      • Nursing education, practice and research in Korea

          Purpose: Korea is one of very fast-changing countries in economy, life style and health problems. This article explores the current situation of Korea first, and then nursing education, nursing practice, nursing research, nursing informatics and expansion of nursing territory and entrepreneurship in Korea will be presented. Finally, this article explores some points for the future of Korean nursing. Method: The literatures related to Korean nursing were reviewed in terms of nursing education, practice and research. Result: There are two types of basic nursing education program, 63 three-year programs and 53 four-year programs. ANPs authorized by the MOHW of Korea is 11 fields. 86.7% of KNA nurses work for hospital & clinic, 3.5% work for national public health care system, and 2.0% of them work as nurse practitioners who can write out prescription at public health center in rural areas. Nurses agree that a gap still exists between research and practice. Nurses do not use research as often as they could. In accordance with the social demand, field of nursing is expanded to hospice, parish nursing, community-based nursing center, nursing for prison, health promotion, etc. Conclusion: According to the understanding of nursing education, practice, research, nursing informatics, expansion of nursing territory and entrepreneurship in Korea, we can imagine the future of Korean nursing. First is unification of nursing education system to four-year program. Second is making Nursing Practice Act as a separate law which is included in the Medical Practice Act now. Third is increase of the entrepreneurship of nurses. In addition to these, globalization, localization, contribution to ubiquitous health, and strong leadership should be considered for the future of Korean nursing.

        임상간호사의 핵심간호술기 교육에 대한 인지 및 교육 요구도

        To investigate hospital nurses’ awareness and educational needs of Core Basic Nursing Skill Education in this study, mixed methods research has been implemented by using both qualitative research method with phenomenological research method applied and quantitative research method with questionnaires at the same time. Above all, in-depth interview has been given to fifteen participants with phenomenological research method in order to learn new nurses’ awareness of education of Core Basic Nursing Skills, and data analysis has been carried out using a method suggested by Colaizzi (1978) among other phenomenological research methods according to the standard procedure. As a result of interview data analysis, 20 theme-clusters and 6 categories have been drawn from 119 meaning formations. The six categories included “Existence of sense of difference from clinical field”, “Importance of first-hand training experience”, “High/low utilization”, “Regretfulness of not being prepared due to a lack of education”, “Advancement of core skill education” and “Value of core skills realized in clinical field.” In the quantitative research implemented to enrich the result of the qualitative research, 512 hospital nurses have completed questionnaires and the items of nursing skills required for new nurses have been investigated. The result shows that defibrillator instructions (16.60%) was the most chosen item that the nursing students regarded as needs for education followed by common use of respirator (14.06%), use of infusion pump (13.28%), use of electrocardiogram (ECG; 13.28%), etc. in the order listed, which would suggest demand for education of use of medical equipment was relatively high. It is, therefore, expected the study result would be usefully applied in order to establish effective nursing education strategies by promoting a better understanding of new nurses’ experience of Core Basic Nursing Skill education and providing baseline data to raise the quality of education and lead the way forward for the future. 본 연구는 임상간호사의 핵심기본간호술기 교육에 대한 인지 및 교육요구도 조사를 위해 현상학적 연구방법을 적용한 질적 연구와 질문지를 활용한 양적 연구를 동시에 시행한 혼합연구방법으로 수행되었다. 우선 신규간호사의 핵심기본간호술기 교육에 대한 인지를 파악하기 위해 현상학적 연구방법을 적용하여 참여자 15인을 대상으로 심층면담을 진행하였으며 자료 분석은 현상학적 연구방법 중 Colaizzi(1978)가 제시한 방법으로 분석 절차에 맞게 분석하였다. 이를 통한 면담자료 분석결과 119개의 의미구성으로부터 20개의 주제묶음, 6개의 범주로 도출되었다. 6개의 범주는 ‘임상 현장과의 괴리감의 존재’, ‘몸으로 경험한 훈련의 중요성’, ‘활용도의 높고 낮음‘, ’교육의 부재로 준비되지 못한 아쉬움‘, ’핵심술기 교육의 도약‘, ’임상에서 발견한 핵심술기의 가치‘으로 도출되었다. 질적 연구 결과를 풍성하게 하기 위해 시행된 양적 연구는 임상 간호사 512명을 대상으로 질문지를 작성하도록 하여 신규간호사에게 필요하다고 요구되는 기본간호술 교육의 항목을 조사하였다, 그 결과 교육의 필요성으로 가장 높게 나타난 항목은 제세동기 사용법(16.60%)이고 그 다음으로는 인공호흡기 상용(14.06%)에 관한 내용이었고 Infusion Pump 사용(13.28%), 심전도 사용(13.28%) 등의 순으로 대부분 장비 사용에 대한 요구가 높게 나타났다. 본 연구결과는 신규간호사의 핵심기본간호술 교육에 대한 경험의 이해를 높이고 핵심기본간호술 교육의 질 향상과 앞으로의 방향 설정에 대한 기초적인 자료를 제공함으로서 효과적인 간호교육전략을 마련하는데 유용하게 활용될 것이라 기대한다.

