        Arachnoid Cyst in Oculomotor Cistern

        Oculomotor cistern is normal anatomic structure that is like an arachnoid-lined cerebrospinal fluid-filled sleeve, containing oculomotor nerve. We report a case of arachnoid cyst in oculomotor cistern, manifesting as oculomotor nerve palsy. The oblique sagittal MRI, parallel to the oculomotor nerve, showed well-defined and enlarged subarachnoid spaces along the course of oculomotor nerve. Simple fenestration was done with immediate regression of symptom. When a disease develops in oculomotor cistern, precise evaluation with proper MRI sequence should be performed to rule out tumorous condition and prevent injury of the oculomotor nerve.

        인플루엔자B 바이러스 감염 후 발생한 단독 동안신경마비 1례

        단독 동안신경마비는 소아에서 매우 드물며 선천성과 외상이 흔한 원인 중 하나이다. 감염 후 발생한 단독 동안신경마비는 인플루엔자 A 바이러스를 포함한 바이러스 질환에서 보고되었으나 소아에서 인플루엔자 B 바이러스와 관련된 보고는 없었다. 본 저자들은 4세 소아에서 인플루엔자 B 바이러스 감염 후 발생한 단독 동안신경마비 1례를 경험하고, 이 증례를 통해 고 용량 스테로이드 치료의 유용성을 제시하고자 하였다. Isolated oculomotor nerve palsy is very rare in children but if present, congenital disease and trauma are the most common etiologic factors. Post-infectious isolated oculomotor nerve palsy has been identified rarely with several viruses including influenza A virus but not influenza B virus, especially in children. A 4-year-old girl was admitted because of ptosis and ophthalmoparesis of her left eye after influenza B infection. Brain magnetic resonance imaging (MRI) demonstrated diffuse thic-kening of the cisternal segment of the left oculomotor nerve. Laboratory tests were negative for other pathogens or malignancy. She was treated with methylpre dniso-lone pulse therapy because of progressive blepharoptosis and ophthalmopa resis. Within 3 months follow up, the ophthalmoparesis and pupillary abnormalities showed complete recovery, and brain MRI also revealed improvement. Here, we present a case of acute isolated oculomotor nerve palsy after influenza B virus infection.

        동안신경마비 사시환자 1례 증례보고

        Oculomotor nerve palsy presents itself with sudden onset unilateral ptosis and inability to turn the eye upward, downward, or inward, which causes visual disturbances. Strabismus caused oculomotor nerve palsy refers to muscle imbalance that results in improper alignment of the visual axes of the two eyes It may be divided into paralytic and non paralytic strabismus. paralytic strabismus is primarily a neurologic problem. Characteristic clinical disturbances result from lesions of the third, fourth, and sixth cranial nerves. Lesions of the third nerve result in a paralysis of lateral or outward movement and a crossing of the visual axes. Objective: This study was designed to evaluate the effects of oriental medicine therapy on a peripheral oculomotor nerve palsy. Methods & Result: The clinical data was analyzed on a patient with oculomotor nerve palsy whose main symptoms were right side ptosis and inability to turn the eye inward. The patient was treated by the electroacupucture(4Hz frequency, intensity was adjusted so that localized muscle contractions could be seen). As the result, symptoms are improved remarkably. Conclusion: The patient showed right eye ptosis and unilateral deviation of the right eye ball in neutral position. After acupuncture treatment and electroacupuncture treatment, the ptosis and deviation of the patient's right eye was recovered six weeks after the onset. The study suggests that oriental medicine therapy is significantly effective on the treatment of peripheral oculomotor nerve palsy.

