        변증논치(辨證論治)와 병증변치(病證辨治) 장단점의 비교고찰 - 고혈압과 두통의 예를 중심으로 -

        Objectives : Comparative review was rendered to evaluate strengths and weaknesses of two common treatment approaches of treatment based on syndrome differentiation and treatment based on syndrome differentiation from disease. A typical symptom of headache from hypertension was chosen for substantial approach of this review. Methods : Pros and cons of two different approaches to disease were evaluated based on literatures and texts focusing treatment based on syndrome differentiation and treatment based on syndrome differentiation from disease. Headache can be correlated with hypertension yet it can be induced by multiple other factors, and headache may/may not accompany hypertension. Hypertension is an example of treatment based on syndrome differentiation from disease and headache can be an example of treatment based on syndrome differentiation. Results : Treatment based on syndrome differentiation can give more autonomy and flexibility in approach to the disease, Intuition, experience, and traditional medical theory can be applied with ease. However, management and eradication of diseases are difficult and standardization of treatment is not easy among practitioners. Treatment based on syndrome differentiation from disease incorporated merits of both eastern and western medicines, achieving more evidence based diagnosis and treatment. Eradication and standardization of disorders are possible with the latter approach. Conclusions : Comparing two systems of treatment based on syndrome differentiation and treatment based on syndrome differentiation from disease with emphasis on hypertension and headache yielded medical values. Treatment based on syndrome differentiation from disease appears to be superior in medical values and effectiveness, but further evaluation and interest are needed to make advancement in Korean traditional medicine.

        구강작열감증후군의 한의학적 치료에 대한 최근 국내외 임상 연구 동향

        Objectives : The purpose of this study is to analyze the oriental medical treatments of burning mouth syndrome, understand the tendency of treatment, and apply it to clinical settings. Methods : We collected case reports, retrospective cohort studies and RCT studies related to oriental medical treatments of burning mouth syndrome using domestic and Chinese databases(CNKI, KISS, RISS, OASIS, KCI). Search terms include 'Burning Mouth Syndrome', 'BMS', 'Burning Tongue', '灼口综合征', and 'oriental medicine', 'oriental medical treatment', 'Korean medicine' and '中医'. A search was conducted by appropriately combining keywords. Results : A total of 27 papers were included in the analysis. Among them, 9 are case studies, 1 is a retrospective cohort study, 1 is a before and after study and 16 are RCT studies. Treatments for burning mouth syndrome included herbal medicine, acupuncture, electro-acupuncture, acupoint injection, auricular acupuncture treatment, external use herbal medicine and gargling. In all studies, symptoms of burning mouth syndrome were alleviated after oriental medical treatments. In 14 RCT studies comparing western medical treatments, the results of the treatment group that included oriental medical treatments were found to be more significant, except for one. Conclusions : As a result of the study, oriental medical treatments are effective in treating burning mouth syndrome. In the future, we hope that clinical research related to oriental medical treatments of burning mouth syndrome will be actively conducted so that evidence-based treatment can be implemented.

        소아 자폐스펙트럼 장애의 추나 치료에 대한 중의학 임상 연구 고찰

        Objectives The purpose of this study is to investigate the effect of Tuina treatment for children with Autism Spectrum Disorder (ASD), and to propose guidance to future studies or application in clinical setting. Methods The articles published from year 2000 to May 2021 by China National Knowledge Infrastructure (CNKI) were searched by setting up field as '中醫學', '中藥', '中西醫結合' and by setting up keyword as '小儿自閉症', '儿童自閉症', '自閉症譜系障碍', 'Autism', 'Autism spectrum disorder' that corresponds to participant and '推拿', '導引', '按摩', 'Tuina', 'massage'. Results Nine selected articles were analyzed and summarized by authors and year published, characteristics, diagnostic criteria, treatment methods and contents, treatment duration, evaluation criteria and results of the study. The articles were also reviewed by reported side effects, and evaluated the quality of the study according to Cochrane Handbook's Risk of Bias 2 (RoB 2) assessment of the Randomized Control Trial (RCT) articles. All studies used Tuina treatment to achieve effective results for pediatric autism spectrum disorder. Conclusion Pediatric Tuina treatment (TM) is a non-invasive treatment that does not stimulate patients with unstable ASD but increase patient adherence, and can be safely and simply operated. Therefore, pediatric Tuina treatment is worth utilizing as a clinical basis for further development of ASD treatment.