      • 국내 간호연구에 대한 교육실태 및 교수연구 지원에 관한 조사연구

        A nationwide survey was taken place in order to study present status of nursing research education and faculty research support in Korean nursing education system. Sixteen out of 19(84.2%) baccaleureate program nursing schools and 26 out of 43 (60.5%) diploma program nursing schools responded to mailed questionnaire. Then data from 42 nursing programs were analyzed for the study. The study results show as below; 1. Nursing research education is increasing in beccaleureate and diploma nursing programs. In diploma nursing programs, 15 programs (71.5%) offered optinal nursing research courses. Most of these courses were opened recently (87% in 1-5 years), and reflects nursing educators' changing view regarding roles of clinical nurses in nuring research. In baccaleureate program, all 16 schools offered nursing research courses, 71.4% as required, 28.6% as optional, and 79.6% of these courses were open before 1985. 2. At least one nursing research course in required in all of master's programs. 3. The quality and quantity of support given to faculty members' research activities was poor in general. Respondents pointed out as difficulty of funding as the biggest problem in research activities. It was suggested that nursing scientists should take effort in order to give wide publicity for excellence and uniqueness of nuring science, especially to other academic societies and research funding organizations. It is suggested that hours of lecture and clinical instruction, support personnel for faculty research, academic activities(in nation and international) and publication should be included in future study of faculty research activities.

        Experience of nursing students about the barriers to patient education: a qualitative study in Iran

        Purpose: Patient education is a dynamic and continuous process that should be implemented during the entire time of hospital stay and even afterward. Studies have shown the typically poor quality of patient education in Iran and its failure to convey the required knowledge and skills to patients. The purpose of this study was to survey the experience of nursing students in regard to the challenges of patient education in hospitals. Methods: This qualitative study was conducted using the conventional qualitative content analysis approach on a sample of 21 undergraduate nursing students (4th semester and beyond), which was drawn from the Qom Nursing and Midwifery School through purposive sampling with maximum variation. Data were collected through semi-structured interviews conducted over a period of 45 to 75 minutes, and were analyzed using the conventional qualitative content analysis. Results: Results were derived from the experiences of 21 nursing students (nine males, 12 females) about the research subject. The primary themes identified in the study were the student-related, patient-related, instructor-related, education environment-related, and curriculum-related barriers to patient educations. Conclusion: Participants believed that patient education in Iranian hospitals is faced with many challenges. Nursing instructors and curriculum planners should ensure more emphasis on patient education at the initial semesters of nursing education curriculum and make sure that it is included in the evaluation of students. Hospital officials should provide a dedicated education environment with suitable facilities, tools, and atmosphere for patient education. Also, special education programs need to be developed for less educated patients.

