        흰쥐의 좌골신경 신경손상에 따른 골격근 중간세사단백의 발현변화 실험적 신경성근병중에서의 desmin과 vimentin으 발현

        중간세사단백인 desmin은 성숙한 근성유의 세포골격을 이루며 Vnnentin은 근재생시 중간엽기원 세포에 나타나기 때문에 중간세사단액의 발현여부로 근재생과정을 주석할 수 있다 신경성근영총에 따른 근재생과정을 중간세사의 변화로 알아보고자 흰쥐의 좌골신경을 압궤손상시킨 후 시기별 (2 4 6, 15 20, 27, 69일)로 장딴지근을 정취해 냉동연속조직절편을 제작한 뒤 연역조직화학적 반응을 시켜 desmin 및 Vlmentin 의 발현변화를 조사하여 다음과 같은 결과를 얻었다 l. 정상적인 흰쥐 장딴지근의 근섬유에서 desmin은 약하게 반응한 반연 Vlmentm 은 반응이 없었고 신경 손상 후 2, 4일째까지 같은 현상을 보였다. 2. 신경 손상후 6일째에 처음으로 desmin과 Vlmentin 에 모두 강한 양성연역반응을 보이는 위축된 근섬유들이 관찰되었다. 3. 신경손상 후 15일째에서 위축왼 근섬유을이 증가되었고 정상 근성유보다 desmm 에 강한 면역반응성을 나타내었다. Desmin과 vlmentm 모두에 양성반응율 보인 위쭉왼 근섬유들이 소수 관찰되었다. 4. 신경손상 후 20일째에서 대부분의 근섬유들이 정상 근성유 보다 너 강한 desmin 양성연역반응을 보였다. 일부 근섬유들은 중심핵을 갖고 있었으며 desmin과 Vimentin에 대한 양성연역반응을 동시에 나타내었다 5. 신경손상 후 27일째에서 대부분의 근섬유들은 정상 크기로 회복되었으나 소수의 근섬유들은 위축된 상태였다 위축된 근섬유은 desmin에는 반응성을 보이지 않은 반면 Vlmentm 에 매해서는 은섬유의 일부가 양성반응을 나타내였다. 6. 신경손상후 69일째에서 비대된 근섬유가 많이 관찰되였고 desmin 및 vinentin에 대한 면역반응은 정상근섬유와 동일하였다. 일부 위축된 근섬유들은 desmin 에 양성 면역반응을 보였으나 Vlmentm 에는 양성 면역반응을 보인 근섬유는 관찰되지 않았다. 위의 결과로 미루어 볼 때 약한 desmm 반응 만을 보이는 은섬유는 정상 강한 desmm잔응만을 나타내는 근섬유는 변성 (degeneration), desmm 과 Vlmentm 이 동시에 나타내는 근섬유는 재생 (regenerahon) 중인 근섬유라고 규정지을 수 있겠다 그러므로 중간세사 단액인 desmin 과 Vlinentm 발현 어부로 신경성근육병증에는 물론 다양한 근육병증의 진단 및 근육발생에도 유용하게 이용될 수 있을 것으로 사료된다.

      • Efficacy and Safety in Genotype 1b HCV Infected Patients Who Are Treated by Ledipasvir/sofosbuvir: A Real Life and Multicenter Study

        Background/Aim: Combination of ledipasvir (LDV)/sofosbuvir (SOF) ± ribavirin (RBV) has been approved in Korea for the treatment of genotype 1b (GT1b) hepatitis C virus (HCV) infected patients with NS5A resistance-associated variants (RAVs). The efficacy and safety were investigated in LDV/SOF±RBV treated patients. Methods: All HCV GT1b patients with NS5A RAV who were treated with LDV/SOF±RBV for at least 4 weeks from March 2016 to January 2017, were retrospectively enrolled from five hospitals. Virologic response was measured at 4 weeks (rapid virologic response, RVR), at 12 weeks (end of treatment response, ETR), and at 12 weeks after the end of treatment (sustained virologic response, SVR12). Safety was assessed by review of adverse events, physical examinations, and laboratory findings. Results: A total of 18 RAV-positive patients were treated with LDV/SOF±RBV at least 4 weeks [n=17, LDV/SOF; n= 1, LDV/SOF+RBV for treatment-experienced patients with liver cirrhosis (LC)]. Sixteen patients had Y93H variants and 2 patients had L31 plus Y93 double-substituted variants. Baseline characteristics were obtained: mean age (58±10 years), gender (male: 44.4%), LC (38.9%), treatment-naive (66.6%), mean AST (66.8±49.0 IU/L), mean ALT (66.2±36.8 IU/L), and mean HCV RNA level (2,789,457±5,007,723 IU/mL). Overall, RVR (14/16, 87.5%, HCV RNA was not checked in 2 patients at 4 weeks), ETR (9/9, 100 %), and SVR (5/5, 100 %) rates were obtained. Patients with SVR (n=5) included 3 patients with LC and 3 treatment-naive patients. There was no case of serious adverse events. Conclusions: LDV/SOF±RBV treatment for HCV GT1b infected Korean subjects with RAV achieved very high SVR rates.

