탄력밴드운동시 NIDDM 환자의 혈장내 Leptin 수준과 Insulin,Glucose 신체구성성분과의 관련성 분석

이규성, 김문회. 탄력밴드운동시시 NIDDM 환자의 혈장내 Leptin 수준과 Insulin, Glucose 및 신체구성성분과의 관련성을 분석. 운동과학, 제11권 제1호, 131-143, 2002. 이 연구는 탄력밴드 운동시 NIDDM환자의 혈장내 Leptin 수준과 Insulin, Glucose 및 신체구성성분과의 관련성 분석하고자 하는데 있다. 연구대상은 00시에 거주하는 제 2형 당뇨 비만노인여성환자 (체지방량이 30% 이상)로 저항성-밴드 운동군(exercise group) 9명, 통제군 (control) 10명으로 총 19명을 대상으로 하였다. 운동프로그램은 저항성 밴드 트레이닝 운동으로 총 12주간 실시하였으며, 초기단계는 7종목, 둘째 단계는 9종목, 세번째 단계에서는 11종목으로 구성하였다. 운동강도는 밴드를 늘린 길이 즉, 저항력에 따라 분류하는데 노인 여성인 점을 감안하여 주로 노랑색 (thin; 20㎝ -0.7㎏, 40㎝ - 1.0㎏, 60㎝ 1.1㎏의 저항력) 과 적색(medium; 20㎝ - 0.9㎏, 40㎝ - 1.6㎏, 60㎝ 2.0㎏의 저항력)을 이용하였다. 운동빈도는 주당 3∼4일로 운동시간은 총 65분 정도로 2∼3set씩 실시하였다. 자료처리 방법은 12주간의 저항성-밴드운동이 제2형 당뇨 노인여성의 집단간, 기간간의 혈청 leptin, insulin, glucose, NEFAs수준 및 신체구성성분의 차이를 알아보기 위해 반복측정에 의한 이원변량분석을 실시하였으며, 운동후 6주와 12주후에 leptin, insulin, glucose, NEFAs수준 및 신체구성성분과의 관련성을 의해 Pearson의 상관분석을 실시하였다. 이 연구결과 집단별 (운동여부), 기간별 (사전, 6주후, 12주후)에 따른 OGTT 결과, 운동군에서 훈련 6주후에 근육내 uptake 비율이 약간 높아진 것으로 나타났으며, 혈청 렙틴, 인슐린과 NEFAs수준는 운동군에서 운동기간이 지남에 따라 감소하는 것으로 나타났다. 또한 leptin과 insulin, glucose, NEFAs수준 및 신체구성성분과의 관련성을 살펴본 결과 렙틴은 체지방량과 NEFA가 높은 관련성이 있는 것으로 나타났다. 이러한 결과는 12주간의 저항성 밴드 트레이닝이 저항성 운동임에도 불구하고 NIDDM 노인환자의 렙틴 수준, 인슐린, NEFAs 수준을 감소시킨 결과를 가져와 지질대사를 향상시키고, 체중 감소를 유도하는 운동으로서 적절하다는 활용 가능성을 확인케 하였다. To evaluate analysis of relationships on plasma leptin and insulin, glucose levels and body composition 20 obese subjects with NIDDM were enrolled in 12 weeks of band-resistance training. The training program consisted of band exercise for 40min at least 5 times per week, with the intensity of exercise maintained at 50% of 1RM. Serum leptin levels decreased significantly in the exercise training group (p<.05), but not in the sedentary group. Body weight and percentage body fat decreased in the exercise training group, but not in sedentary group. However, The concentrations of plasma insulin and NEFAs did not differ between the groups either before or after exercise training. This study demonstrates that band-resistance training in obese women with NIDDM subjects reduces serum leptin levels dependent of changes in body fat mass during 12 weeks of band-resistance training

The Significance of Caspase-Cleaved Cytokeratin 18 in Pleural Effusion

Background: Apoptosis plays a role in the development of pleural effusion. Caspase-cleaved cytokeratin 18, a marker for epithelial cell apoptosis, was evaluated in pleural effusion. Methods: A total of 79 patients with pleural effusion were enrolled. The underlying causes were lung cancer (n=24), parapneumonic effusion (n=15), tuberculous effusion (n=28), and transudates (n=12). The levels of M30, an epitope of caspase-cleaved cytokeratin 18, were measured in blood and pleural fluids using enzyme-linked immunosorbent assay along with routine cellular and biochemical parameters. The expression of M30 was evaluated in the pleural tissues using immunohistochemistry for M30. Results: The M30 levels in pleural fluid were significantly higher in patients with tuberculosis (2,632.1±1,467.3 U/mL) than in patients with lung cancer (956.5±618.5 U/mL), parapneumonic effusion (689.9±413.6 U/mL), and transudates (273.6±144.5 U/mL; all p<0.01). The serum levels were not significantly different among the disease groups. Based on receiver operating characteristics analysis, the area under the curve of M30 for differentiating tuberculous pleural effusion from all other effusions was 0.93. In the immunohistochemical analysis of M30, all pathologic types of cancer cells showed moderate to high expression, and the epithelioid cells in granulomas showed high expression in tuberculous pleural tissues. Conclusion: Caspase-cleaved cytokeratin 18 was most prominently observed in tuberculous pleural effusion and showed utility as a clinical marker. The main source of M30 was found to be the epithelioid cells of granulomas in tuberculous pleural tissues.
선천성 무홍채증 환자에서 인공수정체 삽입술 후 발생한 전방 내 섬유혈관막 1예

Purpose: To report a case of an anterior fibrovascular membrane following cataract extraction and intraocular lens implantation in a patient with congenital aniridia. Case summary: A 13-year-old girl presented with congenital aniridia and cataracts in both eyes. She underwent cataract extraction by phacoemulcification with intraocular lens implantation. Six months after cataract surgery, a progressive anterior chamber fibrovascular membrane was noted in both intraocular lens and rudimentary iris. Surgical excision of the fibrovascular membrane was performed, but there was recurrence after five weeks in both eyes. Subsequent surgical intervention on both eyes involved intraocular lens explantation combined with membranectomy to prevent recurrence and phthisis. Surgical findings indicated that the fibrovascular membrane involved the retrolenticular space, and histopathological evidence indicated that the extensive fibrotic tissue originated from the root of the rudimentary iris. Conclusions: Patients with congenital aniridia should be monitored carefully for the development of intraocular fibrosis after intraocular lens implantation. If a fibrovascular membrane is noted, early surgical intervention is recommended, and the explantation of the intraocular lens should be considered during surgical intervention to prevent recurrence and complications.