        Exploring Experiences of Psychiatric Nursing Simulations Using Standardized Patients for Undergraduate Students

        Purpose: This study was conducted to explore and understand the experiences of nursing students related to psychiatric simulation using standardized patients and to identify the value of such simulations in relation to the psychiatric clinical practicum. Methods: This study was designed using qualitative research. Eleven undergraduate students were individually interviewed regarding their experiences with psychiatric nursing simulation. A thematic content analysis was conducted to derive themes from subthemes, which were derived from contents of the interview. Results: Four themes were derived from nine subthemes by means of thematic analysis of nursing students’ experiences regarding psychiatric nursing simulation with standardized patients compared to clinical psychiatric nursing practicum. The topics studied included gaining practice without fear, gaining confidence in clinical practicum, being embarrassed by the gap and being in need of further simulation. Conclusion: Exploration of student responses comparing simulation and clinical practicum enhances integration of dichotomous experiences that bridge the theory-practice gap, which magnifies the benefits of the simulation.         간호학 전공 대학원생들의 연구윤리에 대한 인식, 자기효능감 및 지식수준

        This study aimed to identify the level of awareness, self-efficacy, and knowledge of research ethics among nursing graduate students. A descriptive survey design was used. A total of 102 nursing graduate students were recruited from one nursing school in Korea. Data collection instruments were developed based on the core instructional areas of Responsible Conduct of Research (RCR) from the Office of Research Integrity at the research team’s institution. Participants completed a self-reported questionnaire consisting of questions on research ethics. The data were analyzed with descriptive statistics using PASW Statistics 18.0 Window program. Participants (mean age=31 years) consisted of master’s (77.5%) and doctoral (22.5%) level students. Only 25.5% of participants had taken a course in research ethics, and 26.5% of participants had experience conducting research as principle investigators. The level of awareness (mean score 2.91 out of 5) and self-efficacy for good research practice (3.15 out of 5) were moderate. The level of knowledge regarding research ethics was low (10.59 out of 16). The area of lowest awareness was ‘protection of intellectual property and patents,’ followed by ‘justifying authorship’;the area of least knowledge was ‘writing research articles,’ followed by ‘criteria for authorship.’ Statistically significant correlations were found between awareness, self-efficacy, and knowledge of research ethics (all p<0.05). These results support the conclusion that educational programs for enhancing awareness, self-efficacy, and knowledge of research ethics for graduate nursing students are needed in order to help students conduct ethically sound research.

        시뮬레이션 기반 실습 교육을 통한 간호학생의 간호전문직관 인식 경험: 포커스 그룹 인터뷰를 중심으로

        This study was undertaken to identify nursing professionalism perceptions among nursing students who had completed a semester of simulation-based education, and to provide the basis for presenting direction of the simulation curriculum that helps in the emergence of a positive nursing professional in future. Data obtained from focus group interviews of study participants determined 17 semantic categories and 6 themes through the process of identifying, comparing, and categorizing meaningful concepts. The perception of nursing students regarding the profession after simulation-based education include 'realization of professionalism', 'ethics and emotions as professional human subjects', 'cultivation of communication skills', 'integrative thinking for problem solving', and 'foundation', and also the importance of a 'foundational knowledge body' and 'reflection of self-capacity'. The results of this study reflect on 5 intuitional areas of a nursing professional: professional self-concept, social awareness, nursing expertise, nursing room role, and independence of nursing. The results of this study indicate that in order to enable nursing students to experience nursing professional intuition through simulation-based hands-on training, it is necessary to actively revise and apply courses for the development of a positive nursing professional intuition through simulation education. 본 연구는 한 학기 동안 시뮬레이션 교육과정을 이수한 간호학생들을 대상으로, 이들의 간호 전문직관 인식 경험을 파악함으로써, 향후 긍정적 간호 전문직관을 형성을 돕는 시뮬레이션 교과목 운영의 방향성을 제시하는 데 근거자료를 마련하고자 수행하였다. 2019년 6월부터 7월까지 서울시에 소재한 일개 간호대학에서 한 학기 동안 시뮬레이션 기반 실습교과목에 참여한 4학년 간호 대학생을 대상으로 포커스 그룹 면담을 실시하였다. 연구 참여자들의 포커스 그룹 인터뷰 후, 자료질적내용분석 방법에 따라 의미있는 개념을 확인하고, 개념들의 비교하며 범주화하는 과정을 통하여 17개의 의미 범주와 6개의 주제가 도출되었다. 시뮬레이션 기반 교육 후 간호학생의 인식한 간호 전문직관은 ‘전문직임을 실감함’, ‘인간 대상 전문직으로서의 윤리의식과 정서’, ‘의사소통 능력의 함양’, ‘문제해결을 위한 통합적 사고’, ‘근거기반 지식체의 중요성’, ‘자기 역량에 대한 성찰’ 이었다. 연구의 결과는 간호 전문직관의 5개 영역인 전문직 자아개념, 사회적 인식, 간호의 전문성, 간호실무역할, 간호의 독자성을 반영하였다. 시뮬레이션 기반 실습교육을 통하여 간호학생들에게 간호 전문직관을 경험할 수 있도록 하였다는 점에서, 시뮬레이션 교육을 통하여 향후 긍정적 간호 전문직관을 형성하기 위한 교과목의 수정과 적용이 적극적으로 시도되어야 할 필요가 있다.

