      • 자세평가 프로그램의 타당화 연구

        The misalignment of the vertebra spine affects muscles, tendons and standing posture. Misalignment of the spine provokes the imbalance of shoulders, pelvis and neck posture. The main purpose of this study was to investigate the validity of the digital posture analysis software. 30 college students participated in this study. Posture was evaluated by radiography film and a digital posture analysis program. All the data was collected and statistically analyzed. Measurement of forward head posture appeared 13.39±2.57 mm in postural evaluation software and 16.34±1.94 mm in radiography film. The interclass correlation coefficient between postural analysis software and radiography film is 0.755 (p>0.05). Measurement of shoulder displacement appeared 0.75±0.31° in postural evaluation software and 1.29±0.29° in radiography film. The interclass correlation coefficient between postural analysis software and radiography film is 0.646 (p>0.05). Finally, the measurement of pelvis displacement appeared 0.31±0.34° in postural evaluation software and 0.85±0.38° in radiography film (p>0.05). The interclass correlation coefficient between postural analysis software and radiography film is 0.808. In conclusion, this study suggests the digital posture analysis software is valid for use in the clinic for analyzing a patient’s posture conveniently.

        Effects of Non-Contact Corrective Exercise Continuum for 12 Weeks on Postural Imbalance in Adults

        [OBJECTIVES] This study was to examine the effect of the 12-week video-based non-contact corrective exercise continuum (CEC) program on the forward head posture, central asymmetry, varus and valgus knees for men and women with postural imbalance. [METHODS] Subjects were recruited openly by the Internet community and measured their posture with a 3D whole body scanner (Medicube WS-210, Korea), and then classified into exercise group (n=32, EG) and control group (n=32, CG). Total subjects were classified into forward head posture exercise group (n=8), central asymmetry exercise group (n=8), varus exercise group (n=8), valgus exercise group and each control group (n=8). For the non-contact home training, 12 weeks of 30 minutes/day, 3 times/week, corrective exercise continuum were conducted. The CEC program is an effective treatment to correct the anatomical alignment of the extremities as suggested by the National Academy of Sports Medicine. [RESULTS] As a result, the distance between the centers according to the forward head posture CEC program in EG, the angle of spinal inclination of the cervical and thoracic vertebrae, and central asymmetry were lowered (p<.05). Body shape distribution balance and shoulder tilt angle were lowered in EG according to the central asymmetry CEC program (p <.05). According to the varus CEC program in EG, the distance between the center of the knee, the tilt angle of the femur and tibia, and body shape distribution were lowered (p <.05). According to the valgus CEC program in EG, the distance between the center of the knee increased, and the tilt angle of the femur and tibia decreased (p<.05). [CONCLUSIONS] The effect of improving the anterior head posture, central asymmetry, varus and valgus knees of postural imbalance according to the CEC program was confirmed. It is thought that the CEC program can restore optimal neuromuscular control by restoring proper joint kinematics and restore posture and balance by reducing the observed muscle tension.

        협응이동훈련이 특발성 측만증 환자의 척주 불균형과 선 자세에 미치는 영향 : 단일사례연구

        This study investigates the effects of Coordinative locomotor training on spinal imbalance and posture in patients with idiopathic scoliosis. This study included two patients with idiopathic scoliosis, each with thoracic type. The study was designed as a single-case study (A-B-A’). The total duration of the study was 8 weeks. In the first week, participants were recruited and consent forms were completed. The second week was the initial baseline period, weeks 3-7 were the intervention period, and week 8 was the final baseline period. Coordinative locomotor training consisted of a 10-minute warm-up, 30 minutes of exercise, and a 10-minute cool-down, performed for a total of 50 minutes per session. Primary outcome measures were measured using moti-physio to assess spinal imbalance. Secondary outcome measures were assessed by evaluating posture from the front, side, and back. Statistical analysis was performed using descriptive statistics and visual analysis with graphs to show changes in percentage using Windows 27.0. The results of this study showed that both participants demonstrated improved spinal imbalance and posture during the intervention period compared to the initial baseline period, and maintained this improvement during the final base line period. This study suggests that applying Coordinative locomotor training can be an effective intervention for improving spinal imbalance and posture in patients with idiopathic scoliosis in a rehabilitation setting.

