芍藥의 補瀉 효능에 대한 고찰 : 역대 문헌의 논설을 중심으로

Objectives : 작약의 상반된 補瀉 효능에 대한 통합적인 설명을 시도하는 것이다. Methods : 본초교과서의 본초사와 중국의학사 서적들을 참고하여 주요 본초서를 선정하였고, 금원사대가 의서들과 종합의서 중에서도 몇 권 선정하였다. 이들 문헌에서 芍藥, 白芍藥, 赤芍藥, 白芍, 赤芍을 키워드로 하여 모든 내용을 검토한 뒤, 補瀉에 대한 내용을 발췌하여 분석하였다. 이렇게 補瀉 작용들을 분류하였을 때 발견되는 상반된 작용들에 초점을 맞춰 문헌에서 이에 대한 기전 설명을 수집하였다. 상반된 작용들 사이에서 기전의 유사성을 파악하고 종합하여 상반된 작용을 아우를 수 있는 상위 기전을 추론하여 파악하였다. Results & conclusion : 작약의 상반된 효능들을 전체적으로 검토한 결과, 작약은 먼저 수렴을 통하여 陰血分으로 氣를 모으고 血中의 氣를 조절하는 것으로써, 血이 원활히 흐르며 瘀血을 없애고 血이 생기도록 하며 熱을 식히는 결과를 가져올 수 있다고 정리할 수 있었다. Objectives : This paper aims to suggest an integrated explanation for the contrary reinforcing/reducing effects of Shaoyao. Methods : From a list of books on the history of herbology and Chinese Medicine as mentioned in the herbology textbook, main texts were selected for further examination, along with some books of the Four Masters of the Jin Yuan period and some general medical texts. These texts were thoroughly examined with keywords such as Shaoyao, Baishaoyao, Chishaoyao, Baishao, Chishao, out of which contents on reinforcing and reducing were selected and analyzed. Next, explanations of the mechanisms of the contrary effects that were found through categorization of the reinforcing and reducing effects were examined. Among the contrary effects, similarities were uncovered and further studied for deduction of an encompassing higher level mechanism. Results & Conclusions : Overall consideration of the contrary effects of Shaoyao revealed that its working mechanism is to collect qi in the yin-blood part through convergence to control qi among blood, to help smooth flow of blood, eliminate blood stagnation and reinforce blood production which in turn cools down heat.

拱辰丹 方義에 대한 고찰 : 『東醫寶鑑』의 내용을 중심으로

Objectives : 본 연구는 임상에 유용하게 활용되기 위해 『東醫寶鑑』의 拱辰丹 내용을 고찰하여 拱辰丹의 方義와 적용 대상과 작용 기전을 밝혀내는 것이 목표이다. Methods : 먼저 한의학 원문 데이터베이스를 활용하여 拱辰丹 관련 자료들을 검토하였다. 또한 『是齋百一選方』과 『東醫寶鑑』의 관련 내용을 분석하고, 拱辰丹의 구성 약재들에 대한 효능을 『東醫寶鑑』과 『중약대사전』을 중심으로 검토하였다. Results & conclusion : 『東醫寶鑑』에서 拱辰丹을 적응증을 연령대와 장부별로 검토한 결과, 연령대별 拱辰丹의 적용 대상은 先天 稟賦가 부족한 소아와 壯年에게는 사용할 수는 있으나 노인에게는 가장 적합하다고는 할 수 없었다. 장부별로는 『東醫寶鑑』 ｢虛勞門｣의 의하면 拱辰丹은 水升火降의 기전에서 水에서 火로 상승시키는 작용이 있어, 肝虛로 인하여 精血이 耗損을 補하면서 기운을 끌어올려 불안하며 두려워하는 精神 증상을 치료할 수 있었다. 따라서 拱辰丹은 젊은 나이에 체력이 약하면서 겁먹고 의욕이 없는 상태에 사용하는 것이 가장 적합하다고 할 수 있다. Objectives : This paper aims to lay out the meaning of Gongjindan, its indications and mechanisms based on relevant contents in the 『Donguibogam』, for better clinical application. Methods : First, Gongjindan related contents were searched in the medical classics database. Next, contents from the 『Shizhaibaiyixuanfang』 and the 『Donguibogam』 were analyzed. Finally, the properties of the Gongjindan ingredients were examined based on the 『Donguibogam』 and the 『Zhongyaodacidian』. Results & Conclusions : Examination of its indications according to the 『Donguibogam』 in terms of applicable age and viscera/bowels, Gongjindan could be applied to children with constitutional insufficiency and elder generations, but it could not be said to be most appropriate for older generations. In regards to viscera/bowels, Gongjindan sends water upwards into fire in the water-rising-fire-descending mechanism, which makes it applicable to symptoms of anxiety and fear by tonifying the consumed Jing and Blood caused by Liver deficiency. To summarize, those who would most benefit from Gongjindan are young adults in weak, fearful and lethargic conditions

Objectives: This study provides basic data for activating the school doctor of Korean medicine (SDKM) program by conducting a questionnaire survey for public health doctor of Korean medicine (PHDKM) Methods: The survey questionnaire was produced by referring to the previous satisfaction survey studies for the Doctor of Korean medicine applying for the SDKM program (SDKMP). The survey questionnaire was composed of 10 questions regarding the satisfaction level, difficulties in progress, and suggestions for activation. The survey was conducted online for a total of 35 PHDKM applying for SDKMP, and 32 responses were collected. Results: Of the 32 subjects, 9 (28.1%) conducted the SDKMP. The total satisfaction level of the SDKMP was 3.69 ± 1.12 points. The lack of a standardized manual and information system was the most difficult in the program's progress (43.8%), followed by the bad cooperation of the public health centers (40.6%), and the lack of personal circumstances (40.6%). The establishment of a system to enlist cooperation with related institutions was suggested the most as a question for activating the SDKMP (71.9%), followed by the expansion of the SDKMP manual and guidance system (53.1%), and the expansion of publicity about SDKMP (37.5%). Conclusions: To activate the SDKMP in a public health environment, it is necessary to improve the system to enlist cooperation with related institutions. Also, the need to expand the standardized manual, the guidance system, and the education to PHDKM about SDKMP.