        간호대학생의 핵심기본간호술 훈련에 적용된 선후배간 동료교수활동 경험 연구 : 선배 동료교수자 경험 중심으로

        Purpose : The purpose of this study was to understand and describe the experience of senior peer tutoring for core basic nursing skills training in nursing students. Method : Data was collected in June 2019 by individual in-depth interviews of seven third-grade nursing students. The transcribed data underwent thematic analysis. Result : The analysis revealed four themes. First, “Senior peer tutoring to improve education and nursing skills performance” illustrates participants’ motivation to apply for senior peer tutoring. Second, “Training and feedback to become a senior peer teacher” describes re-learning and practice nursing skills under professor coaching through a peer teacher competency empowerment program before senior peer tutoring. Third, “Win-win nursing skills education field with junior students” delineates the experience of growing together as senior peer teachers help and teach junior. Last, “Development through self-reflection” illustrates self-awareness what needs to be improved and reserve to be a good nurse in the clinical field. Conclusion : These findings suggest that cross-age peer tutoring can supplement limitations of same-age peer tutoring in core basic nursing skills education. Therefore, it is necessary to develop competency empowerment programs for senior peer teachers to improve confidence in education and nursing skills performance.

        교육전담간호사가 주도하는 임상현장교육 프로그램이 신규 간호사의 간호업무성과와 교육 만족도에 미치는 효과

        Purpose: This study examained the effects of a clinical nurse education program led by clinical nurse educators on nursing performance and education satisfaction for newly graduated nurses. Methods: This study used a non-equivalent control group pre-test_post-test design. The clinical nurse education program was conducted for the experimental group. The experimental and control groups were measured using self-reported questionnaires as pre and post-surveys. The preliminary homogeneity test of general characteristics and dependent variables between the experimental group and the control group was analyzed using a X2 test, Fisher's exact test, independent t-test, Mann-Whitney U test and ANCOVA. Results: The experimental group showed higher nursing performance than the control group (F=4.50, p=.039). The experimental group showed higher educational satisfaction than the control group (F=4.91, p=.031). Conclusion: The clinical nurse education program led by clinical nurse educators improved the nursing performance of new nurses and increased the education satisfaction.

        간호대학생의 실무역량 강화를 위한 현장실습 개선방안: 포커스 그룹 인터뷰 연구

        Purpose: To determine the current status and demand for innovation of clinical nursing practicum and to suggest strategies to improve clinical nursing education to strengthen clinical nursing competencies. Methods: Focus group interviews were conducted with 21 participants who could provide sufficient information on nursing practice education. Collected data were analyzed using content analysis. Results: The following four main themes were identified: ‘revision of nursing curriculum to demonstrate competencies as a nurse after graduation’, ‘activating clinical nurses’ participation in undergraduate education’, ‘strengthening education and training after obtaining a license to demonstrate clinical competencies of new nurses’, and ‘institutional support for the introduction of various clinical practice systems’. Conclusion: Results of this study showed that curriculum revision to improve clinical nursing competencies should be based on integrated clinical practice model. Securing sufficient clinical nurse educators belonging to the practice institution is most important. In addition, political and financial support should be provided to strengthen academic-practice partnership.
