        파킨슨병 변증 유형 및 지표 분포에 대한 전향적 다기관 관찰연구 프로토콜

        Objectives: This study investigated the pattern identification (PI) and clinical index of Parkinson’s disease (PD) for personalized diagnosis and treatment. Methods: This prospective observational multi-center study recruited 100 patients diagnosed with PD from two Korean medicine hospitals. To cluster new subtypes of PD, items on a PI questionnaire (heat and cold, deficiency and excess, visceral PI) were evaluated along with pulse and tongue analysis. Gait analysis was performed and blood and feces molecular signature changes were assessed to explore biomarkers for new subtypes. In addition, unified PD rating scale II and III scores and the European quality of life 5-dimension questionnaire were assessed. Results: The clinical index obtained in this study analyzed the frequency statistics and hierarchical clustering analysis to classify new subtypes based on PI. Moreover, the biomarkers and current status of herbal medicine treatment were analyzed using the new subtypes. The results provide comprehensive data to investigate new subtypes and subtype-based biomarkers for the personalized diagnosis and treatment of PD patients. Ethical approval was obtained from the medical ethics committees of the two Korean medicine hospitals. All amendments to the research protocol were submitted and approved. Conclusions: An objective and standardized diagnostic tool is needed for the personalized treatment of PD by traditional Korean medicine. Therefore, we developed a clinical index as the basis for the PI clinical evaluation of PD.         농촌 지역 노인 우울증 및 치매 예방을 위한 사회적처방의 효과평가

        Purpose: This study was to evaluate the effects of a social prescribing program on cognitive function and depression of the elderly in the rural community. Methods: A non-equivalent control group pretest-posttest design was used for this study. The experimental group (n=76) completed a 8-week intervention of social prescribing, while the control group (n=39) only participated in pre- and post- need assessment from April 2021 to October 2022. A Wilcoxon signed rank test was conducted to evaluate the effectiveness of a social prescribing program on the level of cognitive function and depression of the participants. Results: The level of cognitive function was improved significantly in the experimental group (p<.05) while there was no significant change in the control group. Depression has been significantly decreased in the experimental group (p<.01), but there was no statistically significant change in the control group. Conclusion: The results of this study suggest that the social prescribing program is effective for improving cognitive function and reducing depression of the elderly living in the rural area. Therefore, a social prescribing program can be utilized as a preventive program for dementia and depression of the elderly.

        오픈 플랜 업무공간에서 나타난 바이오필릭 디자인 특성 연구

        This study suggested a new direction for office design by examining the changes of workplace in the office environment after COVID-19 and analyzing the characteristics of biophilic design detailed elements in open-plan workplace. For this purpose, 17 cases were selected among 71 cases completed from January 2020 to December 2021 in USA, and biophilic design detailed elements were derived in the workplace. Also biophilic design detailed elements analyzed separately by office design components. This study is summarized as follows. First, 20 biophilic design detailed elements were derived out of 31 biophilic design detailed elements in the workplace of 17cases. Among design components composed office environments, the wall showed the highest application rate of biophilic design detailed elements. In addition, the most applied biophilic design detailed elements were found to be wood finishes, brown colors, plant pots, blue colors. Second, it is important to secure as many places as possible to see natural lighting and natural scenery when arranging furniture. Therefore, the location of the window should be considered when planning a workspace, and an environment should be created to maintain illumination similar to natural light. Third, natural elements such as plant pots, movable objects such as natural-form crafts, photos of nature, and stand lighting should be appropriately placed in workplace. In addition, it is necessary to plan to secure the autonomy, variability, and mobility of the space and provide a creative and flexible work environment.

