        The Effect of Socio-Cultural Attitude towards Appearance on the Medical Skin Care Customer Behaviors' Intention: With a Focus on the Sequential Mediating Effect of the Medical Skin Care Involvement and the Appearance Related Quality of Life

        Purpose: The purpose of this study is such that, the socio-cultural attitude towards appearance and the effect of medical skin care customer behaviors’ intention were analyzed, and the effect of medical skin care involvement and the appearance related quality of life sequentially was studied, while the basic data was presented for stra-tegic counseling and promotional marketing expansion of medical skin care according to socio-cultural attitude towards appearance and customer behaviors’ intention. Method: In this study, a statistical analysis was performed by using the SPSS 25.0 and PROCESS macro for SPSS programs as an empirical analytical method for 507 copies through an online questionnaire in the Seoul metro-politan area targeting men and women in their 20s and 60s, and the independent sample t-test was used to confirm the difference among the major variables according to gender, and the model 6 analysis was performed by using the SPSS Process Macro to verify the research model. Results: In terms of the results of this study, first, as a result of confirming the differences among the major variables according to gender, significant differences were found in the socio-cultural attitude towards appear-ance, appearance related quality of life, and the customer behaviors’ intention. The socio-cultural attitude to-wards appearance did not directly affect the customer behaviors’ intention, but a high extent of medical skin care involvement increased appearance related quality of life and affected the customer behaviors’ intention where the sequential mediating effect was found, and in terms of the difference between male and female pathways, it was found that the higher the quality of life related to appearance, the higher the customer behaviors’ intention. Conclusion: According to the results of this study, women with high skin care behavior according to the socio-cultural attitude and aesthetic desire and men with a weakened relationship between quality of life and customer behaviors’ intention were appropriate for the intention to purchase according to their appearance, depending on gender, and from a positive point of view, it is necessary to approach the counseling services and marketing that can increase the intention to purchase through the external beauty. The increase in the appearance related qual-ity of life according to the extent of medical skin care involvement affects the customer behaviors’ intention, which provides the service information that can promote a stable life and provides self-care through the various beauty lifestyles through the skin care, and hence, it is considered that it would be necessary to provide the information that not only self-satisfaction but also the overall quality of life is affected.

        라이프스타일에 따른 소비자의 스킨케어 선호도에 관한 연구 -일반스킨케어와 메디컬스킨케어를 비교대상으로-

        Modern people has grown attentive for appearance, which has been reflected in their life-style. This trend tends to polarize skin care market as market of general skin care and medical skin care. The goals of this study are to present marketing strategy for meeting needs of detailed market by doing an analysis of purchasing behavior between general skin care and medical skin care according to this customer`s changed life-style. In order to achieve the goals, a questionnaire survey was performed for randomly selected 500 customers using the skin care. These surveys were analyzed as with SPSS-WIN 19.0. and performed as frequency, factor, reliability, cluster, cross analysis and ANOVA. The customer`s 5 life-style factors, which are classified as health & economic-oriented life-style, leisure-oriented life-style, appearance & achievement-oriented life-style and convenience-oriented life-style, showed statistically significant difference. According to groups, it appeared to have a difference in preferred skin care style. Especially, appearance & achievement-oriented lifestyle was preferred from both general and medical skin care. Among the skin cares, face care in general skin care and laser therapy in medical skin care were occupied most of skin cares. In conclusion, the results of this study are to expect to be utilized as fundamental resource which plan differentiated marketing strategy for meeting of life-style of customers using skin care services.

        대학병원 피부과 방문 환자들의 피부 질환 치료를 위한 의료 서비스 이용 행태에 대한 설문조사

        Background: Analyzing the medical care utilization behavior and conception of disease treatment of dermatologic patients is important. However, the Korean literature has few studies reporting this information. Objective: The aim of this study is to evaluate medical care utilization behavior and conception of disease treatment of dermatologic patients. Methods: A written questionnaire concerning personal information, treatment behavior, treatment conception, patient satisfaction, and patient compliance was completed by 442 outpatients who visited the department of dermatology at a university hospital. Results: According to the patients, satisfactory outpatient consultation time for dermatology was 14.2 minutes for the first-visit consultation, and 9.92 minutes for the second-visit consultation. Most (76.0%) patients initially sought the dermatologic clinics for dermatoses, but only 39.8% of patients knew how to distinguish dermatologists from other doctors. Among the participants, 26.7% of patients directly visited a tertiary medical center without visiting primary clinics. Before visiting the hospital, 52.3% of patients sought disease information, and 28.3% of patients obtained information about doctors. Some respondents (39.7%) had a negative attitude about dermatologic medicine because of lay referral. Men were highly satisfied with explanations of drugs and had more positivity towards drugs, but showed low compliance to applying topical medication. Patients of older age, lower education level, and lower economic status had lower satisfaction with explanations given during the examination. Conclusion: We advise dermatologists to play a major role in enlightening patients and constructing proper information delivery systems via diversified routes to prevent unreasonable medical care utilization behavior and groundless negative conceptions about dermatologic treatment.         노인의 사회자본과 건강위해 행위 및 건강수준이 의료서비스 이용에 미치는 영향