        In this article we manually searched all the scriptures in 『Qianjin Fang (Invaluable Prescriptions)』and collected all the content about facial-traumatic diseases to provide literature support to the herbal treatment to cosmetology of TCM. 『Qianjin Fang (Invaluable Prescriptions)』, as a monumental work formed before Tang(唐) Dynasty, also an important and valuable literature, will provide more evidence or clues for herbal treatment to cosmetology of TCM, which will be helpful and meaningful in clinical practice.

        Multimode participation of traditional Chinese medicine in the treatment of COVID-19

        Background The outbreak of COVID-19 has swiftly spread across China and all over the world, resulting in severe contagious pneumonia. However, no specific anti-COVID-19 drugs or methods are available for the treatment of this acute and fatal disease. In recent years, as the efficacy and safety of traditional Chinese medicine (TCM) have been universally acknowledged, it has been brought to a crucial status domestically and overseas for the treatment of COVID-19. Methods We searched relevant literature, electronic databases, and official statements, diagnoses and protocols to retrieve studies and applications related to traditional Chinese medicine for COVID-19 in terms of regulations and policies, clinical evidence, preclinical rationale and big data analysis and then summarized the discovery and development of potential drugs and their targets. Results Clinicians, researchers, governments, the public, colleges, institutes and companies collected and classified associated policies, regulations and actual contributions, searched clinical trials and preclinical experimental outcomes from databases, studied potential TCM drugs with possible mechanisms, retrieved numerous big data analysis method and gathered pooled results of compounds along with their effective targets to make traditional Chinese medicine vital to cover all stages of patients in the treatment and control of COVID-19. Conclusion Traditional Chinese medicine provides new evidence to support the clinical value of TCM for COVID-19.         소아 골절의 한약 치료에 대한 최신 중의학 임상 연구 동향 -대조군 연구 논문을 중심으로-

        Objectives The purpose of this study is to analyze the clinical research literature of TCM (traditional Chinese medicine) which is applicable to pediatric fracture and to broaden the field about clinical application of Korean medicine treatment for pediatric fracture. Methods We searched randomized controlled trials about TCM treatment of pediatric fracture from the CNKI (China National Knowledge Infrastructure) January 2013 to December 2017. We searched the literature and analyzed the treatment methods and the results. Results 31 papers were selected from 63 studies. In most studies, the total effective rate of the treatment group was significantly higher than that of the control group, and the bone healing time of the treatment group was significantly lower than that of the control group. Methods of herb treatment include internal medicine (IM), fumigation (FG) and external application (EA). The most commonly used herb medicine were Angelicae Gigantis Radix (當歸), Carthami Flos (紅花), Drynaria Fortunei (骨碎補), Paeonia Lactiflora Pallas (芍藥), Persicae Semen (桃仁), Lycopodii Herba (伸筋草), Ligusticum Chuanxiong Hort (川芎), Olibanum (乳香), Salviae Miltiorrhizae Radix (丹蔘), and Panax Noto Ginseng (三七). Conclusions The use of herb medicine in pediatric fractures has resulted in faster and more effective restoration of fracture union than the Western medical therapy alone. Based on the results of this study, it is possible to widen the scope of Korean medicine if additional clinical studies on pediatric fracture were conducted.