        동해 울릉분지 가스 하이드레이트 매장 지역의 메탄산화 미생물 군집 조성 및 분포

        동해 울릉 분지 내 메탄 하이드레이트가 매장된 지역에서 혐기적 메탄 산화와 연관된 미생물 군집 특성을 이해하기 위해 메탄 누출이 있는 대륙사면 정점(UBGH2-3)과 메탄 누출이 없는 분지 정점(UBGH2-10)에서, (1) 퇴적물의 지화학적 성분 및 황산염 환원율을 측정하였으며, (2) 기능성 유전자 분석을 통해 혐기적 메탄 산화 미생물 및 황산염 환원 미생물 군집의 정량 및 다양성 분석을 수행하여 그 결과를 비교하였다. 황산염-메탄 전이지대(sulfate and methane transition zone, SMTZ)는 UBGH2-3에서 0.5~1.5 mbsf (meters below seafloor), 그리고 UBGH2-10에서는 6~7 mbsf에 분포하는 것으로 나타났다. 두 지역의 SMTZ에서 측정된 황산염 환원율은 UBGH2-3의 1.15 mbsf에서 $1.82nmol\;cm^{-3}d^{-1}$으로 나타났고, UBGH2-10의 SMTZ에서 황산염 환원율은 $4.29nmol\;cm^{-3}d^{-1}$으로 높은 값을 보였다. 총 미생물 16S rRNA gene과 기능성 유전자인 mcrA (methyl coenzyme M reductase subunit A) 및 dsrA (dissimilatory sulfite reductase subunit A)의 정량 PCR 결과 두 정점의 SMTZ 부근에서 상대적으로 높게 검출되었다. 그러나 UBGH2-10지역에서 mcrA 유전자는 SMTZ 아래인 9.8 bmsf에서 가장 높게 검출되었다. mcrA 유전자의 다양성 분석 결과 두 지역의 SMTZ와 그 아래 퇴적층에서 혐기성 메탄산화 고세균(ANME: Anaerobic MEthanothoph) 군집인 ANME-1이 우점하였다. 한편, ANME-2 군집은 메탄 누출이 발생하는 UBGH2-3지역의 2.2 mbsf 층에서만 관찰되었고, 더불어 dsr 유전자 다양성 분석 결과 ANME-2와 컨소시엄을 이루는 Desulfosarcina-Desulfococcus (DSS) 이 나타났다. 본 연구 결과, ANME-1과 ANME-2은 동해 심부 퇴적 환경에서 혐기적 메탄 산화 및 생성 과정에 관여하는 중요한 고세균 군집으로 사료되며, 또한 ANME-2/DSS 컨소시엄은 메탄이 누출되는 지역인 UBGH2-3과 같이 혐기적 메탄 산화가 활발한 곳에서 메탄 거동을 조절하는 중요한 미생물 그룹으로 인식된다. To elucidate the microbial consortia responsible for the anaerobic methane oxidation in the methane hydrate bearing sediments, we compared the geochemical constituents of the sediment, the rate of sulfate reduction, and microbial biomass and diversity using an analysis of functional genes associated with the anaerobic methane oxidation and sulfate reduction between chimney site (UBGH2-3) on the continental slope and non-chimney site (UBGH2-10) on the basin of the Ulleung Basin. From the vertical profiles of geochemical constituents, sulfate and methane transition zone (SMTZ) was clearly defined between 0.5 and 1.5 mbsf (meters below seafloor) in the UBGH2-3, and between 6 and 7 mbsf at the UBGH2-10. At the UBGH2-3, the sulfate reduction rate (SRR) in the SMTZ exhibited was appeared to be $1.82nmol\;cm^{-3}d^{-1}$ at the depth of 1.15 mbsf. The SRR in the UBHG2-10 showed a highest value ($4.29nmol\;cm^{-3}d^{-1}$) at the SMTZ. The 16S rRNA gene copy numbers of total Prokaryotes, mcrA, (methyl coenzyme M reductase subunit A), and dsrA (dissimilatory sulfite reductase subunit A) showed the peaks in the SMTZ at both sites, but the maximum mcrA gene copy number of the UBGH2-10 appeared below the SMTZ (9.8 mbsf). ANME-1 was a predominant ANME (Anaerobic MEthanotroph) group in both SMTZs of the UBGH2-3 and -10. However, The sequences of ANME-2 were detected only at 2.2 mbsf of the UBGH2-3 where high methane flux was observed because of massive amount of gas hydrate at shallow depth. And Desulfosarcina-Desulfococcus (DSS) that is associated with ANME-2 was detected in 2.2 mbsf of the UBHG2-3. Overall results demonstrate that ANME-1 and ANME-2 are considered as significant archaeal groups related to methane cycle in the subsurface sediment of the East Sea, and ANME-2/DSS consortia might be more responsible for methane oxidation in the methane seeping region than in non-seeping region.

      • Clinical correlation of nodal yield and disease recurrence in patients with node negative, early stage cervical cancer

        This study was aimed to evaluate the clinical impact of nodal yield on recurrence in patients with node negative, early stage cervical cancer. Retrospective chart review was performed for 93 women with early stage (clinical stage 1A2 to 1B1) cervical cancer, who underwent radical hysterectomy and pelvic lymphadenectomy between 2003 and 2011. According to the cut-off value (nodal yield > 27) which has been determined by the receiver operating characteristic curve, patients were divided into group A (nodal yield ≤ 27) and group B (nodal yield > 27). Patients in group B were associated with recurrence (p=0.058). Also, the disease-free survival was poor in group B, although it failed to show significance (87.1 ± 3.67 months vs. 67.9 ± 4.42 months, p=0.210). Clinicopathologic characteristics in both groups were not different, except for a larger tumor size of group B (p=0.044). Cox`s regression analysis revealed that a higher amount of nodal yield (p=0.049, 95% CI 0.050-0.997) and deep stromal invasion (p=0.045, 95% CI 0.048-0.963) were poor prognostic factors. Our findings show that an excessive pelvic lymphadenectomy had no benefit in reference to disease recurrence and survival.