        수기요법과 바른생활자세 병행요법이 신체전면의 불균형 개선에 미치는 효과

        In this study, we examined if the method of using massage therapy combined with manual for correct posture could improve the imbalance of body. To address a clinical trial was conducted for eight weeks targeting the total number of twenty one aged above 30 and under 60 years old women whose office worker. As for the clinical trials experimental groups involves: the control group (5 subjects), test group 1 (8 subjects) groups that receive body balance correction massage therapy once a week, and test group 2 (8 subjects) that receive body balance correction massage therapy once a week and perform the correct posture in daily life according to the manual. Test group 1 and test group 2 has obtained the following results: at the change of the anterior body, in the changes of the gradient shoulder showed a tendency to decrease and decrease in the height of left and right shoulders (p<.05), respectively; tendency to decrease and decrease (p<.01) in angle and height of left and right pelvic gradient, respectively; tendency to decrease and decrease (p<.01) in left­deviation in terms of left and right body balance; and tendency to decrease in length of legs (13.3% more decrease in test group 2). Thus, it was confirmed that body imbalance was improved more effectively when body balance correction massage therapy combined with manual for correct posture in daily life.

        수기요법과 바른생활자세 병행요법이 신체 측면과 후면의 불균형 개선에 미치는 효과

        The purpose of this study was to verify if the method of using massage therapy combined with manual for correct posture in daily life can be an intermediary cause in improving the imbalance of body. To address a clinical trial was conducted for eight weeks targeting the total number of 21 aged above 30 and under 60 years old women whose office worker. As for the experimental groups involves: the control group, group 1 that receive body balance correction massage therapy once a week, and group 2 that perform the correct posture in parallel with group 1 method. As a result, at the change of the lateral body: group 2 showed a decrease in the gradients of pelvic (p<.05) and the height difference (p<.05). At the change of the posterior body: in the changes of the vertebral flexure, group 1 showed a decrease in the gradient (p<.05) and group 1 and 2 showed a decrease in the height difference (p<.05). The gradient of scapula, group 1 and 2 showed a decrease (p<.01), (p<.001) and height difference (p<.01), (p<.001), respectively. In the change of pes introversion & extroversion, left and right feet of group 1 showed a decrease: in the gradient (p<.001), in height difference (p<.01), (p<.001) respectively. And left and right feet of group 2 showed a decrease: in the gradient (p<.01), (p<.001), in height difference (p<.05), (p<.001) respectively. Thus, it was confirmed that body imbalance was improved more effectively when body balance correction massage therapy is performed in parallel with the correct posture in daily life as manual.

        지방공무원의 체형불균형 분석 연구(사례 연구)