        $K-BEtest^{\circledR}$, 새로운 생물학적 동등성 시험 통계처리 프로그램의 개발

        A computer program for personal computers, $K-BEtest^{\circledR}$, was developed to analyze bioequivalence data in accordance with Korean Guidelines for Bioequivalence Test (KGBT). This program is user-friendly, interactive, Hangul-compatible and supports $2{\times}2$ cross-over design as well as $2{\times}2$ Latin square design with various significance levels. This program is able to calculate AUC, $C_{max}$ and $T_{max}$ parameters from the blood drug concentration-time profile of individual subjects and evaluate the parameters statistically for the bioequivalence by ${\pm}20%$ rule, the F-test, the Non-centrality test and 90% confidence intervals. All procedures are supported with graphic interface, interactive menu and outputs in Korean. In this paper, two experimental data sets were analyzed by the program and detailed process was demonstrated. The $K-BEtest^{\circledR}$ program appears to be very effective for analyzing bioequivalence data and can be widely used with convenience and accuracy.

      • Plenary Session 2 : PS-2-5 ; A nationwide seroepidemiology of hepatitis C virus infection in South Korea

        Background: Hepatitis C virus (HCV) infection leads to significant public health problem. The aim of this study is to reveal nationwide seroprevalence of HCV infection by a large-scale survey in South Korea. Methods: From January to December 2009, a total of 291,314 adults (≥20 years) underwent health check-up in 29 centers. The data of anti-HCV and biochemical tests were obtained from all participants. Among subjects with positive anti-HCV, such data as HCV RNA, genotypes and treatment detail were additionally obtained. Results: The overall seroprevalence of anti-HCV was 0.59% (1,718/291,314) in South Korea. Anti-HCV prevalence in females (0.60%, 729/112,506) was higher than that in males (0.55%, 989/178,808) (p=0.014). Gradual increase of anti-HCV positivity was observed, from 0.26% in twenties to 2.09% over seventies. The age-adjusted anti-HCV prevalence varied in different areas; higher in Busan, Jeonnam and Gyeongnam (1.34-1.86%), middle in Seoul and surrounding districts (0.42-0.77%), lower in Jeju (0.25%). Based on the Census 2009 of Korea, the age, sex, and area-adjusted anti-HCV positive rate in South Korea was estimated to be 0.78%. Among the 1,605 anti-HCV positive subjects, qualitative or quantitative serum HCV RNA was measured only in 478 people, of whom 268 (56.1%) patients had detectable HCV RNA in serum. Among them, the residents in Gyeongsang, Seoul, Gyeonggi area showed higher frequency of positive HCV RNA than other areas, and 252 patients (94.0%) were diagnosed as chronic hepatitis, 11 patients (4.1%) as cirrhosis, and 5 patients (1.9%) as hepatocellular carcinoma in 5 (1.9%) patients. A total 61 patients received antiviral therapy after 2009 and overall sustained virologic response was achieved in 42 (84%) patients. Conclusions: Our data suggest that South Korea is one of the countries of relatively low overall HCV prevalence, which increase with age, especially over 5th decade. Further study on the risk factors and geographic difference of HCV epidemiology is warranted.

