive impairments. Conclusions Findings from this study indicate that the herbal mixture may be a promising therapeutic option for individuals with subjective memory complaints.

        SH-SY5Y 세포에서 도네페질과 병용투여시 신경보호 효과를 나타내는 한약재의 in vitro 선별 연구

        Objectives: The purpose of this study was to evaluate the neuroprotective effects of donepezil and 33 kinds of herbal extract combinations in SH-SY5Y cells with Aβ <sub>25-35</sub> treatment. Methods: MTT assay was performed to measure the cell viability of each herbal extract combined with donepezil against Aβ-induced neurotoxicity. The most active extracts were then subjected to assess the effects on CREB phosphorylation and COX-2 expressions through the western blot analysis. Results: There were eight herbal extracts representing significant increase on the cell viability: 1) Erycibe obtusifolia, 2) Polygonum multiflorum, 3) Polygala tenuifolia, 4) Illicium verum, 5) Santalum album, 6) Loranthus parasticus, 7) Platycladus orientalis, and 8) Zanthoxylum piperitum. Especially, when Santalum album and donepezil were treated together, the phosphorylation of CREB significantly increased and COX-2 protein expression was significantly inhibited. Conclusions: Among the screened herbal extracts, combination treatment of each of the eight herbs and donepezil showed neuroprotective effects in SH-SY5Y cells. Additionally, the combination of Santalum album and donepezil suggested cognitive improvement by up-regulation of p-CREB and down-regulation of COX-2.