        Fine-scale Microbial Communities Associated with Manganese Nodules in Deep-sea Sediment of the Korea Deep Ocean Study Area in the Northeast Equatorial Pacific

        Despite its potential significance for industrial utilization, any activities associated with the mining of manganese (Mn) nodules might have substantial impacts on benthic ecosystems. Because microorganisms respond quickly to changing environmental conditions, a study of microbial communities provides a relevant proxy to assess possible changes in benthic ecosystems associated with mining activities. We investigated fine-scale microbial community composition and diversity inside and on the surface of Mn nodules and in nearby deep-sea sediments in the Korea Deep Ocean Study (KODOS) area located in the Clarion-Clipperton Fracture Zone (CCFZ) of the northeast equatorial Pacific. Although microbial cell density was lower within nodules (3.21 × 106 cells g-1) than in sediment (2.14 × 108 cells g-1), nodules provided a unique habitat for microorganisms. Manganese-oxidizing bacteria including Hyphomicrobium and Aurantimonas in Alphaproteobacteria and Marinobacter in Gammaproteobacteria were abundant in nodules, which implied that these bacteria play a significant role in nodule formation. In contrast, Idiomarina in Gammaproteobacteria and Erythrobacter and Sulfitobacter in Alphaproteobacteria were abundant in sediments. Meanwhile, Thaumarchaeota, a phylum that consists of ammonia-oxidizing chemolithoautotrophs, were the predominant archaeal group both in nodules and sediment. Overall, microbial communities in Mn nodules were unique compared to those observed in sediments. Furthermore, the phylogenetic composition of microorganisms in the KODOS area was distinguishable from that in the nodule provinces claimed by China and Germany in the CCFZ and nodule fields in the central South Pacific Gyre, respectively.

        C57BL/6 Mice에서 이소플라본의 지질강하, 항산화, 항염증효과를 통한 항동맥경화 효과

        5주령의 C57BL/6 mice종 암컷 40수를 2주간의 적응기를 거쳐, 평균 체중 $17.9{\pm}0.9\;g$인 실험동물을 완전임의배치로 한 군당 10수씩 4군으로 나누어 실험식이는 동맥경화식이(control)에 이소플라본을 첨가한 실험식이를 각각 8주간 급여하면서 사육하였다. 이소플라본의 첨가량은 식이 100 g 당 10 mg(IF-10)과 40 mg(IF-40) 및 100 mg(IF-100)이 되도록 조정하여 제조하였다. 이소플라본의 섭취는 체중증가량에 영향을 미치지 않는 것으로 나타났으며 식이효율은 각 군 간의 유의적인 차이를 보이지 않았다. 이소플라본 섭취에 의한 혈장 TG 농도는 대조군에 비해 이소플라본을 첨가한 식이를 섭취한 군에서 감소하는 추세를 보였다. 총 콜레스테롤 수준은 모든 실험군에서 증가하는 경향을 보였으나 8주 후의 농도 변화는 대조군에 비해 이소플라본을 첨가한 식이를 섭취한 군에서 감소하는 추세를 보였다. 세포부착물질인 MCP-1과 지질과산화물인 TBARS의 생성량은 이소플라본의 첨가량이 높을수록 현저하게 감소되는 결과를 보여 농도의존적인 효과를 보였다. 간의 TG와 총 콜레스테롤 농도는 이소플라본의 첨가량이 높을수록 감소하는 것으로 나타났으며(p<0.05), 간의 항산화 영양소인 glutathione 함량은 이소플라본을 100 mg 제공한 IF-100에서 유의적으로(p<0.05) 높은 수준을 보였다. 간의 항산화 효소계 활성은 대조군에 비해 이소플라본을 첨가한 식이를 섭취한 군에서 대체로 높게 나타났으며 특히 이소플라본 100 mg을 첨가한 식이를 섭취한 IF-100에서 현저하게 높은 수준을 보였다. 이상의 결과들로 미루어 볼 때 이소플라본의 섭취가 혈장의 중성지방을 저하하고 항염증, 항산화효과를 보여 동맥경화예방에 효과가 있는 물질로 사료된다. This study was carried out to investigate the effect of isoflavone on the atherogenic effect in C57BL/6 mice. C57BL/6 female mice, 5 weeks of age, were fed on chow diets for 2 weeks during adjustment period. Mice weighing approximately $17.9{\pm}0.9\;g$ were divided into 4 groups and were fed on the experimental diets containing isoflavone for 8 weeks. Experimental groups were control (atherogenic diet), IF-10 (atherogenic diet with isoflavone 10 mg/100 g diet), IF-40 (atherogenic diet with isoflavone 40 mg/100 g diet) and IF-100 (atherogenic diet with isoflavone 100 mg/100 g diet). Food efficiency ratio was not different among the experimental groups. Plasma triglyceride (TG) concentrations were lower after 4 weeks in isoflavone supplementation groups than in control group, whereas monocyte chemoattractant protein-1 and thiobarbituric acid reactive substances (TBARS) levels of plasma were significantly (p<0.05) decreased in the isoflavone supplementation groups in a dose dependent manner. Both hepatic TG and cholesterol levels were significantly lowered in IF-100 than control. Hepatic glutathione concentrations were higher in the IF-100 group than in the other groups. Hepatic antioxidant enzyme activities including glutathione-reductase, glutathione-peroxidase, catalase, and Mn-superoxide dismutase were significantly higher in the isoflavone supplemen-tation groups in a dose dependent manner. From the above results, it is concluded that isoflavone may reduce the risk of atherosclerosis via hypolipidemic, anti-oxidative and anti-inflammatory effects.