        Objectives This study aimed to investigate the effect of social capital, health risk behavior and health status on medical care utilization by the elderly. The data and Research method Data were obtained from the 4th wave survey of the Korea Welfare Panel Study. 4,087 household members aged 65 years and over were subject to analysis. Descriptive statistics are used to describe the basic features of the data in a study. we performed a structural equation modeling(SEM) analysis to evaluate the effect of social capital and mediating effect of health risk behavior and health status. Results Results showed that factors related to medical care utilization of the elderly were different depending on types of service (inpatient and outpatient service) except health status. Age, higher social capital, more health-risk behavior and poorer health status were associated with increased use of inpatient service. Social capital was found to have a positive direct effect on it. Also, social capital had an indirect effect on reducing use of inpatient services by improving health status. On the other hand, lower age and higher household income tended to increase use of outpatient service, while higher social capital and higher health status were inversely related. Social capital had a direct effect and an indirect effect on reducing use of outpatient service and, at that time, health status played a mediating role. Conclusions Social capital may contribute to improve health status and indirectly reduce medical care utilization of the elderly by enhancing their health status. These results provide evidence that more policy and strategy considerations should be needed for the elderly to strengthen their social capital in order to enhance their levels of health and more efficient utilization of medical care.

      • K-DRG 대상환자의 진료행태와 진료비 분석에 관한 연구

        This study was intended to analyze the medical care behavior and medical care bills of K-DRG patients. For this study, samples were derived from discharge abstract file and medical claim bill file of patients who were discharged with the K-DRG(cataract extra tonsillectomy. appendectomy, c/s, vaginal delivery)from the 3^(rd) general hospital(A, B) from January to March, 1996. In order to analyze the medical care behavior, discharge abstracts of 2025 patients from A&B hospitals were retrieved and to analyze the medical care bills, 1259patients form B hospital were retrieved. The followings are the results of this analysis 1. The B hospital had higher composition rates and longer average LOS, especially cataract extraction had longer average LOS in spite of lower severity rates than the A hospital. 2. While the A hospital had higher rates in admission via ER(delivery room) ICU care, & spinal anesthesia(vaginal delivery), the B hospital had higher rate in admission via C OPD, transfer & consultation rate and longer waiting days for surgery. 3. In the medical care bill analysis(A hospital, severity 0) by hospital days, DRG 0390 could produce the higest income during #HD 2?3?4, DRG 0590?0591 were during #HD 3?4?5?6 and the left DRGs were from #HD 1. 4. The A hospital had longer average LOS & higher medical care bills than the national standard suggested by Ministry Health & Welfair Affairs. But estimated medical care bills, substituted the national standard LOS for the A hospital LOS by SIGMA PLOT & TABLE CURVE PROGRAM, were lower cost than the national standard except for appendectomy. The methodology of the study will give suggestive ideas to meet the changes of the DRG situation in the hospitals.

        신포괄수가에 영향을 미치는 의료행태 요인 분석 - 내과 입원환자 중심으로

        Purpose: The purpose of this study is to investigate medical care behaviors influencing accuracy of the payment based New diagnosis-related groups (DRG) compared to fee for service (FFS) in hospitalized patients with medical illness. Methodology: In order to estimate the difference in medical costs between New DRG and FFS depending on medical care behaviors, medical records and hospital claims data (n=4,232) were utilized, which were collected from a single public hospital during the first-half of 2018. Data were analyzed by descriptive statistics, t-test, chi-square test, and multivariate binary logistic regression. Findings: The average difference in medical costs between New DRG and FFS were KRW 506,711±13,945 with incentives and KRW -51,506±12,979 without incentives, respectively. Forty-four point two percent (44.2%, n=1,872) of total subjects were shown to have negative compensation in overall medical costs with New DRG compared to the costs with FFS. Medical care behaviors that affected on the negative compensation were the presence of severe bed sores on admission, medical consultations, death, operations, medications and laboratory or imaging tests with unit price over KRW 100,000, hospital-acquired complications or underlying comorbidities, elderly patients (≧65 years), and hospitalized for more than average inpatient days defined by New DRG (p<0.001). The difference in average medical cost between New DRG and FFS for a group with mild illness was KRW –11,900±10,544, whereas it was KRW –196,800±46,364 for a group with severe illness (p<0.0001). Practical Implications: These findings suggest that New DRG payment model without incentives may incompletely cover the variation of medical costs in real clinical practice. Therefore, policy makers need to consider that the current New DRG reimbursement should be focused and refined to improve accuracy of payment on medical care resources utilized in severe and complex medical conditions.

        Factors Influencing Behavioral Intention for Mobile Applications of Health Care Escort Service

        Mobile applications of health care escort services (MA-HCES) are applications for booking escorts that accompany patients and offer a range of services during medical visits, including home delivery, queue registration, and face-to-face consultations. Young single-person households are the users of MA-HCES, making it necessary to investigate their behavioral intention to use MA-HCES. This study aims to develop and validate a research model based on the Theory of Planned Behavior (TPB) to identify factors influencing the behavioral intention of this group toward MA-HCES. The research model includes subjective norms, perceived behavioral control, attitude, emotional preference, behavioral intention, and privacy concerns. These factors respectively represent the social pressures felt by individuals when performing the behavior, their perceived ease or difficulty of the behavior, their overall evaluation of the behavior, the emotions that drive the behavior, the inclination to perform the behavior, and their concerns about the extent of personal privacy exposure to others. Data were collected through a survey of 322 young single-person households, and a structural equation model (SEM) was employed to test the research model. The results indicate that subjective norms, perceived behavioral control, attitude, and affective preference positively influence behavioral intention. Subjective norms and emotional preference also have positive influence on attitude. Privacy concerns have a negative influence on attitude but do not influence behavioral intention. This study establishes an innovative model that can predict the behavioral intention of young singleperson households to use MA-HCES, contributing to both practice and theory.
