        A Case of Asymptomatic, Localized, and Idiopathic Diffuse Alveolar Damage

        Diffuse alveolar damage (DAD) is a histological change in lung tissue, and is generally caused by an acute lung injury, which is characterized by bilateral and widespread damages. Localized DAD occurs very rarely. The causes for DAD are numerous, but the chief cause is acute interstitial pneumonia or acute exacerbation of idiopathic interstitial pneumonia, in cases of idiopathic manifestation. The 82-year-old patient, in this case study, showed a DAD lesion in only 1 lobe. The patient was otherwise healthy, with no previous symptoms of DAD. He was admitted to our medical center owing to localized infiltration, observed on his chest radiograph. Laboratory studies showed no signs of infections. DAD was confirmed by a surgical lung biopsy. The patient received corticosteroid treatment and had gradually improved. We report the case of a patient with localized, idiopathic DAD that cannot be classified as acute interstitial pneumonia or acute exacerbation of idiopathic interstitial pneumonia.

        COVID-19 감염병 대응 의료진용 개인보호복 착용 시 온열 심리반응: 기온과 보호수준의 영향

        We aimed to evaluate discomfort levels while wearing personal protective equipment (PPE) of healthcare workers to protect from COVID-19 in a hot and humid environment. Six types of PPE (0.18~1.77 clo in thermal insulation; 0.227 ~ 0.319 kPa·m²·W<SUP>-1</SUP> in water evaporative resistance) were chosen and eight subjects (23.1 ± 3.3 y in age) were participated in a neutral (25ºC, 50%RH) and a hot and humid environment (33ºC, 70%RH). A trial consisted of 90 min (10-min rest, 60-min walking and 20-min recovery). Thermal sensation, thermal comfort, humidity sensation, shortness of breath and ratings of perceived exertions (RPE) were recorded. The results showed that the PPE were classified as three levels in terms of discomfort in 33ºC and 70%RH (Low, Middle, and High). Local thermal comfort, shortness of breath and RPE were more valid to classify the discomfort level, rather than overall thermal sensation or comfort. Furthermore, there were no relationships between thermal insulation (or water evaporative resistance) and overall thermal sensation or comfort. Using a powered air purifying respirator was beneficial in the neutral environment only. To conclude, the three levels of discomfort can be applied to improve subjective perceptions of healthcare workers who wear PPE in hot and humid environments.

        건강하고 젊은 성인에서 주파수 차이를 이용한 불일치음전위: 잠복기와 전위

        잠복기와 전위는 MMN의 측정 매개변수이다. 이 매개변수의 수치는 자극방안에 따라 민감하게 변한다. MMN 검사를 위해 주파수 차이를 자극변이로 이용하는 자극방안이 잘 알려져 있다. 이 자극방안을 이용하여 건강하고 젊은 성인에서 MMN 검사를 하고 매개변수의 참고치를 구하는 것이 본 연구의 목적이다. 저자들은 10명의 정상성인을 모집하였다. 이들의 연령은 평균 25.5세이고, 3명은 여성 7명은 남성이었다. MMN 검사를 위한 청각 자극방안에서 표준소리의 주파수는 1,000 Hz, 변이소리의 주파수는 1,032 Hz였다. MMN의 평균잠복기와 평균전위가 Fz에서 202 ms 와 1.88 ㎶, Cz에서 207 ms와 1.46 ㎶, C3에서 212 ms와 1.10 ㎶, 그리고 C4에서 214 ms와 1.45 ㎶였다. MMN의 잠복기와 전위사이에 상관성은 관찰되지 않았다. 본 연구는 적용하기 쉬운 표준자극방안으로 검사한 MMN의 잠복기와 전위의 참고치를 제시하였다. 이 정보는 임상과 기초연구에서 MMN의 유용성을 높일 것이다. Latency and amplitude are the measurement parameters of mismatch negativity (MMN). The values of the parameters vary sensitively with the stimulus paradigm. A paradigm using the frequency difference of sounds for the MMN study is well known. This study obtained the reference values of the parameters in healthy young adults by performing the MMN study using the frequency paradigm. The authors recruited ten healthy adults. Their average age was 25.5 years; three were female, and seven were male. On the auditory paradigm for the MMN study, the frequency of sound was 1000 Hz for the standard stimulus, and 1032 Hz for the deviant stimulus. The mean values of latency and amplitude of MMN were 202 ms and 1.88 ㎶ at Fz, 207 ms, and 1.46 ㎶ at Cz, 212 ms, and 1.10 ㎶ at C3, and 214 ms and 1.45 ㎶ at C4. There was no correlation between the latency and amplitude of MMN. This study presented the reference values of the latency and amplitude of the MMN using a standard paradigm that is easy to apply. This information may make the MMN useful for clinical applications and basic research.

