Purpose: Various kinds of local flap or free flap have been used for coverage for soft tissue defects with bone exposure over the ankle and dorsum of foot. Adipofascial flaps, nourished by vascular plexuses of the subcutaneous tissue and deep fascia originating from the local perforators of the major vessels, appear particularly to be indicated for the reconstruction of these areas. Our experience with this flap on the dorsum of foot and ankle has also been quite encouraging.Methods: The design of the flap is determined by the size and the location of the defect. The base of the flap is chosen depending on the availability of the soft tissue around the defect. The ratio of the area of the flap to the area of the base wound be more reliable to predict the survival of the turnover flap by the conventional length-to-width ratio. Nineteen patients with defect over the dorsum of the foot and ankle were resurfaced with adipofascial turn-over flaps and skin graft.Results: The average age of the patients was 38.2 years(3-81 years). The flap size was from 2×3cm to 8×5cm. The average follow-up time was 6 months. All flaps survived completely except one case who suffered distal necrosis of the flap. The additional skin graft was required for partial skin loss in the five cases. Other functional impairment was not noted. Conclusion: Dissection of the local adipofascial turnover flap is quite easy, quick, requires less time and sacrifice of surrounding muscle itself, and maintains major arteries. In most cases, donor-site morbidity is minimal with an acceptable scar, and both functional and esthetical results were satisfactory. Therefore, Adipofascial flap could be an option for the difficult wounds around the foot and ankle.
이차성 구순열 비변형 환자에서 비변형과 비폐쇄와의 상관관계

Patients with secondary cleft lip nose deformity usually complain not only aesthetic problems but also functional problems such as nasal obstruction. However, there have been few studies on nasal airway problem in secondary cleft lip nose deformity patients and no reports for the correlation between degree of the deformity and nasal airway obstruction. The authors selected 34 patients with secondary cleft lip nose deformity and measured the degree of morphological deformity by photo analysis and the degree of nasal airway obstruction by VAS score and acoustic rhinometry. Almost all patients complained of more obstructive symptom on the affected side. Functional anatomic structure and volume of the nasal airway examined by acoustic rhinometry showed that narrowest point was located above the nostril and pathological obstruction was more significant in patient group in both side and the area of the narrowest point and the total nasal volume was smaller in patient group. The result that alar contour difference has correlation with total nasal volume but not with objective symptom shows that there may be other factors affecting the nasal obstruction. Therefore the authors conclude that not only the correction of the external morphologic deformity but also the consideration of nasal airway obstruction is essential when performing operation on the secondary cleft lip nose deformity patients.

In order to clarify the shape control mechanism of nonmetallic inclusions in molten steel, the morphology changes of oxide and sulfide inclusions were investigated by Ca treatment in Al-deoxidized steel at 1600℃. The Ca addition of suitableamount into Al-deoxidized steel considerably improved the degree of steel cleanliness because the alumina cluster formed after Al-deoxidization changed to liquid phase calcium-aluminates which float out rapidly from steel bath. And also, when total oxygen content in molten steel was less than about 20ppm, MnS sulfide and Al₂O₃oxide were modified to C type inclusions such as (Ca, Al, Mn) O-(Ca, Mn) S or CaO-Al₂O₃-CaS

Patency of the radial artery plays an important role in the survival of radial forearm fasciocutaneous free flap and artery conduit of coronary artery bypass graft procedure. Even though Allens' test has been used for evaluating the patency of radial artery, the studies on the correlations between risk factors for atherosclerosis and histopathologic findings of radial arteries are rare, until now. Therefore, the authors investigated the correlations between these two factors, and tried to estimate the feasibility of the radial artery in high-risk groups for artherosclerosis.The risk factors for atherosclerosis and lipid profiles were investigated in 38 patients by history taking, physical examinations and blood analysis. And 38 cases of segments of vessel were harvested during the elevation of the flap. The degrees of vessel medial sclerosis were estimated by R values(by Kobayashi and colleagues) that is the median value between the thickness of the intima and that of the media.The measured mean R value was 0.210±0.05. Thirty one cases belonged to Grade I(R<0.25), 7 cases belong to Grade II(0.25<R<0.5), none of cases belong to Grade III(0.5<R<0.75) or Grade IV. Only age factor of the patients demonstrated a positive relationship with R values(Correlation coefficient=0.3675, p<0.05).In conclusions, the radial artery in the radial forearm flap demonstrate no significant narrowing of the lumen due to medial or intimal thickening and radial artery can be used in conduit of coronary arterial bypass graft or forearm free flap regardless of the risk factors of artherosclerosis.

Oil is an important energy source in transportation, agriculture and industry. However, as petroleum consumption continues to increase, the depletion of oil has accelerated, prices have risen, and environmental problems have become an issue of the current generation. Therefore, in various papers, the fuel conversion of biodiesel has been carried out to solve this problem, and the expensive commercialized vegetable oil is used as a raw material for the production of biodiesel, which makes it difficult to commercialize biodiesel internationally. Therefore, in this study, microwave-assisted biodiesel production was performed to save raw materials and time as a solution to this problem. Experiments were carried out using methanol at various acid values. In order to apply it industrially, optimization and analysis of the biodiesel manufacturing process were performed using the central composite design method of the response surface methodology.