        이 연구는 A시에 근무하고 있으며 체형불균형 측정에 참여한 지방공무원 1024명 중 근무 경력이 10년 이상에 해당되는 공무원(남성:523명, 여성:289명; 총 812명)을 대상으로 자세분석을 통해 체형불균형의 양상을 평가하고자 진행되었다. 연구에 참여한 연구대상자의 직장 근무경력은 남성 24.02±7.28(년), 여성 23.77±6.52(년)이며, 평균연 령은 남성:49.8±6.26(세); 여성:48.22±5.73(세), 근무 중 앉아서 생활하는 시간은 남성: 8.61±2.48(시); 여 성:9.23±2.17(시)이다. 분석 결과 통증이 있거나 좋지 않은 부위가 있는 남성은 359명(68.64%), 여성은 233명 (80.62%)으로 여성 공무원이 남성 공무원보다 약 12%정도 높은 비율로 근골격계 통증을 호소하고 있었다. 또한, 규칙적 인 운동을 하고 있는 직원은 남성 256명(48.95%), 여성 132명(45.67%)으로 조사되었다. 정적체형 분석(관상면, 시상 면) 결과 통증이 있는 남성 359명 중 비운동자 192명(53.48%)의 목 근육의 긴장도(PCMT)를 살펴본 결과 5.46±2.99kg이었고, 통증이 있는 여성 233명 중 비운동자 126명(53.08%)의 목 근육의 긴장도는 3.33±1.64kg로 나타났다. 이러한 연구 결과를 토대로 오랫동안 앉아서 근무하는 대표적인 직업형태인 공무원은 자세습관, 운동습관 등이 일자목과 같은 체형불균형에 상당 연관성이 있으며 이러한 체형불균형은 근골격계 질환에도 영향을 미칠 수 있다는 것을 확인할 수 있었다. The purpose of this study was to evaluate body imbalance through posture analysis system for 812 civil servants(man:523; women:289) working at A city. Participants in the study had (men: 21.1±8.4 years; women: 23.77±6.52 years) of work experience with an average age of men: 49.8±6.26years; women: 48.22±5.73years. As a result of this study, There were 359 men(68.64%) & 233 women(80.62%) with pain and 256 men(48.95%) & 132 women(45.67%) who exercised regularly. Static body measurements (coronal and sagittal) showed 4.46±2.99kg of the posterior cervical muscle tension(PCMT) of 192 non-exercises(53.48%) among the man 359 patients and 2.33±1.64kg of PCMT of 126 non-exercises(53.08%) among the man women 233 patients with pain. Posture analysis and questionnaire were closely related to posture imbalance such as forehead posture and musculoskeletal problem, which is a representative occupational form of employees who have been sitting for a long time.

        매트 운동과 걷기 운동 프로그램이 신체불균형 비만초등학생의 신체정렬과 보행능력에 미치는 영향

        본 연구는 2017년 6월부터 2017년 8월까지 12주간 매트 운동과 걷기 운동 프로그램이 신체불균형 비만초등학생(EG그룹/20명, NEG그룹/20명)의 신체정렬 및 보행능력에 미치는 영향을 알아보기 위하여 그룹별 신체정렬 및 보행능력을 알아보았다. 매트 운동과 걷기 운동 운동 프로그램 후 신체정렬의 변화는 EG 그룹에서 미간, 어깨높이, 배꼽, 골반높이, 양측 무릎에서 통계적으로 유의한 차이가 나타났으며, 그룹 간 차이에서는 사후검사에서 통계적으로 유의한 차이가 나타났다. 매트 운동과 걷기 운동 프로그램 후 보행능력에 관한 연구에서는 EG 그룹에서 보행속도, 분속수, 보폭 에서 통계적으로 유의한 차이가 나타났다. 그룹 간 차이에서는 사후검사에서 보행능력의 모든 변인이 통계적으로 유의한 차이가 나타났다. 위의 결과로 보아, 매트 운동과 걷기 운동 프로그램이 신체불균형 초등학생의 신체정렬 및 보행능력의 변화에 긍정적인 효과가 있는 것으로 나타났다. This study was to investigate the effects of mat exercises and gait exercises on obese elementary students posture alignment pursuant and gait abilities in the body imbalance of obese elementary students. The participants were divided into two groups: mat exercises and gait exercise group (EG, n=20) and non exercise group (NEG, n=20). The EG took part in the mat exercises and gait exercise for 12 weeks. The results of this study were as follows: First, there were significantly increased in Posture Alignment pursuant in EG compared with NEG after 12 weeks. Second, there were significantly increased in gait abilities in EG compared with NEG after 12 weeks. As conclusions, this study confirmed that the mat exercises and gait exercises program could improve the posture alignment pursuant and gait abilities of body imbalance of obese elementary students

      • 턱을 앞으로 내민 자세와 견관절 불안정의 치료

        Purpose : The purpose of this study was conducted to find treatment of forward head posture and shoulder instability with proprioceptive neuromuscular facilitation concept. Methods : This is literature study with books, seminar note and international course. Results : Forward head posture and shoulder instability has related each other. Forward head posture will make muscle instability, weakness and stiffness on neck and shoulder girdle. It will make pain also. Important muscle are suboccipital muscles, omohyoid muscle, sternoclaidomastoid muscle, scaleni, pectoralis minor, levator scapular and digastric. Conclusion : Treatment of the forward head posture and shoulder instability is provided. It is that treatment of stiff muscle with eccentric muscle work, muscle elongation, muscle strengthening at the structure level and at the functional level for daily activities.