        수상 회전식 태양광 발전시설 설치에 따른 농업용 저수지의 수질변화 평가

        본 연구는 수상 태양광 발전시설의 설치로 인한 농업용 저수지의 수질변화를 평가하기 위해 경기도 안성시 금광저수지에 위치한 수상 회전식 태양광 발전시설에서 발전시설 설치에 따른 차광구역 6지점과 비차광구역 4지점을 선정하여 1년 동안 총 16회에 걸쳐 차광으로 인한 수질변화를 시간과 수심 별로 분석하였다. 이를 위해 수온, pH, DO, Chl-a, BGA 항목을 0.3 m, 1 m, 3 m, 5 m의 수심별로 측정하고, 표층의 시료를 채수하여 COD, TN, TP 항목을 분석하였다. 연구결과, 금광저수지 내 10곳의 측정지점 간의 관측된 전 수질항목에서 차이는 유의확률(p - value) 0.05 이상으로 유의수준(α=0.05)에서 서로 다르지 않다고 통계학적으로 분석되었다. 이러한 결과를 토대로 측정지점을 차광구역(site 1~6)과 비차광구역(site 7~10)으로 그룹화 후 시간 및 수심에 따른 변화를 확인하였다. 차광구역과 비차광구역 간의 수온, pH, DO, COD, TN, TP, Chl-a, BGA의 계절 및 수심에 따른 차이는 유의한 수준에서 통계학적으로 다르지 않았다(p > 0.05). Chl-a와 BGA의 경우, 7월에 비차광구역보다 차광구역에서 일부 높은 농도가 관측되었으나 이는 기록적인 가뭄과 낮은 저수량, 발전시설 구조물에 부착된 부착조류의 과다성장으로 인한 일시적 현상으로 전체 수질은 통계학적으로 유의할 만한 차이가 없는 것으로 조사되었다. 이러한 결과는 수상 회전식 태양광 발전시설의 설치로 인한 저수지 수면의 차광이 전체 수면적 대비 0.5% 미만으로 일사량 유입 감소효과는 취송 및 방류를 통한 저수지 수체의 혼합 효과 대비 미미했기 때문인 것으로 판단된다. 향후, 수상 태양광 발전시설 설치로 인한 수질변화를 면밀히 연구하기 위해서는 보다 넓은 면적의 태양광 발전시설의 설치로 인한 차광과 함께 장기적인 수질 및 수생태계 관측이 필요할 것으로 판단된다. To evaluate the water quality changes in agricultural reservoir covered with floating photovoltaic solar-tracking systems, the water quality variations with time and depth were monitored on both six sites for light blocking zones and four sites for light penetration zones after the installation of floating photovoltaic solar-tracking systems in Geumgwang reservoir at Anseong-si, Kyeonggi province. For one year with 16 monitoring events, water quality parameters [i.e., water temperature, pH, dissolved oxygen (DO), chlorophyll-a (Chl-a), and blue-green algae (BGA)] were monitored at depths of 0.3 m, 1 m, 3 m, and 5 m, while chemical oxygen demand (COD), total nitrogen (TN), and total phosphorus (TP) were monitored at depths of 0.3 m. Statistically, the difference in all water quality parameters was not significantly different (p > 0.05) at the level of significance of 0.05. Based on these results, the water quality data from light blocking zones (site 1~6) and light penetration zones (site 7~10) were clustered, and were compared with time and depth. As a result, the difference in water temperature, pH, DO, COD, TN, TP, Chl-a, and BGA between light blocking zones and light penetration zones was not significant (p > 0.05) with different time and depth. For Chl-a and BGA, some data from light blocking zones greater than light penetration zones were temporary observed due to the severe drought, low water storage rate, and over growth of periphyton. However, this temporal phenomenon did not impact the water quality. Considering the small water surface area (≤0.5%) covered by floating photovoltaic solar-tracking systems, the mixing effect of whole Geumgwang reservoir caused by Ekman current and continuous discharge were more dominant than the effect of reduced solar irradiance. Further study is warranted to monitor the changes in water quality and aquatic ecosystems with greater water surface area covered by floating photovoltaic solar-tracking systems for a long time.