        IgG4 related disease is a fibro-infiammatory disease, characterized by lymphoplasmacytic infiltration of various organs and frequently affected middle-aged males. Massive pleural effusion in children is an uncommon feature in IgG4 related pulmonary disease (IPD). We report a case of young male with extensive IPD presenting mediastinal mass, massive pleural effusion and mesenteric lymphadenopathy. A 16-year-old male was admitted for treatment on a recurrent bilateral pleural effusion. He has a history of admission to other hospital because of similar symptoms last year. At that time, Chest CT showed bilateral pleural effusion and anterior mediastinal soft tissue density. Video- assisted thoracoscopic mediastinal biopsy revealed angiomyolipoma. The follow-up chest CT findings were not specific but progressive increase of pleural effusion may suggest diffuse pleural involvement of IgG4-related disease. Abdomen CT showed diffusely increased attenuation of the mesentery and enlarged mesenteric lymph nodes, which was also associated with IgG4-related disease. Our histopathological review of prior mediastinal biopsy was consistent with IgG4 related sclerosing disease rather than angiomyolipoma, which included papillary proliferation of reactive mesothelial cells and dense collagenous . brosis with stromal dense lymphoplasmacytic infiltration. [IgG4-/IgG-(+) plasma cell>40%, IgG4: maximal count 62/HPF, HMB-45(-)] In addition, serum IgG level was elevated (626mg/dl, normal range 700~1600mg/dl), as was the serum IgG4 level (1650mg/dl, normal range 110~1570md/dl), thereby con. rming a diagnosis of IgG4 related disease. The characteristic of his pleural fiuid was exudate and chylothorax (triglyceride 551mg/dl). He was treated with high dose steroid. The present case is the first case of extensive IPD in a 16-year-old male in our country. We recommend that clinicians consider IgG4-related disease as a possible differential diagnosis from the children with massive pleural effusion, elevation of serum IgG4/IgG and lymphoplasmacytic in. ltrated in pathology.

        Alternative Lengthening of Telomeres in Primary Pancreatic Neuroendocrine Tumors Is Associated with Aggressive Clinical Behavior and Poor Survival

        <P><B>Purpose:</B> Alternative lengthening of telomeres (ALT), a telomerase-independent telomere maintenance mechanism, is strongly associated with ATRX and DAXX alterations and occurs frequently in pancreatic neuroendocrine tumors (PanNET).</P><P><B>Experimental Design:</B> In a Korean cohort of 269 surgically resected primary PanNETs and 19 sporadic microadenomas, ALT status and nuclear ATRX and DAXX protein expression were assessed and compared with clinicopathologic factors.</P><P><B>Results:</B> In PanNETs, ALT or loss of ATRX/DAXX nuclear expression was observed in 20.8% and 19.3%, respectively, whereas microadenomas were not altered. ALT-positive PanNETs displayed a significantly higher grade, size, and pT classification (all, <I>P</I> < 0.001). ALT also strongly correlated with lymphovascular (<I>P</I> < 0.001) and perineural invasion (<I>P</I> = 0.001) and the presence of lymph node (<I>P</I> < 0.001) and distant metastases (<I>P</I> = 0.002). Furthermore, patients with ALT-positive primary PanNETs had a shorter recurrence-free survival [HR = 3.38; 95% confidence interval (CI), 1.83–6.27; <I>P</I> < 0.001]. Interestingly, when limiting to patients with distant metastases, those with ALT-positive primary tumors had significantly better overall survival (HR = 0.23; 95% CI, 0.08–0.68; <I>P</I> = 0.008). Similarly, tumors with loss of ATRX/DAXX expression were significantly associated with ALT (<I>P</I> < 0.001), aggressive clinical behavior, and reduced recurrence-free survival (<I>P</I> < 0.001). However, similar to ALT, when limiting to patients with distant metastases, loss of ATRX/DAXX expression was associated with better overall survival (<I>P</I> = 0.003).</P><P><B>Conclusions:</B> Both primary ALT-positive and ATRX/DAXX-negative PanNETs are independently associated with aggressive clinicopathologic behavior and displayed reduced recurrence-free survival. In contrast, ALT activation and loss of ATRX/DAXX are both associated with better overall survival in patients with metastases.       • Genome sequence of mungbean and insights into evolution within Vigna species

        Mungbean (Vigna radiata) is a fast-growing, warm-season legume crop that is primarily cultivated in developing countries of Asia. We constructed a draft genome sequence of mungbean to facilitate genome research into the subgenus Ceratotropis and to enable a better understanding of the evolution of leguminous species. The draft genome sequence covers 80% of the estimated genome, of which 50.1% consists of repetitive sequences. In total, 22,427 high confidence protein-coding genes were predicted. Based on the de novo assembly of additional wild mungbean species, the divergence of what was eventually domesticated and the sampled wild mungbean species appears to have predated domestication. Moreover, the de novo assembly of a tetraploid Vigna species (Vigna reflexo-pilosa var. glabra) provided genomic evidence of a recent allopolyploid event. To further study speciation, we compared de novo RNA-seq assemblies of 22 accessions of 18 Vigna species and protein sets of Glycine max and Cajanus cajan. The species tree was constructed by a Bayesian Markov chain Monte Carlo method using highly confident orthologs shared by all 24 accessions. The present assembly of V. radiata var. radiata will facilitate genome research and accelerate molecular breeding of the subgenus Ceratotropis.