        뇌성마비 환자의 자세 불균형 탐지를 위한 스마트폰 동영상 기반 보행 분석 시스템

        Gait analysis is an important tool in the clinical management of cerebral palsy, allowing for the assessment of condition severity, identification of potential gait abnormalities, planning and evaluation of interventions, and providing a baseline for future comparisons. However, traditional methods of gait analysis are costly and time-consuming, leading to a need for a more convenient and continuous method. This paper proposes a method for analyzing the posture of cerebral palsy patients using only smartphone videos and deep learning models, including a ResNet-based image tilt correction, AlphaPose for human pose estimation, and SmoothNet for temporal smoothing. The indicators employed in medical practice, such as the imbalance angles of shoulder and pelvis and the joint angles of spine-thighs, knees and ankles, were precisely examined. The proposed system surpassed pose estimation alone, reducing the mean absolute error for imbalance angles in frontal videos from 4.196° to 2.971° and for joint angles in sagittal videos from 5.889° to 5.442°.

        보건의료 학생들을 위한 자기조절 자세교정 운동프로그램 개발: Intervention mapping protocol 활용

        The purpose of this study is to develop a posture correction exercise program based self-regulation using the intervention mapping protocol (IMP) in healthcare professional students. Participants of this study were healthcare professional students at a university in Korea. To develop a posture correction exercise program based self-regulation, literature review and focus group interview for 10 university students majoring in healthcare were conducted in 2016. Then, the program was developed based on IMP. The 6 stages of the IMP, consisting of 1) needs assessment, 2) creation of objectives, 3) selection of theory-based methods and practical strategies, 4) development of the program, 5) development of an adoption and implementation plan, and 6) development of an evaluation design, were utilized in this methodological study to develop the exercise program. For application of the theory-based strategies such as self-regulation, including self-observation, goal-setting, social support, environmental aids, reinforcement, and prevention of recurrence, the intervention program included health education, posture correction exercises, setting long- and short-term goals, and self-help groups. Additionally, group education and one-on-one counselling, and a mobile application were used as methods to implement this exercise program. It is expected that these posture correction exercise program based on self-regulation would be widely available as an intervention tool to improve the musculoskeletal health. 본 연구의 목적은 보건의료 학생들을 대상으로 Intervention mapping protocol (IMP)을 적용한 자기조절 전략 기반의 자세교정 운동 프로그램을 개발하는 것이다. 연구 대상자는 한국 일개 대학교의 보건의료 대학생이다. 자세교정 운동 프로그램 개발을 위해 문헌분석및 10명 학생의 포커스그룹 인터뷰를 2016년에 시행하였다. 자세교정 운동 프로그램은 IMP의 6단계를 적용하여 개발되었다. IMP의 6단계는 1) 요구도 사정, 2) 목표 설정, 3) 이론 기반 중재방법 및 수행전략 결정 4) 프로그램 개발 5) 적용 및 수행계획 개발 6) 평가 디자인 개발로 시행되었다. 운동 자기조절 전략의 하위 개념인 ‘자기관찰’, ‘목표설정’, ‘사회 적지지’, ‘환경적 도움’, ‘강화’, ‘재발방지’ 전략에 따라 건강교육, 자세교정운동, 장단기목표 설정, 자조모임 등을 중재방법으로 선택하고, 집단교육, 1:1상담, 모바일 앱을 수행전략으로 하여 중재 프로그램을 구현하였다. 자기조절 전략 기반의 자세교정 운동 프로그램이 근골 격계 건강 증진을 위한 중재 도구로서 다양한 영역에서 활용될 수 있기를 기대한다.