        Clinical and Endoscopic Features of Metastatic Tumors in the Stomach

        Background/Aims: Metastasis to the stomach is rare. The aim of this study was to describe and analyze the clinical outcomes of cancers that metastasized to the stomach. Methods: We reviewed the clinicopathological aspects of patients with gastric metastases from solid organ tumors. Thirty-seven cases were identified, and we evaluated the histology, initial presentation, imaging findings, lesion locations, treatment courses, and overall patient survival. Results: Endoscopic findings indicated that solitary lesions presented more frequently than multiple lesions and submucosal tumor-like tumors were the most common appearance. Malignant melanoma was the tumor that most frequently metastasized to the stomach. Twelve patients received treatments after the diagnosis of gastric metastasis. The median survival period from the diagnosis of gastric metastasis was 3.0 months (interquartile range, 1.0 to 11.0 months). Patients with solitary lesions and patients who received any treatments survived longer after the diagnosis of metastatic cancer than patients with multiple lesions and patients who did not any receive any treatments. Conclusions: Proper treatment with careful consideration of the primary tumor characteristics can increase the survival period in patients with tumors that metastasize to the stomach, especially in cases with solitary metastatic lesions in endoscopic findings.         Effect of Ce and La Substitution on the Microstructure and Magnetic Properties of Hot-deformed Nd-Fe-B Magnets

        Nd in hot-deformed Nd-Fe-B magnets was partially substituted by Ce and La for the purpose of reducing the materials cost and balancing the utilization of rare earth resources. Initial melt-spun ribbons with the nominal compositions of (Nd1-xMx)13.6FebalB5.6Ga0.6Co6.6 (x = 0, x = 0.2/M=Ce, x = 0.3/M=Ce, Ce+La, wt.%) were prepared by a single-roller melt-spinning method, and pulverized into powders. The powders were then hot-pressed at 973 K under 100 MPa, and deformed at 973 K until 75 % of height reduction was achieved. The magnetic properties of hot-deformed magnets were decreased with the substitution of Ce for Nd. In addition, simultaneous substitution of Ce and La for Nd resulted in much lower magnetic properties. This tendency was almost the same as initial melt-spun powders. The deterioration on magnetic properties by substation of Ce and La for Nd in hot-deformed Nd-Fe-B magnets could be attributed to not only inferior intrinsic properties of (Ce/La)₂Fe14B to Nd₂Fe14B phase but also the effect of Ce/La substitution on microstructure such as density and grain alignment during hot-deformation. Although the magnetic properties were deteriorated by substitution of Ce and La for Nd in hot-deformed Nd-Fe-B magnets, the cost performance was largely enhanced from 2.32 to 2.62 MGOe×kg/$ by 13 % when increasing the content of Ce substituted for Nd from 0 to 0.3 wt.%.