        비정형유방증식에 대한 최근 중의 약물치료 동향에 대한 문헌연구

        Objectives: We conducted a literature study on the treatment trends in China to find out the possibility of Oriental medicine treatment of atypical hyperplasia of breast (AHB). Methods: RCTs (randomized controlled trial) on AHB were collected from CNKI (China National Knowledge Infrastructure). The search words were “乳腺增生”, “乳腺囊性增生”, “乳癖”, “中医”, “中药” and “中西医结合”. The search period was limited from July 2006 to May 2017. Finally, we selected 107 RCTs which were clinical studies to find out the effectiveness of Chinese herbal medicine in comparison with Western medicine. After reviewing, we investigated Chinese herbal medication guide, Chinese treatment method and prescriptions. And the correlation between the treatments and the medicinal herbs was investigated to be useful in the clinical practice. Results: 1. The administration of herbal medicine was 58.9 percent in 63 cases, followed by menstrual cycles, and 41.1 percent in 44 cases, regardless of menstrual cycles. 2. In the basic frequency analysis between the treatment and the medicinal herb, the frequency of dissipate binds (散結) was the highest. Next, there was a high frequency of therapies such as activating blood-activating (活血), relieve pain (止痛), soothe the liver (疏肝), regulate qi (理氣), resolve phlegm (化痰), soften hardness (軟堅), resolve depression (解鬱), move qi (行氣) of frequency was high. In herbal medicine, bupleuri radix (柴胡), cyperi rhizoma (香附子), angelicae gigantis radix (當歸), fritillaria thunbergii bulb (貝母), paeoniae radix alba (白芍藥), prunellae spica (夏枯草), corydalis rhizoma (玄胡索) showed high frequency. 3. We finded out the correlation between the frequent treatment methods and the medicinal herbs using Text Mining. Conclusions: These findings are thought to help implement Korean traditional medicine treatments for AHB.

        COVID-19 중의 진료지침에 수록된 예방 처방 분석

        Objectives : This study collected and analyzed information related to preventive formulas from continuously published and revised COVID-19 treatment guidelines in various regions of China. Methods : We collected treatment guidelines published in different regions of China and categorized formulas for prevention and medical observation period listed in them according to the editions. The categorized preventive formulas were compared by type and target group. Results : Herbal medicines used for prevention included formulas derived from Korean medical classics, such as Okbyeongpungsan(玉屏風散) and Eunkyosan(銀翹散). The newly composed formulas, totaling over 100, were created by adding, subtracting, and combining formulas such as Sang-gukeum(桑菊飮), Eunkyosan(銀翹散), Sasammaekmundongtang(沙參麥門冬湯), Okbyeongpungsan(玉屏風散), Gwakhyangjeong-gisan(藿香正氣散), and Soshihotang(小柴胡湯). Patent medicines including Huoxiangzhengqi capsule(藿香正氣膠囊), Lianhuaqingwen capsule(連花淸瘟膠囊), Shufengjiedu capsule(疏風解毒膠囊), and Jinhuaqinggan granule(金花淸感顆粒) were frequently used, mainly targeting close contacts. These medicines were used differently depending on the specific population group, such as the general population, the elderly, children, pregnant women, and patients with underlying diseases, and were also applied differently according to the individual’s constitution. Conclusion : We were able to identify various background factors contained in the guidelines for the use of preventive formulas presented by TCM group, and understand the social conditions that enabled the group to provide such guidelines. Through this, thorough preparation should be made so that the Korean Medicine can actively respond to another future pandemic.

        간섬유화는 다양한 만성 간질환에 기인하는 간실질의 결합조직의 과도한 증식을 말하며, 간경화로 발전하는 중간과정이다. 중국과 한국에는 많은 간섬유화 환자가 있지만 현재까지 서양의학에서 간섬유화를 효과적으로 치료할 수 있는 치료법은 발견되지 않고 있다. 최근 수년간 중국전통의학의 임상과 실험연구에서 많은 발전이 있었다. 전통의학이론을 바탕으로 활혈거어(活血祛瘀), 익기(益氣)의 효능이 있는 한약제가 항섬유화 효능을 보였다. 전통중국의학에서 간섬유화에 대한 일반적인 치료는 유효 성분, 단방 및 복방 처방의 세 부분으로 나뇐다. 우리는 중국전통의학에서 간섬유화의 효과적인 치료와 관련된 작용기작을 소개하였다.