      • 제2형 당뇨병 환자에서 발생한 자발적 안구마비 2예

        Diabetics are predisposed to certain acute mononeuropathies, including a cranial neuropathy involving ocular motor nerves. Oculomotor nerve palsy is the most common cranial neuropathy in diabetes mellitus. Affection of several nerves in one eye can occur, rarely. Such as, the third and the sixth or the third and the fourth. The clinical characteristics of diabetes-associated ophathalmoplegia include abrupt onset, frequent occurrence of short-lived ipsilateral pain, sparing of pupillary reflex, resolution in most cases within a few months. Clinicopathological studies have suggested that diabetic ophathalomoplegia results from microvascular ischemia of an oculomotor nerve in its subarachnoid, cavernous segment or mid brain. Pupillary sparing is a single feature of diabetic third nerve palsy, and it has been widely used to distinguish diabetic oculomtor palsy from extrinsic compressive lesion of the third nerve, such as an aneurysm in the carotid siphon. No specific treatment is necessary. We experienced two cases of diabetic spontaneous ophthalmoplegia, one affected oculomotor nerve and the other affected partially oculomotor nerve and trochlear nerve, so we report these cases with review of the literatures.

        양측 해면정맥동내 뇌동맥류를 동반한 동안신경 섬유종 : 증례보고 Case Report

        The authors report a rare case of neurofibroma of the oculomotor nerve combined with the bilateral intracavernous aneurysms of the internal carotid artery. A 38-year-old female presented with a 2 month history of ocular pain and ptosis on the left side. Computed tomogram and magnetic resonance image showed a parasellar enhancing mase Angiogram reveaied bilateralintracavernous aneurysm of the internal carotid artery and multiple beaded appearance of the internal carotid artery. The tumor was completely removed via left Dolenc approach and the oculomotor nerve was reconstructed by end-to-end anastomosis, with the sural nerve. Ipsilateral intracavernous aneurysm was clipped using an encircling clip.

        Upon review, it is noted that recurrent painful ophthalmoplegic neuropathy (RPON) is a rare neurological syndrome characterized by recurrent unilateral headaches and painful ophthalmoplegia of the ipsilateral oculomotor nerve. As seen on brain MRI, thickening and enhancement of the oculomotor cranial nerve can be observed in these cases. We experienced a case of RPON in an adult patient who showed thickening and enhancement of the oculomotor nerve on gadolinium-enhanced 3D-FLAIR image. The authors report a case of RPON with a review of the literature.

        중간뇌 해면기형으로 인한 단독 눈돌림신경마비

        A 36-year-old previously healthy woman developed diplopia and right-sided ptosis. Neurological examinations were normal except for palsy in the right third cranial nerve with pupil involvement. CT and MRI revealed a cavernous hemangioma with recent bleeding in the right midbrain. This case indicates that midbrain cavernous hemangioma can be a rare cause of isolated pupil involving palsy of the third cranial nerve. A 36-year-old previously healthy woman developed diplopia and right-sided ptosis. Neurological examinations were normal except for palsy in the right third cranial nerve with pupil involvement.         Cavernous Angioma of the Oculomotor Nerve

        Cavernous angiomas of the cranial nerves are rarely reported. We report a case of a 33-year-old man affected by a cavernous angioma originated in the oculomotor nerve with it's palsy. Preoperative radiological findings are difficult to differentiate it from meningioma or neurinoma. Postopertive pathological report discloses it as cavernous angioma. We discuss radiological, pathological features and management of this vascular lesion of the cranial nerve.

        해면정맥동 외측벽의 미세해부

        In the microsurgical dissection of the lateral wall of the cavernous sinus. interrelationships of its nerves have been studied in 100 Korean adult half heads. And in the serial coronal section of the cavernous sinus in 10 half heads, the structures of the lateral wall have been studied. The relationships of the nerves of the lateral wall were classified to five types according to the course of the trochlear nerve. Each length of the Parkinson's triangle and the angle between trochlear and ophthalmic nerves were measured. These data were compared with those of the other races. The lateral wall of the sinus was composed of two layers. The orientation of the fibers of the deep layer was variable in regions. The oculomotor nerve was enveloped with sheath extended from dura mater and it was connected with deep layer of the lateral wall. The trochlear nerve was enveloped with sheath formed by division of innermost part of the deep layer. The venous sinus was found between the two layers of the lateral wall. The neurosurgical significance of the lateral wall of the sinus was discussed.
